Natura umana, tecnica, volontà di immortalità: lezioni dall'epoca tragica dei Greci
In the age of the death of God, paradise is only projected and enacted by the seemingly unlim-
ited power of technoscience. As a testament to it, the dream to defeat death is coming closer to
become true, thus enabling humankind to achieve the status of Homo Deus. Present-day will to
technopower revolves around two tenets : first, if a limit can be overcome, then it should be ; sec-
ond, any purpose, meaning and justification of the actions of technoscience is to be found only
in those actions themselves. If the paradise of technoscience has to exist, then the very existence
of the limits of finiteness represents a repression of human vital forces in their strive towards
endless self-overcoming, and a constraint to erased at all cost. But tragedies such as Prometheus
bound and Antigone show that ancient Greeks, arguably the inventors of will to power, thought
the opposite was true : indeed, only in its finiteness lies the root of humankind’s incredible ca-
pacity to overcome (by means of tèchne) the constraints of existence. Therefore, finiteness and
death should be carefully cherished like humanity’s most precious gifts, not destroyed in the
race to achieve an impossible divine status

The E3 ligase RNF32 controls the IκB kinase complex and NF-κB signaling in intestinal stem cells
Nuclear factor-kappa B (NF-κB) signaling is a central pathway regulating a plethora of
cellular functions. Here, we find that RNF32, a RING E3 ubiquitin ligase whose
expression is enriched in murine intestinal stem cells, regulates the activity of the IkB
kinase (IKK) complex, the signal integration hub for NF-κB activation. The E3 ligase
activity of RNF32 depends on Calmodulin, the primary calcium sensor in eukaryotic
cells. Increased levels of intracellular calcium ion (Ca2+) induce RNF32 binding to
Calmodulin, RNF32 activation, and autoubiquitylation. In turn, polyubiquitin chains
conjugated to RNF32 recruit NEMO, the regulatory subunit of the IKK complex.
Moreover, Ca2+ rise triggers RNF32 phase separation, which is required for the
formation of NEMO condensates and IKK activation. Finally, we show that RNF32 is
required for NF-κB activation triggered by bacterial lipopolysaccharides. Collectively, our findings uncover a mechanism controlling NF-κB signaling in the intestinal
epithelium

Late outcomes of Viabahn self-expandable covered stent for the elective treatment of popliteal artery aneurysms
Objective: In the present study, we aimed to evaluate in detail the late outcomes of the overall endovascular cohort of the PARADE study, with a focus on factors that could influence such outcomes as these may provide useful insights for patients and clinicians alike. Methods: Between January 2010 and December 2023 patients with nonacute elective popliteal artery aneurysms undergoing endovascular exclusion with the Viabahn stent graft were included in a multicenter retrospective cohort study (40 sites from 10 countries). A cut-off of 15 procedures was used to define a participating center as high volume (>15) or low volume (<15). Results: During the 14-year studied period, 326 patients were treated who met inclusion criteria for the present study. Patients were predominantly male (304, 93.3%) with a mean age of 74.6 ± 9.2 years. Most patients were asymptomatic (221 [67.8%]); 56 (17.2%) had intermittent claudication and 49 (15%) chronic limb-threatening ischemia. Acute technical success was not obtained in two cases (0.6%), owing to residual type Ia endoleak (one case), and residual type Ib endoleak (one case). Of these, one patient underwent an open conversion, whereas the other one was followed up because unfit for any type of reintervention. At 30 days, two patients died with an overall 30-day mortality rate of 0.6%. Both were not cardiovascular deaths related to interventions. In addition, 30-day rates of major adverse cardiovascular events, graft occlusion, and procedure-related reinterventions were 1.2%, 3.7%, and 5.2%, respectively. No patient underwent early major amputation. The 5-year Kaplan-Meier estimates of primary patency, secondary patency, freedom from reinterventions(s), and amputation-free survival were 65.8% (95% confidence interval [CI], 61.7%-71.9%), 84.9% (95% CI, 78.7%-89.1%), 70.5% (95% CI, 66.2%-74.8%), and 98.2% (95% CI, 96.4%-99.6%), respectively. Amputation-free survival was adversely affected by active smoking (P = .011), chronic kidney disease (P < .001), poor run-off status (P = .042), and low number of cases for each center (<15) (P = .011). Multivariate analysis reported an approaching significance for active smoking (hazard ratio, 3.460; 95% CI, 2.6-6.1; P = .051), and confirmed the association with chronic kidney disease (hazard ratio, 7.413; 95% CI, 5.4-9.3; P = .006). Conclusions: The findings from this study show that endovascular repair using the Viabahn stent graft may provide a feasible technical option for elective treatment of popliteal artery aneurysm. Some patient-related and procedure-related factors were identified, including chronic kidney disease, that were associated with higher rates of long-term complications
Surgical and Oncological Outcomes of Minimally Invasive Left Pancreatectomy for Pancreatic Cancer: Robotic vs. Laparoscopic Approach
Objective: This study compares the surgical and oncological outcomes of minimally invasive robotic (RLP) and laparoscopic (LLP) left pancreatectomy in pancreatic cancer (PC) patients.
Methods: Data from patients who underwent minimally invasive left pancreatectomy between 2013 and 2023 were analyzed. Two groups were identified: RLP and LLP. Perioperative outcomes were compared, including operative time, blood loss, conversion rate, and postoperative complications. Oncological outcomes included margin status, lymph node retrieval, lymph node status, overall survival (OS), and disease-free survival (DFS).
Results: Fifty-four patients were divided into the LLP (n = 39) and RLP (n = 15) groups. The median operative time was shorter for LLP than RLP [260 min vs. 366 min, p = 0.007]. Blood loss and conversion rates were comparable (p > 0.05). In the LLP group, significantly more lymph nodes were harvested (29 vs. 19, p = 0.05), and a higher percentage of positive lymph nodes was noted (72% vs. 40%, p = 0.033). No significant difference was found in the R0 resection status (82% vs. 73%, p = 0.358). After a median follow-up of 26 months, OS (23 months vs. 34 months, p = 0.812) and DFS (17 months vs. 16 months, p = 0.635) were similar.
Conclusion: RLP provides outcomes identical to LLP in treating body-tail pancreatic cancer, with further studies needed to confirm its long-term oncological efficacy
Phenotype-Genotype Correlations in Early-Onset Myelin Protein Zero-Related Neuropathies
Background and objectives: Myelin protein zero (MPZ)-related neuropathies include demyelinating CMT1B and later-onset axonal forms (CMT2I/J). CMT1B pathogenic variants act through gain-of-function, destabilizing MPZ and activating the unfolded protein response (UPR), or loss-of-function, disrupting MPZ homomeric interactions in myelin. This study investigates early-onset (<18 years) CMT1B due to MPZ variants in a large Italian cohort, shedding light on clinical progression and genotype-phenotype correlations, with relevance to emerging UPR-targeted therapies. Methods: We analyzed cross-sectional clinical and genetic data from 75 patients across 7 Italian centers. MPZ variants were categorized as destabilizing or non-destabilizing based on published data. To explore the relationship between the degree of protein destabilization and clinical severity, missense variants were evaluated for stability alterations using the DUET online tool, with negative ∆∆G values indicating greater destabilization. Statistical correlations between clinical and molecular features were assessed. Results: This study presents a comprehensive clinical characterization of one of the largest cohorts of early-onset MPZ-related neuropathies reported to date. The Charcot-Marie-Tooth Examination Score (CMTES) correlated with age at assessment (r s = 0.56; p < 0.001), suggesting disease progression in this cohort. Destabilizing variants were associated with earlier onset (p < 0.001), upper limb involvement (p < 0.001), impaired ambulation (p < 0.01), scoliosis (p < 0.01), and faster progression as measured by CMTES/age (0.24 vs 0.11 CMTES/year). After excluding the c.245A>C, p.Y82S outlier (highly destabilizing but not UPR-activating), ∆∆G values showed significant correlations with age at onset (r s = 0.54, p = 0.030), CMTES (r s = -0.82, p < 0.001), and progression (r s = -0.60, p = 0.032). Discussion: Stratification of MPZ variants into destabilizing and non-destabilizing is crucial for predicting severity and prognosis and tailoring therapeutic strategies. This approach has significant implications for upcoming UPR-modulating treatments and underscores the necessity of integrating molecular and clinical insights for optimal patient care

