    Interdental Papilla Regeneration with Hyaluronic Acid: a Narrative Review

    Peri-implantitis is a growing concern as dental implants become increasingly popular. This review paper aims to present the current understanding of peri-implant disease and provide insights into its management and prevention. The study followed PRISMA guidelines and conducted an extensive electronic search for relevant articles. Only systematic reviews published in Q1 and Q2 journals were considered. Eighteen systematic reviews have been selected from the 63 initial studies. The prevalence of peri-implantitis is estimated at around 19.53% of patients with implants and 12.53% of implants placed. Specific bacteria and MMP-8 levels have been associated with peri-implantitis, highlighting potential diagnostic markers. The review emphasized the need for consensus in research to accurately estimate the epidemiological parameters of peri-implantitis. Additionally, the adjunctive use of local antibiotics showed promising results in improving probing pocket depth and bleeding on probing compared to surgical treatment alone. Overall, this paper provides a comprehensive overview of the current knowledge on peri- implantitis and emphasizes the importance of precise diagnostic markers and effective treatment modalities

    Dialettica dei materiali della creazione. Theodor Adorno e le aporie dell'immersività

    La tesi presenta l'elaborazione di una teoria dialettica dei materiali della creazione artistica, e affronta un problema dirimente dell'estetica contemporanea attraverso tale costellazione concettuale. Essa si focalizza dunque sul concetto di immersività, inteso precipuamente nel suo significato di materiale della creazione. Immersività, nel senso pregnante che difendiamo, non sarà il significante maestro entro cui recepire gli effetti spettacolari delle opere d’arte prodotte con le nuove tecnologie, ma una modalità di fruizione, e creazione, che coinvolge in profondità la coscienza estetica e critica dell’individuo. Per riportarlo al suo concetto, occorrerà dunque in primo luogo decostruire la sua nozione irriflessa, che ne riduce la comprensione al solo lato spettacolare, nel segno di un male inteso effetto coinvolgente che dovrebbe condurre il fruitore all’interno dell’opera stessa non più come semplice soggetto della contemplazione ma come agente interattivo. Questa tendenza, riscontrabile in molta arte del nostro tempo, ha tuttavia come tacita condizione necessaria l’abolizione della distanza tra l’opera e lo spettatore, il cui presupposto teoretico è l'abolizione del carattere di autonomia dell'opera d'arte, revocando con ciò lo statuto filosofico stesso della contemplazione; la quale è invece momento determinante non solo del rapporto estetico che il soggetto instaura con l’opera, bensì dell’opera stessa nella sua istanza sorgiva. Ciò perché la natura medesima dell’opera d’arte è conflittuale, nella misura in cui essa conserva dialetticamente la stessa lotta tra soggetto e oggetto che il reale riproduce mediante la sua fruizione. Per chiarire questo punto e dimostrare come un giusto intendimento circa la nozione di immersività possa salvaguardare entrambi i momenti anziché sopprimerli con l’indistinzione del soggetto nell’oggetto, ci rivolgeremo ad Adorno: alla sua analisi delle cause aristoteliche, contenuta nel corso del 1965 sui concetti fondamentali della metafisica classica, e al suo scritto paradigmatico su Wagner per chiarire la nozione di "effetto", benché il termine di confronto primario sia il suo scritto più teoreticamente fecondo: la Teoria estetica.This thesis delineates a dialectical theory of the materials of artistic creation, and addresses a crucial problem of contemporary aesthetics through this conceptual constellation. It thus focuses on the concept of immersivity, to be understood primarily in its meaning of material of creation. Immersivity, in the salient sense that we defend, will not be the master-signifier through which the spectacular effects of works of art produced with new technologies are apprehended; rather, it is a mode of fruition and creation that profoundly engages the aesthetic and critical consciousness of the individual. In order to reconnect with its conceptual foundation, it is imperative to deconstruct its unreflective notion, which limits its comprehension to the spectacular dimension, leading to an ill-conceived interactive effect that should engage the user within the work itself, no longer as a passive subject of admiration, but as an active participant. This tendency, which may be observed in a significant portion of contemporary art, implicitly assumes the abolition of the distance between the work and the spectator. The theoretical premise underlying this assumption is the dissolution of the autonomous character of the work of art, consequently invalidating the philosophical status of contemplation – which is a process that not only dictates the aesthetic rapport that the subject establishes with the work, but also the work itself in its genesis. This is due to the inherent conflictual nature of artistic production, which, in its dialectical essence, mirrors the perpetual struggle between subjectivity and objectivity that is inherent in the real. In order to elucidate this point and demonstrate how an accurate comprehension of the notion of immersivity can ensure both moments are present, rather than being suppressed by the indistinction of the subject in the object, we will refer to Adorno's analysis of Aristotelian causes (contained in the 1965 course on the fundamental concepts of classical metaphysics), and his paradigmatic writings on Wagner to clarify the notion of "effect", although the primary term of comparison is his most theoretically fertile work: Aesthetic theory

