Co-developing a planetary health module for pediatrics: a student-led initiative with peer teaching and evaluation of planetary health literacy
Objectives
The aim of the study was to co-develop and implement a student-driven, interactive, and competence-based planetary health education (PHE) module on climate-sensitive health counselling (CSHC) in pediatrics and to evaluate pediatric planetary health literacy among medical students at a medical faculty in Germany.
Study design
This is a mixed methods study with a qualitative co-developing process of a teaching intervention in the pediatric curriculum of medical students and a pre-post evaluation. The questionnaires assessed whether the module could enhance students' planetary health literacy and evaluated its didactic design.
Methods
A teaching module titled “Child Health in Times of Planetary Crises” was co-developed, integrating insights from interviews with pediatricians practicing CSHC. Measurable learning objectives were created using Bloom's taxonomy. The module included an online course and a seminar that applied CSHC in pediatric scenarios, focussing on preventive and adaptive consultation. Pre- and post-course questionnaires assessed planetary health literacy, covering knowledge, understanding, and self-assessed competence, along with feedback on the course design. Statistical analysis included paired t-tests, Pearson correlations, and median comparisons.
Results
Students rated the module as “important”, “interesting” and “neglected in the rest of the curriculum”. Their planetary health literacy improved significantly across knowledge, comprehension of pediatrician's special responsibilities, and self-assessed competence to advise patients on adaptive and preventive measures (p < 0.01). The module's design received positive evaluation.
Conclusion
The module effectively enhanced planetary health literacy among medical students. Competence-based education on medical response strategies is essential for addressing children's vulnerability to growing health risks posed by planetary crises. We recommend integrating planetary health education into discipline-specific teaching and cross-disciplinary subjects
Nectin cell adhesion molecule 1 (Nectin-1) and the plasma membrane citrate carrier (pmCiC) – potential predictive biomarkers
Cancer remains a major global health challenge, with metastatic cancer accounting for a significant proportion of cancer-related deaths. Identifying appropriate treatment approaches is often complex and varies by cancer type. Predicting metastasis and response to treatment is critical to optimising patient care. Biomarkers, which detect changes indicative of disease,
play an important role in diagnosis and treatment prediction of cancer. They are already an integral part of cancer care, supporting early detection and the development of personalised therapies. Advancing biomarker research is critical to improving the efficiency of cancer treatment and patient outcomes. The aim of this thesis is to address the critical need for
improved methods of cancer treatment decision making by exploring the potential of biomarkers, focusing on two novel candidates, Nectin cell adhesion molecule 1 (Nectin-1) and
the plasma membrane citrate carrier (pmCiC).
Malignant melanoma is an aggressive form of skin cancer with a high metastatic rate. Oncolytic
virotherapy is a promising approach for the treatment of advanced melanoma, with talimogene
laherparepvec (T-VEC) being the first oncolytic virus approved by the United States (US) Food
and Drug Administration (FDA). T-VEC selectively targets tumour cells and stimulates an
immune response. Although effective, treatment with T-VEC requires multiple injections and
can cause local and systemic side effects that can be debilitating. In addition, a proportion of
patients do not benefit from treatment. This study identified Nectin-1 as a predictive marker of
melanoma response to oncolytic virotherapy and demonstrated a significant correlation
between Nectin-1 expression and tumour regression in both melanoma cell lines and patient
samples. High levels of Nectin-1 were associated with effective T-VEC-induced melanoma cell
killing, suggesting its potential as a biomarker to optimise virotherapy efficacy in various
tumours.
