    Improving Pediatric Asthma Outcomes Through Pictorial Asthma Action Plans

    Problem: Asthma is the leading chronic disease of children and the leading cause of childhood emergency department (ED) visits, hospitalizations, and missed school days. Asthma action plans (AAPs), including photographs or pictures of asthma medications, symptoms, triggers, and devices, have improved parents’ understanding of their child’s asthma and have increased their comfort level in caring for their child. Methods: An interventional pre-post study with a mixed-method research design, at a large, urban, primary care pediatric clinic with two locations, evaluated the effectiveness of a pictorial AAP on a child’s asthma control. A pictorial AAP was provided, and a childhood asthma control test (C-ACT) tool was completed when a child was enrolled in the study from January-February 2023. Two months later, another C-ACT and the Pictorial AAP Perceptions Questionnaire were completed. During March-May 2022 and March-May 2023, retrospective and prospective chart reviews were completed to examine how many asthma-related ED visits occurred. Results: Eighteen participants completed the study. A paired sample t-test determined the significance between the C-ACT scores pre-post intervention (p=0.002), and the scores improved by a mean of 5 points. There were eight ED visits before the pictorial AAP was given and two occurred after. Although the reduction in ED visits was not statistically significant, there was a clinically significant reduction in ED visits. Based on the responses to the Pictorial AAP Perceptions Questionnaire, most parents endorsed the pictorial AAP. Conclusion: The pictorial AAP intervention may be a favorable standard to decrease asthma morbidity and mortality

    Self-Care Practices in Doctoral Nursing Students and Effects on Educational Experience

    Problem: Doctoral nursing students are often faced with stressors on top of their schooling, one of which is continuing to work while pursuing their education. This in combination with normal life stressors can be challenging. The purpose of this project was to survey doctoral nursing students on what their current self-care practices. Methods: An exploratory needs assessment project was created to gather data from Doctor of Nursing practice (DNP) and Doctor of Philosophy (PhD) nursing students in an urban, mid-sized, Midwestern, public university to identify current self-care practices, barriers to self-care, preference of self-care practices, and the impact these practices have on their educational experiences. Results: 50 completed surveys were completed by the doctoral nursing students. Household income as a variable is statistically significant predictive of a higher category of number of times self-care is performed a week, χ2(4) = 14.352, p = .006. The odds of male vs female student having a higher number of times self-care practiced each week was 9.828, a statistically significant effect. The odds of a student who works in nursing education, compared to other nursing professions, having a higher number of self-care practices each week was -20.148, a statistically significant effect. Implications for Practice: There is a need for additional data of the self-care practices of doctoral nursing students. Implementation of an MBSR type program during the transitional years two and three of a traditional four-year doctoral nursing program is needed due to the drastic decrease in self-care between these years

    Orthostatic Vital Signs in Bone Marrow Transplant Patients

    Abstract Background: Orthostatic hypotension is a potentially serious risk factor that can cause falls in adults, especially those receiving a bone marrow transplant. The odds of falling increase if diagnosed with a hematological form of cancer while receiving treatment due to side effects of chemotherapy and medications. Methods: This retrospective record review investigated the completion rate of orthostatic vital signs in bone marrow transplant patient’s days 10 to 17. The aim was to discover 2% of positive orthostatic cases in a 6-month period. The primary outcome measures included the number of orthostatic vital signs obtained and secondary outcome measures included the number of falls. Results: The overall findings included 33 females and 50 males (N=83) in a 6-month period. There were 18 different diagnoses that were treated with 5 different types of bone marrow transplants. In October-December 2022, there was a total compliance rate of 50% and from January-March 2023 there was a compliance rate of 53%. A total compliance rate for a 6-month period of 51%. A chi-square analysis was performed on the data from 2022 vs 2023 and p=.904. The orthostatic compliance was not significantly significant. There were 106 positive orthostatic cases caught which reached the discovery of 2% of cases. There were also 8 total falls, and 3 falls were between the 10 through 17-day timeframe within the 6-month time frame. Implications for Practice: Overall, the results were not statistically significant, but future PDSA cycles will help to facilitate earlier identification of patients who are orthostatic, prompting timely intervention, and subsequent fall and injury prevention

    Encouraging Human Papillomavirus Vaccine Uptake: A Healthcare Improvement Project

    Abstract Problem: Human papillomaviruses are the causative agents of \u3e95% of cervical cancers, most oropharyngeal and anogenital cancers, yet the vaccination status of the population remains critically low despite Gardasil9 offering robust protection against HPV-related disease. Methods: This quality improvement project utilized a descriptive, observational design to assess the effect of the HPV Encouragement Bundle, a two-step intervention to improve HPV vaccine uptake. Quantitative data was collected via retrospective chart review to assess the effect of the intervention on first-dose administration of Gardasil9 (series initiation), as well as overall Gardasil9 vaccine uptake. Results: Following implementation, Gardasil9 first dose uptake increased from 0% to 8.45%, and overall Gardasil9 uptake increased from 0.8% to 12.68%, in pre- and post-intervention groups, respectively. A Chi-square test of independence was performed to examine the relationship between intervention groups and Gardasil9 administration. A statistically significant relationship was established between intervention groups and Gardasil9 administration, with alpha value of .05, χ2(1) = 14.19, p \u3c .001. Persons in the post-intervention group were more likely to be administered a dose of Gardasil9. Implications for practice: Widespread use of the HPV Encouragement Bundle is a cost-effective, inclusive intervention that could be used to improve Gardasil9 vaccine uptake

