Reconciliação da medicação : perceção dos conhecimentos, práticas e atitudes pelos médicos de família da região de saúde de Lisboa e Vale do Tejo
Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2023Objetivos: caracterização da perceção dos médicos de família (MF) da Região de Saúde de Lisboa e Vale do Tejo (LVT) em relação aos conhecimentos, atitudes e práticas (na primeira consulta após alta hospitalar) sobre RecMed, e exploração de possíveis factores associados ao conhecimento e à prática (na primeira consulta após alta hospitalar) da RecMed.
Métodos: estudo quantitativo, observacional, transversal e descritivo, com uma componente exploratória analítica. Todos os MF das Unidades de CSP na Região de Saúde de LVT foram convidados, por e-mail, a preencher um questionário, sendo incluídas 208 respostas. Utilizou-se o Software SPSS v.28 para a análise dos dados.
Resultados: A maioria dos MF já tinha ouvido falar do termo «RecMed» (n=150; 72,1%). A quase totalidade dos MF identificou o processo de RecMed como uma ferramenta importante na melhoria da segurança e adesão à medicação, considerando que o tempo dispensado a actualizar a lista de medicação é relevante para o doente. Na primeira consulta após alta hospitalar, metade dos MF realizam a RecMed em mais de 76% destas consultas, encontrando, por vezes, discrepâncias (n=90; 43,3%), sendo que dificilmente conseguem estabelecer contacto com o médico hospitalar quando existem dúvidas. Entre os MF que praticam RecMed em mais de metade das consultas após alta hospitalar, a proporção que conhece o processo de RecMed é superior àqueles que não conhecem (75,7% vs 24,3%).
Conclusão: Os MF que conhecem a RecMed e a aplicam na prática clínica encontram, com maior frequência, discrepâncias na lista de medicação do doente. Identificou-se uma insuficiente comunicação entre CSP e centros hospitalares, o que pode levar à perpetuação de erros de medicação.Objectives: characterization of family doctors' (MF) perception of RecMed knowledge, behaviours and practices (during the first appointment following hospital discharge) in the Health Region of Lisboa e Vale do Tejo (LVT).
Methods: quantitative, observational, cross-sectional and descriptive study, with an analytical exploratory component. All MF in the LVT Health Region's CSP Units were invited via email to complete a questionnaire, which received 208 replies. The software SPSS v.28 was used for data analysis.
Results: The majority of MF (n=150; 72%) had heard of the term «RecMed». Considering that the time spent updating the prescription list is pertinent to the patient, almost all MF rated the RecMed procedure as a key instrument to enhance medication safety and adherence. In the first appointment following hospital discharge, more than 76% of the MF performed RecMed, occasionally identifying discrepancies (n=90; 43,3%) and having trouble getting in touch with the hospital doctor when there are questions. Among the MF who practice RecMed in more than half of the appointments after hospital discharge, the proportion who knows the RecMed process is higher than those who don’t (75,7% vs 24,3%).
Conclusion: MF that know RecMed and apply it in the clinical practice find, more frequently, discrepancies in the patient’s medication list. An insufficient communication between CSP and hospital centers was identified, which can lead to the perpetuation of medication errors
Carcinomas salivary-like da glândula tiroideia : a propósito de um caso clínico
Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2023Os carcinomas Salivary-like constituem carcinomas extremamente raros (<1%) primários da glândula tiroideia, que partilham semelhanças com os encontrados primariamente nas glândulas salivares. Este tipo de carcinomas apresenta uma predileção por doentes do sexo feminino, com histórico de Tiroidite de Hashimoto, e numa percentagem relevante em simultâneo com o Carcinoma Papilar da Tiroide. Exibem um comportamento indolente e outcome favorável pós-cirurgia, possuindo um número muito reduzido de casos agressivos reportados.
