Isolated Non-Resolving Vulvar Lesion as a Presentation of Disseminated Histoplasmosis in a Woman with HIV
Disseminated histoplasmosis is most commonly caused by Histoplasma capsulatum and is a known opportunistic infection in immunocompromised patients. The clinical presentation of histoplasmosis varies from asymptomatic to a progressive disseminated form. Pulmonary and CNS involvement is common in disseminated histoplasmosis. Rarely, disseminated disease can present as vulvar lesion in the absence of pulmonary symptoms, causing a delay in diagnosis. As per our PubMed literature search, there have only been two reported cases where vulvar lesion was the only presenting symptom of disseminated disease. In our patient, a histopathological diagnosis was made with staining showing budding yeast forms of histoplasma
Natural Groundwater Background Levels of Nitrate and Landfill Effects (Apulia, Southern Italy)
High nitrate concentration of fresh groundwater of a coastal karstic aquifer, not far from the Apulian regional capital (Southern Italy), caused public concern and alarm from the authorities. The attention of local communities focused on the effect of a group of landfills, the use of which started from 1975, using improved technological and safety devices to reduce groundwater pollution risks
The cylinder seal as a challenge for quantitative investigation, electronic cataloguing and digital visualization
In ancient Western Asiatic studies glyptic has been historically the preferred subject for quantitative experiments and investigations. In most cases this led to stimulating and complex challenges which deserve to be critically discussed and analysed in order to find a proper use in the field of recently developed technologies and models. Cylinder seals in particular compel the scholars to face the close connection between the development of an optimal representation of the artefacts in primary publications and the building of strategies for their quantitative investigation. A synthesis of past experiences and present issues is presented here
Pyogenic Ventriculitis Associated with Urosepsis: A Rare Case Report
Pyogenic ventriculitis is an infection of the cerebral ventricles usually associated with neurosurgery or head trauma. There are less than 10 reported cases of community-acquired pyogenic ventriculitis in adults. We present the case of a 71-year-old man with a medical history of type 2 diabetes mellitus, hospitalized due to urosepsis caused by E. coli. Because he had a fluctuant level of consciousness, he underwent magnetic resonance imaging that diagnosed pyogenic ventriculitis. He was treated with ceftriaxone 2 g 12/12h for a total of 6 weeks and recovered without neurological deficits. Pyogenic ventriculitis is a clinical challenge due to its rarity, atypical presentation and variety of aetiological microorganisms
Inferior Vena Cava Syndrome: A Rare Complication
Inferior vena cava syndrome is rare and often difficult to diagnose because of its rarity and consequent low suspicion. We describe the case of a 28-year-old female patient with a history of nephroblastoma of the right kidney, stage IV, with a favourable histology with epidural metastasis (D5-D9), diagnosed at 3 years of age. The patient underwent treatment with surgery, chemotherapy and radiotherapy. The patient suffered from sudden low back pain worsening over 2 weeks, with progressive inability to walk. The pain radiated to the front of the thighs. Concomitantly, oedema of the lower limbs with cephalocaudal progression was observed. At admission to our institution, the physical examination showed peripheral oedema, abdominal wall venous collaterals, an inability to walk due to low back pain in the supine position, with no neurological deficits. Lumbar MRI showed exuberant epidural venous congestion. The hypothesis of inferior vena cava thrombosis (IVCT) was considered and confirmed by angio-CT.IVCT is prevalent in patients with congenital anomalies of the inferior vena cava, occurring in approximately 60–80% of these cases, and most published series on inferior vena cava syndrome refer to thrombotic complications in this subgroup of patients. There are currently no guidelines defined or validated to guide the diagnosis and approach to IVCT. With this case, we would like to draw attention to a rare disease that should be suspected in all patients with inferior vena cava disease, whether resulting from congenital disease or after surgical procedures
Propofol-Induced Hepatitis
Objectives: To present a rare case of propofol-induced hepatitis.Materials and methods: A 59-year old man was referred to our department because of suspicion of toxic hepatitis after propofol anaesthesia for endoscopic colonoscopy. Results: The patient had jaundice, increased transaminases demonstrating liver necrosis, and liver stiffness of 18 kPa. Liver biopsy revealed bridging necrosis and initial post-collapse fibrosis. Following therapy with steroids and N-acetyl cysteine, the patient was discharged on the seventh day after admission in good general condition. Conclusion: Although propofol is considered safe, it can cause acute hepatitis, the seventh published case of which is reported here. Importantly, treatment with N-acetyl cysteine, a radical scavenger, but especially with steroids resulted in hepatic improvement
Electrochemical sensing of glucose by chitosan modified graphene oxide
Graphene oxide (GO) coated electrodes provide an excellent platform for enzymatic glucose sensing,
induced by the presence of glucose oxidase and an electrochemical transduction. Here, we show that
the sensitivity ofGOlayers for glucose detection redoubles upon blendingGOwith chitosan (GO
+Ch) and increases up to eight times if covalent binding of chitosan toGO(GO−Ch) is exploited. In
addition, the conductivity of the composite materialGO−Ch is suitable for electrochemical
applications without the need ofGOreduction, which is generally required forGObased coatings.
Covalent modification ofGOis achieved by a standard carboxylic activation/amidation approach by
exploiting the abundant amino pendants of chitosan. Successful functionalization is proved by
comparison with an ad-hoc synthesized control sample realized by using non-activatedGOas
precursor. The compositeGO−Ch was deposited on standard screen-printed electrodes by a dropcasting
approach. Comparison with a chitosan-GO blend and with pristineGOdemonstrated the
superior reliability and efficiency of the electrochemical response for glucose as a consequence of the
high number of enzyme binding sites and of the partial reduction ofGOduring the carboxylic
activation synthetic step
