The YmdB Phosphodiesterase Is a Global Regulator of Late Adaptive Responses in Bacillus subtilis
Bacillus subtilis mutants lacking ymdB are unable to form biofilms, exhibit a strong overexpression of the flagellin gene hag, and are deficient in SlrR, a SinR antagonist. Here, we report the functional and structural characterization of YmdB, and we find that YmdB is a phosphodiesterase with activity against 2′,3′- and 3′,5′-cyclic nucleotide monophosphates. The structure of YmdB reveals that the enzyme adopts a conserved phosphodiesterase fold with a binuclear metal center. Mutagenesis of a catalytically crucial residue demonstrates that the enzymatic activity of YmdB is essential for biofilm formation. The deletion of ymdB affects the expression of more than 800 genes; the levels of the σD-dependent motility regulon and several sporulation genes are increased, and the levels of the SinR-repressed biofilm genes are decreased, confirming the role of YmdB in regulating late adaptive responses of B. subtilis.
Toward a further understanding of drivers of customer loyalty across economic conditions, industries, firms, and customers
Binnen de dienstensector concurreren bedrijven hevig met elkaar om de loyaliteit van de klant te winnen. Er zijn drie marketingstrategieën die klantloyaliteit effectief beïnvloeden, te weten ‘kwaliteitswaarde’, ‘merkwaarde’ en ‘relatiewaarde’. De impact van die strategieën op klantloyaliteit hangt echter sterk af van de markt- en bedrijfscontext, zoals economische omstandigheden en concurrentie. Yi-Chun Ou onderzocht cruciale contextfactoren die de relatie tussen strategieën en klantloyaliteit beter kunnen verklaren. Tijdens recessies is vooral het versterken van de prijskwaliteitverhouding een effectieve strategie is om klanten te behouden, concludeert Ou.
Zij onderzocht ook waarom bepaalde loyaliteitsstrategieën in sommige sectoren beter werken dan in andere sectoren. Ou toont aan dat loyaliteitsstrategieën effectiever zijn in sectoren waar klanten een contract hebben (zoals de telecom- en verzekeringssector), dan bij sectoren waar klanten geen contract hebben (zoals de retail). Ook concludeert zij dat het belang van loyaliteitsstrategieën toeneemt in sectoren met een hevige concurrentie. Opvallend genoeg profiteren marktleiders minder van het werken aan loyaliteit.
Bedrijven spelen sinds enige tijd meer in op de emoties van klanten, en proberen deze positief te beïnvloeden. Ou betwijfelt of dat een juiste strategie is. Positieve emoties leiden inderdaad tot een hogere loyaliteit, maar ze zorgen er ook voor dat de effecten van bijvoorbeeld het merk en de relatiewaarde afnemen. Met de resultaten van de drie studies in haar proefschrift kunnen bedrijven context-specifieke loyaliteitsstrategieën ontwikkelen om zo succesvol klanten te behouden.
Customer loyalty is important for firms to compete in service industries. The literature indicates that three marketing activities effectively influence customer loyalty. They are value equity, brand equity, and relationship equity, i.e., customer equity drivers (CEDs). However, contextual factors, such as economic climate and competitiveness, can impact the importance of the drivers. This dissertation focuses on examining crucial but untested contextual situations on the link of CEDs and loyalty.
First, we consider economic conditions as a contextual factor and ask how firms can retain customers during recessions, by investigating the moderating role of consumer confidence. We show that value equity is an effective loyalty strategy for service industries during recessions (i.e., relatively lower consumer confidence).
Second, we consider industry- and firm characteristics and ask why CEDs work better in some industries/ firms. We are able to recommend managers which industry- and firm characteristics should be seriously taken into account when developing loyalty strategies and which should be not. For example, we find that loyalty strategies work better in contractual settings (e.g., insurance and mobile phone service providers) than non-contractual settings (e.g., retailing).
Finally, as nowadays firms aim to create strong customer emotions, we study their effects on loyalty and how they influence the role of service value, brands and relationships. Importantly, we show that inducing positive emotions reduces the impact of service value, brands, and relationships. In sum, firms may use the findings of this dissertation to develop context-specific loyalty strategies and thereby strengthen competitive advantages.

