    P53 IHC Result as a Prognostic Tool in MDS

    Background & Objective: Some of the patients with myelodysplastic syndrome (MDS) are categorized as good prognosis based on the Revised International Prognostic Scoring System (IPSS-R). However, these patients may have poor clinical outcomes. It seems that the current diagnostic tools and IPSS-R cannot consider genetic factors for determining the prognosis of MDS patients. Methods: This cross-sectional study included all adult MDS patients of both genders who were admitted from March 2015 to March 2020 to the Hematology wards of two educational tertiary hospitals in Iran (Namazi and Faghihi, affiliated with Shiraz University of medical sciences). Study data included relevant retrospective data from medical records and the results of immunohistochemical p53 staining on bone marrow biopsies. Results: Of the 84 patients, 65 (77.4%) showed p53 expression in bone marrow. They had shorter median survival than those without p53 expression. Considering both variables of P53 IHC results and IPSS-R score, the patients who died with low-risk IPSS-R score presented high p53 expression. Conclusion: This study shows that the investigation of p53 expression by IHC at the time of diagnosis is a valuable indicator of survival rate in MDS patients. These data suggest that the immunohistochemical analysis of p53 can be a prognostic tool for MDS and should be used as an adjunct test to make decisions on the best therapeutic choice

    The perspectives and experiences of patients with COVID-19: a qualitative study

    Background. The expansion of COVID-19 and its unique transmission speed created an emergency situation in global health. This disease not only causes public health concerns but also has many psychological consequences for patients. In this situation, maintaining the mental health of individuals is essential. Therefore, focusing on the human aspects of the threat perceived by patients could lead the way for the provision of empathetic and effective care. Objectives. The aim of this study was to investigate the perspectives and experiences of patients with COVID-19. Material and methods. In this qualitative study, 27 patients with COVID-19 were purposefully selected and interviewed in semi-struc-tured in-depth manner. Interviews continued until data saturation. All interviews were recorded and analysed using the MAXQDA 10 software and the contractual content analysis method via the Graneheim and Lundman approach. The accuracy and robustness of the research was obtained based on Guba and Lincoln criteria. Results. The codes extracted from the interviews led to the emergence of two main themes including “negative emotions” and “sense of cohesion” and five categories of stigma experience, psychological distress, perceived threat, positive perception and spirituality. Conclusions. Individuals experience critical crises in the face of stressful life events, including COVID-19, from negative emotions to psychological adjustment. Going through the crisis stage and achieving stability and peace requires a change of attitude towards every-thing around the person and the use of spiritual forces to achieve adaptation

    A Systematic Review and Meta-analysis on the Role of Statins in the Prevention of Mortality Following Pancreatic Cancer

    BACKGROUND: Pancreatic cancer (PC) is a type of cancer with a high incidence and case-fatality rate. OBJECTIVE: This study aimed to evaluate the role of statins in preventing mortality following PC based on scientific evidence with systematic review and meta-analysis method. METHODS: This meta-analysis considered studies published from 1980 till the end of 2022 in ISI Web of Science, Scopus, PubMed, Cochrane, Science Direct, Google Scholar, and Embase databases. Funnel diagrams and Begg's and Egger's tests were used to assess the publication bias. RESULTS: In general, this meta-analysis has included 19 studies (13 cohort studies, 4 case-control, and 2 randomized clinical trials (RCTs)) and a total of 100,888 patients with PC. The risk of mortality of PC in statin users in total was 0.86 (95% CI: 0.80 - 0.92, P-value <0.001); in the case-control studies, it was equal to 0.53 (0.34-0.83); in the cohort studies, it was equal to 0.87 (0.82-0.92, P-value <0.001); in RCTs, it was equal to 1.19 (0.99-1.42, P-value <0.001); in studies with good quality score category, it was equal to 0.92 (0.86-0.99, P-value <0.001), and in articles of the moderate quality score category, it was equal to 0.73 (0.64-0.84, P-value [removed

    Trends of national and sub-national burden attributed to kidney dysfunction risk factor in Iran: 1990-2019

