Sr. Madonna Bishop
In her oral history, Sr. Madonna Bishop discusses her life as a Sister of Saint Francis, having a unique background as both a school teacher and a licensed practical nurse (LPN). Initially reluctant to enter the religious life, Sr. Bishop became open to becoming a sister after her encounter with the sisters of Saint Mary’s Academy, Indianapolis, whom she described as having a “sincere goodness.” Awarded degrees in English and Reading, Sr. Bishop taught numerous subjects at the grade school and high school levels before returning to school at the age sixty-four to become an LPN, completing an internship at Wishard Hospital in Indianapolis. As a nurse, Sr. Bishop cared for her mother and, for thirteen years, her beloved sisters of Saint Francis in the infirmary at Oldenburg. Three libraries in Indianapolis provide a unique example of collaboration by hosting a National Library of Medicine traveling exhibit at three locations within the city. Hosting the exhibit, “Harry Potter’s World,” will provide greater awareness of the libraries and their host institutions through a fun educational atmosphere and bring attention to National Library of Medicine resources through Harry Potter stories
Bovine Aortic Arch Shortens Available Clamping Distance for Extended End-to-End Repair of Infant Coarctation of the Aorta
Bovine aortic arch is an anatomical variant in which the innominate and left common carotid arteries share a common origin. It is generally considered to be benign. The current literature estimates its prevalence may be as high as 30%. Coarctation of the aorta is a congenital heart defect characterized by narrowing of the aortic arch, usually near the insertion of the ductus arteriosus. Untreated coarctation of the aorta can lead to hypertension, poor perfusion of the lower extremities, and left ventricular hypertrophy and failure. Resection with extended end-toend anastomosis from a left thoracotomy remains the standard of care in the absence of arch hypoplasia. Extended end-to-end anastomosis requires the placement of a proximal clamp just distal to the innominate artery and a distal clamp on the descending aorta. The distance between the proximal clamp and the coarctation represents the amount of vessel the surgeon has available for reconstructing the aorta with this technique. Student Use of Virtual Microscopy Laboratory Modules in an Integrated Medical Curriculum
In conjunction with recent curricular changes at US medical schools, histology instruction has undergone significant changes. One major change has been the steady shift from traditional microscopy laboratory sessions to the integration of virtual microscopy programs. This shift has been matched by a decrease in total histology course contact hours, in part due to incorporation of histology into integrated medical curricula. Histology at MUCOM is fully integrated into a systems-based curriculum during the pre-clinical years. Additionally, self-directed virtual histology laboratory modules created in Digital Slidebox take the place of formal microscopy laboratory sessions. These modules were designed to be interactive, collaborative, opennote/ open-book, and provide both formative and summative feedback via practice and graded quizzes respectfully. The purpose of this project was to determine if students utilize the self-directed virtual microscopy laboratory modules in the way they were designed to be used and explore any discrepancies. Two voluntary, anonymous Qualtrics surveys were used to collect data on how students use the virtual microscopy laboratory modules within the NEMS course during the Spring 2016 semester. 68 students responded to the first survey which focused on the first three histology lectures; 33 students responded to the second survey which focused on the last three histology lectures. Results indicated that students do favor group study with more students working in groups for the graded quizzes compared to the practice quizzes. Most students referred to the lecture PowerPoints while working on the quizzes; and about two-thirds of students used Google search. Surprisingly, about 80% of students prefer to take the practice quiz only once. Additionally, students who only occasionally worked in study groups found the practice quizzes more difficult than those who always or never worked in groups. Additional studies are planned to investigate patterns of use over time throughout the pre-clinical curriculum
Loss of the Nutrient Sensor Tas1R3 Leads to Reduced Bone Resorption
The taste receptor type 1 (TAS1R) family of heterotrimeric G protein-coupled receptors participates in monitoring energy and nutrient status. TAS1R member 3 (TAS1R3) is a bi-functional protein that recognizes amino acids such as L-glycine and L-glutamate or sweet molecules such as sucrose and fructose when dimerized with TAS1R member 1 (TAS1R1) or TAS1R member 2 (TAS1R2), respectively. Loss of TAS1R3 expression causes impaired mTORC1 signaling and increased autophagy, indicating that signaling through this receptor is critical for assessing nutrient needs. It was recently reported that deletion of TAS1R3 expression in Tas1R3 mutant mice leads to increased cortical bone mass and trabecular remodeling, but the underlying cellular mechanism leading to this phenotype remains unclear. The cellular mechanism of the increased bone mass was investigated by independently confirming that TAS1R family members are natively expressed in bone and that Tas1R3 mutant mice exhibit high bone mass. Cells of the bone-forming lineage in vitro express Tas1R family members, but levels of the bone formation marker Procollagen Type I N-terminal Propeptide are unchanged in the serum of 20-week-old Tas1R3 mutant mice as compared to controls. In contrast, levels of the bone resorption marker Collagen Type I C-telopeptide are reduced greater than 60% in Tas1R3 mutant mice. Consistent with this, Tas1R3 and Tas1R2 are expressed in primary osteoclasts and their expression levels positively correlate with differentiation status. Collectively, these findings suggest that high bone mass in Tas1R3 mutant mice is due to uncoupled bone remodeling with reduced osteoclast function and provide rationale for future experiments examining the cell type-dependent role for TAS1R family members in nutrient sensing in postnatal bone remodeling
