Linking Knowledge Transfer and Competency Development: The Role of Lectures in a Family Medicine Curriculum
(1) Background: Medical education is moving from a cognition-based to a competency based model in Germany. Traditional learning activities (LAs) are questioned. Some
stakeholders criticise traditional LAs for not facilitating deep learning or operational com petency transfer required in practical contexts. This qualitative study aims to take a closer
look at the role of lectures in competency-based medical education from a student’s point
of view. (2) Methods: Three semi-structured group interviews were held with students
from the family medicine curriculum in the summer semester of 2021. Questions focused
on the three lectures in this family medicine curriculum and on students’ experiences with
lectures in general. One additional expert interview was held with one of the lecturers. The
video-recorded interviews were transcribed verbatim and analysed using content analysis.
(3) Results: Interview participants highlighted entertainment, the provision of a social
and physical learning environment, and the completion of knowledge from books and
educational websites as important roles of lectures. Lectures on demand were used by
interviewees for time- and space-independent repetition. Lecturer-dependent qualitative
differences between lectures were identified by interviewees. Important differences were
the extent of interaction, as well as the enthusiasm and preparation of the lecturer. (4) Con clusions: Even though literature suggests that lectures may be a less effective learning
activity, under certain circumstances, several aspects make them an essential element of
modern curriculum development. By raising interest in a subject, providing a space for
discussion and social interaction, interactive lectures appear to be a helpful link between
knowledge acquisition and practical training of competencies

A uniaxial magnetic balance was used to directly measure the change in ferromagnetic volume fraction of the fatigue specimens as the fatigue load was applied. Informed Yet Unvaccinated: Investigating COVID-19 Vaccine Hesitancy Among Syrian University Students Using the 5C Framework
Background: Vaccine hesitancy (VH) remains a pressing global health concern, particularly in low-resource settings, where vaccination remains the primary means of protection against infection. The urgency of this issue became evident during the COVID-19 pandemic. The present study aimed to elucidate the determinants of vaccine hesitancy among university students in medical and non-medical fields in Syria by utilizing the 5C framework (confidence, complacency, constraints, calculation, and collective responsibility). Methods: A structured interview-administered questionnaire collected responses from 4722 students at five universities in Syria. The questionnaire assessed sociodemographic factors, COVID-19 vaccination status, vaccination experience, sources of information, beliefs in vaccine-related conspiracies, attitudes toward vaccine policies and attributes, and the 5C psychological antecedents. Internal consistency and factor analysis of the Arabic 5C scale were performed to ensure construct validity. Statistical analyses included descriptive statistics, logistic regression, and multivariate multiple regression. Results: Our findings revealed that 64% of participants had not received the COVID-19 vaccine, with official sources (e.g., WHO, Ministry of Health) being the most trusted. The highest 5C score was for calculation (5.86, sd = 1.21), followed by confidence (5.29, sd = 1.26). Belief in vaccine conspiracies was common, particularly regarding profit motives and genetic modification. Only three of the 5C—complacency, calculation, and collective responsibility—significantly, predicted vaccination behavior, while all the 5C were influenced by contextual factors. Non-medical students showed significantly higher hesitancy (OR = 1.60, 95% CI [1.39–1.84, p < 0.001]) compared to their medical counterparts. Hesitant respondents displayed significantly, higher complacency, increased calculation, and reduced collective responsibility scores. Conspiracy beliefs eroded confidence and magnified perceived barriers, whereas trust in official sources and favorable views of the vaccine’s attributes strengthened collective responsibility and acceptance. The regression models explained 2.8% to 11.2% of variance across the 5C, with collective responsibility showing the highest explanatory power (adjusted R2 = 0.112). Conclusions: Despite high self-reported knowledge, significant VH persisted—a paradox that highlights the limits of information alone. Given the cross-sectional design and the modest explanatory power of the models, these recommendations are tentative. These findings highlight the need for trust-based interventions targeting populations in conflict-affected areas
