Over the Counter Overdose: A Case of Tylenol Toxicity
We report a case of a 51-year-old man presenting to the Emergency Department (ED) with suicidal ideations (SI) endorsing an intentional overdose of acetaminophen. Just as important is the clinical presentation; it is equally important to obtain a detailed history of patient\u27s presentation leading to the ED visit when evaluating a suicide attempt. Tylenol (acetaminophen) is a common over the counter (OTC) medication available to the public. Management of Tylenol overdose follows a standard outline of things to observe and intervene on. In this case, we discuss how Tylenol overdose is managed in a clinical setting through a patient who had an intentional overdose
A NOVEL PERFUSED CONTUSION SPINAL CORD INJURY MODEL TO ASSESS RAPID MECHANICAL PROPERTIES CHANGES AFTER BLOOD SPINAL CORD BARRIER BREAKDOWN
Blood-spinal cord barrier (BSCB) disruption exacerbates the tissue damage caused by spinal cord injury (SCI), but the mechanisms and dynamics of how the barrier breakdown affects the mechanical properties of the tissue remain unclear. The perfused bovine ex vivo indentation injury model described here represents a new platform to investigate the short-term effects of altered blood flow and vascular permeability following traumatic spinal cord injury. Our results indicate that injured cords exhibit changes to bulk perfusion, as evidenced by laser speckle contrast imaging, in addition to decreased barrier function shortly after injury. Indentation tests that simultaneously simulate a crush injury and provide force-indentation data reveal that tissue softening initiates as early as 30 minutes when perfused with whole blood. Perfusing with resuspended RBC (rRBC) instead of whole blood mitigates the decreased stiffness, highlighting the importance of white blood cells and plasma proteins in mediating the extracellular matrix mechanical response after SCI. Immunohistochemical staining indicates increased levels of extravasated Evans blue, ED-1, and vimentin in injured sections, suggesting mechanisms underlying the rapid response in mechanical properties following injury. This research provides novel insights into the immediate mechanical and vascular changes following spinal cord injury
Evolution of the substance use landscape: Implications for contingency management.
Contingency management (CM), which involves the delivery of incentives upon meeting behavioral goals, has the potential to improve substance use treatment outcomes. The intervention allows for flexibility through numerous modifiable components including changes to incentive magnitude and schedule, target behavior, and intervention structure. Unfortunately, numerous changes in the substance use landscape have occurred in the past 10 to 15 years: Substances are more potent, overdose risk has increased, new substances and methods of use have been introduced, and substance classes are increasingly being intentionally and unintentionally mixed. These developments potentially undermine CM outcomes. We explored recent substance use changes due to legislative, regulatory, social, and economic factors for four substance classes: stimulants, opioids, tobacco, and cannabis. We discuss potential adjustments to the modifiable components of CM for future research in response to these changes. By continually adapting to the shifting substance use landscape, CM can maintain optimal efficacy
Bilio-Cecal Stent Migration Presenting as Massive Rectal Bleeding.
Endoscopic biliary stenting is a well-established intervention for the treatment of biliary, hepatic, and pancreatic disorders. The common indications include strictures, neoplasms, stones, infections, and bile leaks. Stents can be occluded, predisposing patients to ascending cholangitis and biliary sepsis. Distal stent migration is another known complication of endoscopic stenting and is usually spontaneous. Bowel perforation, abscesses, bleeding, and pancreatitis are rare complications of distal stent migration and are usually limited to the duodenum. A Tale of Two Low-Income, First-Generation Engineering Students: Well-being, Belonging, and the Role of Chosen Family
Belonging has been identified as an important, if not the most important, factor impacting the success of college students, especially (multi)marginalized students in engineering programs. Well-being and belonging are deeply entwined, helping to explain how the strength of students affinities lead them to feel a sense of true happiness that guide their holistic success. In this presentation, I present two differing narrative stories from Samantha and Rocky, two non-white, low-income, first-generation women in engineering. At the time of their narratives, both women were in the fourth year of their engineering degrees; however, Samantha was graduating and Rocky was not. In fact, Rocky was struggling to belong and maintain a sense of overall wellness. This study emerged from an exploration of how intersectional socioeconomic inequality impacts students\u27 broader success in engineering. Based on a model that was used to choose these two participants (and four others), no student was meant to be successful, yet all the students but Rocky were. In this presentation, I show the ways in which well-being and belonging, or a lack thereof are present in their stories. I further show the impact of traumas, resources, and access to supportive traditional and chosen families on their trajectories
Building Resiliency, Fostering Success, and Celebrating Intersectionality in First-Year Seminar Students
First year students enter college with barriers and these barriers are often amplified for first-generation students. This presentation will highlight the approach Rowan University took to adapt their traditional first year seminar course for first-generation students. The presenter will share an approach which combines institutional mission, campus literacy, skill development, resiliency, and identity exploration. Institutional evidence of the models success (and struggles), as well as tips to implement the model will be shared
Impermanence and the First Generation College Student Dilemma
We often cling to the notion that more stability is needed in our advising practice. After all, advising can be chaotic and unpredictable. However, the elusive nature of stability can be frustrating. What if we embraced uncertainty and constant change instead? How might this enable us to remain attentive to our students? In discussing a frequently misunderstood population in higher education, first-generation college students, we aim to provide a new perspective on the holistic nature of advising. These students face formidable obstacles throughout their undergraduate journeys, often in silence. As advisors, we must adapt and support them through the ever-evolving landscape of their academic and personal experiences. One way we can do this is by introducing the topic of impermanence.
