    Quinones as Multifunctional Scaffolds for Oxidative, Reductive, and HAT Photocatalysis

    Photoredox catalysis, which enables both electron and hydrogen atom transfer, has become a powerful tool for activating chemical bonds and synthesizing complex molecules under mild conditions. Typically, photocatalysts are optimized either for oxidative or reductive reactions within a limited redox window (less than 3.1 V) and for hydrogen atom transfer (HAT) reactions, with few frameworks capable of mediating both pathways for high redox-demanding reactions (covering more than a 5 V redox window) without requiring special conditions. Herein, we report the use of quinones as multifunctional scaffolds in light-driven redox transformations, offering access to a redox window of approximately 5 V using visible light. The quinone scaffold's versatility facilitates a wide range of radical and ionic processes under both oxidative and reductive conditions, in addition to enabling HAT reactions. By keeping the parameters, i.e. the reaction partners, constant, such transformations can be carried out under just two reaction conditions. Oxidative transformations and HAT reactions occur under ambient air, while activation of the chromophore for reductive transformations can be achieved using an inorganic base (Cs2CO3) via a simple acid-base deprotonation event. This dual capability highlights the potential of quinones as scaffolds to extend their utility in photoredox catalysis

    Einfluss von Akute-Phase-Proteinen auf die Granulozytenfunktionalität in vitro

    PMNs wurden mit verschiedenen Konzentrationen von FKS, Ionomycin, CRP, Fibrinogen und Ferritin inkubiert und anschließend verschiedene Granulozytenfunktionalitäten mittels Durchflusszytometrie oder „Live-Cell-Imaging“ detektiert. Hierbei handelt es sich in der Durchflusszytometrie um den oxidativen Burst und die Expression der Oberflächenantigen CD11b, CD62L und CD66b. Das „Live-Cell-Imaging“ zeichnete das Migrationsverhalten, die ROS-Produktion und Intensität, die Freisetzung von MPO und die NET-Formation auf. Es wird gezeigt, dass ein FKS-Anteil von 10 % in dem Medium, in welches die PMNs nach der Isolation aufgenommen werden, zu einer längeren Aktivität der PMNs führt. Durch eine längere Aktivität der PMNs können diese besser untersucht werden. Ionomycin hat einen moderaten Einfluss auf die PMNs. Es führt zu einer vermehrten Expression von CD11b und CD66b und zu einer verminderten Expression von CD62L. Diese Arbeit zeigt außerdem, dass die Akute-Phase-Proteine CRP, Ferritin und Fibrinogen einen moderaten Einfluss auf die Granulozytenfunktionalität ausüben. CRP verstärkt die Intensität der ROS-Produktion. Ferritin führt zu einer mäßigen Steigerung der ROS-Produktion, insbesondere nach Aktivierung mit TNFα/fMLP oder PMA. Dies wurde mittels Durchflusszytometrie detektiert. Fibrinogen hat hauptsächlich Auswirkungen auf die Expression der Antigene CD11b, CD62L und CD66b. Diese Effekte auf die PMNs sind alle moderat und zeigen gewisse Tendenzen. Extreme Beeinflussungen konnten nicht festgestellt werden. Um die genaue Bedeutung der APPs in der Immunabwehr im Zusammenhang mit den PMNs zu verstehen, sollten zukünftig weitere Versuche zu diesem Thema durchgeführt werden. Das Verhalten der PMNs ist noch lange nicht verstanden und hält noch viele ungeklärte Fragen bereit. Beispielsweise könnte in den Versuchen FKS durch humanes Serum ersetzt, die Art der Granulozytenisolierung verändert oder die Auswirkung von weiteren APPs auf die PMNs untersucht werden

    Untersuchung der Unterdrucktherapie mit Instillation durch Analyse der Wundflüssigkeit

