Chemical interaction and molecular growth of a highly dipolar merocyanine molecule on metal surfaces: A photoelectron spectroscopy study
Sub-1-min relaxation-enhanced non-contrast non-triggered cervical MRA using compressed SENSE with deep learning reconstruction in healthy volunteers
Background: We evaluated the acceleration of a three-dimensional isotropic flow-independent magnetic resonance angiography (MRA) (relaxation-enhanced angiography without contrast and triggering, REACT) of neck arteries using compressed SENSE (CS) combined with deep learning (adaptive intelligence, AI)-based reconstruction (CS-AI). Methods: Thirty-four volunteers received 3-T REACT MRA, acquired threefold: (i) CS acceleration factor 7 (CS7), scan time 1:20 min:s; (ii) CS acceleration factor 10 (CS10), scan time 0:55 min:s; and (iii) CS-AI acceleration factor 10 (CS10-AI), scan time 0:55 min:s. Two radiologists rated the image quality of seven arterial segments and overall image noise. Additionally, a pairwise forced-choice comparison was conducted. Apparent signal-to-noise ratio (aSNR) and contrast-to-noise ratio (aCNR) were measured, and image sharpness was assessed using the edge-rise distance (ERD). Multiple t -tests and nonparametric tests with Bonferroni correction were performed for comparison to CS7 as the reference standard. Results: Compared to CS7, CS10 showed lower image quality ( p 0.999). Compared to CS7, aSNR and aCNR were lower in CS10 ( p < 0.001) and the ERD was longer ( p = 0.004), while CS10-AI provided better aSNR and aCNR ( p = 0.001) and showed no difference in ERD ( p = 0.776). Conclusion: Sub-1-min CS-AI cervical REACT MRA was acquired without compromising image quality. Relevance statement The implementation of a fast and reliable non-contrast MRA has the potential to reduce costs and time while increasing patient comfort and safety. Clinical studies evaluating the diagnostic performance for stenosis or dissection are needed. Trial registration DRKS00030210 (German Clinical Trials Register; https://drks.de/). Neoadjuvante Chemotherapie gefolgt von radikaler vaginaler Trachelektomie als experimentelles fertilitätserhaltendes Konzept bei jungen Frauen mit einem invasiven Zervixkarzinom >2 cm und dringendem Kinderwunsch
Zielsetzung:
Eine Konisation/Re-Konisation, einfache oder radikale Trachelektomie sind international etablierte operative Konzepte zum Fertilitätserhalt für junge Patientinnen mit einem frühinvasiven Zervixkarzinom unter 2cm Größe und bestehendem Kinderwunsch. Eine Fertilitätserhaltung bei Patientinnen mit größeren Tumoren ≥2cm wird aufgrund der geringen Datenlage und der erhöhten Rezidivrate als experimentell eingestuft. International werden in
dieser klinischen Situation bei ausgeprägtem Kinderwunsch 2 Konzepte vertreten: eine upfront radikale abdominale Trachelektomie oder eine neoadjuvante Chemotherapie gefolgt von einer einfachen/radikalen Trachelektomie.
Das Ziel der vorliegenden Arbeit ist es, das onkologische Outcome sowie die geburtshilflichen Ergebnisse von Patientinnen mit einem Zervixkarzinomen über 2cm Größe nach
laparoskopischer Lymphadenektomie gefolgt von einer neoadjuvanten Chemotherapie und anschließender radikaler vaginaler Trachlelektomie zu evaluieren, die von einer gynäkologisch onkologischen Arbeitsgruppe in zwei Zentren (Charité Berlin und Asklepios Kliniken Hamburg)
behandelt wurden.
Methoden:
Demografische, histologische, fertilitäts- und follow-up Daten aller Patientinnen, die zwischen 02/2006 und 06/2020 bei einem Zervixkarzinom der Stadien (FIGO 2018) IB2, IB3 oder IIA1 eine laparoskopische Lymphadenektomie, gefolgt von einer neoadjuvanten Chemotherapie und radikalen vaginalen Trachelektomie an der Charité Berlin und den Asklepios Kliniken Hamburg erhalten haben, sollen analysiert werden.
