    Trattamento delle tossicodipendenze presso il Tribunale del Québec a Montréal

    L’articolo rappresenta la conclusione dell’esperienza di studio e di ricerca effettuata, nell’estate del 2017, presso l’Institut Universitaire sur les Dépendances di Montréal (Centre intégré universitaire de santé et de services sociaux CIUSS du Centre-Sud-de l’île-de-Montréal). In particolare, il lavoro empirico si è concentrato sull’analisi delle caratteristiche, degli obiettivi e dell’andamento del PTTCQ (Programme de Traitement de la Toxicomanie de la Cour du Québec à Montréal). A tal fine, sono state effettuate undici interviste semi-strutturate rivolte a testimoni significativi (ricercatori, operatori sociali, giudici e avvocati) impiegati nell’ambito di tale programma. La ricerca ha permesso altresì di metterne in evidenza i punti di forza, le criticità e le prospettive future. Résumé L’article est le résultat de l’expérience acquise dans le cadre d’une rechercherecherché réalisée pendant l’été 2017, à l’Institut Universitaire sur les Dépendances - Centre intégré universitaire de santé et de services sociaux CIUSS du Centre-Sud-de l’île-de-Montréal. La recherche sur le terrain a été centrée sur l’analyse des caractéristiques, des objectifs et des performances du PTTCQ (Programme de Traitement de la Toxicomanie de la Cour du Québec à Montréal). À cette fin, l’auteur a conduit onze entretiens semi-directifs avec des intervenants (chercheurs, travailleurs sociaux, juges, avocats) qui travaillaient dans le cadre de ce programme. La recherche a également mis en évidence les avantages, les inconvénients et les perspectives d’avenir pour ce programme. Abstract This article is the result of the experience gained through the research conducted, during summer 2017, at the University Institute of Drug Addiction in Montreal (Institut Universitaire sur les Dépendances - Centre intégré universitaire de santé et de services sociaux CIUSS du Centre-Sud-de l’île-de-Montréal). In particular, the empirical research focused on the analysis of the characteristics, objectives and performance of the PTTCQ (Programme de Traitement de la Toxicomanie de la Cour du Québec à Montréal - Drug Addiction Treatment Program at the Court of Quebec in Montreal). To this end, the author conducted eleven semi-structured interviews with stakeholders (researchers, social workers, judges, lawyers) working in this program.     A Rare Presentation of Ogilvie's Syndrome

    Ogilvie’s syndrome or acute colonic pseudo-obstruction is characterized by massive colon dilation in the absence of mechanical obstruction or toxic megacolon. The phenotype associated with secretory diarrhoea is rare and is related to increased potassium channel activity in the colon, inducing excessive potassium loss, with increased sensitivity to normal serum aldosterone levels. The recommended therapy is potassium-sparing agents. We present the case of an 85-year-old patient who was admitted at the emergency department with prostration, abdominal distension and diarrhoea, corresponding to functional colonic dilation precipitated by severe hypokalaemia. Resolution of the condition only occurred after spironolactone was administered for suspected primary hyperaldosteronism, which was not proved as the patient showed normal aldosterone serum levels. The pathophysiological mechanism of abnormal potassium secretion in this scenario corresponds to ‘relative hyperaldosteronism’ caused by increased sensitivity of colonocytes to aldosterone

    Parsonage-Turner syndrome, also known as neuralgic amyotrophy, is a rare disorder characterized by painful clinical manifestations mainly involving the upper limbs. This syndrome seems to be triggered, among other factors, by some viral infections, although its pathophysiology remains unclear. Moreover, it has rarely been related to hepatitis E virus infection.     Terlipressin-related Ischaemic Necrosis of the Skin: A Rare Complication

    Terlipressin is used for the treatment of bleeding oesophageal varices and hepatorenal syndrome in patients with cirrhosis. Adverse effects are usually minimal. However, potentially serious side effects such as skin necrosis involving the extremities, scrotum, trunk and abdominal skin can rarely occur. Our patient had greater skin involvement than other described cases. We present the case of a patient with extensive skin necrosis, unrelated to the infusion site, in the lower and upper limbs, scrotum and abdomen after the use of terlipressin. Skin necrosis secondary to terlipressin is a rare complication and early identification is essential so the drug can be immediately suspended

    Systemic lupus erythematosus (SLE) is a chronic multi-systemic immune-mediated disease with confusing symptoms and delayed diagnosis. We report the case of a 32-year-old man with a persistent Venereal Disease Research Laboratory (VDRL)-positive reaction treated for syphilis 5 years previously, who was admitted for rash, weight loss, pancytopenia, inflammatory syndrome, and an important spontaneous prolongation of activated partial thromboplastin time (aPTT). Antiphospholipid antibodies were identified in the patient and he was diagnosed with SLE. The unrecognized false positive VDRL reaction and the delayed diagnosis of SLE were harmful as the patient had developed renal and cardiac complications by the time of diagnosis

    Fluorine-18-fluorodeoxyglucose positron emission tomography/ computed tomography (18FDG-PET/CT) has been used to diagnose vasculitis, tuberculosis and malignancy. Because PET/CT scan show hotspots during an activation of clinically suspected lesions, it is widely used for diagnosis. However, there are rare cases of PET/CT images for vasculitis combined with tuberculosis. Here we report a case of an eosinophilic granulomatosis with polyangiitis in a patient with disseminated non-tuberculosis mycobacterial infection in multiple sites mimicking metastatic malignacy and describe the associated PET/CT scan findings before and after treatment

    Penile Rings: No Innovation without Evaluation

    Penile strangulation is a rare clinical entity which if left untreated can cause serious urogenital problems. We present the case of 45-year-old male patient who presented with penile strangulation after applying a metallic ring. Strangulation of the external male genitalia is a serious problem that requires urgent intervention in order to avoid serious complications. If left untreated, it can result in gangrene and penile amputation. Management of penile strangulation is a real challenge for the treating surgeon and should be treated as an emergency in order to avoid vascular compromise
