    The role of microRNAs in lung cancer: Implications for diagnosis and therapy

    Lung cancer is the first cause of cancer death in the world due to its high prevalence, aggressiveness, late diagnosis, lack of effective treatment and poor prognosis. It also shows high rate of recurrence, metastasis and drug resistance. All these problems highlight the urgent needs for developing new strategies using non-invasive biomarkers for early detection, metastasis and recurrence of disease. MicroRNAs (miRNAs) are a class of small noncoding RNAs that regulate gene expression post-transcriptionally. These molecules found to be abnormally expressed in increasing number of human disease conditions including cancer. miRNAs could be detected in body fluids such as blood, serum, urine and sputum, which leads us towards the idea of using them as non-invasive biomarker for cancer detection and monitoring cancer treatment and recurrence. miRNAs are found to be deregulated in lung cancer initiation and progression and could regulate lung cancer cell proliferation and invasion. In this review, we summarized recent progress and discoveries in microRNAs regulatory role in lung cancer initiation and progression. In addition, the role of microRNAs in EGFR signaling pathway regulation is discussed briefly. © 2020 Bentham Science Publishers

    The therapeutic potential of quercetin in parkinson�s disease: Insights into its molecular and cellular regulation

    Parkinson�s disease (PD) is a chronic and progressive neurodegenerative disorder characterized by the progressive death of dopaminergic neurons in the substantia nigra pars compacta (SNc). PD is a multifactorial disorder, with several different factors being suggested to play a synergistic pathophysiological role, including oxidative stress, autophagy, underlying pro-inflammatory events and neurotransmitters abnormalities. Overall, PD can be viewed as the product of a complex interaction of environmental factors acting on a given genetic background. The importance of this subject has gained more attention to discover novel therapies to prevent as well as treat PD. According to previous research, drugs used to treat PD have indicated significant limitations. Therefore, the role of flavonoids has been extensively studied in PD treatment. Quercetin, a plant flavonol from the flavonoid group, has been considered as a supplemental therapy for PD. Quercetin has pharmacological functions in PD by controlling different molecular pathways. Although few studies intended to evaluate the basis for the use of quercetin in the context of PD have been conducted so far, at present, there is very little evidence available addressing the underlying mechanisms of action. Various principal aspects of these treatment procedures remain unknown. Here, currently existing knowledge supporting the use of quercetin for the clinical management of PD has been reviewed. © 2020 Bentham Science Publishers

    Antiurolithiatic effects of pentacyclic triterpenes: The distance traveled from therapeutic aspects

    Globally, approximately 12 of the population is inflicted by various types of urolithiasis. Standard treatments are available both to avert and treat urolithiasis, but with significant adverse side effects. Pentacyclic triterpenes represent a group of naturally occurring compounds which holds immense potential as therapeutic for treating kidney stone. This review aims to provide an integrative description on how pentacyclic triterpenes can effectively treat calcium oxalate urolithiasis through various mechanisms such as antioxidant, anti-inflammatory, diuretic, and angiotensin-converting enzyme inhibition. Some of the pentacylic triterpenes which shows promising activities include lupeol, oleanolic acid, betulin, and taraxasterol. Moreover, future perspectives in the development of pentacyclic triterpenes in formulations/drugs for urinary stone prevention are highlighted. It is anticipated that compiled information would serve as a scientific baseline to advocate further investigations on the potential of pentacyclic triterpenes in urolithiasis remediation. © 2020 Wiley Periodicals, Inc

    The elevation of S100B and downregulation of circulating miR-602 in the sera of ischemic stroke (IS) patients: the emergence of novel diagnostic and prognostic markers

    Ischemic stroke (IS) is a major cause of mortality and disability. However, no reliable prognostic or diagnostic biomarker has been utilized to date. Here, we have evaluated the serum S100B concentration and miR-602 expression as potential biomarkers for IS. Fifty-two IS patients and 52 age- and sex-matched healthy volunteers were enrolled. Blood samples were collected from all patients at the time of admission, 24 and 48 h later, at the time of discharge, and 3 months later. Real-time (RT) PCR was used to measure the serum level of miR602. We also measured the serum concentration of S100B using ELISA. As compared with healthy subjects, IS patients had a higher level of serum S100B and lower serum miR-602. ROC curve analyses revealed that miR-602 (AUC = 0.8168; P < 0.0001) and S100B (AUC = 0.8699; P < 0.0001) had acceptable ability to differentiate between IS patients from healthy subjects. Furthermore, serum S100B was a reliable predictor of the survival outcome at 3 months (P = 0.021). The expression of miR-602 was significantly higher in patients with bigger NIHSS scores. The lower levels of miR-602 and higher concentration of S100B in the sera of IS patients could be associated with clinically significant diagnostic utilities. S100B could be also introduced as a reliable prognostic marker for stroke and implemented in future research. © 2020, Fondazione Società Italiana di Neurologia

    Electrochemical-Based Biosensors: New Diagnosis Platforms for Cardiovascular Disease