Ergebnisse:
31 Patienten mit einem mittleren Lebensalter von 29.5 Jahren (26-40 Jahre) unterzogen sich nach ausführlicher Aufklärung einer neoadjuvanten Chemotherapie (NACT) gefolgt von einer
radikalen vaginalen Trachelektomie (RVT), sofern nachgewiesen wurde, dass die lokoregionären Lymphknoten tumorfrei waren. Von diesen Patienten waren 26 (84 %) kinderlos.
Die initialen Tumorstadien waren IB2 (n=27), IB3 (n=3) und IIA1 (n=1). Bei der am Anfang stehenden obligatorischen pelvinen Lymphadenektomie wurden durchschnittlich 33 (11-47) Beckenlymphknoten entfernt wurden. Bei einer Patientin wurde nur eine Sentinel-Lymphonodektomie durchgeführt.
Das sich anschließender NACT-Regime bestand aus 2xTIP (Paclitaxel, Ifosfamid und Cisplatin) bei 17 Patienten, 3xTIP bei 8 Patienten, 2xTP (Paclitaxel und Cisplatin) bei 4
Patienten, 2x Cisplatin-Monotherapie bei einer Patientin und 2xTP gefolgt von 2xTIP bei einer weiteren Patientin. Bei der radikalen vaginalen Trachelektomie wurde in 17 OP- Präparaten 8 (55 %) noch Resttumorgewebe nachgewiesen. Die mediane Größe des verbliebenen Tumorrestes nach der neoadjuvanter Chemotherapie betrug 12 mm (1-60 mm).
Ein Fertilitätserhalt war bei 27 Patienten möglich; zwei Patientinnen wurde nach RVT eine adjuvanten Radiochemotherapie aufgrund von high-risk histologischen Merkmalen angeraten, während zwei andere Patientinnen sich nach der neoadjuvanten Chemotherapie bei fehlendem Tumoransprechen einer radikalen Hysterektomie mit adjuvanter
Radiochemotherapie unterzogen.
Von den 18 (67 %) Frauen, die eine Mutterschaft anstrebten, wurden 13 schwanger (72 %). Es kam zu 12 Lebendgeburten bei 10 Frauen mit einem medianen fetalen Gewicht von 2490
(1640-3560) Gramm und 5 Fehlgeburten.
Nach einer medianen Nachbeobachtungszeit von 94,5 (6-183) Monaten traten 3 Rezidive (11,1 %) auf, eine Patientin (3,7%) verstarb an der Krankheit.
Schlussfolgerung:
Eine neoadjuvante Chemotherapie, gefolgt von der radikalen vaginalen Trachelektomie, kann Patientinnen, die einen Fertilitätserhalt bei einem Zervixkarzinom über 2 cm Größe anstreben und bei denen histopathologisch keine Tumorzellen in den Lymphknoten nachgewiesen wurden, angeboten werden mit einer hohen Wahrscheinlichkeit für eine nachfolgende
Schwangerschaft. Dieses experimentelle fertilitätserhaltende Konzept weist, verglichen mit den Ergebnissen der primären radikalen Trachelektomie bei einer Tumorgröße unter 2cm, in
dieser Fallserie wie auch in der Literatur eine höhere Rate an Rezidiven und Todesfällen auf
The genomic implications of sex and its absence in old taxa
Reproduction is a fundamental characteristic of organisms, and sexual reproduction is nearly universal in eukaryotes. The last eukaryotic common ancestor (LECA) is thought to have been capable of sexual reproduction, as this mode of reproduction is common across many different groups of eukaryotes. Sexual reproduction has three critical processes: meiosis, recombination and cell fusion, all of which are considered ancient aspects of sexual reproduction.