In the second part of the present study, we investigated whether pmCiC expression is
associated with increased disease aggressiveness of cancer and can serve as a prognostic
marker. Citrate, a key metabolite in the citric acid cycle, is crucial for lipid synthesis, epigenetic
regulation, and adenosine triphosphate (ATP) production. Cancer cells of various origins use
pmCiC to take up extracellular citrate to support their metabolism, including fatty acid
synthesis, mitochondrial activity, protein synthesis, and histone acetylation. Extracellular
citrate is also linked to a metastatic phenotype in cancer cells. Our investigation of pmCiC
expression showed that it increases with tumour stage and is highly expressed at metastatic
sites as well as in the tumour microenvironment. This highlights the potential role of
extracellular citrate in metastasis and tumour progression. We found a strong correlation
between pmCiC expression in both cancer cells and the surrounding stroma with tumour stage.
Consequently, pmCiC expression may serve as a prognostic marker, although further research
is needed for validation. Our findings also introduce citrate as a critical metabolite in tumour
progression, with citrate synthesis and release by cancer associated fibroblasts being integral
to tumour-stroma interactions and metastasis. Blocking citrate uptake by cancer cells reduced
cancer spread and stromal transformation, providing a novel therapeutic avenue. Our study
sheds light on the complex role of citrate in cancer metabolism and microenvironment
interactions. Extracellular citrate emerges as a key player in tumour progression, with potential
implications for metastasis and therapeutic interventions.
Further research into Nectin-1 and citrate metabolism may improve oncolytic virotherapy and
provide new strategies for cancer treatment and metastasis prevention
Plausibilität und Zuverlässigkeit von Fahrerangaben zur Vorunfallphase: Ein Vergleich mit Erkenntnissen aus dem Event Data Recorder der Unfallfahrzeuge
Die Aussagen von Unfallfahrern werden häufig verwendet, um Verkehrsunfälle und ihre Ursachen zu untersuchen. Diese können durch eine Vielzahl von Faktoren verzerrt sein. Um die Nutzbarkeit der Angaben im Rahmen der Verkehrsunfallforschung richtig einschätzen zu können, ist ein umfassendes Verständnis ihrer Güte erforderlich. In Deutschland ist in einer zunehmenden Anzahl von Fahrzeugen ein Event Data Recorder (EDR) verbaut, der in den letzten fünf Sekunden vor einem Unfallereignis verschiedene Informationen aufzeichnet. Diese objektiven Daten über die Vorunfallphase bieten die Möglichkeit, die Qualität der Fahrerangaben auf einer verbesserten Vergleichsgrundlage zu bewerten. In der vorliegenden Arbeit wurden die Angaben von 115 Autofahrern, die im Kontext der AARU Verkehrsunfallforschung erhoben worden sind, systematisch mit den Erkenntnissen aus den EDR-Daten des jeweiligen Unfallfahrzeugs verglichen. Ziel war es, die Güte der Angaben zu bewerten und ein besseres Verständnis für die Art und Häufigkeit von Abweichungen zu erlangen. Der Fokus des Vergleichs lag auf dem Unfallhergang, der gefahrenen Geschwindigkeit und der Reaktion vor dem Unfall. Es wurde eine nicht-interventionelle retrospektive Registerstudie mit Unfällen durchgeführt, die durch die AARU Verkehrsunfallforschung analysiert worden sind. Der Großteil der Fahrer schilderte die Unfallentstehung plausibel, wobei in den meisten Fällen Abweichungen hinsichtlich von Teilaspekten des Unfallhergangs vorlagen. Die Geschwindigkeitsangaben der Fahrer waren insgesamt eher ungenau und die meisten Fahrer berichteten eine niedrigere Geschwindigkeit als sie laut EDR-Daten gefahren waren. Viele Fahrer hatten Schwierigkeiten, sich an ihre Vermeidungsreaktionen zu erinnern