Ökonomische Analyse ausgewählter Aspekte des Sanierungs- und Insolvenzrechts
Die vorliegende Arbeit befasst sich mit den verschiedenen Sanierungsmöglichkeiten für Unternehmen im Rahmen von Unternehmenskrisen – abhängig vom jeweiligen Stadium. Besonders seit der COVID-19-Pandemie, aber auch bedingt durch andere globale und wirtschaftliche Herausforderungen, treten öfter finanzielle Schieflagen bei Unternehmen auf. Eine wichtige Neuerung wurde in diesem Zusammenhang mit dem Gesetz über den Stabilisierungs- und Restrukturierungsrahmen (StaRuG) geschaffen, auf dessen Basis für Unternehmen weitgehend eigenständig die Möglichkeit besteht, außerhalb eines Insolvenzverfahrens, aber mit verbindlichem Rechtsrahmen eine Sanierung durchzuführen. Der Schuldner entscheidet hierbei selbst, welche Instrumente zur Restrukturierung seines Unternehmens in Anspruch genommen werden sollen. Hiervon hängt sodann auch der Grad der gerichtlichen Beteiligung ab. Um die Bedeutung und Möglichkeiten der Inanspruchnahme der jeweiligen Maßnahmen zu betrachten, wird im Rahmen der vorliegenden Arbeit die gesamte Entwicklung eines in Schwierigkeiten geratenen Unternehmens, beginnend mit dem Auftreten von ersten Anzeichen einer Unternehmenskrise, mit den möglichen Sanierungsmaßnahmen analysiert und kritisch beleuchtet; die Vermeidung einer Insolvenz ist dabei von besonderem Interesse, da der Stabilisierungs- und Restrukturierungsrahmen an diesem Ziel anknüpft.This study deals with the various refinancing options available to companies in the context of corporate crises, depending on the respective stage. Especially since the COVID-19 pandemic, but also due to other global and economic challenges, financial distress within companies has been occurring more frequently. A significant innovation in this regard was created with the Stabilization and Restructuring Framework Act (StaRuG), on the basis of which companies largely have the opportunity to pursue a restructuring outside of insolvency proceedings, but within a binding legal framework. The debtor itself decides which instruments for restructuring of its company should be employed. The degree of judicial involvement then depends on this. To examine the importance and possibilities of using the respective measures, the present work analyzes and critically evaluates the entire development of a company in distress, starting with the appearance of initial signs of a corporate crisis, with the possible restructuring measures; preventing insolvency is of particular interest, as the Stabilization and Restructuring Framework aims at this objective
Randomisierte, kontrollierte, monozentrische Studie über die Reduktion des Beschlags einer Laparoskopieoptik mit Endoflator® 50 (KARL STORZ SE & Co. KG) im Vergleich zu nicht vorgewärmter Optik
Einleitung
Die Grundlage für eine sichere und effiziente Anwendung der Laparoskopie sind klare Sichtverhältnisse. Der Beschlag der laparoskopischen Optik ist einer der Hauptgründe für schlechte Sichtverhältnisse und ist bedingt durch die physiologisch höhere Temperatur und Feuchtigkeit der Peritonealhöhle im Vergleich zur Umgebung. Der Endoflator 50® verfügt über beheizbare Schläuche, die das Kohlenstoffdioxid über eine integrierte Gasheizung während der Insufflation auf Körpertemperatur erwärmen. Wir führten eine Prüfer-initiierte, randomisierte, kontrollierte Studie über die Reduktion des Beschlags einer Laparoskopieoptik mit Endoflator® 50 im Vergleich zu konventioneller Insufflation durch.
Material und Methoden
Zwischen Oktober 2022 und Juli 2023 wurden insgesamt 72 Patientinnen am Universitätsklinikum des Saarlandes und der Medizinischen Fakultät der Universität des Saarlandes, Klinik für Frauenheilkunde, Geburtshilfe und Reproduktionsmedizin, Homburg, Saarland, Deutschland rekrutiert. Diese wurden 1:1 in die Gruppe A (Endoflator 50®) und Gruppe B (Kontrolle) randomisiert. Der Endoflator 50® wurde von der Firma KARL STORZ SE & CO. KG für die Dauer der Studie kostenlos zur Verfügung gestellt. Primärer Endpunkt war der Schweregrad des Beschlags der Optik nach 1, 10 und 30 Minuten. Dieser wurde anhand einer visuellen Analogskala (0= freie Sicht, 10= größtmöglicher Beschlag) von zwei verblindeten Ärzten bewertet. Für die Analyse wurden für diese Werte der Mittelwert berechnet. Ausgewählte sekundäre Endpunkte sind die Anzahl und die Gesamtzeitdauer der Reinigungen der Optik (in Sekunden). Die Gruppenauswertung erfolgte mittels Mann-Whitney-Test (nicht-parametrische Variablen). Die statistische Signifikanz wurde auf p < 0,05 festgelegt. Die statistische Analyse erfolgte mittels Jamovi (Version 2.3.28.0).
