    Case Report: Dysphagia in Inclusion Body Myositis Leading to Respiratory and Gastrointestinal Complications

    Inclusion Body Myositis (IBM) stands as a rare and complex neuromuscular disorder (NMD) characterized by progressive muscle weakness and atrophy. Among its cardinal symptoms are dysphagia and respiratory distress, which are the most common cause of death in this disease. While the differential diagnosis of respiratory distress is vast and includes aspiration, pneumonia, acute coronary syndrome, emphysema, and congestive heart failure, a clinician should recognize that respiratory distress can also be secondary to dysphagia in NMDs like IBM and can quickly become life threating. Here we present the case of a 68-year-old female with a history of IBM who presented for respiratory distress, was found to have severe dysphagia, and subsequently required intubation and percutaneous endoscopic gastrostomy (PEG) tube placement

    Case Report: Erysipelas Diagnosed in the Emergency Department

    Here we present a case of erysipelas. Erysipelas is a common and potentially dangerous infection. It involves the dermal layer of the skin but may also extend to the superficial cutaneous lymphatics. It is characterized by a relatively fast onset and a well-defined border. The most common sites are lower extremities with face being second. Much feared and often fatal in the pre-antibiotic era, it responds well to antibiotics today and most people have a full recovery without complications

    We report a case of an 80-year-old female who presented to the emergency department as altered mental status and diagnosed with pneumoperitoneum requiring immediate surgical intervention. The differential for altered mental status is vast but for a patient who was initially hypoglycemic on ED arrival, suspected drug overdose was at the forefront until the patient was intubated for airway protection and chest radiograph for confirmatory endotracheal tube placement showed pneumoperitoneum

As a result, an estimated 732,000 deaths, 21 million hospitalizations, and 322 diagnosed cases of diseases have been prevented between 1993 and 2013. •Vaccines remain a point of contention with pediatricians and their patients\u27 caregivers •Many parents maintain hesitancy to give vaccines; to a number of reasons. Perceived risk and fear of side effects from vaccines more than the disease itself, can cause out-of-proportion concern •Misinformation spread through the media as well as social media platforms can contribute to hesitancy seen during the COVID-19 vaccination effort

    How Psychological and Nonpharmacological Treatment Modalities Reduce the Disease Burden of Amplified Musculoskeletal Pain Syndromes in Pediatrics

    The prevalence of pediatric chronic musculoskeletal pain ranges from 8.5% to 40%.1 This literature review examined the treatment of amplified musculoskeletal pain syndrome (AMPS), which is a general term that represents a spectrum of pain disorders for patients who may not satisfy specific diagnostic criteria.2 Specifically, sources were analyzed to compare the efficacy of individual components in the nonpharmacological, interdisciplinary treatment of AMPS against pain medications. These components included cognitive behavioral psychotherapy (CBT), physical therapy (PT), as well as occupational therapy (OT). There is limited information that assesses these components individually, but there is ample evidence that when used in conjunction, a multidisciplinary approach to AMPS is successful. In particular, the functional disability index (FDI) is indicative of decreased disease burden found in the nonpharmacologic treatments investigated. This review demonstrates the efficacy and importance of CBT, PT, and OT in AMPS treatment

    Intra-accumbens Microinfusion of the Dopamine D3 Receptor Partial Agonist (±)VK4-40 Does Not Affect Basal Locomotion in Mice

    The opioid epidemic remains a pressing public health crisis, prompting the search for alternative pharmacotherapies for Opioid Use Disorder (OUD). This study explores the potential of the dopamine D3 receptor (D3R) partial agonist, (±)VK4-40, as a novel treatment option. We investigated the impact of intra-nucleus accumbens (NAc) microinfusion of VK4-40 on basal locomotion in mice. Results indicate that VK4-40 did not significantly alter basal locomotion, suggesting that its therapeutic effects may not be mediated through disruptions in generalized motor function. Future research will focus on elucidating the neuropharmacological mechanisms underlying VK4-40\u27s therapeutic actions and exploring its effects on psychostimulant-induced hyperlocomotion. These findings provide valuable insights into potential D3R-targeted pharmacotherapies for managing OUD

    Pros, Cons, and the Barriers to Implementing a Universal Healthcare System in the United States

    Background: The United States has the most expensive healthcare system in the world. Despite that, it also has worse health outcomes than that of several other countries. The United States is also the only wealthy/developed country without universal healthcare. Universal healthcare, also known as a single-payer healthcare system, refers to the concept that the government finances and governs healthcare for most, if not all residents of the country. The United States currently has a multi-payer system, which means that healthcare is financed through various sources such as the public and private sectors. Purpose: The purpose of this is to investigate the potential benefits and drawbacks of a universal healthcare system as well as identifying some of the potential barriers towards implementing a universal healthcare system in the United States, based on current, peer-reviewed literature. Methods: Peer-reviewed articles relevant to the topic were collected from databases such as PubMed. PRISMA criteria were used to identify relevant publications after applying inclusion and exclusion criteria. Data regarding health outcomes and healthcare expenses among different countries were compared and analyzed to help determine the effectiveness of universal healthcare. Results: Based on the studies, it can be inferred that the United States would significantly benefit from a universal healthcare system as studies show improved health outcomes as well as reduced healthcare expenditure, partially due to an emphasis on primary and preventative care, thus increasing access to overall care. However, there are also a few major barriers such as political lobbying that have prevented a significant overhaul of the current healthcare system in the United States. Conclusion: Universal healthcare would provide many benefits for the United States; however, political considerations pose a significant barrier that will need to be overcome before universal healthcare is implemented. That being said, there has been an increase in support for universal healthcare among Americans, in which small incremental steps can eventually lead the way there

    This presentation will discuss a new, innovative program, ProfPals, that is designed to help all students (including First Generation Students) at Rowan feel a sense of connection and belonging while facilitating student engagement with the Thrive dimensions of well-being.ProfPals is a unique program in that it allows students to develop and build social connection with other students as a means to enhancing their overall well-being and success at Rowan. Due to ProfPals recent development, this presentation aims to discuss the goals and vision of the program while also exploring feedback from attendees to elevate the ability for it to be a successful program for students, and in particular our First Generation Students

    Computational study of binding of oseltamivir to neuraminidase mutants of influenza A virus

    Oseltamivir (OTV), which targets the neuraminidase (NA) of Influenza A virus (IAV), has been reported to develop resistance. Here, we performed a computational study on the binding modes of OTV in the wild-type and popular mutants of IAV NA (E119A, E119D, E119G, H274Y, I117T, I117V, I117V-E119A, K150N, N294S, R292K, V116A, and Y252H). The Arg118, Glu119, Asp151, Arg152, Glu276, Arg292, and Arg371 were identified as crucial interacting residues with the drug. The energy decomposition analysis showed that with few exceptions, the dispersion interaction is the dominant interaction, followed by the charge-transfer and polarization interactions. The affinities for OTV were greatly reduced in all mutant systems, particularly in R292K and Y252H. The present study may be substantial in the design of new OTV analogs with better affinities to overcome the present drug resistance