Loss of BMPR2 Expression in Skeletal Progenitor Cells Reduces Age-Related Bone Loss
Osteoporosis is a disease of low bone mineral density that affects 10 million Americans and accounts for 1.5 million fractures annually. With an additional 34 million Americans at risk for developing the disease, osteoporosis is both a significant health problem and a considerable socioeconomic burden. Current first-line therapies involve anti-resorptive agents but many patients would benefit from augmenting bone formation as well as inhibiting bone loss. We recently reported that targeted deletion of the type 2 BMP receptor BMPR2 in skeletal progenitor cells of the limb bud using Prx1-Cre (Bmpr2-cKO mice) leads to dramatically increased bone mass and bone formation rate by ten weeks of age in the absence of changes in osteoclast number or function (Lowery et al 2015). In the present study, we examined the impact of Bmpr2 deletion on age-related bone loss in Bmpr2-cKO mice. Consistent with our previous results, 55-week-old female Bmpr2-cKO mice exhibit approximately four-fold higher bone mass in the tibia than control mice. Moreover, the age-related decline in bone mass from 15 weeks to 55 weeks of age in female Bmpr2-cKO mice is reduced approximately twofold compared to control mice. Bone mass of the L5 vertebrae, which is outside the Prx1-Cre expression domain, is unchanged in Bmpr2-cKO mice compared to control mice at all ages examined. Quantification of the serum bone turnover markers Procollagen Type I N-terminal Propeptide (PINP) and Collagen Type I C-telopeptide (CTx) suggest that high bone mass in aging female Bmpr2-cKO mice is preserved due to a sustained increase in bone formation rate to at least 35 weeks of age with no alteration in bone resorption. Collectively, our findings provide insight into the mechanisms regulating age-related bone loss and suggest that strategies aimed at controlling signaling through BMPR2 have the potential to impact bone mass in the aging adult skeleton
Not a Little Confusing : Francis Silvestri of Ferrara\u27s Hybrid Thomist Doctrine of Analogy
Fifty-plus years ago, Ralph McInerny’s The Logic of Analogy characterized Francis Silvestri of Ferrara’s doctrine of analogy as a confusing hybrid of the thought of Thomas Aquinas and of Thomas Cajetan. Since then, scholarship on fifteenth-century Thomism has flourished, thanks especially to the efforts of Ashworth, Bonino, Hochschild, Riva, and Tavuzzi. In light of these decades of scholarship, in this article I reconsider Francis Silvestri’s doctrine of analogy. I attempt to show the merits of his contribution to the Thomist tradition’s ongoing reflection on analogy, especially the dispute among Thomists and with Scotists over abstracting an analogous concept, the unity of the concept used analogously, and the use of analogy in demonstration. I argue that Francis’s hybrid succeeds in finding a place for analogy of attribution in names said analogously of God and creatures while still meeting Cajetan’s standards for answering Scotist objections to demonstration through analogous terms
Accessibility of ethics education and the formation of Catholic healthcare providers
Specifically in the medical field, Catholic providers rarely receive extensive education regarding the applications of moral theology to their work. Those knowledgeable on the topic have often gone to great lengths to acquire such formation. Both crises have a direct impact on the medical field as tensions rise between demands within the profession and providers’ beliefs
A method to switch from oral dopamine agonists to rotigotine in patients with restless legs syndrome and mild augmentation.
BACKGROUND: We examined the short- and long-term efficacy and tolerability of a cross-titration algorithm from oral dopamine agonists to the rotigotine transdermal patch in patients dissatisfied with their restless legs syndrome (RLS) treatment, predominantly with mild augmentation.
METHODS: Patients with RLS (n = 20) were recruited at a single site. The cross-titration consisted of decreasing oral dopaminergic agents (ropinirole by 1 mg or pramipexole by 0.25 mg) and increasing rotigotine by 1 mg every two days. Efficacy and adverse events (AEs) were assessed at one, three, six and 12 months after the switch.
RESULTS: Patients had moderate-severe RLS symptoms at the baseline (mean international restless legs syndrome (IRLS) score 19.4 ± 5.5); 85% had augmentation and 45% reported afternoon RLS symptoms. The baseline mean pramipexole equivalent dose was 0.6 ± 0.3 mg. At Week 5, 85% (17/20) had successfully switched from their oral dopamine agonist to rotigotine (mean dose 2.5 ± 0.6 mg; change in IRLS score: -6.7 ± 8.4, p = 0.002); 14 patients were CGI-I responders (much or very much improved). Three patients withdrew due to lack of efficacy. Twelve months after cross-titration, 10 patients continued on rotigotine, of whom four required either higher doses of rotigotine or supplemental RLS medication compared with their optimal Week 5 dose; five patients withdrew due to AEs and two due to lack of efficacy.
CONCLUSION: A cross-titration to rotigotine was efficacious after five weeks in 70% of patients dissatisfied with RLS treatment, most of whom had mild augmentation. At one year following the medication switch, 50% had discontinued rotigotine due to lack of continued efficacy or side effects