Ergebnisse
Insgesamt wurden 72 Patienten in die Analyse eingeschlossen [medianes Alter 41, (18-83 Jahre]. Bezüglich des Alters oder anderer Patientencharakteristika wie BMI oder ASA-Klassifikation bestand kein signifikanter Unterschied zwischen den Gruppen. Häufigste Operation war die totale laparoskopische Hysterektomie. Der primäre Endpunkt war nach 1 Minute [Gruppe A Median 1 (1-2), Gruppe B Median 4 (3-8), p < 0,001], nach 10 Minuten [Gruppe A Median 1 (1-2), Gruppe B Median 4 (3-7), p < 0,001] und nach 30 Minuten [Gruppe A Median 1 (0-2), Gruppe B Median 4 (2-7), p < 0,001] statistisch signifikant. Die Anzahl und die Dauer der Kamerareinigungen war in Gruppe A ebenfalls geringer im Vergleich zu Gruppe B [Median 2 (0.5-3.0) vs. 4 (2-6), p=0,001; Median 15 (2.5-27.5) vs. 40 (25-60), p < 0,001].
Schlussfolgerung
Unsere Arbeit zeigt, dass die Nutzung des Endoflator 50® zu einer signifikanten Verringerung des Beschlages und zu einem verringerten Aufwand bei der Kamerareinigung führt. Anzahl sowie Dauer der Reinigungen können durch die Vorwärmung des Kohlenstoffdioxidgases reduziert werden.Objective
The fogging of the laparoscopic lens is a widespread obstacle in minimally-invasive surgery, caused by the temperature and humidity difference between the peritoneal cavity and the surrounding area. At the same time, maintaining visual clarity is fundamental in laparoscopic procedures not only for efficacy but also for patient safety. The Endoflator 50® has heatable tubes that warm the carbon dioxide to body temperature via an integrated gas heater during insufflation. We conducted an investigator-initiated, randomized, controlled trail on the reduction of fogging of laparoscopic optics using the Endoflator® 50 compared to conventional insufflation.
Methods
We recruited a total of 72 patients at Saarland University Hospital, Department of Obstetrics, Gynecology and Reproductive Medicine, Homburg, Saarland, Germany between October 2022 and July 2023. Participants were randomized to group 1 (Endoflator 50®) (n = 35) and group 2 (controls) (n = 37). The primary endpoint was the severity of lens fogging after 1, 10 and 30 minutes, which was evaluated on a 10-point inverted visual clarity scale ranging from 0 (clearest) to 10 (foggiest) by two blinded attending physicians after the end of the data collection. Selected secondary outcome measures were the number of lens cleanings and the total time required to clean the lens. The group analysis was performed using the Mann-Whitney test (non-parametric variables). A p value of < 0.05 was considered statistically si- gnificant. Jamovi (Version 2.3.28.0) was used for the statistical analysis.
Results
All 72 recruited patients were included in the analysis. Baseline characteristics including age, body mass index, surgical procedure, ASA classification, and total operating time showed no statistical difference
between the groups. The most common indication was endometriosis. The primary endpoint was statistically significant after 1 minute [group A median 1 (1-2), group B median 4 (3-8), p < 0.001], after 10 minutes [group A median 1 (1-2), group B median 4 (3-7), p < 0.001] and after 30 minutes [group A median 1 (0-2), group B median 4 (2-7), p < 0.001]. Cleaning effort was lower in the Endoflator 50® group compared to the control group with significantly less lens cleanings and less total time required to clean the lens (all, p ≤ 0.001).
