    The texts read in this volume question the characteristics of the éphémères creations, the relationships and the differences that are entrenched with the previous works, that surpass the parfois and inspire souvent. If this phenomenon is particularly evident in the largest regions of Italy, our objective is to expand our horizon of research towards other European countries such as France, the République region, Poland, the Pays-Bas et l'Espagne, in order to understand the dynamics of artistic changes across the continent. The presenters study the contexts in which these are not the manifestations, the objectives, the dialogue between clients, artists and the public. Is the role of time another element in the heart of our reflection: what traces remain of the inventions conçues pour des parties? Are those strategies put in place to transmit the memory

    Factors affecting the unused short peripheral catheters: A neverending debate? A cross-sectional study in hospitalized patients

    Background: Concern about unused vascular access devices has increased, but data on idle short peripherally intravascular catheters in medical-surgical units is still scarce. We aimed to assess the prevalence of unused short peripheral catheters in hospitalized patients and identify risk factors and reasons that lead nurses to maintain them in situ.Methods: A cross-sectional observational study was performed in September 2022. Clinical observation (24h) and medical records review were assessed to collect patient demographics and catheter-related information. Nurses were asked the main reason for continuing the use of the unused catheter. The primary outcome was the prevalence of unused short peripheral catheters, which were no longer used for intravenous fluids or medications in the previous 24 h. Multivariate logistic regression has been performed to evaluate risk factors for unused catheters. The STROBE checklist was used to report our study.Results: Seven hundred and sixty-four patients were included; out of the 621 patients with at least one short peripheral catheter, one-third had at least one unused catheter. The risk of having an unused peripherally intravascular catheter was almost double when the catheter was positioned in the emergency department. Catheters of smaller calibers and in situ for fewer days were less likely to be unused. The main reasons for keeping a peripherally intravascular catheter unused in situ were clinical conditions.Conclusion: Unused short peripheral catheters are still quite frequent. While specific clinical needs justified some unused short peripheral catheters, a proportion remained unexplained and potentially avoidable. Nurses and policymakers can use the findings to inform decision-making and protocols to manage peripherally inserted catheters to reduce unused short peripheral catheters and, consequently, complications and costs and improve patient safety and quality of care

    Oral health in patients with cleft lip and palate: a systematic literature review and meta-analysis of periodontal and dental disease and oral microbiota (part 1)

    Introduction Orofacial cleft impacts jawbone and dental development and function, often with consequences for oral health. The first in this two-part systematic review of the literature on oral health in persons with cleft lip and/or palate focuses on periodontal parameters and composition of oral bacterial flora, while the second analyzes data on dental caries.Materials and MethodsFour databases (PubMed, Cochrane, Scopus, Web of Science) were searched for studies that compared periodontal parameters, caries index, and microbiota composition between persons with cleft lip and/or palate and healthy controls. The Newcastle-Ottawa scale and the Joanna Briggs Institute checklist were applied to evaluate study quality.ResultsA total of 24 studies underwent systematic review, 18 of which entered meta-analysis. Twenty studies included a control group, while four included only persons with orofacial cleft. The first part (18 studies) compared periodontal parameters and oral microbiota.ConclusionsMeta-analysis of periodontal parameters (plaque index, gingival index, bleeding index, probing pocket depth, clinical attachment level) revealed an association between orofacial cleft and greater risk for poor oral health and periodontal disease. Good oral health relies on correct home oral hygiene and regular visits to the dentist starting at an early age

    Theta Burst Stimulation Modulates Exercise Performance by Influencing Central Fatigue and Corticospinal Excitability

    Purpose: Theta-burst stimulation (TBS) over the primary motor cortex modulates activity of the underlying neural tissue, but little is known about its consequence on neuromuscular fatigue (NMF) and its neural correlates. This study aimed to compare the effects of facilitatory versus inhibitory TBS on the NMF and excitability/inhibition of the corticospinal pathway in an unfatigued/fatigued muscle. Methods: The effects of three TBS protocols (facilitatory/intermittent: iTBS; inhibitory/continuous: cTBS, and sham: sTBS) were tested on exercise performance, neuromuscular function, corticospinal excitability and inhibition in twenty young healthy participants. Transcranial magnetic and peripheral electrical stimulations were used at baseline, following TBS (unfatigued state), and after a fatiguing sustained contraction (fatigued state) at 35% of the maximal voluntary isometric contraction (MVIC) of the elbow flexors. Results: Time-to-task failure was shorter for cTBS (142±51 s) and longer for iTBS (214±68 s) compared with sTBS (173±65 s) (P < .05). In an unfatigued state, cTBS reduced MVIC and voluntary activation (VA), increased motor-evoked potential (MEP), and silent period (SP) (P < 0.05), while iTBS did not cause any change. In a fatigued state, MVIC and VA decreased in all TBS sessions (P < 0.05). However, the reduction in VA was larger after cTBS (Δ-18±18%) compared with iTBS (Δ-3±5%), and sTBS (Δ-9±9%) (P < 0.001). Furthermore, the increase in MEP and SP were greater for cTBS (P < .05), compared to iTBS and sTBS (P < .05). Conclusions: Facilitatory TBS augments exercise performance that is independent of central parameters and corticospinal mechanisms whilst inhibitory TBS attenuates exercise performance through an exacerbation in the development of central fatigue and possibly intracortical inhibition

    Background: The dura mater, a fibrous membrane protecting the brain and spinal cord, is prone to lacerations during spinal surgery, often leading to complications such as cerebrospinal fluid (CSF) leakage. Anatomical differences in the spinal dura mater, particularly in regions under high biomechanical stress, suggest that chronic inflammatory processes may compromise its structural integrity. This pilot study investigates the molecular and anatomical characteristics of the spinal dura mater, focusing on its response to inflammation and the challenges of surgical repair. Methods: The study involves the collection of 40-60 cadaveric spinal dura mater samples and 100 preoperative blood samples from patients undergoing major spinal surgery. Histological and biomolecular analyses will assess tissue integrity, elastin, collagen content, and the expression of inflammatory biomarkers (heat shock proteins [HSPs] and cytokines). Multiplex assays, mass spectrometry, and Western blot techniques will analyze biomarker profiles in both dura mater and blood samples. Comparative and correlation analyses will be performed to identify common molecular patterns between cadaveric tissue and patient blood samples, with statistical tests applied to detect significant differences. Results: Preliminary power analysis justifies the sample sizes, aiming to detect moderate effect sizes (Cohen's d = 0.5) with 80% power. Histological and biomolecular results will focus on the impact of chronic inflammation on spinal dura integrity, specifically its thinning and vulnerability to laceration. Key findings will include biomarker profiles correlating with tissue damage and post-surgical complications like CSF leakage. Conclusion: This pilot study aims to enhance our understanding of the inflammatory processes affecting the dura mater and their role in frequent lacerations during spinal surgery. Findings are expected to guide future clinical practices in dura repair strategies, potentially improving surgical outcomes and reducing complications related to CSF leakage
