Exploring on factors that influence mothers to choose epidural analgesia during the labor process
This study aims to identify factors influencing pregnant women in choosing epidural analgesia during labor. Statistics from Raja Permaisuri Bainun Hospital (HRPB), Ipoh, revealed a rise in epidural usage from 2019 to 2021, indicating a growing demand and underlying factors for it. Purpose: To determine demographic characteristics and external factors influencing mothers’ choice of epidural analgesia during labor. Methodology: This quantitative, descriptive cross-sectional study involved 30 respondents, calculated using Epical 2000 software for a 95% confidence interval. Inclusion criteria encompassed primigravida or multigravida mothers with spontaneous vaginal delivery. A questionnaire was adapted from Harkins et al. (2006) which consists of 10 questions for part A and 15 questions for part B to gather research data. Result: The largest group was aged 23-27 years (33.3%), followed by 28- 32 years (26.7%), 33-37 years (20.0%), 38-42 years (13.3%), and 18-22 years (6.7%). The mean age was 29.63 years with a standard deviation of 5.568 years, indicating a predominantly young population with some age variability. Most of the respondents’ information came from medical experts, and the main motivation for selecting an epidural was to reduce and control labor discomfort. Conclusion: This study provides essential baseline data for health education on pain management during labor. It enhances nurses’ understanding of factors influencing women’s preference for epidural analgesia, facilitating improved patient care
Pendekatan Fiqh Sabahi dalam memperkukuh kesepaduan komuniti majmuk di Sabah: Satu tinjauan awal
Kajian ini bertujuan meninjau peranan pendekatan Fiqh Sabahi dalam memperkukuh kesepaduan komuniti majmuk di Sabah. Sabah merupakan negeri yang terkenal dengan kepelbagaian etnik, budaya dan agama, menjadikan aplikasi fiqh lokal sebagai satu keperluan penting dalam usaha membangun keharmonian sosial. Fiqh Sabahi merujuk kepada pendekatan fiqh yang beradaptasi dengan realiti setempat berdasarkan prinsip keadilan, toleransi, kepedulian terhadap muallaf dan keperluan dakwah yang hikmah. Tujuan utama penulisan ini ialah untuk menganalisis bagaimana prinsip-prinsip Fiqh Sabahi diaplikasikan dalam konteks kehidupan masyarakat majmuk di Sabah, serta menilai sejauh mana prinsip-prinsip tersebut menyumbang kepada pengukuhan hubungan sosial antara Muslim, muallaf, dan bukan Muslim. Kajian ini juga bertujuan mencadangkan strategi pengukuhan kesepaduan berasaskan nilai-nilai fiqh yang sesuai dengan latar budaya setempat. Kajian menggunakan pendekatan kualitatif melalui tinjauan awal berbentuk temubual separa berstruktur dengan pemimpin komuniti, asatizah, dan individu berpengaruh di beberapa daerah terpilih di Sabah. Hasil awal menunjukkan bahawa prinsip Fiqh Sabahi seperti al-taysir (memudahkan), al-ta'ayush (hidup bersama secara harmoni), dan al-‘adalah (keadilan sosial) banyak diamalkan dalam hubungan masyarakat setempat. Dapatan kajian ini dijangka memberi manfaat kepada agensi dakwah, institusi pendidikan Islam, dan pembuat dasar dalam merangka program pembangunan komuniti majmuk yang lebih berkesan. Kajian ini turut membuka ruang kepada penyelidikan lanjutan mengenai potensi fiqh lokal dalam memperkukuh keharmonian nasional secara lebih luas
Exploring the use of e-Portfolio in a teacher education programmed from the lenses of university lecturers
Technology has become an essential part of education by enhancing the process of teaching, learning and assessment. Students are expected to be conversant with the use of technology in school, as such teachers are also expected to demonstrate competency in technology usage. In line with the prevalent use of technology in education, this study introduced the use of e-Portfolio in a teacher education programmed at a public university in Malaysia, focusing on the practicum phase. The e-Portfolio was named PRO folio (short for professional e-Portfolio) and was employed by undergraduates and lecturers from the Teaching English as a Second Language (TESL) programmed. The undergraduates were also pre-service teachers, and their role was to upload their practicum materials onto their individual PRO folio. On the other hand, the lecturers provided feedback regarding the work that the undergraduates posted. After the completion of practicum, qualitative data collection was collected through semi-structured interviews that involved a total of three lecturers who volunteered for the interview. Purposive sampling was employed by selecting only the TESL lecturers who supervised the undergraduates and used the PRO folio. The data were imported to NVivo 12, and thematic analysis was employed to analyses the data. The findings revealed that the use of PRO folio contributed to an enhanced practicum experience for the lecturers and pre-service teachers. The study demonstrated that the use of technology in teacher education programmed can contribute positively to the learning experiences of pre-service teachers
Policy gaps in school safety and student protection: A comparative case study of recent school violence in Malaysia