Conclusion
We were able to show that the use of the Endoflator 50® leads to a significant reduction in laparoscopic lens fogging and less required effort to clean the laparoscopic lens. The number and duration of clea- nings can be reduced by preheating the carbon dioxide gas
How pore formation in complex biological membranes is governed by lipid composition, mechanics, and lateral sorting
The primary function of biological membranes is to enable compartmentalization among cells and organelles. Loss of integrity by the
formation of membrane pores would trigger uncontrolled depolarization or influx of toxic compounds, posing a fatal threat to living
cells. How the lipid complexity of biological membranes enables mechanical stability against pore formation while, simultaneously,
allowing for ongoing membrane remodeling is largely enigmatic. We performed molecular dynamics simulations of eight complex
lipid membranes including the plasma membrane and membranes of the organelles endoplasmic reticulum, Golgi, lysosome, and
mitochondrion. To quantify the mechanical stability of these membranes, we computed the free energy of transmembrane pore
nucleation as well as the line tension of the rim of open pores. Our simulations reveal that complex biological membranes are
remarkably stable, however, with the plasma membrane standing out as exceptionally stable, which aligns with its crucial role as a
protective layer. We observe that sterol content is a key regulator for biomembrane stability, and that lateral sorting among lipid
mixtures influences the energetics of membrane pores. A comparison of 25 model membranes with varying sterol content, tail length,
tail saturation, and head group type shows that the pore nucleation free energy is mostly associated with the lipid tilt modulus,
whereas the line tension along the pore rim is determined by the lipid intrinsic curvature. Together, our study provides an atomistic
and energetic view on the role of lipid complexity in biomembrane stability
Informativity and linearization in reference production
In visually-situated referential communication tasks, speakers must select relevant visual properties and determine their linear order within a syntactic structure in order to encode a message that enables the listener to successfully identify the intended referent. While previous studies have primarily focused on the influence of informativity on property selection, especially overspecification, little is known about how informativity affects the linearization of property order, particularly when syntactic variation is involved. This thesis investigates whether and how the informativity of property words, as determined by visual-situated contexts and quantified via Referential Entropy Reduction, influences syntactic linearization. Five referential communication experiments investigate whether informativity modulates speakers' syntactic choice between pre-nominal and post-nominal modifications in German, when describing referents in visual scenes depicting animals performing actions. Additionally, the project explores the role of communication engagement by reinforcing perspective-taking and comparing web-based and face-to-face interaction settings. The results reveal two groups of speakers: *Group Consistent*, who are insensitive to informativity and adhere to a fixed syntactic structure, in line with a speaker-oriented, heuristic production approach; and *Group Varied*, who vary the use of syntactic structures to adjust property orders based on informativity, favouring an informative-first linearization strategy that facilitates target identification for listeners. The proportion of *Group Varied* speakers increases with communication engagement, particularly in the most engaging face-to-face interactions and when perspective-taking is reinforced. This thesis advances our understanding of referential production and communication efficiency. Examining informativity, a speaker-external, listener-oriented factor, provides a clearer distinction between the speaker-oriented and listener-oriented views of reference production, as reflected in the different linearization strategies adopted by the two speaker groups. The distribution of these two groups is mediated by perspective-taking and communication engagement. The informative-first linearization preference offers novel evidence for the role of communication efficiency and audience design in shaping syntactic choices during the early grammatical encoding phase of language production.Diese Dissertation untersucht den Einfluss von Informativität auf die syntaktische Linearisierung von Objekteigenschaften in der referenziellen Produktion. In visuell eingebetteten referenziellen Kommunikationst-Tasks konstruieren Sprecher Ausdrücke, um einem anwesenden Hörer zu helfen, ein spezifisches Zielobjekt in einer visuellen Szene zu identifizieren. Dieser Prozess umfasst zwei zentrale Komponenten: 1) die Eigenschaftsauswahl, also die Auswahl relevanter Eigenschaften, die eine eindeutige Zielidentifikation unterstützen, und 2) die Eigenschaftsreihenfolge, also die Anordnung der gewählten Eigenschaften in einer syntaktisch linearisierten Form. Während frühere Studien besonderes Augenmerk auf Überspezifizierung (ÜS) bei der Eigenschaftsauswahl gelegt haben – bei der Sprecher mehr Informationen einfügen als notwendig (z.B. ein blaues Dreieck”, obwohl nur ein Dreieck vorhanden ist) – ist wenig darüber bekannt, wie Eigenschaftsreihenfolge als Linearierungsprozess in der referenziellen Produktion funktioniert, insbesondere wenn syntaktische Variation möglich ist, etwa bei prä-nominalen versus post-nominalen Strukturen (z.B. der weinende Hase vs. der Hase, der weint). Indem referenzielle Kommunikation als kooperativer Prozess (Clark & Wilkes- Gibbs, 1986; Grice, 1975) und als ein Akt der Informationsübertragung (Sperber & Wilson, 1995) verstanden wird, der auf Prinzipien der Informationstheorie beruht (Shannon, 1948), untersucht diese Dissertation, ob die referenziellen Äusserungen von Sprechern durch Informativität im Sinne kommunikativer Effizienz beeinflusst werden. Genauer gesagt wird erforscht, ob Informativität die Linearisierung von referenziellen Eigenschaften beeinflusst, wenn diese Linearisierung mit syntaktischen Variationen in der referenziellen Enkodierung einhergeht. Decreased Expression of the EAAT5 Glutamate Transporter at Photoreceptor Synapses in Early, Pre-Clinical Experimental Autoimmune Encephalomyelitis, a Mouse Model of Multiple Sclerosis
Multiple sclerosis (MS) is a prevalent neuroinflammatory and neurodegenerative disease that affects the CNS, leading to structural and functional alterations in both the white and gray matter of the brain. Among the systems frequently impacted by MS is the visual system, which is particularly susceptible to changes linked to disease progression. Glutamate excitotoxicity has been hypothesized as a contributing factor to the pathogenesis of MS. To explore this, we focused on the role of the EAAT5 (SLC1A7) glutamate transporter, a key presynaptic component located near glutamatergic photoreceptor ribbon synapses. EAAT5 is essential for regulating glutamate levels and maintaining synaptic integrity.
