Effects of Acetaminophen (Paracetamol) on vasogenic brain edema and blood-brain barrier disruption in experimental model of transient focal cerebral ischemia in rats
Background & objective : Ischemic stroke as third leading cause of death and disability in the most of the human communities has complicated pathophysiology and there is no effective treatment against it.inflammation and oxidative stress are mostimportant pathophysiological mechanisms in ischemic stroke and subsequent development of brain edema . we examined the effects of post-ischemic treatment with acetaminophen on cerebral infarction , sensorimotor dysfunctions , formation of edema and blood – brain disruption in a rat model of ischemic storke.
Methods: male Sprague-Dawley rats were divided into three main groups (n=16) , sham,control ischemic, acetaminophen treated (400mg/kg) ischemic groups. Transient focal cerebral ischemia was induced by 60-min-long occlusion of the left middle cerebral artery followed by 24-h-long reperfusion.Sensorimotor dysfunctions was evaluated at the end of the reperfusion period. Thereafter, the animals were randomly used for measurement of the infarct volumes and investigation of ischemic brain edema formation using a wet/dry method. Blood-brain barrier permeability was assessed by Evans blue extravasation technique.
Results : Induction of cerebral ischemia in the control group produced considerable brain infarction in conjunction with ceverely impaired sensorimotor functions. Treatment with acetaminophen significantly reduced the infarct volume and improved the above functions. The water content and EB concentratin in the left (lesioned) hemisphere were considerably elevated in the control ischemic group. Acetaminophen significantly lowered the water content and protected BBB integrity in the ischemic Lesioned hemisphere.
Conclusion: post – ischemic treatment with acetaminophen can noticeably decrease ischemic brain injury, improve sensorimotor dysfunctions ans attenuate edema formation likely via protecting BBB integrity
Comparative study of the efficacy of Remifentanil alone and in combination with Dexamethasone in controlling labor pain
Background and Objective: Labor pain is often severe. Unrelieved labor pain can have an adverse effect on the physiologic status of women. Pain during labor is accompanied by fear, which is related to slower progress of labor. Accurate measurement and appropriate management of pain is a significant problem for attendant medical and nursing personnel. There are choices to be made during pregnancy about options available for pain relief in labor; each method has its own risks and benefits, with variations in effectiveness, availability and acceptability.The purpose of this study was to comparsion effect of remifentanil with and without dexamethasone among women undergoing normal vaginal delivery
Methods: In this clinical trial study, 90 women with term pregnancy were observed. In all of the cases, labor pains started spontaneously and had fetal and maternal indication for normal vaginal delivery. They were divided into two groups randomly. The first group received remifentanil with dexamethasone and the second group received the remifentanil. Data of labor pain and outcome of delivery collected by means of questionnaires.
Results: Pain severity according to VAS score was significantly lower in patient received remifentanil and dexamethasone after 30, 60 and 120 min post intervention VS subjects with remifentanil . Duration of labor's active phase in remifentanil and dexamethasone group was significantly lower than other group. Analysis of the pooled risk differences showed that none of the side effects investigated were significantly different between two groups unless nausea, vomiting and headache that was significantly higher in remifentanil users. There wasn't significant difference regarding in FHR. Significant difference was not observed among the groups regarding apgar in 1th and 5th minute between two groups. Analysis of the maternal complications between two groups showed that there were significantly different between two groups about heartbeat, SBP and DBP.
Conclusions: Use of remifentanil and dexamethasone has more beneficial effects for many parturient women compared to remifentani
Correlations between mutations site in MEFV gene and phenotype of periodic disease, gout, palindromic rheumatisem and recurrent synovitis of hip
Background and objective: Auto-inflammatory syndromes have a distinct group in rheumatologic diseases. The aim of in this study, Correlations between mutations site in MEFVgene and phenotype of periodic disease, gout, palindromic rheumatisem and recurrent synovitis of hip.
Methods: This study is a case-control study. In this study, after a clinical examination and selection of patients: a) gout, b) palindromic syndrome and c) recurrent sinusitis, 32 patients were enrolled. All patients received complete history and demographic data, duration of symptoms, duration of diagnosis, type of disease, etc, and were obtained in checklist of each patient. Then blood samples were taken from the patients and taken to the Genetic Laboratory for examination of the varieties or mutations obtained for the MEFV gene. After determining the MEFV gene and its sequencing in patients, all of the data entered the SPSS v21 statistical program and finally we analyzed the data.
