    The Role of Organizational Justice and Spirituality in the Workplace on Organizational Commitment

    هدف از پژوهش حاضر رابطه عدالت سازمانی و معنویت در محیط کار با تعهد سازمانی در بین معلمین آموزش و پرورش شهر اردبیل بوده، که با روش تحقیق توصیفی از نوع همبستگی انجام شده است. جامعه آماری شامل تمامی معلمین آموزش و پرورش شهر اردبیل بوده که جهت بدست آوردن حجم نمونه از فرمول کوکران استفاده شد و 160 نفر به صورت تصادفی ساده از بین آنها انتخاب گردید. برای گردآوری داده ها از پرسشنامه های عدالت سازمانی نیهوف و مورمن، معنویت سازمانی کینجرسکی و اسکریپینگ و تعهد سازمانی آلن و می یر استفاده شد. یافته ها نشان داد که در فرضیه اصلی بین عدالت سازمانی و معنویت در محیط کار با تعهد سازمانی رابطه مستقیم و معناداری وجود دارد و در فرضیه های فرعی بین ابعاد عدالت سازمانی، عدالت رویه ای و مراوده ای و ابعاد معنویت با تعهد سازمانی رابطه مستقیم و معنا داری وجود دارد و همچنین از یافته های بدست آمده می توان چنین نتیجه گرفت که در بین ابعاد عدالت سازمانی، متغیر عدالت مراوده ای و در بین ابعاد معنویت در محیط کار، متغیر جذابیت کاری نسبت به دیگر متغیرها، پیش بینی کننده قوی تری برای تعهد سازمانی می باشند

    Analysis of Apoptotic Genes Expression in HL60 Cells Exposure to Origanum vulgare Nanoparticles

    Origanum vulgare is perennial herb with high antioxidant components. Acute promyelocytic leukemia (AML-M3) cell line, HL60 is used for AML-M3 in vitro investigations. Acute promyelocytic leukemia (APL) often arisen from chromosomal translocation between 12 and 17 chromosomes (t (q24:q21) (15:17)) and leading to PML–RARA fusion protein formation. Following PML- RARA formation, apoptosis and differentiation processes have been arrested in myeloid precursor cells and these cells undergo uncontrolled proliferation in promyelocytic stage. Apoptosis and differentiation induction with ATRA (all-Trans Retinoic Acid) and ATO (Arsenic Trioxide), is first line of APL treatment. ATO with induction of intracellular ROS production lead to cancer cells death. In oreder to investigation of the effect of Origanum vulgare extract and Origanum vulgare mediated silver nanoparticles on HL60 cells death, Origanum vulgare nanoparticles were synthesized using green synthesis method. Then, synthesized nanoparticles were characterized using physiochemical tests, such as SEM microscopy, FTIR analysis, UV- vis spectroscopy and D.L.S technology. For baseline study, HL60 and B lymocytic leukemia, NALM6 cell line, were treated by Origanum vulgare extract. Based on lower IC50 in HL60 cells, studies continued on HL60 cells. Dose and time dependent graphs for the effect of Origanum vulgare and its nanoparticles in HL60 cells after the cytotoxicity test, MTT and IC50 values determination were ploted in each groups. Then expression of the apoptotic genes, including Caspase8, Caspase9, Bax, Bcl-2 and Nrf2 which responsible of intracellular ROS levels, changes were investigated using Real time PCR. For analysis of cell death stages including ealy apoptosis, late apoptosis and necrosis, the cells were stained with Acridin orange/Ethidium bromide cell staining assay and shown with phase contrast microscope. The results indicate thet the Origanum vulgare and its nanaoparticles are effective in HL60 cells death. Origanum vulgare nanoparticles is effective than Origanum vulgare extract and both agents induced apoptosis in internal and external pathways. However, analysis of genes expressions have shown different apoptosis induction pathways with Origanum vulgare and Origanum vulgare nanoparticles. The stages of early and late apoptosis and necrosis is dose dependent and were different in Origanum vulgare extract and nanoparticles treated HL60 cells