In this study, we utilized the experimental autoimmune encephalomyelitis (EAE) mouse model of MS to investigate whether alterations in EAAT5 contribute to the observed structural and functional changes at photoreceptor ribbon synapses during the disease. Through a combination of qualitative and quantitative immunofluorescence microscopy and Western blot analyses, we assessed EAAT5 expression and localization at various stages of disease progression.
Our findings revealed a significant reduction in EAAT5 at photoreceptor synapses in EAE retinas compared to controls, an alteration detectable as early as nine days post-immunization. This reduction was corroborated by Western blot data, which demonstrated decreased EAAT5 protein expression in EAE retinas. These results point to early disruptions in glutamate transport at photoreceptor synapses during the pre-clinical phase of EAE, suggesting a potential role for EAAT5 in the synaptic dysfunctions observed in MS.
This study highlights the importance of EAAT5 in maintaining synaptic homeostasis and raises questions about the mechanisms underlying its dysregulation in MS. Further research is needed to elucidate the precise pathways through which EAAT5 alterations contribute to neurodegenerative and neuroinflammatory processes, particularly in the context of the visual system's vulnerability in MS. These findings contribute to a deeper understanding of the molecular underpinnings of MS and may inform future therapeutic strategies targeting synaptic dysfunction in this disease.Multiple Sklerose (MS) ist eine weit verbreitete neurologische Erkrankung, die sowohl durch neuroinflammatorische als auch neurodegenerative Komplikationen gekennzeichnet ist. Diese Krankheit führt häufig zu erheblichen Veränderungen in der strukturellen und funktionellen Integrität der weißen und grauen Substanz des Gehirns. Besonders anfällig für Störungen, die mit dem Fortschreiten der MS einhergehen, ist das visuelle System. Ein vermuteter Faktor in der Pathogenese der MS ist die Glutamat-Exzitotoxizität. Die vorliegende Studie konzentriert sich auf den EAAT5 (SLC1A7) Glutamat-Transporter, der eine entscheidende Rolle bei der Regulation von Glutamat an den Synapsen spielt und sich in der Nähe der aktiven Zonen der Photorezeptor-Ribbon-Synapsen befindet.
In meiner Dissertation habe ich das experimentelle Modell der autoimmunen Enzephalomyelitis (EAE) bei Mäusen verwendet, um zu untersuchen, wie Veränderungen in EAAT5 die Struktur und Funktionalität von Photorezeptor-Ribbon-Synapsen bei MS beeinflussen könnten. Ich setzte qualitative und quantitative Methoden ein, darunter Immunfluoreszenzmikroskopie und Western-Blot-Analysen, um die Expression und Lokalisation von EAAT5 am neunten Tag nach der Immunisierung während der präklinischen Phase der EAE zu messen.
Meine Ergebnisse zeigen einen signifikanten Rückgang von EAAT5 an den Photorezeptor-Synapsen in von EAE betroffenen Retinas im Vergleich zu Kontrollen, beginnend ab dem neunten Tag nach der Immunisierung. Dieser Rückgang wurde durch Western-Blot-Analysen bestätigt, die eine reduzierte EAAT5-Proteinexpression zeigten. Solche frühen Beeinträchtigungen im Glutamattransport deuten auf eine mögliche Beteiligung von EAAT5 an den synaptischen Anomalien bei MS hin.