Results: In this study, the mean age of patients with gout was 53.41 years. The majority of patients were male and thumb was involved most commonly joint. It was also observed that 20.8% of patients had MEFV positive gene and the most common genotype E148Q in 16.7% of patients. Among 5 patients with palindromic syndrome, the mean age of patients was 14.6 years and Most of them were male and hip and knee were the most commonly involved joints. It was also observed that only 1 (20%) patients had MEFV gene and the V726A / Wt gene was reported. Of the 3 patients with Periodic Synovite, the mean age of the patients 15.66 years and most of them were male. It was also observed that all three patients with periodontal sinus had MEFV gene mutations including R761H / Wt, V726A / Wt and A744S / Wt.
Conclusion: In the present study, MEFV gene was observed in all patients with Periodic Synovite. Also, a high percentage of patients with gout and palindromic syndrome had positive gene mutations
Targeting the mitochondrial apoptosis pathway by a newly synthesized COX-2 inhibitor in pediatric ALL lymphocytes
Aim: Acute lymphoblastic leukemia (ALL) is known as a barely curable malignancy. Particular mutations involved in apoptosis may have a main role in the onset of ALL in the pediatric patients. It has been proven that cycloxygenase-2 is capable of impairing the apoptosis pathway through mitochondria in tumor cells. Methodology: In this study, we investigated selective toxicity of a newly synthesized chalconeferrocenyl derivative as a selective cycloxygenase-2 inhibitor in ALL and healthy B-lymphocytes, and also isolated mitochondria obtained from them. For this purpose, we evaluated the cellar parameters like viability, apoptosis/necrosis, caspase-3 activation and ATP content, and also mitochondrial parameters like mitochondrial membrane potential decline, reactive oxygen species formation, cytochrome C release and mitochondrial swelling. Conclusion: Our results implied that this compound can selectively induce cellular and mitochondrial toxicity in cancerous ALL B-lymphocytes and obtained mitochondria from them without any detrimental effects on healthy subjects
Salophen copper(II) complex‐assisted click reactions for fast synthesis of 1,2,3‐triazoles based on 1,4‐naphthoquinone scaffold, anti‐bacterial evaluation and molecular docking studies
The salophen copper(II) complex was successfuly used for the efficient synthesis of new 1,2,3‐triazoles based on the 1,4‐naphthoquinone scaffold. The reaction of 2‐chloro‐3‐(prop‐2‐yn‐1‐yloxy)‐1,4‐naphthoquinoneor2,3‐bis(prop‐2‐yn‐1‐yloxy)‐1,4‐naphthoquinone with aromatic azides in the presence of a low copper catalyst loading (1 mol%) afforded 2‐chloro‐3‐((1‐phenyl‐1H‐1,2,3‐triazol‐4‐yl)methoxy)‐1,4‐naphthoquinone or 2,3‐bis((1‐phenyl‐1H‐1,2,3‐triazol‐4‐yl)methoxy)‐1,4‐naphthoquinone, respectively. The advantages of these reactions are short reaction times, high‐to‐excellent reaction yields, operational simplicity, and mild experimental conditions. The new 1,2,3‐triazoles obtained were screened for their in vitro anti‐bacterial activities and were subjected to molecular docking studies
Performance Evaluation of Hospitals Affiliated With Guilan University of Medical Sciences, Based on a DEA Approach and in the Presence of Undesirable Factors Before and After Health Sector Development Plan
Introduction: Change in the health system is always considered as the main concern of governments which in turn may have many positive and negative effects. The health sector development plan is one of the programs aiming at improving the therapeutic and economic dimensions of families, given priority by the 11th government of Iran, and is expected to improve performance of the medical sciences universities and their affiliated hospitals, in addition to develop their educational, health, research and cultural systems.
Objective: The aim of this study is to evaluate technical efficiency of 22 public hospitals of Guilan university of medical science before (April 2013 to March 2014) and after (April 2014 to March 2015) implementation of health sector development plan, using data envelopment analysis approach and in accordance with the production of undesirable factors.
Materials and Methods The current research is a descriptive-analytical type and also a cross sectional one. The research population includes 22 public hospitals of Guilan University of Medical Sciences, with enumeration chosen as the sampling method and the data were collected from documents. Finally, the collected data were analyzed based on the coding the DEA weak disposability model in GAMS software.
Results: The findings indicate that before implementation of health sector development plan (2013- 2014), 14 hospitals (63%) were efficient, while after its implementation (2014-2015), number of efficient hospitals increased to 16 units (73%). The average of technical efficiency also computed as 0.954 and 0.958, respectively.
Conclusion: Although, observing the effects of changes in social systems often requires time lapse, however, one year after the implementation of the health system development plan, the average of performance in public hospitals of Guilan province was improved.

Comparison of health status of medical equipment levels and pseudo-homogeneous environmental levels in hospitalized areas of Imam Khomeini Hospital of Ardebil by observational and microbial method in 1396
Background & Objective: The transmission of infection from the equipment and the environment to patients is largely through contact. The aim of this study was to determine the health status of medical equipments and pseudo-homogeneous environmental levels in Imam Khomeini Hospital (RA) in observational and microbial methods in 1396.