Protists are considered the closest eukaryotes to the LECA and have evolved into many clades. Their abundance and primitive characteristics are crucial for elucidating the origins and evolution of sexual reproduction. However, most of the protists were considered to be asexual or facultative sex because of less sexual behavior or gender observed. This may be due to the protists being too diminutive to directly observe and trace in nature, and examining their entire lifespan when culturing most unicellular protists in laboratory presents a challenge. Detecting potential sexual processes by identifying meiosis-related genes and their expression dynamics may serve as a valuable approach during cell fusion.
Fisculla terrestris (Thecofilosea, Rhizaria, SAR Supergroup) is a unicellular, shell-bearing amoeba living in soils that can be reared in the lab. Starvation triggers cell fusion and cell disintegration when food re-supplied are observed in F. terrestris. To check the presence of meiosis-associated genes and the dynamics of their expression during cell separation and aggregation, we sequenced the transcriptomes of the two treatments of starvation and food supply. We found that genes related to recombination are upregulated in the starving treatment compared to the food supply treatment. This might imply the exchange of genetic material and cryptic sex.
Contrasting unicellular protists, most metazoans are known to reproduce sexually. However, there are a few metazoans that have stopped reproducing sexually and reproduce obligately asexually. Although asexuality was regarded as a dead end in evolution due to harmful mutation accumulation, some organisms still survived and evolved in the long-term without sex: "ancient asexual scandals," such as oribatid mites, bdelloid rotifers, and darwinulid ostracods. Studying how these organisms escape extinction and adapt to the environment can help us to understand why sexual reproduction is important and how these species evolved.
To elucidate the enigma of asexual evolution following a prolonged absence of sex, we used genome and haplotypic assemblies of P. peltifer to detect genomic features as a consequence of asexuality over time. We detected the “Meselson effect” in the P. peltifer genome, which indicated long-term asexual reproduction. The haplotypes independently evolved, leading to one allele relaxed and the other allele conserved, which was also reflected on the patterns of differently expressed alleles (DEAs) and horizontal gene transfer (HGT). About 2% of genes showed a HGT origin, and they might provide new traits like cell wall degradation and pesticide resistance, strengthening the mites as decomposers in soil. Selection analysis showed that most of the alleles are under purifying selection, potentially alleviating deleterious mutation accumulation, while certain genes under positive selection may facilitate adaptation. Our results provide insight into long-term asexual genomic features and the potential mechanism for ancient asexual persistence and evolution.

Der Einsatz von Ectoin-haltiger Inhalationslösung zur Therapie der akuten Bronchitis
Als eine der häufigsten Krankheitsentitäten stellen Atemwegsinfekte eine enorme alltägliche Her-
ausforderung für unsere Gesellschaft dar. Die Herausforderung besteht darin die vielfältigen Diffe-
rentialdiagnosen zu erkennen und die richtige Therapie auszuwählen. Eine eindeutige Diagnose-
stellung ist nicht immer einfach oder gar unmöglich.
Individuell verursachen sie teilweise enormen Leidensdruck, eine erhebliche Einschränkung in der
Lebensqualität und können potenziell letal verlaufen. Gesellschaftlich rangieren Atemwegsinfekte
unter den häufigsten Todesursachen weltweit und verursachen dadurch enorme Kosten für das Ge-
sundheitssystem. Zu den unkomplizierten Atemwegsinfekten zählt die akute Bronchitis.
Sie wird anhand klinischer Kriterien diagnostiziert. Fast immer geht sie mit Husten einher. Hinzu
kommen zahlreiche andere unspezifische Symptome. Sie verläuft selbstlimitierend und fast immer
unkompliziert. Eine weiterführende Diagnostik ist in den meisten Fällen daher nicht sinnvoll. Thera-
peutisch wird ein symptomatisches Konzept empfohlen. Die verfügbaren Therapieoptionen sind
zahlreich ohne das eindeutige Empfehlungen durch die Leitlinien vorliegen.