Diese Untersuchung unterstreicht die Rolle von EAAT5 bei der Aufrechterhaltung des synaptischen Gleichgewichts und schlägt ein Ungleichgewicht von EAAT5 als möglichen Mechanismus vor, der zu den neurodegenerativen und neuroinflammatorischen Manifestationen der MS beiträgt. Diese Erkenntnisse vertiefen unser Verständnis der molekularen Grundlagen der MS und könnten zukünftige therapeutische Strategien zur Zielsetzung der synaptischen Dysfunktion bei dieser Krankheit informieren
Risk factors for survival after lung transplantation in cystic fibrosis: impact of colonization with multidrug-resistant strains of Pseudomonas aeruginosa
Background
Lung transplantation is the ultimate treatment option for patients with advanced cystic fibrosis. Chronic colonization of these recipients with multidrug-resistant (MDR) pathogens may constitute a risk factor for an adverse outcome. We sought to analyze whether colonization with MDR pathogens, as outlined in the German classification of multiresistant Gram-negative bacteria (MRGN), was associated with the success of lung transplantation.
Methods
We performed a monocentric retrospective analysis of 361 lung transplantations performed in Homburg, Germany, between 1995 and 2020. All recipients with a main diagnosis of cystic fibrosis (n = 69) were stratified into two groups based on colonization with Pseudomonas aeruginosa in view of MRGN before transplantation: no colonization and colonization without (n = 23) or with (n = 46) resistance to three or four antibiotic groups (3MRGN/4MRGN). Multivariable analyses were performed including various clinical parameters (preoperative data, postoperative data).
Results
CF patients colonized with multidrug-resistant pathogens (Pseudomonas aeruginosa) classified as 3MRGN/4MRGN had poorer survival (median survival 16 years (without MRGN) versus 8 years (with MRGN), P = 0.048). Extracorporeal support (P = 0.014, HR = 2.929), re-transplantation (P = 0.023, HR = 2.303), female sex (P = 0.019, HR = 2.244) and 3MRGN/4MRGN (P = 0.036, HR = 2.376) were predictors of poor outcomes in the multivariate analysis. Co-colonization with the mold Aspergillus fumigatus was further associated with mortality risk in the 3MRGN/4MRGN group (P = 0.037, HR = 2.150).
Conclusion

Die Rolle der Aktivierung des renalen sympathischen Nervensystems auf die Entwicklung eines atrialen proarrhythmischen Substrates in einem hypertensiven Rattenmodell mit chronischer Niereninsuffizienz
1 Zusammenfassung
Die arterielle Hypertonie und die chronische Nierenerkrankung (CKD) sind durch gemeinsame Risikofaktoren und pathophysiologische Vorgänge eng miteinander verbundene Erkrankungen. Dabei führt die arterielle Hypertonie wie die chronische Nierenerkrankung zu einer Überaktivität des sympathischen Nervensystems. Diese erhöhte sympathische Aktivität spielt eine relevante Rolle bei der Entstehung von Vorhofflimmern und erhöht durch strukturelle, biochemische und elektrische Remodeling-Vorgänge die Inzidenz atrialer Arrhythmien. Aufgrund weltweit hoher Prävalenz der betrachteten Erkrankungen und einem gesteigerten Einfluss auf Morbidität und Mortalität war das Ziel dieses experimentellen Modells der kardiovaskulären Komorbiditäten bei CKD, den Einfluss der Aktivierung des renalen sympathischen Nervensystems auf die Entwicklung eines atrialen proarrhythmischen Substrates in einem Hypertensiven Rattenmodel mit CKD eingehend zu untersuchen.
Methoden: Männliche Spontan Hypertensive Ratten (SHR) erhielten für insgesamt 16 Wochen ausschließlich 0,25 % Adenin-haltiges Spezialfutter, um eine CKD zu induzieren. Nach vier Wochen Fütterung mit Adenin-Futter erfolgte bei der Hälfte der Tiere (n=4) eine beidseitige renale Denervation zur Modulation des sympathischen Nervensystems (Ad+RDN: Interventionsgruppe). Die verbleibenden SHR (n=4) wurden einer Sham-Operation unterzogen (Ad). Als Kontrolltiere (Ko) dienten vier SHR, welche mit einer normalen Haltungsdiät ernährt wurden und nach vier Wochen einer Sham-operation unterzogen wurden. Die Entwicklung des Blutdrucks über den Versuchszeitraum wurden wöchentlich mittels Katheter-basierter Telemetrie durchgeführt. Zur Überprüfung der Nierenparameter wurde alle zwei Wochen die Konzentration von Kreatinin und Harnstoff jeweils im Serum und Urin erfasst. Zur Bestimmung des elektrischen Remodelings wurden EKG-Aufzeichnungen verwendet. Zur Erfassung des strukturellen und biochemischen Remodelings wurde nach 16 Wochen das Gewebe von Niere und Herz mittels Western Blots, Polymerase-Kettenreaktion und Gewebe-Histologie untersucht.
