    Medication Therapy Management Implementation in Conjunction with a Rural Health Clinic in Minnesota: A CQI Impact on Diabetes Metrics

    The purpose of this continuous quality improvement project was to implement a pharmacist-led Medication Therapy Management (MTM) clinic in rural, central Minnesota and assess the impact on patients’ healthcare over a six-month period. Patients were identified through the clinic’s diabetes registry, which included patients 18 through 75 years old with a diagnosis of diabetes not meeting all five D5 goals, as recommended by the Minnesota Community Measurements, independently referred by the provider, or be identified by their insurance company as a patient that would benefit from an MTM visit. The D5 focuses on blood pressure and blood sugar control, appropriate statin and aspirin use, and cessation of tobacco products. Referred patients were offered and scheduled for an appointment with the pharmacist. MTM visits were provided both in person and via telephone. The pharmacist identified and documented drug therapy problems (DTPs) and implemented changes, via the system’s clinical pharmacy collaborative practice agreement, to improve patient outcomes. All MTM visits, DTPs, and composite D5 scores were analyzed after the conclusion of the study period. Thirty-one visits were provided by the pharmacist at the clinic between October 1st, 2021 and March 31st, 2022, with 22 unique patients seen. Seventy-three DTPs were identified, with 69 DTPs documented as closed. Of the 22 unique patients seen, 13 patients were reviewed for D5 metrics. Referral processes were created, implemented, and modified over the six-month period to increase patient volume

    This work includes studies which are under the umbrella of the Natural Sciences. Areas of study such as Biology, Physics, and Astronomy all strive to better understand the natural world. When we wish to understand a particular phenomenon, it is often helpful to observe it through the lens of more than one discipline because they are interrelated and often interdependent. Physics, Astronomy, and Biology are interwoven throughout this project. In fact, some cited sources herein are co-authored by a biologist and a physicist. Astrophysicist, Neil DeGrasse Tyson, puts it this way, “Sciences, like people, undergo phases as they mature. What we know, and how we know it, changes at the same time. Until the 19th century, for example, biologists primarily concerned themselves with cataloging the species of life on Earth; today, on the other hand, they care about the molecular and physical processes that govern life. Biology, in other words, has absorbed chemistry and physics into its ranks (Tyson and Trefil 2021, 57).

Conclusions/Implications: Results from this QI project suggest a higher BMI class is associated with postoperative complications-wound infection. However, given the small sample size no conclusions could be drawn This QI project should be replicated on a larger scale The process for recruitment and intervention is established.     Catheter-Associated Urinary Tract Infection (CAUTI) rates increased in FY21 compared to FY20, and many units continue to not meet their National Database of Nursing Quality Indicators (NDNQI) target goal even through FY22. A group of CentraCare stakeholders gathered, including bedside nursing staff, urology techs and nursing leadership, to discuss barriers and areas for opportunity. Representatives from BARD®, our urinary catheter supply company, were invited to conduct an analysis on our urinary catheter/ CAUTI reduction practices.