    The relationship between acidosis and hypercapnia with Cor pulmonale in patients with chronic obstructive pulmonary disease

    Introduction: Cor pulmonale or right ventricular (RV) enlargement is cardiovascular complication of the chronic obstructive pulmonary disease (COPD). Hypoxic pulmonary vasoconstriction, hypercapnia, acidosis and pulmonary vascular remodeling are suggested as possible mechanisms of cor pulmonale. In this study, we aimed to evaluate the correlation between acidosis and hypercapnia with corpulmonale in patients with COPD. Methods: In this cross-sectional analytical study, 100 patients (56 men and 44 women with mean age of 66.53 ± 10.63 years) with moderate to severe COPD exacerbation were included. Complete history taking and physical examination as well as atrial blood gas, pulmonary function test (PFT) and echocardiography were performed. Disease severity was defined according to global initiative for obstructive lung disease (GOLD), Modified Medical Research Council (mMRC) and COPD Assessment Test (CAT) criteria. Patients with cor pulmonale were defined and findings were compared between patients with and without cor pulmonale. Results: Forty-two patients had cor pulmonale. There was no significant difference in hypercapnia between groups. Cor pulmonale patients, compared to non-cor pulmonale, had significantly lower forced expiratory volume in the first second (FEV1) (P = 0.020), higher tricuspid regurgitation (TR) (P = 0.001) and pulmonary hypertension (P = 0.020). There was significantly negative correlation between RV thickness with FEV1/forced vital capacity (FCV) (r = -0.239, P = 0.010) and RV size with FEV1/FVC (r = -0.312, P = 0.002) and positive correlation with partial pressure of carbon dioxide (PaCO2) (r = 0.312, P = 0.002) and bicarbonate (HCO3) (r = 0.258, P = 0.009). Conclusion: Cor pulmonale in the course of COPD accompanies with adverse outcome. These patients have worse spirometry and left ventricle echocardiographic findings, but have no difference in arterial blood gas (ABG) findings.     Evaluation of the success rate of ESWL in patients with urinary stones admitting to ardebil sepehr salamat clinic in 2017

    Background and objective: Urinary stones (kidney stones, ureter, bladder, and urethra) are one of the most common and earliest known human diseases. The prevalence of kidney stones is estimated to be around 1-15% throughout life, which varies according to age, gender, race, and geographical location. The prevalence of stones in men is 2-3 times higher than in women. Extra Corporeal Shock Wave Lithotripsy (ESWL) is known as the most common method for treating urinary stones (70%) and selective treatment of small urinary stones (7 to 25 mm). Methods: This interventional comparative study was carried out with easy sampling (up to 200 persons) reaching the required roof. Patients with urinary stones with a diameter of 6-30 mm were candidates for ultrasound scaling. After obtaining written consent, They entered the study. Patients with coagulation disorders, urinary or other organ transplantation, pregnant women, uncontrolled blood pressure patients, and those who are contraindicated for drug use have been excluded. The process was carried out by an operator and by the Dornier Compact Delta II smashing machine. Two weeks later, the patients were re-visited and ultrasound were performed and the success rate of stone mass was measured and recorded. Complete cleansing of the urinary system or the remaining 4 mm stone is considered as a complete success, and the presence of a stone larger than 4 mm and smaller than the original stone as a relative success, and failure to break the stone is considered as a failure. Results: In stones below 10 mm, the stones in the upper calyx were 100% successful in treatment, while 66% of the medial calyx stones were complete. 92% of the lower calyx stones were fully treated and 78% of the primary ureteral stones were fully treated and 82% of the stones were fully treated in the pelvic floor and 85% of the UPJ rocks were complete. The difference was statistically significant (P <0.001). In the case of stones over 10 mm, the upper calyx stones were 100% complete with complete treatment. In the case of mid- calyx stones, 66% of the stones were complete and in the lower calyx area, 60% of the stones were fully treated and 68% complete in the primary ureteral stones, 72% of the pelvic gastric ulcers and UPJ rocks were treated thoroughly. The difference between the success rate of outbreak treatment in rocks over 10 mm based on the rock formation site was statistically significant (P <0.008). Conclusion: The results of the data analysis showed that extracorporeal lithotripsy procedure in patients with urinary system stones is very desirable and convenient and the success rate is at acceptable leve