und berichteten in den EDR-Daten ersichtliche Reaktionen nicht. Von den Fahrern angegebene Reaktionen waren hingegen meist tatsächlich erfolgt und die Erinnerung daran weitgehend zuverlässig. Auf Basis der Ergebnisse wurden Rückschlüsse für die Bewertung und Nutzung der Fahrerangaben im Rahmen der Verkehrsunfallforschung gezogen. Die Fahrerangaben weisen meist eine ausreichende Plausibilität für die Unfallursachenbewertung auf. Hierfür bieten die Aussagen der Fahrer, trotz ihrer Qualitätseinschränkungen und der vermehrten Verfügbarkeit objektiver Daten, weiter einen wichtigen Mehrwert. Aufgrund der limitierten Zuverlässigkeit ist für die Verwendung jedoch eine Plausibilisierung mit weiteren Unfallinformationen erforderlich. Zudem muss eine Einordnung unter Berücksichtigung der möglichen Verzerrungsprozesse erfolgen. Dafür bieten die Erkenntnisse dieser Arbeit eine wichtige Grundlage
Right answers to wrong questions: The dysfunctional nature of information needs
People frequently experience difficulties when seeking information to complete tasks. To overcome these difficulties, people require help. Regarding struggles with information needs, past research focuses on unclear information requests, such as ambiguous, under-specified, and ill-defined queries, and repairing these by user-led strategies (e.g., clarification). In an exploratory qualitative study where information clerks were interviewed, we, however, found that well-formed and seemingly reasonable requests can conceal misconceptions inquirers have (e.g., about what information is required for their current task) and, therefore, interfere with information seeking and task completion, too. Besides being more difficult to identify than unclear requests, such hidden misconceptions also undermine current user-led repair strategies as they cause inquirers to believe they are making appropriate requests. Understanding misconceptions in information seeking and requests concealing these is, therefore, essential to building more effective information systems. Our study contributes to addressing this task: It is the first to provide empirical insights into how misconceptions can negatively influence information requests, information-seeking conversations, and task completion. Ultimately, our findings highlight that inquirers' perceived information needs can present an unreliable and even counterproductive basis for task support, implying that researchers and professionals should rethink the prevailing focus on user requests in designing information systems
Von der Handschrift zu FAIRen Daten: Computergestützte Erschließung historischer Wetteraufzeichnungen aus St. Emmeram
Obwohl der Digitalisierung historischer Quellen zunehmend Aufmerksamkeit zukommt, stellt die computergestützte Erschließung handschriftlicher Dokumente mit komplexen Layouts und individuellen Schreibstilen weiterhin eine große Herausforderung dar. Das vorgestellte Forschungsprojekt widmet sich den Potenzialen aktueller Methoden der Handwritten Text Recognition (HTR) an einem einzigartigen Quellenbestand von Wetteraufzeichnungen des Klosters St. Emmeram in Regensburg (1774–1827).
Ziel ist ein KI-gestützter Workflow, der eine (semi-)automatische Transkription und strukturierte Erschließung dieser Quellen ermöglicht und ihre nachhaltige wissenschaftliche Nachnutzung sichert. Neben technischen Fragen der Texterkennung werden Interoperabilität, Nachnutzbarkeit und Kontextualisierung der Daten adressiert. Die resultierenden Datensätze fördern sowohl die Rekonstruktion historischer Wetterereignisse als auch Analysen der sozioökonomischen Auswirkungen in der Oberpfalz. In Verknüpfung mit weiteren digitalen Quellenbeständen können sie zudem neue Betrachtungsweisen landesgeschichtlicher Forschung eröffnen.
