    Can the Mortality Rate be Reduced in the Diabetes Induction Model in Rats? A Protocol Study

    The use of animal models of diseases is essential for the study of the effects of various drugs and the discovery of new drugs. One of the most common problems in researches on diabetes in animal models is the high rate of mortality after diabetes induction. This leads to disrupt the grouping of diabetic animals for interventional evaluations. We introduce a protocol to reduce the rate of mortality in diabetic rats. To do this, we used alloxan at dose of 185 mg/kg to induce diabetes in rats. In this study in addition to provide 5% glucose solution in drinking water from 2 to 12 hrs post alloxan injection, 2 ml of 50% dextrose solution orally gavaged with 2 hrs intervals up to 12 hrs post diabetes induction, and also from 12 to 24 hrs following alloxan injection 2 ml of 5% glucose solution was given by gavage every 2 hrs. Furthermore, from 6 to 48 hrs post alloxan injection the rats were orally received 1 ml of Ringer's serum with 2 hrs intervals via gavage route. Moreover, one unit of Novolin 70/30 (70% NPH and 30% insulin regular) was subcutaneously injected in the back of the neck area for 5 consistence days from the second day post alloxan injection. In the sixth day following alloxan injection the level of glucose was at level of 280 mg/dL which considered as diabetic level. Our findings showed that by abovementioned interventions the rate of mortality significantly reduced (2 percent) and diabetes well established

    The use of medicinal plants in the prevention of COVID-19 using the Health Belief Model: A survey based on the Iranian population

    BACKGROUND: Because of the spread of coronavirus disease 2019 (COVID-19), the preventive measures have increased, such as focusing on the use of medicinal plants in most communities, including Iran. The purpose of this study was to identify the knowledge, attitude, and performance of individuals toward the use of medicinal plants and to identify the predictors of the use of medicinal plants in the prevention of COVID-19. MATERIALS AND METHODS: This descriptive-analytical study (February-April 2021) was performed on 3840 Iranian men and women aged 20-70 years selected as a multi-stage cluster study. At the first stage, all provinces were divided into five regions: North, South, East, West, and Center. In the second stage, a provincial center and a city were randomly selected from each region (North: Sari, Babol; South: Bushehr, Bandar Genaveh; East: Mashhad, Sabzevar; West: Hamedan, Toisarkan; Center: Yazd, Ardakan). Data were collected by a researcher-made scale based on the Health Belief Model (HBM). Data analysis was performed applying Pearson correlation coefficient, logistic regression, and linear regression. RESULTS: The results showed that people have relatively high knowledge and positive attitude toward the use of medicinal plants in prevention of COVID-19. The most important reason for positive attitude was the perceived benefits with the mean of 75.06. Also, half of the people had poor performance. Correlation coefficient showed that the use of medicinal plants with perceived sensitivity (p = 0.000, r = 0.3), perceived benefits (p = 0.012, r = 0.126), perceived barriers (p = 0.000, r = 0.179), and perceived self-efficacy (p = 0.000, r = 0.305) had a significant correlation. The strongest correlation between perceived self-efficacy was observed with the use of herbs in prevention of COVID-19. The HBM constructs can predict 26 of the variance for the use of medicinal plants in the prevention of COVID-19, among which perceived self-efficacy (β = 0.230) was the most powerful predictor. CONCLUSION: Based on the results, the predictive role of self-efficacy constructs for the use of medicinal plants in prevention of COVID-19 has been confirmed according to the HBM. Therefore, methods of increasing self-efficacy such as training programs and providing appropriate intervention models can be used not only as promoters of using medicinal plants in prevention of COVID-19 but also for improving people's performance in the proper use of medicinal plants

    Factors influencing the delay in childbearing: A narrative review

    Background: Delay in parenthood and the related consequences for health, population, society, and economy are significant global challenges. This study was conducted to determine the factors affecting delay in childbearing. Materials and Methods: This narrative review was conducted in February 2022 using databases: PubMed, Scopus, ProQuest, Web of Science, Science Direct, Cochrane, Scientific Information Database, Iranian Medical Articles Database, Iranian Research Institute for Information Science and Technology, Iranian Magazine Database, and Google Scholar search engine. The search terms used included 'delayed childbearing,' 'delayed parenthood,' 'delayed fertility,' 'delay of motherhood,' 'parenthood postponement,' 'deferred pregnancy,' 'reproductive behavior,' and 'fertility.' Results: Seventeen articles were selected for final evaluation. The factors were studied at micro and macro levels. The factors in micro level fell into two classes: personal and interpersonal. Personal factors included extension of women's education, participation in the labor market, personality traits, attitude and personal preferences, fertility knowledge, and physical and psychological preparation. The interpersonal factors included stable relations with spouse and other important people. The macro level included supportive policies, medical achievements, and sociocultural and economic factors. Conclusions: Policy-making and enforcement of interventions, such as improvement of the economic conditions, increased social trust, providing adequate social welfare protection, employment, and support of families using such strategies as creating family-friendly laws, taking into consideration the conditions of the country will reduce the insecurity perceived by the spouses and contribute to a better childbearing plan. Also, improving self-efficacy, increasing couples' reproductive knowledge and modifying their attitude can be helpful to better decision-making in childbearing

