De la différence de l’interprétation du meurtre du conjoint dans le système pénitentiaire canadien selon que le coupable est femme ou homme
Cet article porte sur des hommes et des femmes qui ont tué un partenaire intime. Il rappelle que ces personnes qui ont été reconnues comme coupables sont amenées à reconnaître elles-mêmes leur culpabilité, quelles qu’aient été les circonstances dans lesquelles l’homicide a été commis. Il montre que l’explication de l’homicide que la personne coupable est amenée à développer varie selon le genre et que cette différence repose sur une vision largement essentialiste qui veut que les femmes se découvrent comme victimes et les hommes, comme contrôlants. Ce résultat a été établi à partir d’une analyse de discours des audiences des meurtriers devant la Commission des libérations conditionnelles du Canada. Il clima e la disponibilità di acque sotterranee in Puglia: effetti recenti e novità in tema di ricarica controllata
Le acque sotterranee pugliesi costituiscono la risorsa idrica preminente per lo sviluppo socio-economico regionale in virtù della scarsa presenza di risorse superficiali, disponibili solo nel Tavoliere. La penuria idrica, sin da epoca storica, ha fortemente condizionato la vita umana, ad esempio favorendo l'ubicazione di villaggi, oggi prosperose città, soprattutto lontano dalla costa, in luoghi in cui le di acque sotterranee risultavano facilmente estraibili. Nel corso del tempo, soprattutto a causa delle modificazioni climatiche, osservate in particolare dal 1980 in poi in tutta l'Italia meridionale (Cotecchia et al. 2004), si è registrato un importante calo della ricarica che, posto in relazione al crescente utilizzo delle risorse, ha determinato condizioni di sovrasfruttamento (2011)
Alteplase for Massive Pulmonary Embolism after Complicated Pericardiocentesis
Background: The occurrence of a high-risk pulmonary embolism (PE) within 48 hours of a complicated pericardiocentesis to remove a
haemorrhagic pericardial effusion, is an uncommon clinical challenge.
Case summary: The authors report the case of a 75-year-old woman who presented with signs of imminent cardiac tamponade due to recurring
idiopathic pericardial effusion. The patient underwent pericardiocentesis that was complicated by the loss of 1.5 litres of blood. Within 48
hours, the patient had collapsed with clear signs of obstructive shock. This was a life-threating situation so alteplase was administered after
cardiac tamponade and hypertensive pneumothorax had been excluded. CT chest angiography later confirmed bilateral PE. The patient
achieved haemodynamic stability less than an hour after receiving the alteplase. However, due to the high risk of bleeding, the medical team
suspended the thrombolysis protocol and switched to unfractionated heparin within the hour. The cause of the PE was not identified despite
extensive study, but after 1 year of follow-up the patient remained asymptomatic.
Discussion: Despite the presence of a contraindication, the use of thrombolytic therapy in obstructive shock after exclusion of hypertensive
pneumothorax can be life-saving, and low-dose thrombolytic therapy may be a valid option in such cases
Detecting Pneumoperitoneum via Point-of-Care Abdominal Ultrasound: To See Beyond Touch
Point-of-care ultrasound is increasingly important in the management of acute medical emergencies. An elderly man was brought to the emergency department after 2 days of fever and urinary retention. He was drowsy and had peri-arrest arrhythmia. He was hypoperfused peripherally with a systolic blood pressure of 45 mmHg and so was managed as for septic shock with no obvious aetiology. Chest and abdominal physical examinations were unremarkable. The source of sepsis was unclear. A point-of-care abdominal ultrasound was performed by the reviewing internist which detected a pneumoperitoneum, leading to a change in diagnosis to a perforated viscus which was confirmed later by a CT of the abdomen and pelvis
A Rare Case of Primary Pulmonary Anaplastic Large Cell Lymphoma
Non-Hodgkin lymphomas are rare causes of primary lung neoplasms and most are B-cell in origin. Anaplastic large cell lymphoma is an exceedingly rare type of primary pulmonary lymphoma, with an aggressive clinical course.
We present the case of an 85-year old male patient who attended our Emergency Department complaining of respiratory and constitutional symptoms, and who was found to have a bronchial mass causing subtotal atelectasis of the left lung. Histological examination showed an anaplastic large cell lymphoma and further investigation revealed that it was limited to the lung. To our knowledge, very few similar cases have been reported in the literature
Linagliptin-Associated Alopecia and Bullous Pemphigoid
Bullous pemphigoid is a chronic autoimmune blistering disease. Recently, several reports suggested dipeptidyl peptidase 4 (DPP-4) inhibitors,
also known as gliptins, were a potential cause of drug-induced bullous pemphigoid but not of both bullous pemphigoid and alopecia areata
together. Here we describe the case of a 68-year-old man with type 2 diabetes mellitus who developed new onset diffuse alopecia on the
scalp with diffuse tense bullae over his body a few months after linagliptin was introduced for better control of his diabetes.
DPP-4 inhibitors are not known to increase the risk of alopecia. To the best of our knowledge, this is the first report of linagliptin-associated
alopecia areata and bullous pemphigoid, which may help demonstrate if there are any links between DPP-4 inhibitors and alopecia
Abrupt Severe Chest Pain and Vomiting: Remember to Think of a Ruptured Oesophagus (Boerhaave Syndrome)
Boerhaave syndrome or spontaneous rupture of the oesophagus is a severe condition commonly misdiagnosed or unrecognized. Prognosis is poor even if the diagnosis is made promptly. We describe a case of Boerhaave syndrome diagnosed after the development of pneumomediastinum and cardiac arrest. Unfortunately, the patient died 48 hours after admission to the Intensive Care Unit. This entity requires a multidisciplinary management approach which may include conservative, surgical or endoscopic procedures

Water-pipe Smoking as a Risk Factor for Transmitting Mycobacterium tuberculosis
A 20-year-old Swiss male presented at the emergency department with acute onset of febrile temperatures and hemoptysis and a 3-month history of productive cough. An X-ray and CT scan of the chest, sputum samples for acid-fast bacilli, polymerase chain reaction(PCR), and cultures for Mycobacteria revealed pulmonary infection with Mycobacterium tuberculosis. None of the classical risk factors for tuberculosis were present, but the patient reported regularly smoking a water pipe. Water-pipe smoking poses a serious risk of M. tuberculosis transmission
Towards a standardization of medits benthos processing: some proposal for a common protocol
The Technical Report summarizes experiences gathered, since 2004, in sampling and processing procedures developed at CNR of Mazara del Vallo Laboratory for benthos studies including MEDITS program. Some critical issues faced during sampling and processing are presented. The work could contribute to the discussion for achieving agreed procedure of a standardized protocol for analyses on benthic organisms