In the most general form, MEMS/NEMS consist of mechanical microstructures, microsensors, microactuators/transducers, and microelectronics, all integrated onto the same silicon chip. A sensor responds to physical stimuli such as pressure, heat, light or motion, and generates an electric signal for detection while an actuator converts an electric signal to motion. The mechanical resonator plays a central role in MEMS or NEMS devices. Examples of mechanical resonator sensors used in MEMS and NEMS are cantilevers, double clamped beams, quartz crystal tuning forks, and microdisk resonators. MEMS with their batch fabrication techniques enable components and devices to be manufactured with increased performance and reliability, combined with the obvious advantages of reduced physical size, volume, weight and cost. However, there are many challenges and technological obstacles associated with miniaturization that need to be addressed and overcome before MEMS/NEMS can realize its overwhelming potential.

Assessing quality of Interdisciplinairy rounds in the intensive care unit
Het meten van de kwaliteit van het multidisciplinair overleg op de intensive care
De technische mogelijkheden om patiënten te bewaken en te behandelen op een intensive care (IC) zijn de afgelopen decennia sterk toegenomen. Naast deze technologische ontwikkelingen is goede samenwerking evenzo belangrijk voor optimale patiëntenzorg, om het risico op fouten en complicaties te verminderen.
Samenwerking in teams is niet alleen zichtbaar in de directe zorg maar ook in regulier overleg: het multidisciplinair overleg (MDO). In dat laatste worden tussen artsen, consulenten en verpleegkundigen afspraken gemaakt over gewenste diagnostiek en behandeling van patiënten en de verdeling en uitvoering van taken.
Internationaal gezien wordt het MDO als een belangrijke kwaliteitsparameter van een moderne, goed georganiseerde IC beschouwd. Eerder waren er geen objectieve instrumenten beschikbaar om de kwaliteit van een MDO te meten, analyseren c.q. te verbeteren.
Ten Have heeft de afgelopen jaren een instrument ontwikkeld om de kwaliteit van het MDO te beoordelen ( de MDO-Beoordelings Schaal). Belangrijk kenmerk van deze schaal is dat het zowel de bespreking van het behandelplan van de patiënt meet (technische aspect), als de samenwerking in het team. Deze schaal bevat tevens 10 essentiële kwaliteitsindicatoren die geschikt zijn om als checklist te fungeren voor een interventie. De interventie omvatte een leiderschapstraining gericht om de kwaliteit van het MDO te verbeteren.
Tevens heeft Ten Have een vragenlijst ontwikkeld om te meten of de verschillende disciplines na het MDO dezelfde verwachtingen hebben ten aanzien van het behandelplan van de patiënt, omdat bleek dat verwachtingen tussen de verschillende disciplines ten aanzien van het behandelplan kan verschillen.
Assessing the Quality of Interdisciplinary Rounds in the Intensive Care Unit
Effective and systematic team communication and coordination are recognized as being crucial for improving quality and safety in acute medical settings such as the intensive care unit (ICU). Communication failures between doctors and nurses lead to increased patient harm, length of stay, and dissatisfaction by care providers. Numerous studies show that collaboration leads to improvement in care and several authors found significant positive relationships between collaboration and patients outcomes. Therefore the Society of Critical Care Medicine has endorsed daily interdisciplinary rounds (IDRs), which are defined as rounds where the appropriate plan of care is agreed on, understood, and executed as planned by all care providers. However, there are no specific structured models or theories found in the literature that describe criteria or goals for IDRs.
The studies constructed by Ten Have within this thesis investigated several aspects of these IDRs in the ICU namely: assessing the quality of IDRs by development of an assessment scale. This scale contains indicators concerning the technical performance of patient plan of care, and the team process. One study investigated the effect of leadership training on the quality of performed IDRs. A survey was developed to measure the perceptions of the care providers after IDRs on important aspects of patients’ plan of care, because of the different perceptions about the patient plan of care among the ICU care providers.
