Current situation and future challenges for nurses requesting RRT at Hospital A
No full textA病院では2020年より院内迅速対応システム (RapidResponse System :R RS) が稼働した。RRS稼働後、院内迅速対応チーム（RapidResponse Team :R RT) の対応数が増加する一方で、 RRT介入基準に該当し何らかの懸念や状態変化を察知していたがRRT要請に繋がらない事例も見受けられた。そこでRRTを要請する病棟看護管理者とリーダーを対象とし、RRSに対する認識およびRRT要請に対する思いなど、現状について独自のアンケートを作成し調査を行った。その結果、RRSへの認識が定着し、その需要が高い反面、アセスメントヘの自信のなさやRRT介入に対する医師の慎重な姿勢などを理由に、RRT要請に対する迷いが生じていることが示唆された。そのため、RRT看護師が継続的かつ能動的にラウンドを行い、病棟看護管理者やリーダーと共に潜在的なRRT要請患者の観察評価と情報共有を行うことで、病棟看護管理者やリーダーの病態アセスメントや急変予兆とその後の対応スキル向上への支援に寄与できるのではないかと考える。今後、職種を越えたRRSに対する理解と他部門との連携を深め、組織的に学び協働し合う文化の醸成が重要である。departmental bulletin pape
Effectiveness of Laparoscopic Uterosacral Nerve Ablation for Chronic Pelvic Pain :A Case Report
Full text linkThe prevalence of chronic pelvic pain (CPP) in women of reproductive age is estimated to be 4-16 ％. Its treatment is often challenging. Evidence supporting the use of laparoscopic uterosacral nerve ablation (LUNA) for CPP is limited. Herein, we report a case of CPP effectively treated with LUNA.A 26-year-old Japanese woman presented with dysmenorrhea and was referred to our hospital. As the structural diseases associated with dysmenorrhea were not evident upon gynecological examination or transvaginal ultrasound, the patient was diagnosed with functional dysmenorrhea. Her symptoms improved after initiating treatment with dienogest. However, she subsequently developed left-sided pelvic pain that progressively worsened. Severe tenderness was noted on the left side of the Douglas pouch. Magnetic resonance imaging suggested pelvic adhesions. However, diagnostic laparoscopy revealed no pelvic adhesions. Instead, findings indicative of endometriosis, such as a peritoneal defect and redness on the left uterosacral ligament, were observed.The mobility of the left uterosacral ligament was limited ; therefore, we performed LUNA. Her CPP markedly decreased from the day following the operation, with no subsequent exacerbation observed.Previous studies reported that LUNA can reduce CPP in patients with mild endometriosis and limited uterosacral ligament mobility. Development of the Gatekeeper Educational Effectiveness Assessment Scale for Assessing the Effectiveness of Suicide Prevention Education Programs for the General Public
No full text目的：一般住民を対象に実施するゲートキーパー（GK）教育研修に着目し、一般住民参加者向けの教育効果が評価できるGK 教育効果評価尺度を開発することとした。方法：GK 教育研修参加者の意識や態度、行動変化を質的帰納的に分析し、質問項目、尺度化、レイアウト作成、内容的妥当性を検討した。分析には、天井効果と床効果検討、I-T 相関分析、因子分析、信頼性検定による内的整合性検討などを行った。結果：研修受講者198 名から回答を得た。因子分析による構成概念妥当性を確認した結果、つなぐ支援に関わる行動（4 項目）、傾聴に関わる態度（3 項目）、自己の心に関わる意識（3 項目）、接し方に関わる態度（3 項目）、支援の準備に関わる意識（4 項目）の17 項目で整理できた。信頼性係数は、全体が0.902、各下位尺度が0.741 から0.838 の値を示した。考察：GK 教育効果評価尺度は、5 下位尺度17 質問項目から構成され、信頼性と妥当性が確認された。Objective：To develop a gatekeeper educational effectiveness assessment scale（ GEEAS） that allows the assessment of educational effectiveness in participants, focusing on gatekeeper（ GK） education and training programs for the general public. Methods：We conducted a qualitative inductive analysis of the awareness, attitudes, andbehavioral changes of participants in GK education and training programs to explore question items, scaling, layout creation, and content validity. The analysis included evaluation of ceiling and floor effects, I-T correlation analysis, factor analysis, and internal consistency using a reliability test. Results：Responses were obtained from 198 education and training participants. The results of factor analysis were sorted into 17 items（ 4, connecting support behaviors ; 3, attitudes toward listening; 3, awareness of one’s own mind ; 3, attitudes toward how to deal with others ; and 4, awareness of support preparation）. The overall reliability coefficient was 0.902, with each subscale ranging from 0.741 to 0.838. Discussion：The GEEAS was comprised of five subscales and 17 question items, and the reliability and validity were confirmed.journal articl