    One of the major reasons for mortality throughout the world is cardiovascular diseases. Therefore, bio-markers of cardiovascular disease are of high importance to diagnose and manage procedure. Detecting biomarkers provided a promising procedure in developing bio-sensors. Fast, selective, portable, accurate, inexpensive, and sensitive biomarker sensing instruments will be necessary for detecting and predicting diseases. One of the cardiac biomarkers may be ordered as C-reactive proteins, lipoprotein-linked phospho-lipase, troponin I or T, myoglobin, interleukin-6, interleukin-1, tumor necrosis factor alpha, LDL and myeloperoxidase. The biomarkers are applied to anticipate cardio-vascular illnesses. Initial diagnoses of these diseases are possible by several techniques; however, they are laborious and need costly apparatus. Current researches designed various bio-sensors for resolving the respective issues. Electrochemical instruments and the proposed bio-sensors are preferred over other methods due to its inexpensiveness, mobility, reliability, repeatability. The present review comprehensively dealt with detecting biomarkers of cardiovascular disease through electro-chemical techniques.     The effect of aromatherapy with Rosa damascena essence on postoperative pain in inguinal hernia repair: A randomized clinical trial

    Background: Despite therapeutic interventions, patients are still dissatisfied with the treatment of postoperative pain. Objectives: The aim of this study was to examine the effect of Rosa damascena essential oil on postoperative pain in patients undergoing inguinal hernia repair surgery. Methods: In a double-blind, randomized trial, 60 patients undergoing inguinal hernia repair surgery were selected consecutively and equally assigned to two groups of experimental and placebo. In both groups, the pain intensity was measured by a visual analog scale in 4, 8, and 12 h after the surgery. Patients in the experimental group received aromatherapy with R. damascena essential oil and those in the placebo group treated with almond oil. In both groups, pain intensity was measured before and 20 min after aromatherapy. Data analysis was performed using the repeated-measures analysis, analysis of covariance, independent-samples t- test, Chi-square, and Fisher's exact tests. Results: The mean pain intensity in the experimental and the placebo groups was 7.10 ± 1.24 versus 7.20 ± 1.10, 4.56± 1.04 versus 4.90 ± 0.84, and 1.30 ± 0.79 versus 2.46 ± 0.68 after the 4th, 8th, and 12th postsurgical hours, respectively. The repeated-measures analysis showed that the intervention was effective on pain reduction, especially in the 8th and 12th postoperative hours when the pain was at moderate-to-mild levels (P < 0.001). Conclusion: Aromatherapy with R. damascena essential oil was effective in relieving mild to moderated postoperative pain. Yet, further studies are needed to confirm this finding

    Development of safety improvement method in city zones based on road network characteristics

    Background and Objective: Extensive studies have so far been carried out on developing safety models. Despite the extensive efforts made in identifying independent variables and methods for developing models, little research has been carried out in providing amendatory solutions for enhancing the level of safety. Thus, the present study first developed separate accident frequency prediction models by transportation modes, and then in the second phase, a development of safety improvement method (DSIM) was proposed. Materials and Methods: To this end, the data related to 14,903 accidents in 96 traffic analysis zones in Tehran, Iran, were collected. To evaluate the effect of intra-zone correlation, a multilevel model and a negative binomial (NB) model were developed based on both micro- and macro-level independent variables. Next, the DSIM was provided, aiming at causing the least change in the area under study and with attention to the defined constraints and ideal gas molecular movement algorithm. Results: Based on a comparison of the goodness-of-fit measures for the multilevel model with those of the NB model, the multilevel models showed a better performance in explaining the factors affecting accidents. This is due to considering the multilevel structure of the data in such models. The final results were obtained after 200 iterations of the optimization algorithm. Thus, to decrease accidents by 30 and cause the least change in the area under study, the independent variable of vehicle kilometer traveled per road segment underwent a considerable change, while little change was observed for the other variables. Conclusions: The final results of the DSIM showed that the ultimate solutions derived from this method can be different from the final solutions derived from the analysis of the results from the safety models. Hence, it is necessary to develop new methods to propose solutions for increasing safety

    Prevalence of needlestick injuries among health-care workers in iranian hospitals: An updated systematic review and meta-analysis