This study explores policy gaps in school safety and student protection through a comparative case study of recent school violence incidents in Malaysia. Using qualitative document analysis, data were gathered from credible online news reports, government policy documents, and institutional publications to assess how existing frameworks address issues of bullying, assault, and student welfare. The findings reveal inconsistencies between policy intent and implementation, particularly in areas of enforcement, accountability, and preventive education. The study highlights the urgent need for a standardized national safety audit and stronger policy mechanisms to ensure that Malaysian schools provide safe learning environments free from violence and neglect
Dari cerita rakyat kepada kepimpinan: Menginspirasikan kanak-kanak melalui Tavantang, hati berani Borneo
Kajian ini mengkaji Tavantang, The Brave Heart of Borneo sebagai cerminan nilai budaya, kepahlawanan, dan jati diri orang asli Borneo sambil menekankan kuasanya untuk memberi inspirasi kepada kanak-kanak untuk menjadi pemimpin masa depan dan penyumbang kepada masyarakat. Melalui analisis tekstual kualitatif yang disokong oleh sumber etnografi dan sastera yang relevan, penyelidikan meneroka bagaimana naratif menyampaikan tema keberanian, persaudaraan, kepimpinan, dan keharmonian dengan alam semula jadi. Penemuan ini menyerlahkan Tavantang sebagai bukan sahaja kisah keberanian individu tetapi juga representasi simbolik ketahanan komunal, integriti, dan penghormatan terhadap tradisi nenek moyang. Dengan meletakkan cerita dalam rangka kerja cerita rakyat Borneo yang lebih luas, kajian ini menekankan kepentingannya dalam memelihara warisan tempatan, memupuk nilai kepimpinan, dan memupuk semangat inspirasi dalam kalangan kanak-kanak untuk memimpin komuniti mereka dengan belas kasihan dan keberanian
Building the maritime paradigm of Sabah, Malaysia using the ecosystem-based management (EBM) model a premilitary study
This study aims to analyze the effectiveness of the Ecosystem-Based Management (EBM) model in developing the maritime paradigm in Sabah, in line with the national blue economy objectives that emphasize the sustainability of marine resources. Sabah, rich in marine natural resources, offers great potential in sectors such as fisheries, marine tourism, marine biotechnology, and marine energy. The main research question is how applicable the EBM model is in Sabah for strengthening marine resource management, improving synergy among various stakeholders, and driving the sustainable development of the maritime sector. The objectives of the study are to explore the potential of EBM in the context of Sabah's maritime sector, identify the challenges faced, and propose strategic steps to overcome these challenges. The study adopts a qualitative approach based on literature analysis and case studies from countries that have successfully applied the EBM model in managing their marine resources. The findings indicate that while the EBM model has significant potential in enhancing the sustainability of marine ecosystems, several challenges such as institutional capacity limitations, conflicts between stakeholders, and a lack of coordination between the public and private sectors need to be addressed for its effectiveness. This study suggests strengthening more holistic policies and regulations, enhancing cooperation between the government, private sector, local communities, and NGOs, as well as investing in technology and research to support effective EBM implementation. Overall, the study provides guidance for developing a more sustainable maritime paradigm in Sabah through the broader application of the EBM model. The way forward recommends a more inclusive approach that focuses on ecosystem conservation, while still encouraging high quality and competitive economic development
Ethnolinguistic pluralism and multiculturalism in contemporary Indonesia: A case study of minority Javanese Muslim migrants in Bali
This article examines how majority-minority relations in Bali, Indonesia, have changed in the last decade, in the context of multiculturalism and multilingualism. It does so through a case study of the socio-cultural practices of minority Muslim migrants in a predominantly Hindu community in the village of Sumerta, which is part of Bali’s capital city of Denpasar. The study shows fresh evidence for the dynamic nature, issues and challenges of interethnic/religious relations at the most local level, a matter of great significance in debates on cultural pluralism, multilingualism, tolerance and modern democracy on the broader national and global levels. Our discussion and analysis make use of methodological and theoretical insights from sociotechnical linguistics, in combination with history and cultural studies. We argue that the cultural-religious and social practices of the different ethnolinguistic groups in this village are based on culturally plural community interactions, where the different ethnic and religious groups remain separate. Intergroup relations, however, show positive forms of cultural and linguistic engagement, primarily symbolic in nature, and thus tend towards multilingualism as an indicator of multiculturalism
A bibliometric analysis of work–family balance: Trends, themes, and future directions (2000–2024)