    Background: Kidney dysfunction is a risk factor for cardiovascular disease and chronic kidney disease. Herein, we aimed to describe the attributable burden of kidney dysfunction at the national and sub-national levels in Iran. Methods: The Global Burden of Disease (GBD) 2019 data were extracted on the deaths, disability-adjusted life years (DALYs), years of life lost, and years lived with disability attributed to the risk factor of kidney dysfunction by age and sex at the national and provincial levels from 1990-2019. Also, risk exposure was reported by summary exposure value (SEV) with a range of 0 to 100. The estimated values were based on a comparative risk assessment framework. Results: In 2019, the age-standardized death rate and age-standardized DALYs rate attributable to kidney dysfunction were 58.2 (95% uncertainty interval of 48.8-68.1) and 1127.2 (981.1-1282.7) per 100,000 population in Iran, respectively. Also, the Sistan and Baluchistan province (1729.3 [1478.3-2006.4]) and the province of Tehran (681.9 [571.4-809.8]) had the greatest and lowest age-standardized DALYs rates, respectively. Nationally, SEVs increased from 22.8 to 26.2. The age-standardized burden attributable to kidney dysfunction had a positive association with age advancement. The attributable age-standardized deaths and DALYs rates in all socio-demographic index regions decreased from 1990-2019. Also, the highest and lowest attributable age-standardized DALYs rates of kidney dysfunction came from ischemic heart disease and peripheral artery disease in 2019, respectively. Conclusion: Although the attributed age-standardized DALYs and death rates decreased from 1990-2019, risk exposure increased and remains a crucial risk factor in Iran. Therefore, policymakers should consider preparing a preventive program that takes into account different levels of prevention of kidney dysfunction

    The effect of implementing Peplau’s theory of interpersonal communication on the quality of life of patients with acquired immunodeficiency syndrome

    Background and objectives: Patients with AIDS face many problems, including problems related to treatment, social and family exclusion, high treatment costs, and drug complications, which affect the quality of life of these patients and change it. The aim was to determine the effect of the application of Peplau's theory of interpersonal communication on the quality of life of patients with acquired immunodeficiency syndrome. Material and method: This quasi-experimental study was performed on 50 AIDS patients referred to Shahrekord Behavioral Diseases Counseling Center. Simple random sampling was performed and then the sample was assigned to two groups experimental and control. The theory of therapeutic communication of Peplau on patients in the experimental group was performed individually and immediately after the intervention and three months later the quality of life questionnaire was completed in both groups. In this research, the data collection tool includes a demographic information questionnaire and a WHOQOL-BREF. The WHOQOLBREF questionnaire measures the four domains of physical health, mental health, social relationships, and environmental health with 24 questions. The chi-square or Fisher's exact test, independent T-test and analysis of variance with repeated measurements were used to compare the quality of life of patients. Results: Data analysis showed no statistically significant difference in the mean score of quality of life between the experimental and control groups before the implementation of Peplau's interpersonal communication theory (p=0.927). But after the intervention, there was a statistically significant difference in the mean score of quality of life between the two groups (p < 0.001). Conclusion: Findings of the study show the positive effects of using Peplau's therapeutic communication model on quality of life. Therefore, this method is recommended as an effective and cost-effective care model for all patients referred to the Shahrekord Behavioral Diseases Counseling Center.     The conservative management of a large hepatic subcapsular hematoma in a pregnant woman with preeclampsia: A case report

    Hepatic subcapsular hematoma is a rare but potentially life-threatening complication that is caused by preeclampsia and hemolysis, elevated liver enzymes, and low platelet (HELLP) syndrome, which may be manifest with nonspecific signs and symptoms. The present case was a 36-year-old woman with a secondary subcapsular liver hematoma as a rare complication of HELLP syndrome. The patient complained of nausea, vomiting, pain in the right upper quadrant of the abdomen, epigastric pain, and severe pain in the right shoulder. On the fourth day after delivery, a computed tomography (CT) scan was performed on the patient, showing a large subcapsular hematoma around the liver. Six weeks after delivery, the follow-up ultrasound exhibited no residual hematoma or free peritoneal fluid, and the patient’s blood pressure was controlled without taking medication.     The effect of combined administration of Ginger (Zingiber officinale Roscoe) and Depakene on pain reduction in patients with migraine headaches compared to Depakene alone