Methods: In this descriptive cross-sectional study, 17 levels of admission to Imam Khomeini Hospital (Ra) were evaluated in a two month observational phase (through checklist and microbial). Data analysis was done by using SPSS software and by comparing it with British standards.
Results: The results of this study showed that the rate of contamination in different parts of the hospital was 75% and 29.15%, respectively. According to statistical analyzes in the microbial method, the most infected ventilators were among the most polluted medical equipment and among the peripheral levels of the most infectious patient ward. At the observation station, among the medical equipment levels, the cliché was the most contaminated case, and among the pseudo-environmental environmental levels, the most infected fridge was determined. The most contaminated part in terms of cardiothoracic microbial monitoring was determined in terms of male in-patient observation
Conclusion: It is suggested that by planning and applying appropriate and preventive methods, implementation of disinfection programs, development of a cleaning and disinfection guide, training and continuous monitoring of it with standard methods of contamination will be minimize
Comparative study of the effects of fennel with Gelofen on the severity of primary dysmenorrhea: A randomized clinical trial
Introduction: Dysmenorrhea is one of the most common gynecological disorders; so that 50 to 70 percent of women suffer it. Dysmenorrhea is often treated by chemical drugs. However, studies showed that almost half of the population in industrialized countries is already using complementary medicine and alternative therapies. Therefore, this study was performed with aim to compare the effects of fennel and Gelofen on severity of primary dysmenorrhea.
Methods: This clinical trial was performed on 72 students residing in dormitories of Islamic Azad University of Ardabil in 2016. Samples with grade 2 and 3 of primary dysmenorrhea were randomly divided in two groups of fennel and Gelofen receivers. The fennel group used 3-5 drop of fennel extract in water or tea three times per day during first 3 days of menstruation and Gelofen group used Gelofen capsule every 6 hours during this period. Both groups used this method for three consecutive cycles. Data collection tool included a dysmenorrhea severity questionnaire according verbal multidimensional scoring criteria. Data were analyzed using SPSS software (version 17) and t-test and chi-square. P < 0.05 was considered significant.
Results: Both fennel and Gelofen reduced the severity of pain from severe and moderate to mild and no pain, but there was no significant difference between the two groups of fennel and Glofen in improvement of dysmenorrhea (p>0.05).
Conclusion: The use of both methods of fennel and Glofen was effective in reducing the pain of primary dysmenorrhea and pain reduction was similar in both groups. Regarding the high prevalence of dysmenorrhea in female, the use of fennel in the relief of dysmenorrhea is recommended
Salmonella Osteomyelitis in a Patient With Systemic Lupus Erythematosus
Although infections occur commonly in patients with systemic lupus erythematosus (SLE), Salmonella osteomyelitis is rarely reported in SLE patients. Hereby, we described an SLE case which was complicated by Salmonella infection. The reported case was referred to our center with right knee swelling and pain from 2 months before her admission. Because of no responding to appropriate therapy and high C reactive protein (CRP) level, we suspected to other differential diagnosis such as infection. Knee MRI was in favor of osteomyelitis of distal femur and proximal tibia. It was approved by the bone specimen culture. Patient’s symptoms were significantly better 1 month after debridement surgery and antibiotic therapy. This case indicates that rare infection site such as bone with unusual organism causes should be considered in SLE patients coming with unexpected symptoms looking like prolonged joint swelling and pain despite appropriate therapy
Knowledge of pregnant women towards multiple sclerosis
Background: The multiple sclerosis (MS) disease is more common in women than men with a ratio of 2.7 to 1. According to Iran MS society, there are about 80,000 MS patients in the country Iran. Having knowledge about MS can reduce significantly the anxiety and stress of pregnant women and promote their life expectancy and the probability of having a successful pregnancy and making pregnancy easier for them.
Methods: This is a descriptive– analytical study that has been done on 150 pregnant women which selected randomly and referred to health centers of Ardabil city at 2017. Data collected by a questionnaire included questions about MS. The collected data were analyzed by statistical methods in SPSS version 19.
Results: Of all women, 20% considered gender as one of the most effective factors in the disease and 44.7% believed that women are more likely to suffer from MS than men. The knowledge of pregnant women about MS with 63.3% was in moderate level and friends and relatives was the most source of their awareness. 43.3% of women believed that knowledge people with MS could marry. 63.3% of women pointed that the health care centers knowledge about MS was inadequate and 68.7% pointed that government's support for patients was significantly low.
Conclusions: Results showed that the awareness of pregnant women about MS was in moderate level. So, planning to increase the awareness of pregnant women in centers and support of women with MS by governmental centers seems necessary