    Background: Needlestick and sharp injuries have been identified as the most dangerous factor among health-care workers. The Centers for Disease Control and Prevention estimates that 385,000 health-care workers experience needlestick each year. Methods: The aim of this study was to determine the prevalence of needlestick injuries (NSIs) in Iranian health-care workers. In this review and meta-analysis, the local and international databases such as Scopus, Medline, PubMed, ScienceDirect, Web of Science, Google Scholar, Scientific Information Database (SID), and Magiran were searched using keywords including prevalence OR needle OR needle stick OR Sharp injury OR Iranian personnel's OR Iranian health care workers OR Iranian hospitals OR Iran. The original researches that determined the prevalence of NSIs among Iranian health-care workers and published from January 2005 to June 2019 were included in the current study. The pooled prevalence of NSIs was determined using a random-effects model with a 95 confidence interval. All analyses were performed using STATA version 11 (Stata Corporation, College Station, TX, USA). P Results: The overall prevalence of NSIs among Iranian health-care personnel was about 50.8 (46.3-55.2). Furthermore, this prevalence in educational, noneducational, both noneducational and educational, and military centers was about 51.1 (46.5-57.7), 40.4.1 (34.2-46.6), 61.0 (32.1-89.9), and 41.5 (23.0-60.0), respectively. The prevalence of NSIs among the nurses was 51.1 (45.4-56.8), which was more than other groups. Conclusions: The prevalence of NSIs in Iranian hospitals was high. Since most of the injuries are caused by nurses, more intervention programs should be designed for nurses in these wards

    Thromboelastography in different mechanisms of injuries/organ injuries in traumatized patients in Southern Thailand

    Background: Trauma is the second most common cause of death in Thailand, relatively with massive blood loss. Coagulopathy plays a role in blood loss. Differences in mechanisms and organs injured can affect coagulopathy stage and blood loss. Thromboelastometry is a measuring instrument for accurate and rapid detection of coagulopathy. We hypothesized that thromboelastometry in traumatized patients who require surgery in Songklanagarind Hospital will help with early detection of coagulopathy and assess anticipated blood loss. Methods: After approval from the Institutional Ethics Committee, patients aged above 18 years who had American Society of Anesthesiologists Physical Status (ASA) IEVE, activated by trauma team and sent for emergency surgery. Anesthesia was induced and maintained, and invasive procedures were done as anesthesiologist's consideration. Thromboelastometry, prothrombin time (PT), partial thromboplastin time (PTT), complete blood count, platelets, arterial blood gas, lactate, and base deficit were assessed and recorded at the emergency room and after Massive Transfusion Protocol (MTP) was activated at 1 and 2 MTP, respectively. Results: Most traumatized patients who were operated on in Songklanagarind Hospital during the study period were male, the most common cause of their injuries was a motorcycle accident, and the most common organ associated with massive blood loss and blood transfusion was the head. After the patients received PRCs of 5 and 11 units, the hematocrit level and platelet count decreased from baseline, while PT and PTT were prolonged. pH, base deficit, and lactate were worse. Clot formation time (CFT), A10, and maximum clot firmness (MCF) of EXTEM were statistically significantly different among the three time periods. Clotting times, CFT, A10, and MCF of INTEM were statistically significantly different among three time periods. A10 and MCF of FIBTEM were statistically significantly different among three time periods. Conclusion: Most baseline laboratory tests in the traumatized patients who received massive blood transfusion and underwent surgery were worse after they received 1 and 2 MTP. These parameters including thromboelastometry could be guided for preparing proper blood components for patients requiring massive transfusion

    A comparative study on the ability of trauma severity-assessing methods in determining the prognosis of patients having accident and referred to Imam Khomeini Hospital of Urmia in 2016

    Introduction: Trauma is the leading cause of death and disability in developing countries. According to the World Health Organization projections by 2020, accidents alone are the second leading cause of missing year around the world. Therefore, the aim of this study was to compare the ability of trauma severity-assessing methods in determining the prognosis of accident patients referred to Imam Khomeini Hospital of Urmia in 2016. Methodology: In this study, the severity of trauma in traffic accident patients referring to Imam Khomeini Hospital of Urmia was determined using the Injury Severity Score (ISS), Revised Trauma Score (RTS), Trauma and Injury Severity Score (TRISS), and A Severity Characterization of Trauma (ASCOT), in a 1-year period, via the census method on 2015 hospitalized accident victims, and the ability of these methods to predict mortality and morbidity, length of stay in hospital, and hospital length charges were investigated by the receiver operating characteristic curve of regression methods. To this end, the hospital records of accident victims who were hospitalized for at least 1 day or whose deaths were examined along with their autopsy reports were the main components of this study. Results: The mean age of the patients was 33.63 ± 18.53 years, and their age range was 1-96 years. The ratio of males to females was 2.73. In addition, in terms of job status (46.8), they were free. The majority of women were homemakers (70.8). In the two gender groups, most accidents occurred within the city, The mean severity of lesions based on ISS system was 16.44 ± 16.28 and 16.8 of ISS injuries were above 25. The mean RTS of the patients was 7.69 ± 0.54, which ranged from 0 to 7.84, and the mean TRISS of the patients was 92.24 ± 15.87. The trauma means via the ASCOT method was 5.35 ± 1.85. The difference in the severity of trauma (calculated by each of the methods of ISS, RTS, TRISS, and ASCOT) was significant between those who survived and those who died. Conclusion: In this study, most of the accidents happened in males, married, undergraduate, and homemaker, and those on the street as well, and the RTS and TRISS methods showed the highest ability in predicting mortality. Given the usefulness and special applications of these methods, their use in designing a national care system in the trauma area is recommended
