Il museo virtuale della Valle del Calore
The Virtual Museum of the Upper Calore Valley is a website which allows visitors to travel in time and space through and have access to various information on monuments, towns, culture, history, wine and food of the Hirpinian territory. By accessing six fictional videos on characters drawn from local history, users can also experience a historical overview, from the Longobard invasion up to the Unification of Italy, through the troubled periods of the Kingdom of Naples. The project works by open source software for video editing, GIS elaboration, and image processing. The browsing platform is based on the earliest release of the Aton framework created by CNR ITABC for browsing large-scale geographical and architectural data, with advanced features for graphic rendering, stereoscopic view, online representation of complex geometry and resolution through powerful paging algorithms. Aton is compatible with every modern HTML5 multimedia standard and is a powerful tool for historical storytelling
Intrahepatic Gallbladder
Gallbladders have been reported in various ectopic locations. An intrahepatic gallbladder is one where the gallbladder lies within the liver parenchyma or has a subcapsular location along the anterior inferior right lobe of the liver. Intrahepatic gallbladders can have impaired function leading to the formation of gallbladder stones. We present the case of a 57-year-old man who was found to have an intrahepatic gallbladder which was not detected before a retrograde open cholecystectomy was carried out without intra-operative or postoperative complications. Awareness of ectopic gallbladdersand their recognition allows proper intra-operative planning
Leydig Cell Hyperplasia Mimicking a Testicular Tumour in a Patient with Klinefelter Syndrome
Background: Klinefelter syndrome (KS) is the most common sex-chromosomal disorder in males. Frequently under-recognized, it occurs in
1 in 500–600 male births. It is caused by the inheritance of at least one additional X chromosome from either parent. Patients often have
uncommon or atypical malignancies.
Patient: We describe the case of a 35-year-old man with 47XXY KS and previous cryptorchidism, presenting with a painful testicular mass.
Histology confirmed Leydig cell hyperplasia.
Discussion: Cryptorchidism is an established risk factor for testicular tumours and occurs six times more commonly in KS than in the general
population. Despite this, large epidemiological studies have shown a reduced burden of testicular cancer in these patients. The presentation
of a hypoechoic lesion on ultrasound will prompt consideration of testicular tumours, however orchalgia represents an atypical presentation.
In patients with KS, Leydig cell hyperplasia is a much more common entity and should be considered early in the differential diagnosis
Circolare applicativa NTC 2018
testo in Redazione.
Ciò ha consentito di mettere a disposizione un volume maggiormente curato e
leggibile, a beneficio di chi dovrà lungamente studiare e consultare per esigenze
professionali le norme tecniche e le istruzioni applicative
Contare a Haghia Triada.
T-Cell/Histiocyte-Rich Large B-Cell Lymphoma: A Challenging Diagnosis
Epstein-Barr virus (EBV) is a double-stranded virus that shows tropism for B-cell lymphocytes. EBV-infected patients usually present with tonsillitis/pharyngitis, cervical lymphadenopathy and fever, but an atypical presentation can mimic lymphoproliferative disease.
We present the case of a 77-year-old woman with asthenia, fever, oral ulcers and peripheral lymphadenopathy. After extensive evaluation, including anatomopathological and immunocytochemical examination of excisional lymph node biopsy samples, it was still not clear whether the patient had EBV infection or diffuse large B-cell lymphoma.
In this case report, the authors describe how it can be difficult to differentiate between two different, although related, entities, making diagnosis of lymphoma highly challenging
Pancreatitis Can Present Like Cancer: Lymphoplasmacytic Sclerosing Pancreatitis in a Patient with a History of Gastric Carcinoma
Autoimmune pancreatitis has been associated with many syndromes in the presence of increased immunoglobulin levels. IgG4 antibodies
are elevated in the context of lymphoplasmacytic sclerosing pancreatitis associated with IgG4-related disease. We present the case of a
74-year-old man diagnosed with autoimmune pancreatitis on a cancer background. Awareness of this condition in the cancer patient is
crucial for timely diagnosis. Infectious complications might have implications for the choice of immunosuppressant
Sphingobacterium multivorum: An Atypical Bacterium in an Atypical Place
We present the case of a 75-year-old woman admitted to hospital because of an infected pressure ulcer. Cultures revealed that the responsible bacterium was Sphingobacterium multivorum, which was successfully eradicated with ciprofloxacin. Over the last few years, there have been reports of new cases of infection caused by bacteria previously not thought to be harmful to humans, like S. multivorum. Previous cases were reported mostly in immunosuppressed patients and the present report is, to our knowledge, the first describing a pressure ulcer infected by this bacterium
Highlighting the Link between Parkinson’s Disease and Malignant Melanoma: A Case Report and Literature Review
The aim of this case report and literature review is to explore the rarely highlighted co-occurrence of malignant melanoma and Parkinson’s disease (PD) and to describe the devastating effects on those affected. We present a case of acute confusion and worsening shuffling gait in a patient with Parkinson’s disease who eventually succumbed to metastatic malignant melanoma, secondary to a primary malignant melanoma on the back that was treated 6 years earlier. More research is needed to better characterize the associations between these two conditions, and to evaluate the effectiveness of prevention strategies such as melanoma screening for adults with PD
Two Dabigatran Fast Reversals in a 4-month Period – a Case Report
Introduction: Idarucizumab is available for immediate reversal of dabigatran-induced anticoagulation in life-threatening bleeding or urgent surgery in patients with non-valvular atrial fibrillation (nvAF).
Case description: We report a case of an 85-year-old female treated with dabigatran for nvAF, submitted to two fast reversal procedures with idarucizumab in a 4-month period. In the first emergency episode, the patient was admitted due to a fall-related cerebral haemorrhage and subdural haematoma. There was a fast reversal of the effects of dabigatran after idarucizumab administration, which allowed stoppage of the bleeding and a decrease in intracranial pressure, with full patient recovery. Four months later, the patient revisited the hospital complaining of diffuse abdominal pain while on the same antithrombotic therapy. Physical examination showed signs of peritoneal irritation and the use of idarucizumab to reverse the effects of dabigatran was decided upon to secure normal bleeding conditions before surgery.
Discussion: Idarucizumab is an efficient, safe and feasible option for dabigatran-treated nvAF patients, when urgent anticoagulant effect reversal is needed