Ergebnisse: Die Fütterung mit 0,25 % Adenin-haltigem Spezialfutter führte zu Versuchsende hin zu deutlich erhöhten Plasma Harnstoff und Kreatinin Werten im Vergleich zur Kontrolle, was auf die Ausbildung einer Nierenschädigung schließen lässt. Diese Adenin-induzierte Nephropathie war mit einer erhöhten renalen Expression der Xanthin-Oxidase, der Häm-Oxigenase 1 (HO-1) sowie reduzierten Katalase-Level assoziiert, was auf eine schwere Schädigung der Nieren schließen lässt. Die renale Denervation führte in den Adenin-gefütterten Tieren zu keiner Verbesserung dieser Nierenparameter. In Adenin-gefütterten Tieren führte eine renal Denervation zu einem deutlich reduzierten renalen Norepinephrin-Gehalt (Ad: 89,47±16,06 ng/mg vs. Ad+RDN: 29,08±13,31 ng/mg, p=0,007) als auch zu einer

verringerten Expression der renalen Tyrosin-Hydroxylase (TH) (Ad: 1,53±0,26 IOD/GAPDH vs. Ad+RDN: 0,73±0,26 IOD/GAPDH, p=0,05). Der mittlere arterielle Druck und die Herzfrequenz blieben trotz bestehender Niereninsuffizienz auf dem Niveau der SHR-Kontrollgruppe.
Die Auswirkung der chronischen Nierenerkrankung auf das pro-arrhythmische atriale Remodeling zeigte sich in Form einer deutlich erhöhten interstitiellen Fibrose im linken Vorhof der Adenin-Gruppe im Vergleich zur Kontrollgruppe (Ad: 12,82±2,87 % vs. Ko: 6,1±0,75 %, p=0,06), einer erhöhten Anzahl atrialer TH-positiver Zellen (Ad: 10,19±1,18 vs. Ko: 4,69±0,23, p=0,01) und einer verlängerter P-Wellen Dauer (Ad: 29,6±1,9 ms vs. Ko: 25,7±0,7 ms, p=0,13). Die renale Denervation führte in den Adenin-gefütterten Tieren zu einer numerischen, aber nicht signifikanten Reduktion der links atrialen interstitiellen Fibrose (Ad+RDN: 8,80±1,29%, p=0,99 vs. Ad), einer verringerten Anzahl an TH-positiven Zellen (Ad+RDN: 6,87±0,89, p=0,42 vs. Ad) und einer Tendenz zur Verkürzung der P-Welle (Ad+RDN: 27,0±0,4 ms, p=0,99 vs. Ad). Das strukturelle und elektrische Remodeling des Vorhofs war unabhängig von Veränderungen des linken Ventrikels. Hier konnten kein Einfluss einer CKD auf Herzgewicht, den linksventrikulären Kardiomyozyten Diameter, die linksventrikuläre BNP-Expression oder die linksventrikuläre interstitielle Fibrose im Vergleich zur SHR-Kontrolle festgestellt werden.
Fazit: Die Adenin-induzierte Nephropathie führte in spontan hypertensiven Ratten zu einer Zunahme der interstitiellen atrialen Fibrose, einer vermehrten sympathischen Innervation des linken Vorhofs und einer Verlängerung der P-Welle. Demnach begünstigt eine Nierenerkrankung die Ausbildung eines pro-arrhythmischen Substrates im Vorhof hypertensiver Ratten. In Adenin-gefütterten SHR führte eine renale Denervation zu erniedrigten Norepinephrin-Konzentrationen und reduzierter TH-Expression in den Nieren. Diese Modulation des sympathischen Nervensystems war verbunden mit einer numerisch deutlich erniedrigten Fibrotisierung, einer verminderten Anzahl an TH-positiven Zellen im linken Vorhof und einer tendenziellen Verkürzung der P-Welle. Demzufolge könnte die renale Denervation bei Bluthochdruck-Patienten mit chronischer Niereninsuffizienz das Auftreten von Vorhofflimmern reduzieren.2 Abstract
Arterial hypertension and chronic kidney disease (CKD) are closely linked diseases due to shared risk factors and pathophysiological processes. Arterial hypertension, like chronic kidney disease, leads to overactivity of the sympathetic nervous system. This increased sympathetic activity plays a relevant role in the development of atrial fibrillation and increases the incidence of atrial arrhythmias through structural, biochemical and electrical remodeling processes. Due to the worldwide high prevalence of these two conditions and an increased impact on morbidity and mortality, the aim of this experimental model of cardiovascular comorbidities in CKD was to investigate the influence of renal sympathetic nervous system activation on the development of an atrial proarrhythmic substrate in a hypertensive rat model with CKD.