    Aufgrund des demographischen Wandels und lebensstilbedingten Erkrankungen wie DM Typ II leiden immer mehr Menschen an komplexen Wunden, deren Therapie eine Herausforderung ist und einer interdisziplinären Herangehensweise bedarf. Die Unterdrucktherapie mit Instillation ist eine etablierte Behandlungsmöglichkeit bei komplexen Wunden. In dieser Studie erfolgt die Untersuchung der Zusammensetzung des Wundexsudats während der NPWTi mit dem Ziel, ein Monitoringtool für die Vorgänge unter dem über mehrere Tage versiegelten Verband zu etablieren. Außerdem soll der Einfluss von pAVK, DM Typ II und der mikrobiellen Besiedelung auf das Wundexsudat untersucht werden. Es wird das Exsudat von 31 durch NPWTi versorgte Wunden untersucht. Die Instillation erfolgt alle vier Stunden mittels isotoner Kochsalzlösung über einen Zeitraum von 20 Minuten. Dazwischen erfolgen Phasen mit kontinuierlichem Unterdruck von -125 mmHg. Die Probenentnahme erfolgt unmittelbar nach einer Instillationsphase. Die Analyse von Kalium, LDH, Phosphat, pH, IL-6, IL-8, TNF-α und der zellulären Bestandteile erfolgt über eine etablierte klinische Routinediagnostik. Die laborchemische Bestimmung von VEGF, bFGF und Gesamtprotein erfolgt im Anschluss gesondert. Die Beurteilung des Wundexsudats im Verlauf für eine einzelne Wunde ist durch die angewendete Methode möglich und wenig invasiv. Zellen, Zytokine und Wachstumshormone können in sinnvoller Konzentration quantifiziert werden. Die statistische Auswertung der Tage null bis drei ergibt einen signifikanten Abfall der Erythrozyten und des pH-Werts an Tag zwei (p=0,038, p=0,033), einen signifikanten Anstieg der Neutrophilen an Tag eins (p=0,028) und der Monozyten an Tag zwei (p=0,037). Des Weiteren steigen die proinflammatorischen Zytokine IL-6, IL-8 und TNF-α (p=0,011, p=0,005, p=0,005) an Tag eins signifikant an. Daraus lässt sich ableiten, dass die Analyse des Wundexsudats aus NPWTi-Einheiten zur Überwachung von Wunden hinsichtlich ihres Inflammationsstatus, oder dem Auftreten von Blutungen ohne Entfernung des Verbandes dienen könnte. Obwohl im Rahmen dieser Studie für pAVK nur wenige und für Diabetes keine signifikanten Unterschiede vorliegen, ist eine Beeinflussung der Wundexsudatzusammensetzung durch diese Faktoren wahrscheinlich.Beim Vergleich der Gruppen bakterieller Befall/C. albicans/kein Keim ergab sich an Tag drei bei den MNCs (p=0,012) und IL-8 (p=0,049) ein statistisch signifikanter Unterschied. Die erhöhten proinflammatorischen Zellen und Zytokine legen den Schluss nahe, dass die mikrobielle Besiedelung Einfluss auf die Wundheilung zu haben scheint. Weitere Studien mit höheren Fallzahlen sind notwendig, um die Ergebnisse zu bestätigen und die Methodik zu optimieren

    Das Tageshospiz – eine neue Möglichkeit der ambulanten palliativmedizinischen Versorgung

    Es bestehen hinsichtlich der sozialen Isolation und der gesellschaftlichen Reintegration von Palliativpatienten immer noch Defizite in der palliativmedizinischen Versorgung. Zusätzlich dürfen auch die Probleme der pflegenden Angehörigen nicht unberücksichtigt bleiben. Das in Deutschland noch recht junge Versorgungsmodell Tageshospize könnte diesen Problemen entgegenwirken, erfordert aber bestimmte Voraussetzungen (u. a. personell, strukturell, inhaltlich). Einheitliche Zertifizierungen für Tageshospize in Deutschland stehen zurzeit noch aus. Erste Empfehlungen existieren zwar bereits, weitere wissenschaftliche Untersuchungen und Begleitprogramme zum Verlauf und den Erfahrungswerten von Tageshospizen sind jedoch notwendig

    Evaluating smartphone-based 3D imaging techniques for clinical application in oral and maxillofacial surgery: A comparative study with the vectra M5