Die EIL-Studie untersucht die Inhalation Ectoin-angereicherten Lösung mit der Inhalation einer rei-
nen Kochsalzlösung als Therapie bei akuter Bronchitis. Die Ergebnisse lassen dabei auf einen po-
sitiven Therapieeffekt in beiden Gruppen schließen. Es gibt Hinweise auf einen Vorteil von Ectoin
gegenüber NaCl, dessen klinische Relevanz noch weiter untersucht werden muss. Die Therapie war
in beiden Gruppen sehr gut verträglich.
Im indirekten Vergleich mit der internationalen Studienlage ist eine Wirkung über der von Placebo
annehmbar. Eine zuverlässigere Aussage ist aufgrund der schweren Vergleichbarkeit der Studien
und fehlender Standards aber nicht möglich.
Die Herausforderungen, die Atemwegsinfekte wie akute Bronchitiden an unsere Gesellschaft stellen,
erfordert einen multimodalen Ansatz. Ein effizientes und kritisch hinterfragtes Zusammenspiel unter
Ausschöpfung bisheriger Erkenntnisse und sinnvoller Anwendung des medizinisch Möglichen ist
gefragt. Gerade unter diesem Aspekt sollte neuen und alternativen Therapieoptionen mit Offenheit
begegnet werden. Denn nur mit einem solchen Ansatz wird es möglich, die uns verfügbaren Res-
sourcen nachhaltig und effizient zu nutzen und gleichzeitig die Versorgung der Patient*innen zu
verbessern, die unter solch alltäglichen Erkrankungen leiden
CAR-E-Score vs. ICE-Score hinsichtlich ICANS Detektion bei der Therapie mit Chimären-Antigen-Rezeptor-T-Zellen
Die CAR-T-Zell Therapie ist mittlerweile eine etablierte und erfolgreiche Behandlung bei Patient*innen mit refraktären oder rezidivierenden malignen B-Zell Erkrankungen. Zu den Nebenwirkungen gehören neben dem Cytokine Release Syndrome (CRS) auch das Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS), welches sich unter anderem durch Verwirrtheit, Tremor, Aphasie und Vigilanzminderung äußert. Aktuell wird ein ICANS mit dem ICE-Score diagnostiziert. An der Uniklinik Köln ist jedoch aufgefallen, dass Patient*innen nach CAR-T-Zell Therapie klinische Zeichen einer Neurotoxizität zeigten, obwohl sie im ICE-Score noch eine volle Punktzahl erreichten. Da der ICE-Score täglich in identischer Form durchgeführt wird und es keine Parallelversionen gibt, erscheint eine herabgesetzte Sensitivität insbesondere für leichte Formen eines ICANS durch dieses Instrument möglich. Im Rahmen dieses Promotionsprojektes sollte untersucht werden, ob mit einem klinischen Test (CAR-E-Score), welcher mittels Anwendung von Parallelversionen täglich variiert, kognitive Defizite, die im Rahmen eines ICANS auftreten, zuverlässiger, sensitiver und früher detektiert werden können.
Der CAR-E-Score besteht aus vier Untertests, die insbesondere Aufmerksamkeit und exekutive Funktionen prüfen (Rechnen, Wortflüssigkeit, Zeitschätzung, Mustererkennung).
Er wurde über ein Jahr bei CAR-T-Zell-Patient*innen in der Hämato-Onkologie der Uniklinik Köln (Medizinische Klinik I) durchgeführt und die Ergebnisse wurden danach mit denen des ICE-Scores verglichen.
In der Auswertung zeigte sich, dass sowohl der CAR-E-Score gesamt als auch der Untertest „Mustererkennung“ in der Lage waren, ein mit dem ICE-Score diagnostiziertes ICANS zu erfassen. Darüber hinaus identifizierte der CAR-E-Score noch weitere Patient*innen mit kognitiven Defiziten. In der klinischen Anwendung des Scores wurde jedoch deutlich, dass zur Beurteilung der individuelle Verlauf der Testwerte des/der jeweiligen Patient*in eine Rolle spielt und durch die Anwendung eines allgemeingültigen Cut-off-Wertes aufgrund einer höheren Variabilität der Testergebnisse die Reliabilität des Testes herabgesetzt wird. Anstatt eines allgemeingültigen Cut-off-Wertes scheint der Nachweis einer Verschlechterung an zwei aufeinander folgenden Tagen im Vergleich zum Testwert vor Therapie besonders geeignet reliabel die kognitive Einschränkung zu detektieren.