O caso apresentado refere-se a uma doente do sexo feminino, de 38 anos diagnosticada com Carcinoma Mucoepidermoide Esclerosante com Eosinonfilia associado a Carcinoma Papilar da Tiroide clássico e Microcarcinoma Papilar, submetida a lobectomia direita com istmectomia. Dois anos pós-operatório foi submetida a tiroidectomia esquerda com identificação e excisão de 2 glândulas paratiroides esquerdas por suspeita de recidiva ou de adenoma/carcinoma da glândula paratiroide, que posteriormente o exame anátomo-patológico veio a descartar: tratava-se de uma transformação nodular adenomatosa do parênquima da glândula tiroideia com focos de Tiroidite de Hashimoto, e um adenoma da glândula paratiroide.
O caso clínico apresentado constitui, portanto, um caso de Carcinoma Mucoepidermoide Esclerosante com Eosinofilia da Tiroide com bom prognóstico, sendo de grande interesse dada a sua raridade, e dado ser o primeiro caso relatado em Portugal.Salivary-like carcinomas are extremely rare (<1%) primary thyroid gland carcinomas, which share similarities with those found primarily in the salivary glands. This type of carcinoma has a predilection for female patients, with a history of Hashimoto's Thyroiditis, and in a relevant percentage of the cases, it appears simultaneously with Papillary Thyroid Carcinoma. They exhibit an indolent behavior and favorable post-surgery outcome, with a very small number of reported aggressive cases.
The case here by presented refers to a 38-year-old female patient diagnosed with Sclerosing Mucoepidermoid Carcinoma with Eosinophilia associated with classic Papillary Thyroid Carcinoma and Papillary Microcarcinoma, who underwent right lobectomy with isthmectomy. Two years post-surgery, the patient underwent left thyroidectomy with identification and excision identification and excision of 2 left parathyroid glands on suspicion of cancer recurrence or parathyroid gland carcinoma, which later the anatomopathological examination came to rule out: it was an adenomatous nodular transformation of the parenchyma of the thyroid gland with foci of lymphocytic thyroiditis, and a parathyroid gland adenoma.
The clinical case presented constitutes, therefore, a case of Sclerosing Mucoepidermoid Carcinoma with Thyroid Eosinophilia with a good prognosis, being of great interest given its rarity, and given that it is the first case reported in Portugal

Caracterização microbiológica de águas residuais hospitalares

conter por exemplo microrganismos oportunistas como Aspergillus fumigatus, Candida auris e
Legionella pneumophila. A. fumigatus e C. auris são considerados pela Organização Mundial de Saúde
como fungos de elevada criticidade devido à sua patogenicidade e resistência a antifúngicos. L.
pneumophila origina patologias como a doença do Legionário e infeções respiratórias, desencadeando
surtos globais.
O trabalho desenvolvido nesta dissertação visa detetar os microrganismos já mencionados em
águas residuais hospitalares e, em afluentes e efluentes das respetivas Estações de Tratamento de Águas
Residuais (ETAR) na área de Lisboa, no âmbito do Plano de Ação para a Gestão das Águas Industriais
Residuais (AgIR).
Foram analisadas 52 amostras, distribuídas por duas campanhas de amostragem em
outubro/novembro/2023 e abril/maio/2024. A metodologia aplicada consistiu na filtração, inoculação e
isolamento das amostras em meio de crescimento apropriado, com observação de parâmetros
microscópicos e macroscópicos. Paralelamente foi extraído DNA a partir das amostras de água e, das
placas de crescimento de espécies fúngicas. O DNA extraído foi submetido a PCR Real-Time (qPCR).
Como método alternativo foi adotado o procedimento de sequenciação da região ITS do DNA
ribossomal (rDNA) para a identificação molecular dos isolados de algumas amostras.
Nas duas campanhas de amostragem foram detetadas/identificadas as três espécies em estudo,
sendo C. auris detetada pela primeira vez em águas residuais no país, apesar de ausência de casos
clínicos de infeção conhecidos neste período. Foi demonstrado que estas espécies são também detetadas
em afluentes e efluentes de ETAR, indicando a potencial disseminação destes microrganismos para o
ambiente.
