Consequently, COVID-19 patients with cardiovascular disease seem to be at higher risk of adverse events. Mondor’s disease is a rare, generally self-limiting, thrombosis of the penis. The pathogenesis of Mondor’s disease is unknown, and it is usually diagnosed through clinical signs and with Doppler ultrasound evaluation.     Thyroid Storm Associated with Testicular Choriocarcinoma

    Thyroid storm is a rare, life-threatening condition that can be caused by various pathologies including serious conditions associated with human chorionic gonadotropin (hCG)-producing tumours. We present the case of a 17-year-old male patient with a 1-month history of dyspnoea and a 3-day history of palpitations and fever. General examination revealed a left testicular mass. Blood tests revealed β-hCG >225,000 mIU/ml, thyroid-stimulating hormone (TSH) 0.02 IU/ml, and thyroxine (T4) 19.07 μg/dl. He was admitted with thyroid storm. Treatment with antithyroid drugs and chemotherapy was started, but the patient died on the third day of admission.This case highlights a rare occurrence of thyroid storm linked to testicular choriocarcinoma, and provides a necessary reminder that, in some instances, hCG at very high levels can exert a thyrotropic effect due to its molecular structure, which is similar to that of TSH

    Masquerading Hypervascular Exophytic Liver Nodule

    Patients with liver cirrhosis are at increased risk of developing hepatocellular carcinoma (HCC) and are placed on routine surveillance for HCC. Diagnosis algorithms are in place to guide clinicians in the evaluation of liver lesions detected during surveillance. Radiological assessments are critical with diagnostic criteria based on identification of typical hallmarks of HCCs on multiphasic computed tomography (CT) and dynamic contrast-enhanced magnetic resonance imaging (MRI). We report a patient with a hypervascular exophytic lesion indeterminate for HCC on CT imaging. While the detection of an exophytic arterially-enhancing lesion in an at-risk patient on CT imaging may prompt clinicians to treat the lesion as HCC without further evaluation, the patient underwent contrast-enhanced MRI with the lesion being eventually diagnosed as an exophytic haemangioma. Thus, no further action was necessary and the patient was continued on routine HCC surveillance

Infatti, per la valutazione di una testimonianza occorre esaminare, da un lato, la narrazione di un fatto, e, da un altro lato, l’attendibilità del testimone, che può essere influenzata, in vario modo, dalle dinamiche dell’attività psichica in rapporto alla percezione ed al ricordo dei fatti, e dalle sue alterazioni a causa dell’età, del sesso, delle emozioni e passioni, del temperamento, delle illusioni e allucinazioni, delle involuzioni e malattie mentali specialmente in relazione all’attenzione e alla memoria. Al fine di evitare l’inquinamento del ricordo è fondamentale utilizzare determinate tecniche di intervista. Il presente lavoro si propone di illustrare la metodologia denominata “intervista cognitiva”, che è stata elaborata con il preciso scopo di ridurre al minimo il rischio di suggestione

    Dare una definizione di tipo psicopatologico di soggetti con comportamenti sessuali devianti non ha trovato terreno fertile e fondamenta salde e pertanto questi comportamenti vengono classificati come inusuali e in contrasto con la morale collettiva; viene anche stilata una classifica in base al livello di pericolosità nei confronti della vittima oltre che del soggetto stesso. In cima a tale lista troviamo la pedofilia ed il sadismo e a seguire tutte quelle scelte sessuali che prevedono fantasie e atti di esibizionismo, travestimento, voyeurismo o masochismo. Pur essendo conosciute e ritenute conseguenza di un disturbo di natura psichica, non sono annoverate all’interno dell’ambito del sistema medico-scientifico ma sono principalmente trattate in sistemi quasi esclusivamente di tipo giuridico. Il presente contributo intende pertanto analizzare il tema sia da un punto di vista descrittivo quanto da una prospettiva più scientifico-trattamentale

To study this problem, we have derived a cylindrical (or concentric) Korteweg-deVries equation (cKdVE) for an incompressible, inviscid, and irrotational fluid.     The role of geological origin of smectites and of their physico-chemical properties on aflatoxin adsorption

    Since 2013, bentonite in the form of dioctahedral smectite is an additive authorised in the EU as a substance for the reduction of the contamination of feed by aflatoxins. Several studies indicate a big difference in the effectiveness of smectites in sequestering aflatoxins. A clear correlation between mineralogical and physico-chemical properties of smectites and aflatoxin adsorption has not been well established. In the effort to identify the most critical mineralogical, chemical, and physical properties that affect aflatoxin adsorption by smectites, 29 samples of bentonites obtained from different sources around the world were evaluated. “As received” samples were divided into two main groups, i.e. hydrothermal (n=14) and sedimentary (n=15) bentonites depending on their geological origin. The characterization studies showed that all samples contained dioctahedral smectite as major mineral; a moderate CEC value (60-116 cmol/kg); the presence of iron; a small organic matter content; a near-neutral pH; and a fine and uniform particle size (<45μm). They differed substantially in their sodium, calcium and magnesium contents, and in the swelling properties depending on the geological origin. Several in vitro adsorption studies showed that they also differed in a significant manner in adsorbing aflatoxin B1 (AFB1). A correlation between geological origin and AFB1 adsorption capacity was found (p<0.001), being sedimentary smectites significantly more effective than hydrothermal ones in adsorbing the toxin at different pH values. The extent of AFB1 adsorption by all samples was negatively and linearly correlated to the extent of desorption, and sedimentary smectites were significantly more effective than hydrothermal smectites in keeping bound the adsorbed fraction of the toxin (p < 0.001). In addition, correlation studies using the Pearson statistical method showed a significant relationship among some physico-chemical properties of smectites and the amounts of adsorbed toxin. In particular, AFB1 adsorption by smectites correlated positively with sodium content and swell index, but negatively with d001-value, magnesium and calcium contents. In conclusion, it seems that the geological origin of smectite is a useful guide for the selection of bentonites for AFB1 detoxification. Sedimentary bentonites containing sodium/swelling-smectite should be preferred to hydrothermal samples as potential aflatoxin binders. Taking into account the geographical origin of our samples, this approach should be applicable to bentonites worldwide