    Addicted to the Beloved? - A Comparison of Craving for and Attention to the Beloved, and Vape Stimuli

    People who are in love seem to be addicted to their beloved. People who are in love have attentional biases for the beloved and people who are addicted have attentional biases for the substance they are using. This raises the question of how similar craving and attention are for the beloved and an addictive substance. However, there are no studies that directly compared craving and attention to the beloved and addictive stimuli. The aims of the current research were to directly compare subjective emotional experiences and sustained motivated attention for the beloved and vape stimuli, in people who are both in love and vape. Participants (N = 11) were over 21 years old, in love with someone who did not vape, and vaped at least once a day. Self-reported levels of craving and valence were significantly stronger for the beloved compared to vaping strangers and neutral strangers. In addition, sustained motivated attention, indicated by the late positive potential (LPP) was significantly larger for the beloved when compared to vaping and neutral strangers. Subjective emotional experiences and sustained attention are stronger for the beloved when compared to an addictive substance, which may mean that love is even more intense than vaping nicotine

    U.S. and China: Microchip Policy Implications

    Microchips have become a crucial influence on a country’s military and commercial sectors; a nation’s capacity to produce large volumes of quality microchips indicates its importance on the world stage. Recently, both the United States and China have promoted many domestic and foreign industrial policies that address each country’s interests in the production and sophistication of its microchips, establishing for themselves a dominant position in the global microchip market. Both countries’ policies aim to simultaneously control/influence microchip production abroad and also stimulate development and production domestically. I have collected commercial, economic, and government budget data provided by various United States federal and private commercial entities that directly refer to these policies. These data, and the policies they relate to, mean that recent US government attention toward Chinese technological advancement, as well as American overreliance upon Taiwan for microchip production, demands concern. Consequently, the US has enacted industrial policies that incentivize scaling up of domestic microchip fabrication facilities, to increase their output and quality, and to create jobs for microchip-specialized workers. These moves are massive financial investments, illustrating the importance of microchips to Chinese and US economic interests. However, such policies, which may appear benign to a domestic audience, are clearly adversarial. Both countries promote these policies as inwardly focused, but they are actually aggressive attempts to prevent the other country from controlling the advanced microchip market. Should tensions between the US and China rise, the many countries that maintain trade with either nation would be ensnared in the fallout

    Mathematical Modeling of EGaIn Droplets Sliding Down an Inclined Plane

    Eutectic Gallium-Indium (EGaIn) is a room-temperature liquid metal alloy that dramatically changes its surface tension and dynamics under an applied electric field. EGaIn has been used heavily in soft electronics engineering due to its high conductivity, malleability, and safety. However, the absence of mathematical modeling in the current literature makes its behavior difficult to understand and predict. In this study, we present a one-dimensional lubrication model for the dynamics of an EGaIn droplet moving along an inclined plane. Our model incorporates essential physical effects and parameters including oxidation, capillary action, diffusion, gravity, and Marangoni effects. In particular, we incorporate effects of the electric field, both through electric forces and changes in oxidation flux. We model the thin oxide skin of the droplet, which modulates the interfacial surface tension, as an insoluble surfactant at the surface. Oxidation, while observable in the physical setting, cannot be well measured, calling for an alternative method to quantify oxidation flux. Utilizing experimental data, we calibrate system parameters and qualitatively obtain numerical simulation results comparable to experimental observations. Stability analysis was conducted to understand the impacts of physical effects on our model. We find azimuthal curvature to be the main contributor in the process of threading that also promotes the formation of satellite droplets. Our model has demonstrated success in reproducing the observed dynamics of an EGaIn droplet and provides a valuable resource for further investigation and uses of EGaIn

    Identifying practices to promote character development in university residential settings: The case of Colegios Mayores/Identificación de prácticas para promover el desarrollo del carácter en contextos residenciales universitarios: el caso de los Colegios Mayores

    In recent years, several initiatives have attempted to integrate character education in universities. However, it is difficult at the university level to find initiatives aimed at generating communities in which their climate or culture contribute to character development, a relatively common approach at the school level in the character education movement. University residential centers may be an ideal setting for generating such communities. However, studies, guides, or programs along these lines are scarce or non-existent. A case worthy of study is that of Colegios Mayores, university residential communities in Spain with a centuries-old tradition that aim to contribute to the integral development of students. However, the literature on the educational dimension of this institution is also scarce and outdated. Contributing to outline its educational dimension, this study reports the results of a survey of directors of Colegios Mayores. The aims of this study are to: 1) explore whether the community of Colegios Mayores agrees in considering six practices as characteristic of this institution; 2) explore the level of implementation of each one of them; and 3) explore if the way in which the community members are involved in these practices responds to the social learning model of Communities of Practice proposed by Etienne Wenger. The questionnaire was answered by 19 directors, representing 15.2% of the population (N=125). Based on the results, it is reasonable to affirm that most directors of Colegios Mayores: 1) consider that the six practices proposed are characteristic of this institution; 2) already implement these practices in their centers; and 3) consider that the Communities of Practice model reflects the way in which the collegial community engages in these practices.