Estes resultados demonstram a importância da monitorização das águas residuais hospitalares e
ETAR na vigilância ativa da disseminação destas espécies com especial interesse na Saúde Pública,
auxiliando as unidades de saúde na implementação de medidas de prevenção e mitigação.Hospital wastewater contains various chemical and biological compounds, including
opportunistic microorganisms such as Aspergillus fumigatus, Candida auris, and Legionella
pneumophila. A. fumigatus and C. auris are considered by the World Health Organization to be fungi of
high criticality due to their pathogenicity and resistance to antifungals. L. pneumophila causes diseases
such as Legionnaire’s disease and respiratory infections, causing global outbreaks.
This dissertation aims to detect the aforementioned microorganisms in hospital wastewater, as
well as in the influents and effluents of the respective Wastewater Treatment Plants (WWTP’s) in the
Lisbon area, as part of the Action Plan for the Management of Industrial Wastewater.
Fifty-two samples were analyzed, distributed across two sampling campaigns in
October/November 2023 and April/May 2024. The methodology applied consisted of filtration,
inoculation, and isolation of the samples in appropriate growth media, with observation of microscopic
and macroscopic parameters. DNA was also extracted from the water samples and from the fungal
species growth plates. The extracted DNA was subjected to qPCR. As an alternative method, the ITS
region of rDNA was sequenced for the molecular identification of isolates from some samples.
In the two sampling campaigns, all three species under study were detected/identified, with C.
auris being detected for the first time in wastewaters in the country, despite the absence of known
clinical infection cases in this period. It was demonstrated that these species are also present in WWTP
influents and effluents, indicating the potential for dissemination of these microorganisms into the
environment.
These results highlight the importance of monitoring hospital wastewater and WWTP’s in the
active surveillance of the spread of these species, which is of particular interest to public health by
assisting health units in implementing preventive and mitigation measures
Configurational analysis of firms' performance : Understanding the role of Internet financial reporting
Internet Financial Reporting (IFR) is the disclosure of financial information through corporate websites. Although a vast amount of the literature analyzes the determinants of IFR, the literature does not adequately address the economic consequences of IFR. This study fills this gap in the literature by answering the following research question: Which configurations of the IFR and the firm's age and risk lead to high or low performance? To answer this question, this study analyzes the impact of IFR through longevity and risk on the Tobin's Q of firms. A fuzzy-set qualitative comparative analysis (fsQCA) sets the methodological ground to explore the role of IFR on the firms' performance. The fsQCA's results indicate that several combinations of IFR with risk and longevity are con figurations for high performance. European wolves are currently exposed to high levels of anthropogenic disturbance and mortality, limiting genetic connectivity between populations, and increasing the risk of inbreeding and genetic diversity loss. This thesis focuses on the northwestern Iberian wolf population, which underwent a severe bottleneck in the 1970s. While the Spanish wolf population managed to recover and expand, the Portuguese subset experienced continued decline, particularly in the region south of the Douro river, where reduced effective numbers, breeding instability, and isolation remain concerning. The overall goal of this thesis is to identify factors acting as barriers to connectivity within this subpopulation, employing a holistic approach. It begins with a literature review outlining previous studies' methodologies and main barriers to wolf connectivity in Europe, serving as a foundation for subsequent empirical analyses. These analyses delve into specific aspects of wolf ecology and conservation in the study area. One chapter focuses on livestock depredation events, addressing the challenges and implications of accurately identifying the predators involved. Another investigates wolf and free-ranging dog’s diets, shedding light on their patterns of prey consumption and livestock depredation, and its implications for wolf conservation. Next, human dimensions are explored to understand the socio-demographic factors and perceptions influencing human attitudes towards wolves. This understanding is crucial for developing effective coexistence strategies in a region where wolf persistence and survival is highly conditioned by anthropogenic factors. Finally, building on the previous findings, the final chapter examines functional connectivity by employing a landscape genetics approach to identify the main factors contributing to genetic distances between individuals. In conclusion, this thesis offers a comprehensive and up-to-date insight into the challenges faced by this threatened population, shedding light on potential factors hindering connectivity. This data may inform new evidence-based conservation strategies aimed at promoting wolves’ long-term persistence and connectivity south of the Douro