    Purpose This study aimed to clarify the applicability of smartphone-based three-dimensional (3D) surface imaging for clinical use in oral and maxillofacial surgery, comparing two smartphone-based approaches to the gold standard. Methods Facial surface models (SMs) were generated for 30 volunteers (15 men, 15 women) using the Vectra M5 (Canfield Scientific, USA), the TrueDepth camera of the iPhone 14 Pro (Apple Inc., USA), and the iPhone 14 Pro with photogrammetry. Smartphone-based SMs were superimposed onto Vectra-based SMs. Linear measurements and volumetric evaluations were performed to evaluate surface-to-surface deviation. To assess inter-observer reliability, all measurements were performed independently by a second observer. Statistical analyses included Bland–Altman analyses, the Wilcoxon signed-rank test for paired samples, and Intraclass correlation coefficients. Results Photogrammetry-based SMs exhibited an overall landmark-to-landmark deviation of M = 0.8 mm (SD =  ± 0.58 mm, n = 450), while TrueDepth-based SMs displayed a deviation of M = 1.1 mm (SD =  ± 0.72 mm, n = 450). The mean volumetric difference for photogrammetry-based SMs was M = 1.8 cc (SD =  ± 2.12 cc, n = 90), and M = 3.1 cc (SD =  ± 2.64 cc, n = 90) for TrueDepth-based SMs. When comparing the two approaches, most landmark-to-landmark measurements demonstrated 95% Bland–Altman limits of agreement (LoA) of ≤ 2 mm. Volumetric measurements revealed LoA > 2 cc. Photogrammetry-based measurements demonstrated higher inter-observer reliability for overall landmark-to-landmark deviation. Conclusion Both approaches for smartphone-based 3D surface imaging exhibit potential in capturing the face. Photogrammetry-based SMs demonstrated superior alignment and volumetric accuracy with Vectra-based SMs than TrueDepth-based SMs

    Das Kompartmentsyndrom nach Unterschenkelfraktur bei Kindern und Jugendlichen - Retrospektive Analyse eines monozentrischen Kollektivs

    Das Auftreten eines Kompartmentsyndroms ist selten, kann allerdings bei Nichterkennen oder zu spätem Handeln zu irreversiblen Folgeschäden führen. Dabei tritt dies, gerade im Zusammenhang mit stattgehabten Frakturen im Unterschenkel, gehäuft auf. In der pädiatrischen Traumatologie stellt die Unterschenkelfraktur eine häufige Frakturart dar. Im Rahmen dieser Arbeit sollen mögliche Faktoren, die das Nichtentstehen eines akuten Kompartmentsyndroms bei Kindern und Jugendlichen begünstigen, herausgearbeitet werden. Dabei erfolgte die retrospektive Analyse von insgesamt 75 Fällen mit Unterschenkelfraktur, zudem fand eine Nachuntersuchung an 41 dieser Patienten statt. Dabei lag in keinem der betrachteten Fälle die Diagnose eines akuten Kompartmentsyndroms nach Unterschenkelfraktur vor. Dies wurde anhand der retrospektiven Datenanalyse sowie der Nachuntersuchung belegt, wobei in zwei Fällen kein eindeutiger Ausschluss möglich war. Zudem wird die Schwierigkeit der korrekten Diagnosestellung eines akuten Kompartmentsyndroms, speziell hinsichtlich der Besonderheiten bei pädiatrischen Patienten diskutiert. In Bezugnahme auf vergleichbare Arbeiten mit ähnlicher Fragestellung, jedoch höherer Inzidenz eines akuten Kompartmentsyndroms nach Unterschenkelfraktur, ist vorrangig ein Unterschied hinsichtlich durchschnittlichem Patientenalter, Unfallschwere sowie Komplexität der Frakturen, hervorzuheben. Dies lässt den Schluss zu, dass ein jüngeres Patientenalter, Niedrigrasanztraumata sowie Stürze und weniger komplexe Frakturen das Nichtentstehen eines akuten Kompartmentsyndroms bei Kindern und Jugendlichen begünstigen könnten

    Preliminary effectiveness of social prescription and virtual patient information in increasing tertiary prevention among cancer patients (ESPRIT): protocol for a single-centre, randomised controlled pilot trial

    Introduction Tertiary prevention through physical activity and psychosocial support can positively impact patient outcomes, such as physical function and quality of life (QoL). However, more research is required on the effectiveness of strategies designed to increase the uptake of tertiary prevention programmes among cancer patients. Here, we present the protocol for a single-centre, randomised controlled pilot trial testing the preliminary effectiveness of social prescription and virtual patient information in increasing tertiary prevention among cancer patients and support persons (SPs) (ESPRIT “Effectiveness of a social prescription and virtual patient information in increasing tertiary prevention” pilot trial). Methods and analysis Cancer patients attending medical oncology units at a university hospital in southern Germany and their SPs will be randomly allocated as a dyad to group A (social prescription (n=36)), group B (virtual patient information (n=36)) or group C (usual care (n=36)). The hospital is part of a Comprehensive Cancer Centre mainly treating patients living in rural areas. Primary outcomes are the uptake of physical activity, participation in social activities and psychosocial support. Secondary outcomes are overall QoL, knowledge of the health benefits of physical activity and psychosocial support and self-efficacy of patients. The outcomes will be assessed at baseline and after 3, 6 and 12 months of follow-up. Physical activity will be assessed using accelerometers and measured by average steps per day within the last 2 weeks after recruitment and at follow-up visits (3, 6 and 12 months). Cost-effectiveness and the time spent in the consultation, as well as potential implementation barriers and facilitators, will also be explored as part of a mixed-methods hybrid design. All data will be summarised descriptively. Regarding the analysis of primary endpoints, the average number of steps per day, as well as the summary score of the social activity log and self-report on the use of psychosocial support, will be compared between the groups (A, B and C) using analysis of variance, followed by Dunnett’s test for pairwise comparisons of the intervention groups against the control group. Mean differences and 95% CIs will be presented as effect estimates. The analysis of secondary endpoints will include appropriate statistical methods such as the χ2 test of independence or linear regression models, which will be used to analyse secondary endpoints and to investigate factors influencing preliminary effectiveness