    Determination and prioritization of factors affecting the occurrence of needle stick injuries among healthcare workers using techniques of Delphi and fuzzy analytical hierarchy process (FAHP)

    Introduction: Needlestick injuries (NSIs) are a major hazard in the workplace for healthcare workers. To prevent these injuries, it is essential to determine the important factors affecting the occurrence of them. This study aimed to identify, classify and prioritize these factors using techniques of Delphi and fuzzy analytical hierarchy process (FAHP). Methods: This descriptive-analytical study was conducted in 2022. Firstly, the factors affecting the occurrence of needlestick injuries were identified by the literature review. Moreover, the Delphi technique was used to identify the factors. 20 experts (physicians, nurses, and occupational health experts) participated in the steps of the Delphi method. Then, these factors were grouped into six groups. In the next step, the fuzzy analytical hierarchy process (FAHP) was applied to prioritize the factors. For this purpose, the pairwise comparison questionnaire was designed and filled out by 20 experts. Finally, data were analyzed using MATLAB software (version 2018a). Results: 42 factors (31 factors extracted from the literature review and 11 factors obtained from the Delphi technique) were identified in this study. These factors were categorized into six groups. Based on the results, the relative weight of non-demographic personal factors, tool and technology factors, job factors, organizational factors, demographic personal factors, and environmental factors were computed by 0.200, 0.185, 0.184, 0.157, 0.142, and 0.133, respectively. Conclusion: These results determined the importance of the factors affecting the occurrence of needlestick injuries. These findings can be useful for planning preventive measures

    The nephrotoxicity of Aristolochia rotunda L. in rats: Mitochondrion as a target for renal toxicity of Aristolochic acids-containing plants

    In recent years, there has been a growing trend in the usage of traditional medicine and herbal treatments. However, the misconception that they are completely safe resulted in irreversible complications and damages. The present study was conducted to investigate the potential renal toxicity of a commonly used drug in Iran's traditional medicine and pharmacy, known as Zaravand Gerd or Nokhod Alvand (Aristolochia rotunda L.). In Iranian traditional medicine, Zaravand Gerd is used as a remedy for respiratory system ailments, back pain, anxiety, headache and septic wounds. Fifty-six male rats were divided into seven groups (n = 8). The first group served as the control and received normal saline, while the second to seventh groups were administered varying doses of the aqueous extract of Zaravand Gerd (0.1, 0.5, 1.25, 2.5, and 5 g/kg) for a period of three weeks. Various parameters were measured to evaluate the potential kidney damage caused by the extract, including serum creatinine and BUN levels, as well as urine protein and glucose levels, which were analyzed using an autoanalyzer. Additionally, kidney tissue samples were examined pathologically, and mitochondria from the kidney tissue were isolated to assess mitochondrial parameters. The results of this study revealed that high doses of Zaravand Gerd extract led to a significant increase in urinary glucose and protein excretion compared to the control group. Pathological examination of the isolated kidney tissues indicated that the concentrations of 2.5 and 5 g/kg of Zaravand Gerd extract resulted in kidney damage and dilation of proximal convoluted tubules. Furthermore, the study demonstrated that high doses of the extract (2.5 and 5 g/kg) caused damage to the mitochondria. Based on the findings of this study, it can be concluded that the administration of high doses of Zaravand Gerd extract, which are not commonly used in traditional medicine, can have toxic effects on the kidneys in rats as an animal model. These results highlight the importance of considering the potential risks associated with herbal medicines and the necessity of usage based on scientific evidence

    Global, regional, and national incidence of six major immune-mediated inflammatory diseases: findings from the global burden of disease study 2019

    Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world. Innovative prevention and integrative management strategy are urgently needed to mitigate the increasing ASR of rheumatoid arthritis and upsurging new cases of other five IMIDs, respectively

    Ovarian cancer (OC) is described as a heterogeneous complex condition with high mortality, weak prognosis, and late-stage presentation. OC has several subgroups based on different indices, like the origin and histopathology. The current treatments against OC include surgery followed by chemotherapy and radiotherapy; however, these methods have represented diverse side effects without enough effectiveness on OC. Recently, mesenchymal stem cell (MSC)-based therapy has acquired particular attention for treating diverse problems, such as cancer. These multipotent stem cells can be obtained from different sources, such as the umbilical cord, adipose tissues, bone marrow, and placenta, and their efficacy has been investigated against OC. Hence, in this narrative review, we aimed to review and discuss the present studies about the effects of various sources of MSCs on OC with a special focus on involved mechanisms

    Evaluation of human error using HEART technique and its reduction based on fuzzy logic (Case study: a gas power plant)

