    Implementación de la simulación in situ en un hospital pediátrico de alta complejidad durante la pandemia covid-19

    Introducción: El aprendizaje basado en simulación se ha usado ampliamente para mejorar la respuesta de los integrantes del equipo de salud ante situaciones de crisis. La pandemia por COVID-19 planteó el desafío de utilizar la simulación in situ como estrategia de capacitación. Objetivo: Describir el impacto del entrenamiento en RCP en pacientes COVID-19 mediante simulación in situ según la autopercepción del equipo de salud participante y comparar los resultados entre aquellos que hicieron un taller previo de manejo de vía aérea y RCP en pacientes con SARS-CoV -2 y los que no lo realizaron. Material y Método: Estudio exploratorio-descriptivo, desde marzo a junio de 2020, por medio de un cuestionario anónimo estandarizado a los participantes luego de 30 escenarios de simulación in situ. Las variables cualitativas se registraron con valores del 0 al 5 donde 1: nada; 2: bajo; 3: medio; 4: alto y 5: máximo. Resultados: De los 55 participantes, el 38 % (n=21) habían realizado previamente un taller de manejo de vía aérea avanzada y RCP en paciente COVID-19. El 40,7% expresó que disminuyó el temor a la asistencia de un paciente sospechoso o positivo en un nivel máximo (31,5% nivel alto). 81,5 % manifestó que le sirvió en grado máximo para reconocer la importancia del trabajo en equipo y designación de roles (13% grado alto). Para el 65% tuvo una utilidad máxima (18,5% alta) en adecuación del carro de paro y elaboración de un kit específico. Se halló diferencia significativa en la disminución del temor a la asistencia al comparar el nivel de respuesta entre los que habían realizado un taller previo versus los que no ( p= 0,013). Conclusión: La simulación in situ resultó ser una herramienta útil que ayudó en gran medida a disminuir el temor, mejorar habilidades comunicacionales y el trabajo en equipo. Los resultados obtenidos fueron aún mejores cuando el escenario fue complementado con un taller de manejo de vía aérea avanzada y RCP.Introduction: Simulation-based learning has been widely used to improve the response of health team members to crisis situations. The COVID-19 pandemic posed the challenge of using on-site simulation as a training strategy. Objective: Analyze the impact of CPR training in COVID-19 patients in situ simulation according to the self-perception of the health team and compare the results between those who did a previous workshop and those who did not. Material and Method: Exploratory-descriptive study, from March to June 2020, through a standardized anonymous questionnaire to the participants after 30 simulation scenarios in situ. The qualitative variables were registered with values from 0 to 5 where 1: nothing; 2: low; 3: medium; 4: high and 5: maximum. Results: Of the 55 participants, 21 (38%) have previously conducted a workshop on advanced airway management and CPR in a COVID- 19 patient. 40,7% expressed that the fear of attending a suspicious or positive patient decreased at a maximum level (31,5% high level). 81,5% stated that it served them to a maximum degree to recognize the importance of teamwork and role designation (13% high degree). For 65% it had a maximum utility (18.5% high) in adapting the stop car and making a specific kit. When we compared the subgroup that carried out the previous workshop with the one that did not, differences were found in most of the variables, highlighting the decrease in fear. Conclusion: The in situ simulation turned out to be a useful tool that greatly helped reduce fear and improve communication skills and teamwork. The results were obtained even better when the scenario was complemented with an advanced airway management and CPR workshop

    ECOPAMPA: A new tool for automatic fish schools detection and assessment from echo data

    Accurate identification of aquatic organisms and their numerical abundance calculation using echo detection techniques remains a great challenge for marine researchers. A software architecture for echo data processing is presented in this article. Within it, it is discussed how to obtain energetic, morphometric and bathymetric fish school descriptors to accurately identify different fish-species. To accomplish this task it was necessary to have a development platform that allowed reading echo data from a particular echosounder, to detect fish aggregations and then to calculate fish school descriptors that would be used for fish-species identification, in an automatic way. This article also describes thoroughly the digital processing algorithms for this automatic detection and classification, as well as the automatic process required for surface and bottom line detection, which is necessary to determine the exploration range. These algorithms are implemented within the ECOPAMPA software, which is the first Argentinean system for marine species identification. Finally, a comparative result over experimental data of ECOPAMPA against EchoviewTM Software Pty Ltd (formerly Myriax Software Pty Ltd), is carefully examined