    Background and aims: Migraine is known as one of the most debilitating diseases with high prevalence worldwide. This study aimed to compare between combined administration of ginger and Depakene (sodium valproate) capsules (intervention group) and the use of Depakene (control group) alone to evaluate the therapeutic efficacy of ginger in the treatment of migraine. Methods: This randomized one-blind clinical trial was conducted with 80 patients suffering from migraine headaches. A total of 40 patients in the intervention group received two ginger capsules of 250mg manufactured by Zintoma (Gol Darou Co.) along with 500 mg Depakene orally daily for sixteen weeks, and 40 patients in the control group received Depakene (500 mg/d) alone. The variables included the severity of the headache, the number of headaches per month, and the sleep quality of patients. Data were analyzed using descriptive statistics: frequency, percentage, mean and standard deviation, and analytical statistics: �2 , independent t test, and pair t tests. Results: For pain intensity, the mean score of pain after the intervention in the intervention group was significant so that it was lower than the mean score in the control group (PPConclusion: Administration of the ginger capsule (500 mg) with Depakene (500 mg) was considered to improve pain severity, disability, and sleep pattern in patients with migraine compared to administration of Depakene alone. Therefore, this combination therapy can be considered a choice in the treatment of these patients

    Differentiation of human primary testicular cells in the presence of SCF using the organoid culture system

    Purpose: Development of organoids using human primary testicular cells has remained a challenge due to the complexity of the mammalian testicular cytoarchitecture and culture methods. In this study, we generated testicular organoids derived from human primary testicular cells. Then, we evaluated the effect of stem cell factor (SCF) on cell differentiation and apoptosis in the testicular organoid model. Methods: The testicular cells were harvested from the three brain-dead donors. Human spermatogonial stem cells (SSCs) were characterized using immunocytochemistry (ICC), RT-PCR and flow cytometry. Testicular organoids were generated from primary testicular cells by hanging drop culture method and were cultured in three groups: control group, experimental group 1 (treated FSH and retinoic acid (RA)), and experimental group 2 (treated FSH, RA and SCF), for five weeks. We assessed the expression of SCP3 (Synaptonemal Complex Protein 3) as a meiotic gene, PRM2 (Protamine 2) as a post-meiotic marker and apoptotic genes of Bax (BCL2-Associated X Protein) and Bcl-2 (B-cell lymphoma 2), respectively by using RT-qPCR. In addition, we identified the expression of PRM2 by immunohistochemistry (IHC). Results: Relative expression of SCP3, PRM2 and Bcl-2 were highest in group 2 after five weeks of culture. In contrast, BAX expression level was lower in experimental group 2 in comparison with other groups. IHC analyses indicated the highest expression of PRM2 as a postmeiotic marker in group 2 in comparison to 2D culture and control groups but not find significant differences between experimental group 1 and experimental group 2 groups. Morphological evaluations revealed that organoids are compact spherical structures and in the peripheral region composed of uncharacterized elongated fibroblast-like cells. Conclusion: Our findings revealed that the testicular organoid culture system promote the spermatogonial stem cell (SSC) differentiation, especially in presence of SCF. Developed organoids are capable of recapitulating many important properties of a stem cell niche

    Food insecurity, body mass index, socio-economic status, and food intake in lactating and non-lactating mothers with children under two years

    Background: food insecurity (FI) is considered as an essential public health problem which may have detrimental effects on people’s health. The aim of present study was to evaluate FI, body mass index, quantity and quality of food intake in lactating and non-lactating mothers with children under two years. Methods: in this cross-sectional study 307 mothers (237 lactating and 70 non-lactating) were participated. Socio-economic and demographic information were gathered by questionnaires. FI of families was evaluated by the questionnaire of the United States Department of Agriculture (USDA) Household Food Security. For assessing quality and quantity of food intake of mothers, dietary diversity score (DDS), diet quality index-international (DQI-I) and nutrient adequacy ratio (NAR) were calculated. Weight and height of participants were measured and body mass index (BMI) was calculated. Finally, Chi-squared test, analysis of variance (ANOVA) and linear regression were used for statistical analysis. Results: in this study the rate of underweight, normal weight, overweight, and obesity in mothers was 0.3%, 39.2%, 42.3%, and 18.2%, respectively. Among the determinants of BMI, household food security status had the greatest effect (Beta=-1.584, P < 0.001) and mother age had the least effect (Beta = 0.101, P = 0.013). Mother’s occupational and educational status, having facilities, physiological status of mother, and house size had significant correlation with NAR. Mother’s occupational and educational status, and having facilities had significant relationship with DDS, too. Also, the significant correlation of Mother’s education, having facilities, and physiological status of mother with DQI-I were found. Conclusions: we found that Household food security status had the most effect on BMI of mothers. In this study, the best nutrient adequacy and dietary diversity were found in the obese group and the most diet quality was found in the normal weight group