    Nucleophilic Functionalization of a Cationic Pentaphosphole Complex–A Systematic Study of Reactivity

    The systematic nucleophilic functionalization of the cationic pentaphosphole ligand complex [Cp*Fe(η4-P5Me)][OTf] (A) with group 16/17 nucleophiles is reported. This method represents a highly reliable and versatile strategy for the design of novel transition-metal complexes featuring twofold substituted end-deck cyclo-P5 ligands, bearing unprecedented hetero-element substituents. By the reaction of A with classical group 16 nucleophiles, complexes of the type [Cp*Fe(η4-P5MeE)] (E=OEt (1), OtBu (2), SPh (3), SePh (4)) are obtained. By transferring this protocol to group 17 nucleophiles, the highly sensitive complexes [Cp*FeP5(η4-P5MeX)] (X=F (5), Cl (6), Br (7), I (8)) could be isolated. All products show exclusively 1,1'-substitution at the cyclo-P5 ring. Interestingly, further studies on the reactivity of the halogenated species revealed their ability to undergo ring-opening reactions with cyclic ethers such as THF and ethylene oxide yielding [Cp*FeP5(η4-P5MeOC4H8X)] (X=Br (9), I (10)) or [Cp*FeP5(η4-P5MeOC2H4X)] (X=Br (11), I (12)), respectively. Furthermore, the use of acyclic ethers such as dimethoxyethane led to the formation of [Cp*FeP5(η4-P5MeOC2H4OCH3)] (13) mediated by C−O bond cleavage, followed by subsequent P−O bond formation, as further enlightened by DFT calculations

    Randomised controlled feasibility trial of retroperitoneal vs transperitoneal robot‐assisted partial nephrectomy: the ROPARN study

    Objectives To assess the feasibility of trial recruitment and confirm that retroperitoneal robotic partial nephrectomy (RRPN) has the same oncological efficacy as transperitoneal robotic partial nephrectomy (TRPN), with time advantages and less peri-operative morbidity, in a randomised controlled trial (RCT). Patients and Methods The study was designed as a single-centre, open-label, feasibility RCT. Patients with suspected localised renal cell carcinoma referred for robotic partial nephrectomy were randomised in a 1:1 ratio to receive either TRPN or RRPN. The primary outcomes were trial feasibility, postoperative mobility and pain perception. Secondary outcomes were intra-operative times, assessment of complications, and comparison of positive surgical margin (PSM) rates. The data on all randomised patients who underwent surgery were analysed descriptively. Results Sixty-one patients underwent TRPN or RRPN (31 vs 30). Postoperative mobility within 24 h after surgery (RRPN: 77 vs TRPN: 71%; P = 0.613), median postoperative pain, assessed using a numeric rating scale (RRPN: 1.5 vs TRPN: 1.8; P = 0.509), and full bowel canalisation within 3 days (RRPN: 100% vs TRPN: 90%; P = 0.315) were more favourable in the RRPN group, but the difference was not statistically significant. In comparison to TRPN, RRPN was associated with shorter operating time (81 vs 105 min; P = 0.005), shorter time on the console (49 vs 73 min; P < 0.001) and shorter time from console to renal artery preparation (7.5 vs 18 min; P < 0.001). TRPN required a shorter time from skin incision to console (16 vs 12 min; P = 0.001). There was no statistically significant difference in tumour complexity, ischaemia time, PSM rate, blood loss or complications between the two groups. Conclusion We present the first data from an RCT comparing RRPN with TRPN. RRPN showed significant time benefits while being a safe alternative to TRPN, with a similar PSM rate. There was less postoperative morbidity in the RRPN group, although this was not statistically significant. These results underline that dorsolateral renal tumours should be preferably resected by RRPN. Further multicentre RCTs are necessary to confirm these results
