    Evaluation einer Spezialambulanz für Schulabsentismus der Klinik für Kinder- und Jugendpsychiatrie

    In der vorliegenden Arbeit wurde eine klinische Inanspruchnahme-Stichprobe von Kindern und Jugendlichen, welche in den Jahren 2012 bis 2018 die Spezialambulanz für schulvermeidendes Verhalten der KJPP Regensburg aufsuchten, evaluiert. Insbesondere wurde die Stichprobe hinsichtlich psychiatrischer Komorbiditäten im Geschlechtervergleich und möglicher Verlaufsprädiktoren genauer untersucht. Die Daten wurden retrospektiv auf der Basis von elektronischen Patientenakten erhoben, wobei sich insgesamt eine Stichprobengröße von N = 174 (n = 99 männlich, n = 75 weiblich) Kindern und Jugendlichen im Alter von 6 bis 17 Jahren ergab. Zur Analyse des zeitlichen Verlaufs der schulischen Fehlzeiten wurden drei Ränge gebildet. Die Rangeinteilung erfolgte angelehnt an Reissner et al. in einen geringen Schulbesuch (90.00 %) und wurde zu drei Zeitpunkten (nach einem Jahr, nach zwei bis drei Jahren, ab dem dritten Jahr) nach Behandlungsbeginn in der KJPP herausgearbeitet. Insgesamt ergab sich in der vorliegenden Studie im Mittel eine statistisch signifikante Rangverbesserung bei 34.64% der Patienten im ersten Jahr. Entgegen den Erwartungen stieg im ersten Jahr auch der Anteil derer, welche nur einen geringen Schulbesuch von unter 10.00 % aufwiesen. Bei einem Großteil dieser Patienten wurde jedoch eine große Compliance-Problematik hinsichtlich der Einhaltung der Therapieempfehlungen deutlich. Als potenzielle Prädiktoren für den Verlauf des Schulbesuchs wurden neben dem Alter und dem Geschlecht beispielsweise das Ausmaß schulischer Fehlzeit zum Zeitpunkt der Ersterhebung, die Diagnosen, die Summe der Therapieempfehlungen und deren prozentuale Einhaltung nach Erstdiagnostik untersucht. Die Ergebnisse zeigen das Geschlecht als einflussreichsten signifikanten Prädiktor auf die Rangveränderung nach einem Jahr. Männlich zu sein machte es 68.50 % unwahrscheinlicher im Rang aufzusteigen und somit im Verlauf weniger schulische Fehltage aufzuweisen. Ebenso konnte auch bei einer höheren Anzahl an Therapieempfehlungen im Rahmen der Erstdiagnostik ein um 44.70 % unwahrscheinlicheres Aufsteigen im Rang aufgezeigt werden. Insgesamt 41.90 % unwahrscheinlicher machte es das Vorhandensein einer höheren Summe an Achse V Diagnosen im Rang aufzusteigen. Die Wahrscheinlichkeit eines Rangaufstiegs nach einem Jahr erhöhte sich hingegen um 61.90 % bei einem höheren Umfang an Fehltagen vor Beginn der Therapie. Bei mehreren weiteren Variablen konnten keine oder nur schwach ausgeprägte Effekte herausgearbeitet werden. Zu den weiteren Erhebungszeitpunkten zeigte sich die Stichprobengröße durch eine Vielzahl fehlender Werte als zu klein für eine aussagekräftige Regression. Hinsichtlich der Komorbiditäten stellte sich eine hohe Prävalenz an psychiatrischen Diagnosen bei Kindern und Jugendlichen mit Schulabsentismus dar. Die durchschnittliche Anzahl externalisierender Diagnosen lag dabei bei den männlichen Patienten statistisch signifikant über der mittleren Anzahl bei den weiblichen Patienten. Insgesamt konnten jedoch keine erheblichen signifikanten Unterschiede zwischen den Geschlechtern herausgearbeitet werden. So zeigte sich im Hinblick auf die Anzahl an Diagnosen auf den einzelnen Achsen des MAS nach ICD-10 kein statistisch signifikanter Zusammenhang sowie ein insgesamt sehr ähnliches Verteilungsmuster der Diagnosegruppen (F1x.x-F9x.x) bei beiden Geschlechtern. Die Ergebnisse verweisen in der Gesamtheit dennoch auf ein sich sehr heterogen präsentierendes Patientenklientel und unterstreichen die hohe Relevanz des Schulabsentismus in der kinder- und jugendpsychiatrischen Patientenversorgung