Despite the growing importance of work-family balance (WFB), the field still lacks large-scale, quantitative, and replicable review studies. To fill this gap, this paper adopts a bibliometric approach to analyze 921 relevant articles published between 2000 and 2024 from the Web of Science database. Utilizing tools such as bibliometric (R package), VOS viewer, and Cite Space, the study maps the academic output and knowledge structure of the WFB field. It aims to review existing research, define the foundational knowledge framework, trace frontier trends, and lay a foundation for future studies. Results reveal a significant increase in WFB-related publications in recent years. A stable network of core authors and journals has emerged, alongside international collaboration. However, the overall influence of the field has declined over the past decade, and breakthrough research remains scarce. Co-citation analysis identifies three major knowledge clusters: work-family conflict (WFC), work-family enrichment, and supportive culture. The journal co-citation network highlights the interdisciplinary potential of WFB research. Keyword co-occurrence analysis, evolution mapping, and factor analysis indicate hotspots include cross-cultural comparisons, industry strategies, gender issues, supportive culture, and outcome variables such as job satisfaction and organizational commitment. These findings help build a theoretical framework and offer insights for policymakers and business leaders
Inhibition of junctional protein disruption by 2,4,6-trihydroxy-3- geranyl acetophenone in lipopolysaccharide-induced endothelial hyperpermeability via GEF-H1/RhoA/ROCK pathway
2,4,6-Trihydroxy-3-geranyl acetophenone (tHGA) is a bioactive phloroglucinol compound found in the leaves of Melicope pteleifolia (Champ. ex Benth.) T.G.Hartley. Our previous study has proven that tHGA exhibited significant in vitro barrier protective effects against lipopolysaccharide (LPS) induction, mainly by inhibiting endothelial hyperpermeability via attenuation of F-actin cytoskeletal rearrangement, as F-actin cytoskeleton is anchored to junctional proteins such as zonula occluden (ZO)-1, occludin, and vascular endothelial-cadherin (VE-cadherin), and they play collaborative roles in the preservation of endothelial integrity. Therefore, the effects of tHGA on these junctional proteins were further investigated, followed by the dissection of signalling pathways mediated by tHGA in suppressing LPS-induced junctional protein disruption during endothelial hyperpermeability. HUVECs were pretreated with tHGA prior to LPS induction. TEER, immunofluorescence staining, Western Blotting, and RT-qPCR were performed to examine the effects of tHGA on junctional proteins in terms of their integrity, localization, protein expression, and gene expression, respectively. Proinflammatory signalling molecules including MLC, NF-κB p65, p38 MAPK, ERK MAPK, and JNK MAPK were assessed to unravel the underlying signalling pathways, followed by molecular docking on human ROCK1 to predict the molecular target of tHGA. tHGA profoundly preserved junctional integrity by inhibiting both delocalization and downregulation of ZO-1, occludin, and VE-cadherin, via inactivation of MLC, NF-κB p65, p38 MAPK, and ERK MAPK, which are mainly diverged from GEF-H1/RhoA/ROCK pathway. ROCK1 was predicted as the molecular target of tHGA. tHGA should be developed as a potential therapeutic remedy for prevention and/or treatment of permeability-related disorders
Pan-immune-inf lammation value and systemic immune-inf lammation index predict the clinical efficacy of biological agents in Crohn’s disease
Background It remains unclear whether pan-immune-inflammation value (PIV) and systemic immune-inflammation index (SII) can predict treatment outcomes in Crohn’s disease (CD) patients receiving infliximab. This study explored the role of PIV and SII in predicting treatment response and prognosis in CD patients receiving biologics. Methods Clinical data of 160 patients with CD were retrospectively collected from Anqing First People’s Hospital in China. Patients were classified into effective and ineffective groups based on therapeutic response. Additionally, 60 healthy controls were enrolled. Serum levels of PIV and SII were measured in all participants. The effectiveness of infliximab was evaluated based on Crohn’s Disease Activity Index. Clinical remission was defined as a Crohn’s Disease Activity Index score <150, while a reduction of ≥70 points from baseline indicated clinical response. Results The effective group included 125 patients (mean age, 34.5 ± 10.2 years; 68 men [54.4%]), and the ineffective group included 35 patients (mean age, 36.1 ± 9.8 years; 18 men [51.4%]). The healthy control group covered 60 individuals (mean age, 33.8 ± 11.1 years; 32 men [53.3%]). Patients in the high SII group demonstrated a markedly elevated hazard ratio (HR = 20.786, P < .001). A higher PIV was associated with better prognosis (HR = 0.386, 95% confidence interval: 0.228–1.655, P = .011). Conversely, a higher SII was associated with worse prognosis (HR = 20.786, 95% confidence interval: 9.412–45.902, P < .001). Conclusions Both PIV and the SII are useful in predicting efficacy of CD patients treated with biologics. Compared with SII, PIV is a better choice. Key messages What is already known on this topic: Infliximab is the most efficacious and widely used biologic for inducing remission in moderate-to-severe Crohn’s disease. SII and PIV have been validated as prognostic indicators in many diseases. What this study adds: Higher PIV and SII both have positive effects on predicting the prognosis of patients with Crohn’s disease. Compared with the SII, the PIV holds greater significance in predicting the clinical outcomes for patients with Crohn’s disease. How this study might affect research, practice or policy: PIV and SII may aid in risk stratification and individualized treatment planning in Crohn’s disease