Genetics of healthy ageing
Veroudering gaat samen met een afname van de fysiologische functies. Deze afname van fysiologische functies kan leiden tot de ontwikkeling van leeftijdsgebonden ziekten, zoals cardiovasculaire ziekte (CVD) of chronische obstructieve longziekte (COPD), en uiteindelijk tot de dood. Er zijn grote verschillen in het tempo van veroudering tussen mensen; mensen worden ziek en sterven op verschillende leeftijden. Genetica lijkt een belangrijke rol te spelen in het tempo van veroudering. Uit eerder onderzoek is gebleken dat de variatie in de menselijke levensduur voor ongeveer 30% erfelijk bepaald is. Dit proefschrift richt zich op de genetische factoren die geassocieerd zijn met sterfte in zijn algemeenheid en met sterfte door specifieke oorzaken (cardiovasculaire en COPD-gerelateerde sterfte) in de algemene bevolking.
Ageing is defined as a decline of physiological functions. Decline of physiological functions ultimately lead to the development of age-related diseases, such a cardiovascular disease (CVD) or Chronic Obstructive Pulmonary Disease (COPD), and in consequence to death. Humans age at different pace; they develop a disease and die at different ages. Among factors influencing human lifespan, genetics seem to play an important role, since the variation in human lifespan is heritable for approximately 30%. This thesis focuses on the genetic factors influencing human all-cause and cause-specific mortality, i.e. cardiovascular and COPD mortality in the general population.

Assessment of Serotonergic Function by Radioligands and Microdialysis : focus on stress-related behaviour and antidepressant efficacy
Serotonine is een chemische stof die betrokken is bij het doorgeven van signalen in de hersenen en speelt o.a. een rol bij het regelen van de gemoedstoestand, zoals in depressie. De meeste antidepressiva zijn erop gericht om serotonine concentraties in het menselijk brein te verhogen, maar deze geneesmiddelen lijken bij een deel van de patiënten niet erg goed te werken. De reden hiervan is niet duidelijk. Daarom is het van belang om te kunnen meten wat er met de signaaloverdracht door serotonine gebeurt onder invloed van stress en na behandeling met antidepressiva. Een methode om dit op een non-invasieve manier te meten is met positron emissie tomografie (PET).
De studies die in dit poefschrift worden beschreven betreffen de validatie van radioactief gemerkte chemische stoffen, of PET tracers, die de aanmaak van serotonine ([11C]5-HTP) en de gevoeligheid van 5-HT2A receptoren voor serotonine ([11C]MDL 100907) in de hersenen kunnen meten. De laatstgenoemde stof bleek zeer geschikt. Hiermee hebben we aangetoond dat 5-HT2A receptoren waarschijnlijk geen essentiële rol spelen bij het omgaan met stress (coping style) en de reactie hierop. Daarnaast hebben we gekeken of de effectiviteit van antidepressiva verbeterd kan worden door aan serotonine heropname remmers (SSRIs) een specifieke 5-HT2C receptor blokker toe te voegen. Door deze geneesmiddelcombinatie lijken de niveaus van zowel serotonine als dopamine, een neurotransmitter betrokken bij motivatie, in de hersenen te kunnen worden verhoogd.
PET lijkt een veelbelovende methode om serotonine signaaloverdracht te meten. Toekomstig onderzoek zal moeten uitwijzen of andere onderdelen betrokken bij serotonine signaaloverdracht belangrijk zijn voor stressgerelateerd gedrag. Daarnaast is het van belang om de effecten van antidepressiva op serotonine signaaloverdracht te meten en deze te relateren aan veranderingen in gedrag en gemoedstoestand.
Serotonin is a signaling compound, or neurotransmitter, in the brain and is involved in the regulation of mood, e.g. in stress and depression. Most antidepressants increase serotonin concentrations in the human brain, but these therapeutic drugs do not have the desired effect in a large number of patients. The exact cause of this lack of efficacy is not clear. Therefore, it is of great importance to measure what happens with serotonin signaling during stress, in depression, and after treatment with antidepressants. Positron Emission Tomography (PET) is a non-invasive technique which may be employed to answer these questions.