Der Vortrag präsentiert erste Ergebnisse zur Texterkennung sowie Layoutanalyse und diskutiert zugleich die methodischen Chancen als auch Grenzen im Umgang mit heterogenen historischen Quellen. Im Zentrum steht die Frage, wie durch eine Verzahnung technischer Innovationen mit geisteswissenschaftlichen Anforderungen ein nachhaltiger und wissenschaftlich fundierter Zugang zu bislang unerschlossenen Quellenbeständen geschaffen werden kann
Interferon gamma modifies nanoparticle tropism and cell uptake
Interferon gamma (IFN-γ) challenges the ability of viruses to distinguish between target and off-target cells i.e. viral tropism. Due to the similarity of viruses and nanoparticles in shape and size, we hypothesized that the effect of IFN-γ on viruses also applies to virus-mimetic nanoparticles. Therefore, we determined the influence of IFN-γ on nanoparticle tropism and cell uptake. For our experiments, we used cyclic RGD-peptide (cRGD) functionalized polymeric nanoparticles and lipid nanocapsules (LNC) to target the αvβ3 integrin receptor on cell membranes of rat mesangial cells (rMCs), HeLa cells and human dermal microvascular endothelial cells (HDMEC). We used a nanoparticle concentration range between 0.1 and 2 nM. The IFN-γ concentrations and IFN-γ incubation times varied in the range of 100 IU–1000 IU and zero to 54 h respectively. When we measured nanoparticle uptake via flow cytometry 24 h after cytokine administration to cells, our findings confirmed that IFN-γ decreased the tropism as well as the uptake of functionalized nanoparticles by target cells. We were able to demonstrate a maximal IFN- γ effect at a concentration of 100 IU of IFN-γ and a 24-h incubation time. The presence of IFN-γ is associated with an upregulation of cholesterol 25-hydroxylase (CH25H). That the CH25H oxidation product, 25-hydroxycholesterol (25HC), is known to inhibit cell-nanoparticle interactions points at the potentially underlying mechanism of the observed INF-γ effect
Data archive of Synergistic olfactory nerve input and cholinergic neuromodulation can activate ERK in rat olfactory bulb vasopressin cells
Social discrimination in rats relies on vasopressin cells (VPCs) intrinsic to the olfactory bulb (OB). We had observed that VPCs responded to electrical stimulation of the olfactory nerve in acute OB slices with inhibitory postsynaptic potentials, and that the neuromodulator acetylcholine could revert these responses to excitation, resulting in action potentials (AP). Moreover, in behaving rats that were exposed to conspecifics, more VPCs were immunopositive to the neural activity marker pERK (pERK+ VPC) than in control rats. However, it is unclear whether these two observations, the increased ERK activation in vivo, and the generation of APs in the presence of ACh in vitro, can be actually mapped onto each other. Here we investigated ERK activation in acute OB slices from transgenic VP-eGFP rats upon either chemical stimulation or tetanic olfactory nerve stimulation. Both KCl and NMDA stimulation resulted in substantial pERK induction across bulbar layers and caused VPC spiking in whole-cell recordings, but only NMDA slightly increased pERK+ VPC percentage. Tetanic olfactory nerve stimulation yielded localized, column-like ERK activation of neurons across bulbar layers. The presence of ACh during tetanic stimulation substantially and specifically increased percentages of pERK+ VPCs within columns, indicating that columnar pERK+ VPCs were preferentially activated by coincident cholinergic neuromodulation and synaptic input.
Our results validate pERK induction as a tool to monitor synaptic VPC excitation in the OB, and imply that depolarization of VPCs alone is insufficient to activate ERK. We propose that synaptically evoked APs are a prerequisite for pERK induction in VPCs
Bedeutung unterstützender Maßnahmen nach erfolgter radikaler Prostatektomie aus Patientensicht - Ergebnisse der retrospektiven Improve-Studie
Einleitung: Trotz medizinischem und technischem Fortschritt stellt das Prostatakarzinom immer noch die zweithäufigste krebsbedingte Todesursache dar. Mittels Etablierung von Prostatakarzinomzentren
wird versucht die Therapiequalität Betroffener zu verbessern.
In der folgenden Arbeit wird untersucht inwieweit sowohl der Einsatz einer technischen Innovation, wie der RARP, als auch der Einsatz prä
- und postinterventioneller supportiver Maßnahmen Auswirkungen auf das Therapiebedauern des Patienten hat. Des Weiteren stellt sich die Frage ob es maßgebliche Unterschiede in Bezug auf das Behandlungsergebnis zwischen ZERTs und NZERTs gibt.
