    The Effects of Exposure to Mild Hyperbaric Oxygen on Specific Physiological Indexes in the In Vitro Fertilization Therapy of Infertile Women

    Currently, few studies have been conducted on the effects of physiological factors during exposure to mild hyperbaric oxygen (MHBO) environment on infertile. Thirteen healthy women and seven infertile women undergoing in vitro fertilization (IVF) were exposed to MHBO environment using an oxygen room. The specific physiological data collected during the study were analyzed. Exposure to MHBO increases blood oxygen saturation and may enhance metabolism, potentially improving the chances of pregnancy in infertile women by improving the outcome of implantation. Heart rate variability (HRV) analysis showed that women in the infertile group exhibited slightly less parasympathetic nerve activity than those in the healthy group after exposure to MHBO. These findings suggest that the blood supply to the reproductive organs in infertile women may not yet be sufficient. We hypothesized that further enhancement of blood flow through exposure could improve implantation success in women with infertile. Furthermore, decreased parasympathetic activity in infertile women may be one of the causes of infertile, as increased sympathetic activity prevents mental relaxation.     Real-time Fluorescence Imaging with IR700 Signal for Monitoring the Therapeutic Effects of Near-infrared Photoimmunotherapy

    Aim : Near-infrared photoimmunotherapy (NIR-PIT) is a new, highly selective tumor treatment that employs a NIR phthalocyanine dye, IRDye 700DX (IR700), conjugated to a monoclonal antibody (mAb). Although NIR light exposure leads to immediate, target-selective necrotic cell death, actual tumor shrinkage occurs several days afterwards. Therefore, it is difficult to detect the immediate therapeutic effects by measuring tumor size. This study investigated IR700 fluorescence changes by means of fluorescence endoscopy as a potential real-time biomarker for monitoring cytotoxic effects during NIR-PIT. Methods : In vivo mouse experiments were conducted with a human epidermal growth factor receptor type 2-expressing, green fluorescence protein (GFP)-expressing, gastric cancer cell line (N87-GFP). IR700 as a photosensitizer was conjugated to trastuzumab. The effects of NIR-PIT were evaluated in a flank xenograft model as well as in a disseminated peritoneal cancer model by GFP and IR700 real-time fluorescence imaging. Fluorescence intensity was compared before and after NIR-PIT for calculations of relative decreasing of fluorescence intensity (DFI). Results : Tumor growth was significantly reduced and survival was significantly improved in the IR700 DFI ≤ 25 % group compared with the IR700 DFI >25 % group in Pearl images (p<0.05) in the flank xenograft model. In the disseminated peritoneal cancer model, an effective NIR-PIT therapeutic response was observed in the IR700 DFI ≤50 % group on fluorescence endoscopy. Conclusion : IR700 fluorescence signal decrease during NIR-PIT may be a potential immediate indicator of the therapeutic effects of mAb-IR700-based NIR-PIT prior to the onset of tumor shrinkage.     A Case of Adult Respiratory Distress Syndrome due to Pasteurella Pneumonia in a Patient Undergoing Tocilizumab Treatment for Rheumatoid Arthritis

    A 58-year-old woman undergoing tocilizumab treatment for rheumatoid arthritis was admitted to our hospital with acute dyspnea. She worked in a dairy farming. Chest computed tomography revealed extensive heterogeneous ground-glass opacities and infiltrative shadows in both lungs. Her respiratory condition rapidly deteriorated, progressing to acute respiratory distress syndrome (ARDS). Laboratory analysis of her bronchoalveolar lavage fluid identified numerous Pasteurella species, thus giving a diagnosis of ARDS caused by Pasteurella pneumonia. With appropriate treatment, she recovered and was discharged after 30 days of hospitalization. Pasteurella species may cause skin infections following animal bites or scratches, and can further deteriorate to severe respiratory infections in immunocompromised individuals. Therefore, Pasteurella might be a potential etiologic agent of pneumonia in immunocompromised patients under certain circumstances.journal articl
