Initiatives to introduce HIYARI-HATTO reports.
No full text当院の集中治療部では、報告されたインシデントレポートのうち、当院で定義されているインシデントの影響レベル0の報告数は、2022年度は3件、2023年度は7件である。影響レベル0の報告数を増やす事を目的として、影響レベル0の事例を「ヒヤリ・ハット」と定義し、「ヒヤリ・ハットレポート」として導入した。ICU看護師へのアンケートの結果からは、ヒヤリハット事例の共有により、他者の経験が学びにつながっている事がわかった。ヒヤリ・ハットレポートは未然に事故を防いだという価値の高い報告であると考える。ヒヤリ・ハットの報告により、安全に出来たという成功体験を表面化して、前向きな報告として共有し承認しあう事で、個々の医療安全対策への動機づけや、事故につながりやすい類似した事例への対応につなげ、危険予測の能力を向上させられる可能性がある。今回のヒヤリハットレポート導入への取り組みにより、現在は月に十数件の報告が継続してあげられるようになった。今後は、うまくいった理由を承認し、概念化するカンファレンスの場や学習する場を設定していくこと、および、ヒヤリ・ハットレポートが継続して報告できるシステムの構築が課題である。departmental bulletin pape
