    Severe Presentation of Cannabinoid Hyperemesis Syndrome With Mixed Acid–Base Disorder and Cardiac Complications: A Case Report

    Background: Cannabinoid hyperemesis syndrome (CHS) is a condition characterized by cyclic abdominal pain, vomiting, and nausea, primarily affecting adolescents and adults with a history of chronic cannabis use. The diagnosis of CHS is clinical, with symptom resolution upon cannabis cessation considered pathognomonic. The overlap of CHS symptoms with other conditions complicates the differential diagnosis, particularly in emergency settings.Case Presentation: We report an unusual case of a 28-year-old man admitted to the Emergency Department of Rovereto (Italy) with limb paresthesia and agitation. Initial evaluation revealed indirect clinical signs of hypocalcemia, and QTc prolongation and severe hypokalemia on electrocardiogram. The arterial blood gas analysis suggested mixed acid-base disturbances. His symptoms improved with aggressive electrolyte correction, benzodiazepine administration, magnesium sulfate administration, and fluid resuscitation. Given the significant risk of arrhythmias, antiemetics known to prolong QTc, such as dopamine antagonists, were contraindicated, and midazolam was used as an alternative for symptom control (both nausea and agitation).Conclusion: This case underscores the importance of recognizing CHS as a potential etiology in patients with recurrent vomiting and a history of chronic cannabis use, even in the presence of atypical findings such as profound electrolyte imbalances and cardiac abnormalities. Given the protracted recovery period associated with CHS, early identification and patient education regarding cannabis cessation are crucial in preventing recurrent episodes

    Long-Term Changes in Parameters of Bone Quality in Kidney Transplant Recipients Treated with Denosumab

    Kidney transplant recipients (KTRs) have an elevated fracture risk. While dual-energy X-ray absorptiometry (DXA) is commonly used to assess areal bone mineral density (aBMD), it does not capture all aspects of bone quality. We investigated the long-term effects on bone DXA-derived indices of bone quality in KTRs treated with denosumab and untreated with denosumab. This is a retrospective study, including KTRs treated with denosumab and untreated age and sex-matched KTR controls. DXA-derived parameters, including trabecular bone score (TBS) and 3D-DXA parameters, were measured at the lumbar spine and femur at baseline and after four years. Hierarchical linear models were used to assess the between-group effect of treatment over time, also adjusting for site-specific aBMDs. We enrolled 23 KTRs treated with denosumab and 23 KTR denosumab-untreated KTRs. Significant between-group differences over time in favor of the denosumab group were observed for TBS (0.843, 95%CI 0.439; 1.248,p < 0.001), trabecular volumetric BMD at the total hip (Tb.vBMD TH) (13.492, 95%CI 1.707; 25.278, p = 0.003), cortical volumetric BMD at the femoral neck (Ct.vBMD FN) (28.766, 95%CI 8.373; 49.158, p = 0.008), cortical surface BMD at the total hip (c.sBMD TH) (10.507, 95%CI 4.140; 16.873,p = 0.002), cortical surface at the femoral neck (c.sBMD FN) (8.795, 95%CI 2.818; 14.771, p = 0.006), and cortical thickness at the total hip (Ct.th.TH) (0.075, 95%CI 0.020; 0.130, p = 0.010). After adjusting for BMD, the differences on TBS and Ct.vBMD FN and c.sBMD FN remained significant. Denosumab treatment in KTRs was associated with better outcomes in terms of bone quality and geometry parameters, independent of changes in aBMD

    Resilient outcomes in people with a history of mental disorder during the COVID-19 pandemic: an international 2-years longitudinal prospective study

    Background: During the COVID-19 pandemic, the global population was exposed to a significant psychological stress, which had an impact on the mental health in individuals with pre-existing mental disorders. Despite their heightened vulnerability, subgroups within this population demonstrated resilient outcomes throughout the pandemic. This study aims to identify predictors of long-term, sustained resilient outcomes among people with a history of mental disorder during the first two years of the pandemic. Methods: In this international 2-year, 5-wave longitudinal online survey, the Patient Health Questionnaire-9, the Generalized Anxiety Disorder Scale, and the PTSD Checklist DSM-5 were used for a proxy measure of psychological distress. As possible predictors of sustained resilient outcomes, we investigated socio-demographic characteristics, economic and housing status, pandemic-related issues, chronic diseases, social support, fear of contamination and personal values which were investigated respectively through the Oslo Social Support Scale, the Padua Inventory, and the Portrait Values Questionnaire. Data were analysed with a Mover-Stayer Latent Transition Analysis model. Results: Nine-hundred and forty-three participants with a mental disorder were included in the analysis. Variables associated with a higher chance of sustained resilient outcomes were older age, maintaining a job, and having more people in the household. In contrast, female gender, losing job, difficulty in meeting basic needs, higher fear of contamination, hedonism, less social support and loneliness resulted in a lower likelihood of presenting sustained resilient outcomes. Conclusion: This study identified factors that predictsustained resilience in people with mental disorders. The newly discovered predictors could prove invaluable in developing strategies to enhance the resilience of people with mental disorders during times of crises, such as pandemics