    Evaluation of the Health Impact of the National Diabetes Program in Paraguay

    The clinical evolution of a diabetic patient is linked to its compliance with non-pharmacological measures such as diet, physical activity, acquiring habits, medical controls and also with its pharmacological treatment. In different Latin American countries such as the Republic of Paraguay, there are National Programs to provide free support and treatment to the diabetic population that mostly have vulnerable social conditions. However, the impact of these government policies is not routinely evaluated. In order to establish the achievements of a state diabetes program, the present study was carried out. Methodology: This is a descriptive, cross-sectional study with an analytical stage. Results: low adherence to non-pharmacological measures such as diet and physical activity was detected. A high percentage of patients were overweight (38%) or obese (46.5%). 75% of the population enrolled in this study did not comply with the program recommendations regarding the time and frequency of physical activities, nor they complied with the minimum 6 daily meals required. Only 37% of the patients had normal levels of glycated hemoglobin and 51.3% of them had pharmacological treatment adherence. Low compliance was observed in relation to regular medical examinations (which were under free coverage by the program) that are transposed in 22% of retinal studies and 60% in 24-hour urine analysis for the early detection of complications such as retinopathy and nephropathy. Conclusion: Despite the economic and logistical efforts carried out by State programs in order to care of people with diabetes mellitus, data showed a low adherence of patients to the proposed pharmacological and non-pharmacological measures. A personalized follow-up strategy is necessary to improve treatment adherence and to obtain better therapeutic results

    Caracterización hidrodinámica e hidroquímica de una cuenca rural utilizando una red monitora con perspectiva ambiental

    El presente trabajo se desarrolló en una cuenca rural del partido de Tandil, Provincia de Buenos Aires, Argentina. Su objetivo fue diseñar una red ambiental de monitoreo del recurso hídrico subterráneo en la cuenca del arroyo Chapaleofú, que integre, más allá de la mirada hidrológica clásica (hidrodinámica e hidroquímica), variables fisicoquímicas y de manejo del recurso en cada sitio de muestreo. De 104 perforaciones censadas se seleccionaron 36 aplicando en un SIG un mallado sobre la cuenca. En cada celda se seleccionó una perforación comparando las siguientes variables: accesibilidad, encamisado, mantenimiento, conductividad eléctrica, distancia a la perforación más cercana, distancia al baricentro de la celda, muestra directa del acuífero y toma de muestra y profundidad del nivel del agua en la misma perforación. La hidroquímica y la hidrodinámica se evaluaron en una campaña donde se tomaron muestras para análisis fisicoquímico y se midieron las profundidades del nivel del agua. Se relevaron variables de manejo del recurso (mantenimiento, estado, uso de suelo en el lote de la perforación y uso de suelo en el establecimiento) y se relacionaron con la potabilidad del agua. Se eliminaron otras 4 perforaciones que presentaron agua no potable. Se seleccionaron 20 perforaciones en el sector de serranías, 7 en el piedemonte y 5 en la llanura. La hidroquímica varió de bicarbonatada cálcica y/o magnésica en la serranía a bicarbonatada sódica en la llanura. No se encontró relación entre el manejo del recurso y su calidad, esto muestra la efectividad en la selección de sitios no contaminados.This work was developed in a rural basin of the Tandil district, Province of Buenos Aires, Argentina. Its objective was to design an environmental network for monitoring the groundwater resource in the Chapaleofú stream basin, which integrates, beyond the classical hydrological view that evaluates the hydrodynamic and hydrochemistry, physicochemical and resource management variables in each sampling site, in a basin located in a rural area. 104 perforations were recorded and 36 were selected by applying a mesh over the basin in a GIS. In each cell, a perforation was selected comparing the variables: accessibility, jacketing, maintenance, electrical conductivity, distance to the nearest perforation, distance to the center of the cell, direct sample from the aquifer, and sampling and water depth in the same drilling. Hydrochemistry and hydrodynamic were evaluated in a campaign in which samples were taken for physicochemical analysis and the depths of the water level were measured. The variables of resource management surveyed (maintenance, condition, use of soil in the drilling lot and use of soil in the establishment) were related to water potability. A second reduction of the perforations was made, removing four that presented poor quality. Twenty boreholes were selected in the hills sector, seven in the piedmont sector and five in the plain. The hydrochemistry ranged from calcium and / or magnesium bicarbonate in the hills sector to sodium bicarbonate in the plain. No relationship was found between resource management and water quality. This shows the effectiveness in the selection of uncontaminated sites