    Impact of metabolic dysfunction severity in steatotic liver disease and its interaction with liver fibrosis on all-cause mortality and multiple hepatic and extra-hepatic outcomes

    Background: In metabolic dysfunction-associated steatotic liver disease (MASLD) and in MASLD with alcohol consumption (MetALD), we investigated the effect of severity of metabolic dysfunction on incident major adverse liver outcomes (MALO), major cardiovascular events (MACE), obesity-related cancers, and all-cause mortality (ACM). Methods: SLD was identified among 502,381 UK Biobank participants using the Hepatic Steatosis Index (HSI) (>36 vs.<30). Metabolic syndrome (MetS) traits and MetS (≥3 traits) using MASLD/MetALD criteria. Cox regression was used to estimate adjusted hazard ratios and 95%CIs [aHR(95%CIs)] of MASLD or MetALD plus 1 to 5 MetS traits with incident MALO, MACE, obesity-related cancers and 5-year/10-year incidence rates versus reference (no SLD/MetS traits). Results: Median follow-up was 148 to 149 months. Comparing MASLD with one versus five MetS traits, respectively, to the reference; for MALO, [aHRs (95%CIs)] were 2.27 (1.03-5.00) and 9.19 (4.98-16.95); for MetALD, aHRs were 1.65 (0.53-5.11) and 8.54 (3.65-19.95) respectively. For MACE, with MASLD; aHRs were 1.51 (1.19-1.92) and 4.81 (4.06-5.69) respectively; with MetALD, aHRs were 1.46 (1.00-2.13) and 4.64 (3.51-6.14) respectively. For obesity-related cancers; with MASLD, aHRs were 1.04 (0.87-1.23) and 1.46 (1.29-1.66) respectively; with MetALD, aHRs were 1.01 (0.79-1.29) and 1.51 (1.24-1.83) respectively. 5-year and 10-year incidence rates also increased progressively with increasing MetS traits. Combining SLD, MetS and high liver fibrosis risk (defined by FIB-4 ≥ 2.67) was strongly associated with MALO in both MASLD and MetALD (aHRs 27.48, (17.72-42.61); 43.36, 20.53-91.58 respectively).     Association of physiotherapy dose with motor recovery in early subacute phase after stroke: results from a multicenter Italian study

    Introduction: The literature investigating the factors associated with functional recovery after stroke suggests that, in the early subacute phase, time is the factor that most significantly drives the recovery. However, it is unclear whether the dose of physiotherapy (PT) delivered is equally associated with recovery of motor function and independence. Methods: A multivariable modeling of data from a multicenter longitudinal prospective cohort study investigating the contents of neurological PT interventions in Italy was developed, with the aim to estimate the association between the dose of PT received in the early subacute phase after stroke and recovery of walking, motor function, and independence. Results: A total of 96 patients were included in the analyses. PT dose seems associated with recovery of manual dexterity, measured by the Box and Block test (BBT), but not with the level of independence nor walking function. The probability of achieving a score higher than 31 at the BBT is 46% (95%CI: 18-76) after 10 hours and 76% (95%CI: 42-93) after 20 hours of PT, respectively.     Motor Skill Development at Preschool Age in Girls and Boys: The Role of Outdoor Free Play

    Background/Objectives: Trajectories of fundamental movement skill (FMS) development start diverging in females and males in early childhood, with determinants of this divergence spanning from individual to social and environmental factors. The present cross-sectional study focuses on the role of free outdoor play and aims to investigate whether sex differences in FMS typically observed in early childhood are associated with participation in free outdoor play. Methods: One hundred and forty-two children aged 4.3 +/- 0.8 yrs were evaluated for locomotor and object control skills (TGMD-3), weight status (BMI), and free outdoor play (parent-reported). Motor skill competence scores were submitted to moderated regression analyses to evaluate the individual and joint effects of sex, outdoor play, age, and BMI; interrelations among these variables were also estimated with network analysis. Results: Results of the moderated regression showed, beyond the expected prediction of motor skill competence (overall and object control skills) by sex and age, also a significant sex x outdoor play interaction, with higher motor skills being predicted by more frequent outdoor play in males only. The network analysis confirmed a positive association between outdoor play and motor skill competence in males but not in females. Conclusions: Males might capitalize on free outdoor play opportunities as early as preschool age to engage in activities that promote their motor and especially object control skill development. Longitudinal studies are needed to test causality and derive practical indications for enabling both males and females to fully exploit the opportunities provided by free outdoor play to exercise both locomotor and object control skills

    Control of high-dimensional collective dynamics by deep neural feedback laws and kinetic modelling

