Impact of Ao-dake-humi, a Japanese Traditional Bamboo Foot Stimulator, on Lower Urinary Tract Symptoms, Constipation, Insomnia, and Hypersensitivity to Cold in Elderly People
Background : Ao-dake-humi (ADH) is a traditional Japanese bamboo foot stimulator that is commonly used to promote general health among the elderly in Japan. Our earlier report revealed that ADH could improve lower urinary tract symptoms (LUTS), constipation, and hypersensitivity to cold (HC). This study investigated the precise effects and mechanisms of ADH on health in elderly people. Methods : In this multi-center, randomized, crossover trial, a simple stepping exercise (SSE) was adopted as a control to exclude the exercise effects of ADH. Elderly people over 60 years of age with clinically diagnosed LUTS, constipation, insomnia, or HC were enrolled. ADH or SSE activities were performed twice a day for 28 days. Before and at 28 days of each regimen, International Prostate Symptom Score (IPSS), Athens Insomnia Scale (a visual analogue scale for constipation and HC), and defecation frequency measurements were assessed for comparisons. After the 28 days of initial treatment, ADH and SSE activities were crossed for each other, and then a second intervention was carried out for 28 days. Results : Of the 37 elderly people (6 male and 31 female) enrolled in the study, 25 participants (4 male and 21 female) were ultimately analyzed. IPSS results improved significantly after ADH. Moreover, ADH ameliorated insomnia and HC significantly more in comparisons with SSE. Constipation status findings were comparable between ADH and SSE. Conclusions : ADH may have a therapeutic advantage over SSE for the management of LUTS, insomnia, and HC, possibly through the mechanism of physical foot stimulation as a neuromodulator.信州医学雑誌 72(4) : 239-249, (2024)journal articl

Gaiko Dialects of Nagano Prefecture
地域語が共通語に受容される過程を「外行」と呼び、その方言を「外行方言」と呼ぶことを述べた。そして、長野県からの外行方言として三例をとりあげ、それらの外行過程を観察、考察した。「ピンピンコロリ(PPK)」は、長野県下伊那郡高森町の教育委員によって造語された。当初は「それを幸せに思う気持ち」が表現された語で、「PPK 運動」という複合語としても使用された。その後、「ピンピンして健康で長生きしてコロリと死ぬ」という様態を指す名詞として長野県内では定着し、全国的にも使用されるようになった。平均寿命の延びで健康長寿が願われる時代に移行したこと、平均寿命が上位に位置する長野県からの発信であったこと、すでにあった共通語の副詞「ぽっくり」にくらべて意味が拡張し名詞としての自由度を獲得したことなどが、全国的な伝播を後押ししたと考えられる。「二年参り」「おつかれさま」についても、地域語の変容をとらえながら、全国に伝播した要因を、社会文化、意識態度、言語それ自体に注目しつつ分析、考察した。From the perspective of observation on the regional language side, the author called the process by which a regional language is accepted into the common language ‘gaiko’, and the dialects of that language ‘gaiko dialects’. Three cases of gaiko dialects from Nagano Prefecture were taken up, and the process of gaiko was observed and discussed. The term ‘pinpinkorori (PPK)’ was coined by a member of the education committee of Takamori Town, Shimoina County, Nagano Prefecture. Initially, the term expressed a feeling of happiness about it, and was also used as a compound term for the ‘PPK movement’. Later, it became established in Nagano Prefecture as a noun referring to the aspect of ‘living a long and healthy life and dying without suffering’, and is now also used nationwide. The fact that the term was introduced from Nagano Prefecture, which has the highest average life expectancy in Japan, and the fact that the meaning of the term was expanded compared to the common adverb ‘pokkuri’, which was already used, gave it greater freedom as a noun, are thought to have contributed to its spread nationwide. The factors that led to the nationwide spread of ‘ninenmairi’ and ‘otsukaresama’ were analysed and discussed, focusing on social culture, attitudes and the language itself, while capturing the transformation of the regional language.Article信州大学人文科学論集 12(1) : 51-67, (2024)departmental bulletin pape

Characteristics of longitudinal changes in quality of life and associated factors in patients post cardiac and thoracic aortic surgery: insights from a prospective cohort study
Background: Although quality of life (QOL) is an outcome of postoperative cardiac rehabilitation (CR), its course and related factors from postoperative hospitalization to the post-discharge period have not been adequately investigated. Additionally, the EuroQol-5Dimension-5Level (EQ-5D-5L) index score has not been characterized over the same period. We aimed to characterize QOL changes assessed by the EQ-5D-5L, over the period from hospitalization to 1 year post-discharge, in patients post-cardiac and thoracic aortic surgery, and investigate the factors associated with these temporal changes. Methodology: This prospective, single-center study included 117 patients who underwent open cardiovascular surgery (median age, 72 years; men, 69%). Patients were assessed for QOL status when transferred to the general ward; at discharge; and at 6 and 12 months after discharge, using the EQ-5D-5L index score and a generalized linear mixed model with random intercepts. Patients were classified into two groups based on score changes postdischarge. Logistic regression analysis evaluated factors associated with QOL decrease post-discharge. Results: The EQ-5D-5L index score significantly increased over time, except between 6 and 12 months postdischarge; “Common activities” was the most common dimension showing score improvement. In 25 patients (21%), the EQ-5D-5L index scores were lower after discharge compared to their scores at discharge. In the logistic regression analysis, Barthel Index pre-admission, preoperative hemoglobin level, and Mini-Mental State Examination-Japanese scores pre-discharge were significantly associated with QOL decline after adjusting for the European System for Cardiac Operative Risk Evaluation II score. Conclusions: Most patients post-cardiac or thoracic aortic surgery experienced improved QOL from postoperative hospital stay to 1 year post-discharge. However, in patients with pre-operative basic activities of daily living, hemoglobin and post-operative cognitive decline may require ongoing comprehensive CR because of reduced QOL.