The studies described in this thesis concern the validation of radiolabeled substances, or PET tracers, that are capable of measuring serotonin synthesis ([11C]5-HTP) and 5-HT2A receptor binding ([11C]MDL 100907). The latter compound appeared to be suitable. Using this tracer, we showed that 5-HT2A receptors don’t play a crucial role in the reaction of animals to social defeat stress or the development of a certain coping style under baseline conditions.
In addition, we investigated if the efficacy of antidepressants can be improved by combining a selective serotonin reuptake inhibitor (SSRI) with a 5-HT2C receptor antagonist. This combination proved capable of increasing the levels of both serotonin and dopamine, a neurotransmitter involved in motivation.
We conclude that PET can be used to assess various components of serotonin neurotransmission. Future research may be aimed at the validation of additional serotonergic ligands, study of the impact of chronic stress on the serotonergic system and assessment of positive effects of the SSRI/ 5-HTC blocker combination on behavior and mood.
Demystification of commonly used measurements in paediatrics
Dit proefschrift gaat over een aantal metingen die vaak gedaan worden binnen mijn vakgebied, de kindergeneeskunde. Dokters worden opgevoed met het adagium “meten is weten”. In dit proefschrift probeer ik deze stelling aan een kritisch oordeel te onderwerpen: Is meten wel weten?
In dit proefschrift onderzoek ik de betrouwbaarheid en het nut van vijf veelgebruikte metingen of procedures binnen de kindergeneeskunde, namelijk het meten van glucose in de urine uit luiers van pasgeborenen met behulp van test-strips, de diagnostische waarde van klinische symptomen (inclusief glucosurie) bij het opsporen van ernstige infecties bij prematuren, de vochtbalans bij zieke pasgeborenen, de beoordeling van de mate van benauwdheid bij kinderen en het gebruik van virale testen bij bronchiolitis.
Alleen de meting van glucosurie met behulp van test-strips bleek in de door ons uitgevoerde studies (onder enkele voorwaarden) betrouwbaar, de overige metingen of procedures waren onvoldoende precies of bleken onvoldoende nuttig te zijn in de klinische praktijk.
Samenvattend klopt het adagium “meten is weten” dus lang niet altijd; eigenlijk klopt het alleen onder de voorwaarde dat je weet wat, hoe, wie en waarom je meet! Meten is weten , maar met onzekerheid! Daarnaast blijkt uit dit proefschrift dat het nuttig kan zijn om veel gebruikte, routinematige metingen onder de loep te nemen. We willen benadrukken dat onze evidence-based practice een belangrijke rol heeft gespeeld bij het creëren van een een zelf-kritische en nieuwsgierige houding een belangrijke rol heeft gespeeld bij het tot stand komen van dit proefschrift.
Measurements are essential In medical practice. Throughout their training, doctors are taught the adage “meten is weten” (the knowledge is in the numbers). In this thesis we evaluated various diagnostic measurements in paediatrics – across a wide age range, from neonates to older children – aimed at clarifying (demystifying) these commonly used procedures. We discuss the general aspects of uncertainty in medicine, since uncertainty is a major reason for performing diagnostic tests in the first place. Furthermore we explain the way evidence-based medicine (EBM) may help doctors to deal with this feeling of uncertainty.
The measurements investigated in this thesis are: 1) test strips for measurement of glucosuria in neonates, 2) diagnostic value of various clinical signs, including glucosuria, in identifying late-onset sepsis in premature neonates, 3) fluid-balance charts in neonates, 4) clinical assessment of severity of dyspnoea in children and 5) viral tests to identify and isolate RSV-infected infants hospitalised for bronchiolitis, for the purpose of reducing nosocomial cross-infection.
Apart from the test strips for measurement of glucosuria, all measurements or procedures we have investigated appeared to be too imprecise or of insufficiently useful in clinical practice.
In general we conclude that the adage “meten is weten” (the knowledge is in the numbers) still holds true, provided that one knows what is being measured, how the measurement is performed, by whom, and for what reason. Furthermore we would like to emphasize the importance of investigating common or routine procedures, and the role EBM has played when investigating this theme.