    Modeling and control of agent-based models is twice cursed by the dimensionality of the problem, as both the number of agents and their state space dimension can be large. Even though the computational barrier posed by a large ensemble of agents can be overcome through a mean field formulation of the control problem, the feasibility of its solution is generally guaranteed only for agents operating in low-dimensional spaces. To circumvent the difficulty posed by the high dimensionality of the state space a kinetic model is proposed, requiring the sampling of high-dimensional, two-agent sub-problems, to evolve the agents’ density using a Boltzmann type equation. Such density evolution requires a high-frequency sampling of two-agent optimal control problems, which is efficiently approximated by means of deep neural networks and supervised learning, enabling the fast simulation of high-dimensional, large-scale ensembles of controlled particles. Numerical experiments demonstrate the effectiveness of the proposed approach in the control of consensus and attraction-repulsion dynamics

    Management of mycosis fungoides and Sézary syndrome with mogamulizumab in combination with psoralen plus UVA: two case reports

    : This report describes the cases of two patients with mycosis fungoides and Sézary syndrome (MF/SS) who achieved clinical benefit with mogamulizumab combination therapies. Case 1 is a 56-year-old male with stage IIIB (T4NxM0B1) MF, which later progressed into SS, with ongoing skin symptoms (erythema, lichenified skin, and pruritis) and axillary and inguinal lymphadenomegaly. Skin-directed and systemic therapies failed to achieve a long-lasting response in this patient. Mogamulizumab (1 mg/kg once weekly for 4 weeks; once every 2 weeks thereafter) yielded temporary improvement in skin symptoms, but progression in the skin was confirmed after ~2 months. Subsequently, the combination of mogamulizumab with psoralen plus ultraviolet light A (PUVA) yielded a partial response; however, PUVA was discontinued due to phototoxicity and mogamulizumab was continued as monotherapy. At the latest evaluation, clinical improvement in the skin and reduced lymphadenomegaly were evident with ongoing mogamulizumab monotherapy; the patient is awaiting allogeneic hematopoietic stem cell transplantation. Case 2 is an 80-year-old male with stage IIIB (T4NxM0B1) granulomatous variant MF who presented with diffuse erythema with desquamation, ectropion, and inguinal lymphadenopathy. Treatment with oral prednisone and bexarotene failed to achieve adequate, long-lasting responses. Mogamulizumab (1 mg/kg once weekly for 4 weeks; once every 2 weeks thereafter) monotherapy yielded an initial improvement, characterized by less intense erythema, but the improvement was not sustained. Mogamulizumab was supplemented with oral prednisone and then PUVA; this combination resulted in improvement in the skin. PUVA was stopped due to unavailability, and methotrexate (10 mg once weekly) was initiated alongside continued mogamulizumab; this led to improvement in erythema. The patient continued mogamulizumab plus methotrexate with improving clinical status, prior to their death, which was deemed to be unlikely to be related to treatment. Our experience suggests that, in principle, mogamulizumab can be used in combination with other therapies; however, further research is needed

    Association of Plasma Lipoprotein(a) With Major Adverse Cardiovascular Events in MASLD With or Without Advanced Liver Fibrosis

    Background and aims: There is uncertainty regarding the role of plasma lipoprotein(a) [Lp(a)] in predicting cardiovascular events in patients with metabolic dysfunction-associated steatotic liver disease (MASLD). We examined the association between plasma Lp(a) concentrations and major adverse cardiovascular events (MACE) in MASLD, stratified by the severity of liver fibrosis. Methods: This study enrolled patients with MASLD from two centres. Lp(a) percentile groups were generated with the reference group set at the 1st-50th Lp(a) percentiles. High Lp(a) levels correspond to the 91st-100th percentile. Advanced liver fibrosis was defined using the fibrosis (FIB)-4 index > 2.67. MACE was defined as myocardial infarction, ischemic stroke, or cardiovascular death. Cox regression was used to assess the association between the Lp(a) percentile groups and MACE. Results: A total of 56 168 patients with MASLD were followed for a median of 5.0 years (IQR: 2.0-8.5 years), and 6136 patients developed incident MACE. There was an inverse association between Lp(a) percentiles and advanced liver fibrosis (91st-100th percentile, adjusted OR = 0.65, 95% CI 0.59-0.72; p < 0.001). In patients with advanced liver fibrosis, there was a lower proportion in the high Lp(a) levels group (p < 0.001), although the incidence of MACE remained high. MASLD patients with advanced liver fibrosis and high Lp(a) levels had a higher risk of MACE (adjusted HR = 1.56, 95% CI 1.27-1.91; p < 0.001) than those with low Lp(a) levels. Conclusions: Plasma Lp(a) levels are lower in MASLD patients with advanced fibrosis, yet their MACE risk remains high, suggesting that relying on Lp(a) alone may underestimate cardiovascular risk if advanced liver fibrosis is not considered