    Genetic characterizations in grapevine: phenology regulation and ancient varieties

    Grapevine (Vitis vinifera spp. vinifera) is an important crop grown worldwide, playing a key role in the economic and historical context of the Verona region. Like many other crops, grapevine is vulnerable to the effects of climate change, particularly rising temperatures, which can accelerate phenological stages and negatively impact the quality of the final product. Consequently, understanding the genetic factors influencing key phenological stages, such as flowering, veraison, and the interval between these events, is critical for developing grapevine cultivars that are better suited to a changing climate. The first aim of this thesis was to identify promising genomic regions involved in the phenology regulations in order to point out interesting candidate genes for more deeper analysis. We conducted a Genome-Wide Association (GWA) analysis to identify Single Nucleotide Polymorphisms (SNPs) significantly associated with flowering time, veraison time, and the interval between these two stages. We characterized a wide germplasm collection of grapevine accessions, from the CREA-VE in Susegana, Treviso (Italy) and constructed a genotypic dataset from new and published data including 6.679 SNPs for 365 unique genotypes. Over a period of 11 years, all cultivars were phenotyped for flowering and veraison times. The distribution of these traits was examined and corrected if necessary. The flowering-veraison interval was derived and analyzed similarly. We conducted GWA analysis using two software, TASSEL 5.0 and GAPIT 3, applying different statistical model, that allowed to identify a total of 228 SNPs associated to the three phenological traits. Twelve SNPs resulted consistently associated to the phenological traits in the analysis conducted with both software. As conclusion, we highlighted genomic regions including putative candidate genes and functional annotation and Geno Ontology (GO)were considered to point out those more interesting for further analysis. The second aim of this thesis was to explore the historical context of viticulture in Verona area by establishing the genetic characterization of archeological grapevine’s seeds excavated from four archeological sites and four different period (from 20th century BC to 13th century CE). To this aim we have set up a pipeline on the archeological remains starting with the optimization of an ancient DNA extraction protocol from grapevine seeds and libraries preparation for NGS sequencing. Once optimized the pipeline was applied on different remains from the four different archeological sites obtaining 37 libraries suitable for sequencing. We conducted bioinformatic analysis of sequencing data in order to validate the authenticity of aDNA and the presence of endogenous DNA. In this way we were able to point out samples more suitable for target enrichment sequencing, in order to retrieve SNPs profiles from ancient varieties to be compared to modern ones

    Toric Aberrometric Extended Depth of Focus Intraocular Lens: Visual Outcomes, Rotational Stability, Patients' Satisfaction, and Spectacle Independence

    : Objective: To evaluate visual outcomes, rotational stability, patients' satisfaction, and spectacle independence after bilateral Toric extended depth of focus intraocular lens (EDOF IOL) implantation. Methods: Prospective observational study including cataract patients with bilateral corneal astigmatism between 0.75 and 3.00 D implanted with Toric EDOF IOLs. After three months distance corrected and uncorrected visual acuity at 4 m (DCVA and UDVA), 80 cm (DCI80VA and UI80VA), 67 cm (DCI67VA and UI67VA), and 40 cm (DCNVA and UNVA), IOL stability by Toric IOL Assistant tool (Osiris T, CSO, Florence, Italy), binocular defocus curves, contrast sensitivity (CS), halometry, reading performance, and subjective and objective (Root mean square-RMS, modulation transfer function-MTF, cut-off and point-spread-function-PSF-Strehl ratio) visual quality were evaluated. Results: Forty eyes from 20 astigmatic patients were enrolled. Mean refractive spherical equivalent and residual cylinder were -0.21 ± 0.74 D and 0.29 ± 0.31 D, respectively. No patients needed additional surgery due to IOL rotation. Binocular UDVA, UI80VA, UI67VA, and UNVA ≤ 0.2 logMAR was found in 90%, 95%, 85%, and 80%. Distance-corrected visual outcomes have overall shown higher performances. All visual acuities at defocus curves were ≤0.125 logMAR between +0.50 D and -2.00 D. PSF-Strehl ratio, MTF cut-off, RMS were 0.26 ± 0.28, 19.82 ± 12.35, 0.31 ± 0.17. Reading analysis reached 125.42 ± 27.21 words/minute, 92.56 ± 7.82, 0.17 ± 0.15 logMAR and 0.50 ± 0.11 logRAD for mean reading speed, visual acuity score, reading acuity, and critical print size, respectively. CS was higher in photopic conditions. Subjective spectacle independence was achieved in 80% of patients. Conclusions: Toric EDOF IOL showed rotational stability and reliable astigmatic correction. It provided spectacle independence and good performance from distance to near distance, reaching high patient satisfaction without undermining binocular quality of vision