    Impact of vitamin C supplementation on serum ferritin level in hemodialysis patients

    Background: Iron deficiency anemia is common in patients on maintenance hemodialysis (HD). Serum ferritin is increased in HD patients. Vitamin C supplementation has shown to improve functional iron deficiency and anemia in HD patients. Objectives: In this study we aimed to evaluate the effect of vitamin C supplementation on serum ferritin level. Patients and Methods: In this randomized clinical trial, 39 HD patients were randomly assigned to receive routine therapy with (n=24) or without (n=15) vitamin C 500 mg IV twice a week after each dialysis session for 2 months. Changes in serum ferritin, iron, total iron-binding capacity (TIBC) and transferrin saturation (TS) before and after treatment were measured in each group. Results: Serum ferritin levels before treatment in case and control groups were 1859±1398.5 and 2567.5±1907.8 ng/mL which was significantly decreased to 1084.8±727.9 and 1313.5±1151.5 ng/mL, respectively (P=0.006 and P=0.007). Serum iron, TIBC and TS did not change significantly in each group after treatment. Comparing the mean changes in serum ferritin, iron, TIBC and TS, there was no significant difference between groups. Conclusions: Vitamin C had no effects on serum ferritin, iron and TS

    Comparison of Levetiracetam and sodium Valproate in the prevention of migraine: a randomized clinical trial study

    Background: Sodium valproate is an anti-seizure drug used for prophylactic use of migraine headaches. Despite the efficacy of this drug due to complications that cause some patients to not tolerate the drug. The purpose of this study was to Comparison of levotiracetam and Sodium valproate in migraine prophylaxis. Methods: This is a clinical trials study. 100 migraineurs who referred to the Alavi neurology clinic and indicated the onset of prophylaxis of migraine were studied in two groups of 50, one of which was a group of levetiracetam and another group of sodium valproate Received. Patients received daily 500mg of drug for 3 months and the effect of the drug on the number of attacks, severity of attacks, and MIDAS score was measured. Collected data analyzed by statistical methods in SPSS version 19. P<0.05 was considered as significant. Results: In both cases, significant improvement was observed in the number of attacks, severity and MIDAS score. In all of the criteria, levetiracetam and sodium valproate were almost identical, except for reducing the number of headache attacks that had a significantly in levothyramine group less than sodium valporoste group (P = 0.0001). Conclusions: This study showed that levothyramineacetamin can be used as a good alternative in those who do not tolerate sodium valproate

    The evaluation efficacy and side effects of IV Ibuprofen and IV Ketorolac in laparoscopic cholecystectomy patients.

    Background and objective: This study assessed the efficacy and tolerability of intravenous ibuprofen and IV ketorolac in the improvement of post-operative pain control and the reduction of opioid usage after laparoscopic cholecystectomy surgery. Prevention and control of postoperative pain are essential. Inadequate treatment of postoperative pain continues to be a major problem after many surgeries and leads to worse outcomes. Methods: It was triple blinded study that included 90 patients who had undergone elective laparoscopic cholecystectomy. Patients were randomly assigned into three groups; Control group (n = 30) received saline solution, Ketorolac group (n = 30) received 30 mg IV Ketorolac and Ibuprofen group (n = 30) received 800 mg IV ibuprofen. All patients received patient-controlled (PCIA) intravenous fentanyl (30µg/kg) analgesia. The first dose of drugs was administered intravenously at the end of surgery and then every 8 hours, for a total of 3 doses after surgery. Postoperative analgesia was achieved by fentanyl and also meperidine (0/5 mg/kg was administered if VAS>3). Visual analog scores (VAS), postoperative opioid consumption, additional analgesic requirements, satisfaction, and side effects such as nausea and vomiting and sedation were recorded. Results: The mean amount of fentanyl and Mepridine use was reduced significantly in the ketorolac and Ibuprofen group compared with the placebo group (P<0/001). The area under the curve of visual analog scale pain ratings and side effects were not different between ketorolac and Ibuprofen groups but compared with placebo, were statistically lower at all time points (P<0/001) Conclusions: This study indicated that Intravenous ibuprofen 800 mg or ketorolac 30 mg was associated with a significant reduction in fentanyl requirements, and it was generally well tolerated for postoperative pain management in patients but there is a non-inferiority between ibuprofen and ketorolac after first 24 hours