Zusammenfassend geht aus der vorliegenden Arbeit hervor, dass insbesondere der Untertest „Mustererkennung“ ein ICANS zuverlässig detektiert und in der Lage ist kognitiv gering eingeschränkte Patient*innen mit Neurotoxizität zu erfassen, die vom ICE-Score nicht identifiziert werden. Er ist jedoch aufgrund der etwas aufwendigeren Anwendung und Auswertung ohne Cut-off-Wert nicht als Ersatz des ICE-Scores geeignet. Wie eine sinnvolle Anwendung und Auswertung genau aussehen könnten, sollte in weiteren Arbeiten untersucht werden.
Ein Vorschlag wäre zunächst, die „Mustererkennung“ zusätzlich zum ICE-Score nach CAR-T- Zell Therapie durchzuführen, um mehr Patient*innen mit Neurotoxizität bzw. ICANS zu erfassen. Dabei würde die „Mustererkennung“ unabhängig vom Ergebnis im ICE-Score bei zweimaliger Verschlechterung im Vergleich zum Testwert vor Therapie zur Diagnose eines Pre-ICANS führen
Research on Artificial Intelligence Technology and Its Reliability in Esophageal Cancer and Robotic Assisted Minimally Invasive Esophagectomy
Focusing on esophageal cancer, this dissertation conducted a meta-analysis to evaluate the general performance of Artificial Intelligence (AI) methods on the diagnosis of Esophageal Squamous Cell Carcinoma (ESCC) in endoscopic images and videos. The results indicated the qualified performance of AI methods in image-based ESCC diagnosis. However, there is still room for improvement of AI methods on video-based ESCC diagnosis. Then a pipeline was proposed and implemented in the University Hospital of Cologne to collect and preprocess Robotic Assisted Minimally Invasive Esophagectomy (RAMIE) video data for data analysis and AI modeling research. Over 200 cases were successfully enrolled and collected by the pipeline in the past three years. For AI modeling research, a Relation Laparoscopic Cholecystectomy Surgical Object Detector (Relation-LCSOD) model was developed to enhance the reliability and accuracy of AI methods in surgical object detection. A series of comparative experiments were performed to validate and test the performance of the proposed model based on two public datasets. The comparison results with baseline models on two datasets demonstrate significant performance improvements of the proposed models, especially for anatomically relevant objects detection. The proposed model can utilize position relation information among surgical objects and effectively reduce redundant bounding boxes. The AI modeling research in this dissertation can promote the trust integration of AI into esophageal cancer research and support AI applications in more clinical practice in the future
Gender inequality in Cameroon: from the perspective of women against women-a household influence
This dissertation offers a new contribution and a new approach by examining the different media and contexts in which women address violence. It also addresses or criticises the use of language to downgrade women. Hence, the aim is to use precisely the language varieties and styles of speaking used by the women in question— which are the women in the English-speaking parts of Cameroon, to present, discuss, and analyse the various aspects of its topic. Using their language varieties, is also a way of reaching back to these women spoken of in this research such that they could be able to recognise their own stories—as it is written in a language, they understand best.
In this study, many qualitative methods were used to get the result. The methods were as follows; In-depth, open-ended interviews, participant observation, discourse analysis, social media and document analysis. The research was carried out through fieldwork (with many visits to the field), internet searches, and desk study.
The results obtained depend on the different methods used. For example, on the part of social media, I realised through this research that vicarious writing, which is the main method this research is based on, is much more put into practice here. Women’s stories were told or written by other women as the owners of these stories couldn’t tell them by themselves. While in household experiences, in as much as all the chapters are interwoven, this particular area (the home) of women’s meetings could act as the pivot for all other meeting platforms in this research. This is because it gives an idea of where most women are coming from, where they got their principles from, and one of the main areas where their emotions could be nurtured.