    Los cambios en el mundo del trabajo se inscriben en una nueva lógica de funcionamiento de las economías capitalistas, dando lugar a mercados laborales informales y precarios con una fuerte inestabilidad laboral y salarial. Estas transformaciones también se observan en la Economía Social, Solidaria y Popular, que se conforma en Argentina por un universo heterogéneo cuyo punto en común es la autogestión para la generación del trabajo y la conformación de estrategias socio-productivas alternativas a la economía de mercado. Estas experiencias productivas se encuentran expuestas a un mayor nivel de precariedad, a la desregulación de los vínculos laborales y a un proceso de intensificación de la jornada de trabajo que lleva a los trabajadores a enfrentar procesos de desgaste y a una mayor desprotección sobre los Riesgos Psicosociales en el Trabajo, en comparación con la empresa tradicional. En el siguiente trabajo nos proponemos como objetivo analizar los procesos de trabajo y los Riesgos Psicosociales en un grupo de emprendedoras asociativas textiles de la Economía Social, Solidaria y Popular de Moreno. Para ello, procuraremos identificar las principales características del proceso de trabajo de las emprendedoras asociativas textiles morenenses, así como los Riesgos Psicosociales a los que se encuentran expuestas. Siguiendo este objetivo, tomaremos el enfoque de los Riesgos Psicosociales y partiremos de una metodología cualitativa de trabajo basada en la observación participante y una serie de entrevistas en profundidad realizadas a emprendedoras asociativas textiles de Moreno e informantes claves de la localidad y del sector

    Diversity of aerobic spore-forming bacteria isolated from fresh bee pollen intended for human consumption

    Bee pollen is the result of the agglutination of pollen grains collected from flowers and mixed with nectar and salivary secretions by honey bees. Bee pollen is a natural product exposed to environmental conditions and also provides a unique microhabitat for yeasts and bacterial com munities. We analyzed 30 fresh bee pollen samples obtained from the main producing areas of Argentina to identify aerobic-spore-forming bacteria. We obtained 73 isolates belonging to 16 different species through isolation on selective and differential media, morphological and bio chemical tests, and PCR and RFLP analysis of genes encoding 16S rRNA. Our data revealed that Bacillus cereus sensu stricto was the most predominant species (50%), followed by Bacillus mega terium (40%) and Bacillus subtilis (40%), respectively. In a minor proportion, Paenibacillus poly myxa (20%), Paenibacillus larvae (17%), Bacillus pumilus (13%), Bacillus licheniformis (13%), Bacillus amyloliquefaciens (10%), Lysinibacillus sphaericus (7%), Bacillus coagulans (7%), Rummelliibacillus stabekisii (7%), Bacillus thuringiensis (7%), Bacillus clausii (3%), Paenibacillus alvei (3%), Bacillus simplex (3%), and Paenibacillus amylolyticus (3%) were also found. Our results showed that Argentinean bee pollen could transmit honey bee diseases due to the presence of viable spores of P. larvae and also spores of toxicogenic B. cereus s.s. and B. megaterium strains

    Inmunodeficiencia común variable: manifestaciones clínicas iniciales

    La inmunodeficiencia común variable (IDCV) es una inmunodeficiencia primaria caracterizada por hipogammaglobulinemia de comienzo tardío, que se manifiesta principalmente con infecciones recurrentes. Objetivo: describir las manifestaciones clínicas iniciales de pacientes con IDCV diagnosticados en el Hospital de Niños Sor Maria Ludovica entre 1981 y 2019. Diecinueve pacientes fueron incluidos. Todos los pacientes tenían historia de infecciones recurrentes, siendo las más frecuentemente observadas la neumonía (74%) y la otitis media (42%). Se documentó diarrea crónica en 9 pacientes (47%), con malabsorción asociada en 6 de ellos. El 32% de los pacientes presentó desnutrición severa y uno de ellos metaplasia gástrica. Un paciente presentó esplenomegalia y otro síndrome de Evans. Bronquiectasias fueron observadas en el 42% de los pacientes al diagnóstico. Conocer las manifestaciones clínicas iniciales de la IDCV es fundamental para el diagnóstico precoz y tratamiento oportuno.Common variable immunodeficiency (CVID) is a primary immunodeficiency characterized by late onset hypogammaglobulinemia, that can manifest as recurrent infections, autoimmunity, digestive disorders and granulomatous disease. Objectives: to describe the initial clinical findings of patients with CVID diagnosed at Hospital de NiñosSorMariaLudovica, between 1981 and 2019. 19 patients were included, 14 were male (74%). All Patients Had a history of recurrent infections, most frequently pneumonia (74%) and acute otitis media (42%). 9 patients suffered from chronic diarrhea (47%), with associated malabsorption in 6 of them. Thirty-two presented with severe malnutrition and 1 patient with gastric metaplasia. One Patient Had Splenomegaly and 1 had Evans´ syndrome. Bronchiectasis were found in 42% of patients at the time of diagnosis. Early suspicion of CVID from pediatricians is essential in order to arrive at a proper diagnosis