    Environmental impact assessment of the transformation of municipal solid waste process of Tabriz City

    Background & Objectives: Waste production is inevitable in human societies. During the past years, many studies have been done on the quantity and quality of municipal waste generation, as well as waste management strategies.The process of change and conversion is one of the most important elements responsible for Municipal Solid Waste Management (MSWM). The present study aimed to investigate the characteristics of Tabriz urban solid waste and, finally, assessment the environmental impacts of proposed scenarios for the management of MSW in Tabriz in 2017. Methods: In this descriptive study, the required data were collected from field observations, library data, physical analysis with 4 sampling steps in 4 seasons based on ASTM standard method. 4 scenarios (energy generation and composting, energy generation, composting, current status) were proposed and environmental impact assessment of proposed scenarios was done using Iranian leopold matrix was conducted. Results: An average of 1438 tons per day of MSW is collected from the city of Tabriz and shipped to the sanitary landfill. The average per capita of waste production for this city is 0.804 kg per person per day. Examining different components of the waste shows that about 59 percent of the total waste is organic matter. The average thermal energy generated from the total waste of Tabriz city is 7964.85 Kj/Kg. The results of the evaluation using the Leopold matrix showed that Scenario 1 (energy generation and composting) with a score of -170 was considered as the best scenario and scenario 4 (current status) with -214 points was the worst management scenario. In addition, Scenarios 2 and 3 are rated -203 and -181, respectively. Conclusion: In the present situation, planning and adopting appropriate measures to reduce negative environmental impacts, composing and energy generation can be appropriate for the management of MSW in Tabriz

    Comparison of Protaper, Medin and RaCe rotary files breaking time in vitro study with RC-prep Lubricant and Sodium Hypochlorite Washing

    Introduction: Root canal instrumentation is an important phase in root canal therapy. Since success in endodontic treatment depends on file defect and fracture, the purpose of this study is Compare of Protaper, Medin and RaCe rotary files breaking time in vitro study with Hypochlorite Lubricant and RC-prep washing. Materials & Methods: In this experimental study, first and second molar teeth, up and down humans with apex closure and channel curvature of less than 30 in three ProTaper groups (30 with hypochlorite washing and 30 with RC-prep) and 60 medin files were selected and examined. Data were analyzed by SPSS software version 22 using independent t-test and one way analysis of variance. Results: The results of this study showed that the average RaCe Rotary File Failure time is higher than Medin and Protaper Rotary Files in washing Hypochlorite; But in case of hypochlorite washing, there was no significant difference in the failure of Protaper Rotary files with Medin and RaCe with Medin and Protaper with RaCe; But when washing with RC-prep, the average Protaper rotary file failure rate was estimated more than the Medin and Race rotary files. Also, the results of the study showed that there is no significant difference in the failure time of failing rotary files of Medin with washing of hypochlorite and RC-prep; but there was a significant difference in the time of failure of the Protaper and Race rotary files. Conclusion: Based on the results of this study, the failure time of Protaper Rotary files by RC-prep washes and RaCe Rotary files are minimized by washing the hypochlorite. However, there was no difference in the failure of Medin rotary files when washing with hypochlorite and RC-prep
