What are Patients at High Risk of Developing Diabetic Foot Lesions Aware of When Selecting Footwear?
No full text本研究の目的は, 糖尿病足病変ハイリスク患者 （以下ハイリスク患者） が足病変を予防するために靴選びでどのようなことを意識しているのか明らかにすることである. 大学病院フットケア外来に通院するハイリスク患者10名 （男性8名・女性2名, 平均年齢63.8±9.2歳, 糖尿病の平均罹病期間19.9±8.9年） を対象に, 半構造化面接による質的記述的研究を行った. その結果, 【足に合う靴の着用】【用途に合わせた靴の着用】【足に合わない靴の着用による弊害】【失敗経験による意識変容】【フットケア外来から受けた療養支援】【既製靴を選ぶ難しさ】の6個のカテゴリーが示された. ハイリスク患者は, 足病変を予防するために自分の足に合う靴を着用したいと考えていたが, 市販靴の中から選ぶことに難しさを抱えていた. フットケア外来や足病診療で行われる糖尿病フットケア療養支援において, 靴の具体的な情報や選び方についての知識提供を継続的に支援していく必要がある.The purpose of this study was to research how patients at high risk of developing diabetic foot ulcers select footwear to prevent foot lesions. This qualitative descriptive study collected data using semi-structured interviews. A total of 10 patients（8 males and 2 females; mean age, 63.8±9.2 years; mean duration of diabetes, 19.9±8.9 years）were enrolled in this study. They were outpatients at the University Hospital Foot Care Outpatient Clinic. Six categories were identified based on the collected data: “wearing well-fitting footwear”, “wearing footwear that serves the required use”, “harmful effects of wearing ill-fitting footwear”, “changes based on past negative experiences”, “medical foot care support at the outpatient clinic,” and “difficulty in selecting ready-to-wear footwear”. High-risk patients wanted footwear that fitted well to prevent foot lesions but faced difficulties in selecting from commercially available footwear. These results suggest that diabetic patients at high risk of podiatric issues should be provided with accurate information on footwear and its selection in foot-care clinics or during podiatric examinations.journal articl