Alla luce delle barriere esistenti, valuta criticamente la capacità della direttiva di garantire il rispetto degli obblighi lungo la filiera e riflette sui possibili effetti della proposta di modifica “omnibus”. Il contributo mette infine in evidenza i margini di azione per gli attori sociali, tra strategie di contenzioso e percorsi di advocacy
Il dollaro e i suoi rivali: dominanza o multipolarità nel sistema monetario internazionale?
A più di cinquanta anni dalla fine del sistema internazionale di cambi fissi nato con la conferenza di Bretton Woods, il dollaro continua ad essere al centro del sistema monetario internazionale anche se il predominio della valuta statunitense non è più assoluto con altre valute, a partire dall’euro, che svolgono alcune delle funzioni tipiche di una valuta di riferimento internazionale anche se in una dimensione più regionale che globale. L’analisi dei fattori che sostengono la forza internazionale di una valuta evidenzia che oggi né l’euro né il renminbi, spesso indicato come il futuro rivale del dollaro, possiedono tutti i requisiti economici e istituzionali necessari alla piena assunzione dello status di valuta internazionale globale. Il ruolo internazionale dell’euro è limitato dalle caratteristiche istituzionali del processo di integrazione europea con una unione monetaria priva di una vera unione fiscale mentre il renminbi soffre della limitata apertura del mercato finanziario e bancario cinese ancora sottoposti a politiche di controllo governative. Malgrado le aspirazioni di paesi come quelli appartenenti al gruppo dei BRICS+, di emancipazione dal sistema dei pagamenti internazionale basato sul dollaro, il mantenimento dell’attuale struttura piramidale del sistema monetario internazionale che vede al vertice il dollaro seguito da euro, yen e sterlina rimane la prospettiva più probabile anche se le conseguenze delle annunciate politiche protezionistiche della nuova amministrazione Trump potrebbero paradossalmente accelerare il fin qui lento processo di riduzione della centralità del dollaro.Fifty years after the demise off the Bretton Woods international monetary system, the US dollar is still the dominant currency even if other currencies, such as the euro and renminbi has been gaining an international status on a regional basis. Looking at the main economic and institutional factors baking the international role of a currency, we see that the institutional limits of the European integration process are constraining the full assumption of global role of the euro, while state control on capital flows, domestic banking system and exchange rate in China should be removed in order to promote a wider use of the renminbi at the international level. Call for of a new currency substituting the dollar by the BRICS+ countries notwithstanding, in the next future, the most likely perspective for the international monetary system is a pyramidal structure with the one true global currency, the US dollar, followed by the euro and few other minor international currency such as the yen and the British sterling. However, as an unintended outcome, the consequences of the renewed protectionist policy of the second Trump’s Administration could undermine the continued dominance of the dollar