    Background and aims: In complex systems, human error has always been identified as an effective factor in most accidents. In the power plant industry, various accidents occur due to human error, affecting the stability of the country’s electricity network and causing financial and human losses. The purpose of this study was to evaluate the human error of personnel operating a V94.2 gas power plant using the fuzzy HEART technique. Methods: This cross-sectional study was performed on gas plant operation personnel. The probability of human error was calculated using fuzzy logic in the HEART technique. The technique was performed in 7 stages: identifying the system/process, identifying existing tasks, assigning nominal probability of human error, identifying error-increasing conditions, estimating impact ratio, quantifying human error potential, and implementing control measures. Finally, the HEART method was improved using fuzzy logic. Results: A total of 13 tasks and 119 sub-tasks were obtained. 24 tasks had a high final human error probability (HEPF), representing 20 of total tasks evaluated. The final human error probability was calculated to be 7.213. Most tasks were in Group D (relatively simple task) of the HEART task general classification. “Insufficient checking” was considered as one of the error-enhancing conditions in more than 98 of tasks and had the highest repetition among error-enhancing conditions. “Checking the openness of the internal power supply’s disconnector” had the highest probability of human error among all activities. Training and justification of the operator or shift engineer were suggested as control solutions for 22 out of 24 tasks with a higher probability of human error. Conclusion: The results showed that training in performing tasks is necessary to reduce the possibility of human error. For tasks with a high probability of human error, retraining and justification of operators, attention to work instructions, and use of work rotation tools can be considered as important measures to prevent human error

    Shear Bond Strength of Colored and Sandblasted Zirconia to Ceramic Veneers Fabricated by the Pressing and Layering Techniques: An In Vitro Study

    Introduction. Porcelain-veneered zirconia (PVZ) restorations are increasingly used due to their optimal esthetics and high strength. However, chipping of porcelain limits the application of PVZ restorations. The aim of this study was to assess the shear bond strength (SBS) of colored and sandblasted zirconia to ceramic veneers fabricated by the pressing and layering techniques. Materials and Methods. Sixty cubic zirconia specimens (10 × 10 × 2 mm) were assigned to three groups according to their surface treatment: (I) control, (II) sandblasting with 50 μm alumina particles (S), and (III) coloring (C). Each group was subsequently divided into two subgroups according to the porcelain-veneering technique: (I) layering (L) and (II) pressing (P). The specimens underwent 10,000 thermal cycles between 5 and 55°C, and their biaxial SBS was measured in an electromechanical universal test machine (0.5 mm/min with 2.5 kN load cell). The failure mode was also assessed under a stereomicroscope. Three samples were randomly selected from each subgroup (n = 18) for examination of zirconia-phase transformation by X-ray diffraction (XRD). Two-way and one-way ANOVA followed by the post hoc Tukey test were used to analyze statistical differences among the groups and subgroups (α = 0.05). Results. The sandblasted zirconia with press porcelain (SP) subgroup showed the highest (24.40 ± 8.16 MPa) and the colored zirconia with press porcelain (CP) subgroup showed the lowest (13.76 ±3.62 MPa) SBS.     Burden of breast cancer and attributable risk factors in the North Africa and Middle East region, 1990–2019: a systematic analysis for the Global Burden of Disease Study 2019

    Background: Breast cancer (BC) is the most common cancer in women globally. The North Africa and Middle East (NAME) region is coping hard with the burden of BC. We aimed to present the latest epidemiology of BC and its risk factors in this region. Methods: We retrieved the data on BC burden and risk factors from the Global Burden of Disease Study 2019 to describe BC status in the 21 countries of the NAME region from 1990 to 2019. We explored BC incidence, prevalence, deaths, disability-adjusted life years (DALYs), and attributable burden to seven risk factors of female BC, namely, alcohol use, diet high in red meat, low physical activity, smoking, secondhand smoke, high body mass index, and high fasting plasma glucose. Decomposition analysis on BC incidence trend was done to find out the contributing factors to this cancer’s growth. Results: In 2019, there were 835,576 (95% uncertainty interval: 741,968 to 944,851) female and 10,938 (9,030 to 13,256) male prevalent cases of BC in the NAME region. This number leads to 35,405 (30,676 to 40,571) deaths among female patients and 809 (654 to 1,002) deaths in male patients this year. BC was responsible for 1,222,835 (1,053,073 to 1,411,009) DALYs among female patients in 2019, with a greater proportion (94.9%) of burden in years of life lost (YLLs). The major contributor to female BC incidence increase in the past three decades was found to be increase in age-specific incidence rates of BC (227.5%), compared to population growth (73.8%) and aging (81.8%). The behavioral risk factors were responsible for majority of attributable female BC burden (DALYs: 106,026 [66,614 to 144,247]). High fasting plasma glucose was found to be the risk factor with the largest effect (DALYs: 84,912 [17,377 to 192,838]) on female BC burden. Conclusion: The increasing incidence and burden of BC in the NAME region is remarkable, especially when considering limited resources in the developing countries of this region.