Material und Methoden: Im Zeitraum April bis Juni 2021 wurden von 20 urologischen Zentren Fragebögen mit 17 items (soziodemographische Daten, Behandlungsstrategien, prä- und postinterventionelle supportive Maßnahmen, Therapieentscheidung) an 1000 Patienten, welche im Median vor 15 Monaten (Interquartilsabstand, IQR: 11–21) prostatektomiert wurden, verschickt. Von den beteiligten Zentren wurde von den Studienteilnehmern die chirurgische Vorgangsweise, der PSA vor der Operation, das Datum der Operation sowie Angaben zur Nervenerhaltung, die ISUP-Klassifizierung, das TNM-Stadium, der R-Status und die Komplikationen nach CDC in pseudonymisierter Form ermittelt. In die anschließende statistische Analyse aus den ermittelten Daten wurde der Versorgungsgrad des Zentrums (universitär vs. außeruniversitär) und die durchschnittliche Fallzahl an durchgeführten RPs pro Jahr zwischen 2018 und 2020 miteinbezogen.
Ergebnisse: Das onkologische, funktionelle und vom Patienten empfundene Ergebnis nach erfolgter RP unterschied sich in dieser Kohorte (75% Rücklaufquote) bei ZERTs und NZERTs nur unwesentlich, was wiederum zu keinem Unterschied bezüglich DR des Patienten führte. Generell ist das DR nach RARP im Vergleich zu ORP jedoch signifikant niedriger, wobei eine
postoperativ bestehende Belastungsinkontinenz das DR signifikant erhöht.
Dieses Ergebnis bezüglich ZERTs und NZERTs spiegelte sich auch in Bezug der angebotenen prä- und postinterventionellen supportiven Maßnahmen wider. Für Patienten scheint jedoch die präoperative Beratung durch den Operateur, eine ausführliche Beratung bezüglich präoperativer
Möglichkeiten zur Beübung des Beckenbodens und eine gute Betreuung durch den Sozialdienst zur schnellen Rehabilitation eine wichtige Rolle für das DR und die Rekonvaleszenz zu spielen während die psychoonkologische Beratung eher als irrelevant eingestuft wird.
Schlussfolgerungen: In zukünftigen prospektiven Studien sollten perioperative Versorgungsstandards mit einem maßgeschneiderten Konzept mit intensivierten Unterstützungsmaßnahmen verglichen werden, um die Auswirkungen dieses Ansatzes auf patientenrelevante Endpunkte wie Lebensqualität und Entscheidungsbedauern zu verschiedenen Zeitpunkten postoperativ zu evaluieren. Zudem sollten, um den potenziellen Spillover-Effekt des Zertifizierungsprozesses auf NZERTs weiter zu untersuchen, die Fallzahl und die Erfahrung des Chirurgen in kleineren regionalen Kliniken ermittelt werden sowie die Qualität der präoperativen diagnostischen Beurteilung untersucht werden
Das digitale Basisdokument. Ergebnisse aus dem Deutschlandweit ersten Reallabor im Rechtswesen
Im vorliegenden Beitrag berichten wir über wesentliche Ergebnisse des Projekts digitales Basisdokument, dessen von den Justizministerien Bayerns und Niedersachsens geförderte Phase im Sommer 2024 zu Ende gegangen ist. Es stellt das deutschlandweit erste Reallabor im Rechtswesen dar. Im Mittelpunkt des Projektes stand die Erprobung eines interaktiven webbasierten Prototyps, mit dem die beteiligten Parteien eines Zivilprozesses den Parteivortrag erfassen können, statt Schriftsätze als Textdokumente auszutauschen. Die Erprobung fand im Rahmen erstinstanzlicher Prozesse an den Landgerichten Hannover, Landshut, Osnabrück und Regensburg von Februar 2023 bis Juni 2024 statt. Die Ergebnisse leiten sich aus mehr als 50 qualitativen Interviews her, die mit den Prozessbeteiligten geführt wurden