Methods: Male spontaneously hypertensive rats (SHR) were exclusively fed a 0.25% adenine-containing diet for a total of 16 weeks to induce CKD. After four weeks, half of the animals (n=4) underwent bilateral renal denervation to modulate the sympathetic nervous system (Ad+RDN: intervention group). The remaining adenine-fed SHR (n=4) underwent sham surgery (Ad). Four SHR, which were fed a normal standard diet and underwent sham surgery, served as control animals (Ko). The development of blood pressure over the experimental period was monitored weekly using catheter-based telemetry. The concentration of creatinine and urea in serum and urine was recorded every two weeks to validate kidney function. ECG recordings were used to determine electrical remodeling. To determine structural and biochemical remodeling, kidney and heart tissue was examined after 16 weeks using western blots, polymerase chain reaction and tissue histology.
Results: Feeding 0.25 % adenine-containing diet led to significantly increased plasma urea and creatinine levels after 16 weeks compared to the control, indicating the development of renal damage. This adenine-induced nephropathy was associated with increased renal expression of xanthine oxidase, heme oxigenase 1 (HO-1) and reduced catalase levels, indicating severe renal injury. Renal denervation did not improve these renal parameters in the adenine-fed animals. Renal denervation resulted in significantly reduced renal norepinephrine levels in adenine-fed animals (Ad: 89.47±16.06 ng/mg vs. Ad+RDN: 29.08±13.31 ng/mg, p=0.007) as well as a reduced expression of renal tyrosine hydroxylase (TH) (Ad: 1.53±0.26 IOD/GAPDH vs. Ad+RDN: 0.73±0.26 IOD/GAPDH, p=0.05). Mean arterial pressure and heart rate remained at the level of the SHR control group despite the presence of renal dysfunction.
In SHR, 16 weeks of adenine-induced CKD was associated with significantly increased interstitial fibrosis in the left atrium compared to the control group (Ad: 12.82±2.87 % vs. Ko: 6.1±0.75 %, p=0.06), an increased number of atrial TH-positive cells (Ad: 10.19±1.18 vs. Ko: 4.69±0.23, p=0.01) and a prolonged P-wave duration (Ad: 29.6±1.9 ms vs. Ko: 25.7±0.7 ms,

p=0.13). In the adenine-fed animals, renal denervation resulted in a numerical but not significant reduction in left atrial interstitial fibrosis (Ad+RDN: 8.80±1.29%, p=0.99 vs. Ad), a reduced number of left atrial TH-positive cells (Ad+RDN: 6.87±0.89, p=0.42 vs. Ad) and a tendency to shorten the P-wave (Ad+RDN: 27.0±0.4 ms, p=0.99 vs. Ad). The structural and electrical remodeling of the atrium was independent of changes in the left ventricle. Here, no influence of CKD on heart weight, left ventricular cardiomyocyte diameter, left ventricular BNP expression or left ventricular interstitial fibrosis could be detected compared to SHR control.
Conclusion: In spontaneously hypertensive rats, adenine-induced nephropathy led to an increase in interstitial left atrial fibrosis, increased sympathetic innervation and prolongation of the P wave. Thus, renal disease favors the development of a pro-arrhythmic substrate in the atrium of hypertensive rats. In adenine-fed SHR, renal denervation led to decreased norepinephrine concentrations and reduced TH expression in the kidneys. This modulation of the sympathetic nervous system was associated with a numerically significantly decreased fibrosis formation, a reduced number of TH-positive cells in the left atrium and a tendency to shorten the P-wave. Consequently, renal denervation could be therapy option to reduce the burden of atrial fibrillation in hypertensive patients with CKD