    Near Infrared–Circularly Polarized Luminescence/Circular Dichroism Active Yb(III) Complexes Bearing Both Central and Axial Chirality

    In this contribution, we present new NIR-CPL/CD active Yb(III) complexes, which are synthesized by combining two ligands characterized by different types of chirality: central and axial chirality. The ligand bearing central chirality is represented by the neutral trans-N,N '-bis(2-pyridylmethylidene)-1,2-(R,R or S,S) cyclohexanediamine (L), whereas the axial ligand is 1,1 '-binaphthyl-2,2 '-diol (BINOL-H2). A combined 1H NMR, thermodynamic (spectrophotometric titrations), and theoretical (DFT structural calculations) study is presented, revealing the high stability in the methanol solution of the homochiral C 2 -symmetric anionic [YbL(BINOL)2]- complexes, in which the two labile BINOLate molecules exhibit the same stereochemistry (R or S). The metal ion-related optical and chiroptical properties of the complexes in the near-infrared spectral region are here presented and discussed: while the axial chirality dominates the circularly polarized luminescence features, the NIR-CD signal is affected by both chiral elements

    Late Languages of Marginal Attestation: Pamphylian, Sidetic, and Pisidian.

    In this section, we introduce three languages attested in a circumscribed geo- graphical area located roughly in the central part of southern Anatolia: Pamphylian, Sidetic, and Pisidian, which are presented in order of relevance to the study of language contact. These languages pose a number of problems in terms of documentation, interpretation of texts, and genealogical filiation. However, they also provide some interesting data for the study of language contact. Pamphylian is a Greek dialect, but, unlike other Greek dialects, its documentation includes only epigraphic evidence (and plausibly some glosses in scholarly tradition); no literary texts have come down to us. Sidetic and Pisidian are Anatolian languages and probably belong to the Luwic branch. They are also attested only by epigraphs, which mostly include personal names. Their chronology is not entirely consistent: whereas Pamphylian and Sidetic display coeval documentation and plausibly were in contact with each other, Pisidian documentation is significantly later

    Osteoporosis and fractures in systemic vasculitides: a systematic review and meta-analysis

    Background/aim: We performed a systematic review and meta-analysis of observational studies aimed at investigating the prevalence of osteoporosis and osteoporotic fractures in subjects affected by systemic vasculitides (SVs) as well as to explore their risk of osteoporosis and osteoporotic fractures when compared to healthy controls. Methods: Scopus, Web of Science and PubMed were systematically searched from inception to February 2024 for observational studies investigating the prevalence of osteoporosis and fragility fractures in adults with SVs. In addition, when available, we assessed the odd ratios (OR) of prevalent osteoporosis and fragility fractures amongst subjects with SVs vs. healthy controls. Data from eligible studies were extracted, and meta-analysis was performed using a random effects model to obtain ORs with 95% confidence intervals (CIs). Subgroup analyses and meta-regressions were also performed. This study was registered in Open Science Framework (DOI: https://doi.org/10.17605/OSF.IO/3G7RJ). Results: Forty studies with 23,358 individuals affected by SVs were included. The overall prevalence of osteoporosis and fragility fractures in the SV patients were respectively 14.64% (95%CI 12.21-18.89), and 17.08% (95%CI 11.42-24.78). The ORs for osteoporosis and fragility fractures in SV patients when compared with healthy controls were 2.92 (95%CI 1.72-4.98) and 2.39 (95%CI 1.34-4.26) respectively. The univariable meta-regression analysis showed a significant association between cumulative glucocorticoids' dosage (total grams) and risk of prevalent osteoporosis (estimate = 0.0995, R2 = 0.24, p=0.0194). Conclusion: SVs are associated with an increased risk for osteoporosis and fragility fractures, suggesting that active vigilance and pre-emptive screening are recommended