From the results gotten, I noticed that almost everything is influenced by power relations. It is about who is more powerful than the other, who has more rights than the other, and who has the right to make certain decisions. Therefore, inequality among women might still have a long way to go, so far power relations is still a big influence
Crystal growth and characterization of 4d and 5d transition metal compounds
In this work, both the flux method for growing single crystals from the melt and the chemical transport reaction method for growing crystals from the gas phase were employed.
The flux method was applied to the crystal growth of quaternary perovskite compounds of the types Ba3MM’2O9 and Ba4MM’3O12. In these structure types, the metal atoms M and M′ occupy distinct crystallographic sites, both in octahedral coordination. The M′ atoms form face-sharing octahedral units: [M’2O9]-dimers in Ba3MM’2O9 and [M’3O12]-trimers in Ba4MM’3O12. These structural units are corner-connected via [MO6]-octahedra, resulting in isolated cluster motifs.
Due to the short metal–metal distances within the [M’2O9]-dimers and [M’3O12]-trimers, the formation of molecular orbitals is possible. Therefore, these compounds are classified as "cluster materials". In this study, the clusters were occupied by either ruthenium or iridium, since these 4d and 5d transition metals exhibit strong spin–orbit coupling due to their high atomic numbers, leading to pronounced splitting of the valence electron levels. The interaction between molecular orbitals and spin–orbit coupling can give rise to novel electronic configurations, making these materials highly promising candidates in the field of quantum materials.
The oxidation states of Ir and Ru are derived from the charge neutrality condition of the compound and are predominantly dictated by the valence of the M-site cation, with Ba2+ and O2- acting as constant components. This enables for targeted tuning of the electron count per dimer or trimer cluster, by selective substitution of differently valent elements on the M-site.
Due to the limited understanding of the phase diagrams of such complex systems, experimental data were collected to identify suitable synthesis conditions. The effect of the BaCl2 flux on the crystal growth process was investigated, and parameters such as temperature, crucible material and chemical composition were optimized. As a result, several iridium- and ruthenium-based single crystals of Ba3MM’2O9 and Ba4MM’3O12 types were synthesized.
Following optimization of the growth parameters, thermal, chemical and structural characterization of the synthesized crystals was carried out. It was found that the ruthenates exhibit greater thermal stability at elevated temperatures compared to their iridium analogues. Chemically, all compounds matched the target stoichiometry, with the exception of systems containing M = Nb and Ta, where partial substitution by Ir and Ru, respectively, was observed.
Structural investigations using single-crystal X-ray diffraction were carried out on Ba3MRu2O9 (with M = Ce, Eu, Tb, Yb and Y), Ba4MRu3O12 (with M = Ce and Ta) and
Ba4TaIr3O12.
For Ba3MRu2O9 with M = Eu and Tb, potential structural distortions and symmetry reductions from P63/mmc to P63mc were observed. For all other compounds of this structure type, the hexagonal symmetry P63/mmc could be confirmed.
Ba4TaRu3O12 crystallizes in space group R-3m, is isostructural with Ba4NbRu3O12, and exhibits Ru–Ta substitution. In contrast, the iridium analogue Ba4TaIr3O12 shows no such substitution. Ba4CeRu3O12 is monoclinically distorted and crystallizes in space group C2/m. Other synthesized trimer-type compounds with M = rare-earth elements showed severe structural disorder along the c-axis. These stacking faults hindered the indexing of the single-crystal data.
In the final part of this work, the crystal growth of Ni2InSbO6 was investigated using the CTR method in combination with a transport balance to measure transport rates. The experimentally determined rates were compared with theoretically calculated values in order to enable controlled crystal growth. The results revealed that the chemical composition of the grown crystals depends on the transport rate and the partial vapor pressures of the starting materials