    Predictors of disease alleviation with mandibular advancement devices in obstructive sleep apnea: a retrospective cohort study

    Background: Obstructive sleep apnea (OSA) can be treated with mandibular advancement devices (MAD), preventing collapse of the upper airway and decreasing apnea-hypopnea index (AHI)/ h. Disease alleviation is expected to vary depending on specific predictors including OSA-severity and cephalometric parameters. This retrospective cohort study aimed to identify predictors of disease alleviation with MAD in adult patients with mild to moderate and severe OSA. Secondary outcomes included assessing the necessity of lateral cephalograms and the therapeutic success in severe OSA-cases. Methods: OSA-patients, treated with MAD at the orthodontic department of the University Hospital Regensburg, Germany, were allocated to mild to moderate (AHI ≤ 30/ h) and severe OSA groups (AHI > 30/ h). BMI, poly(somno)graphic, demographic and cephalometric variables were evaluated before (T0) and after 3 to 6 months of MAD-treatment (T1). Applying linear regression analyses, predictors were identified, following an assessment of their effect on disease alleviation by independent two-tailed t-tests for continuous, and absolute and relative frequencies for categorical variables. Then, the need for cephalometric analysis and the disease alleviation in severe OSA-patients were evaluated. Results: Sixty-six predominantly male patients (mean age 55 ± 11 years; male:female = 52:14) were stratified to mild to moderate (n = 45) and severe (n = 21) OSA-groups. Regression analysis revealed baseline-AHI as a significant and relevant predictor, whereas few cephalometric parameters proved significance with small effect sizes (absolute AHI/ h-reduction, univariate model: -0.64 (95% CI: -0.75; -0.53), p < 0.001, R² = 0.666). Compared to mild to moderate OSA-cases, severe OSA-patients had a significantly higher AHI (19.1 ± 11.7 vs. 6.0 ± 4.0, p < 0.001) at T1, but also a higher disease alleviation according to absolute AHI-reduction (-26.1 ± 16.0 vs. -9.6 ± 6.4, p < 0.001), indicating comparable treatment success in all OSA-degrees. Conclusions: Disease alleviation with MAD in adult OSA-patients can be predicted with specific poly(somno)graphic parameters (especially baseline-AHI), whereas cephalometric variables appeared inappropriate. Still, lateral cephalograms are helpful in the diagnostics and follow-up of MAD-treatment, e.g. for evaluating side effects. Providing good patient selection, therapeutic success can be achieved in both severe and mild to moderate OSA

    Quantitative chest computed tomography: regional differences in dual-energy-derived virtual vs. true non-contrast scans

    Aim Quantitative analysis of chest computed tomography (CT) usually requires non-contrast scans. Multi-energy CT-derived virtual non-contrast (VNC) images may obviate additional true non-contrast (TNC) scans. We aimed to quantitatively compare virtual and true non-contrast images, with a focus on regional differences in pulmonary tissue. Materials and Methods This bi-center retrospective, IRB-approved study included consecutive patients who received contrast-enhanced dual-energy and non-contrast chest CT scans within one year between April 2018 and December 2022. Scans were co-registered and semi-manually segmented into various volumes of interest: whole lung, center/periphery, ventral/dorsal half and upper/middle/lower third. The mean lung density, high and low attenuation volumes and the lower 15th percentile were computed from the VNC and TNC scans for each volume. Correlation and agreement were assessed using Pearson’s and intraclass correlation coefficients and Bland–Altman analysis. Subgroup analyses involved body mass index and markers of pulmonary emphysema. Results Forty-eight patients were included (26 females, median age: 63 years, interquartile range: 58–69). Pearson’s and intraclass correlation coefficients > 0.9 demonstrated high correlation and agreement for the mean lung density, low attenuation volume and the lower 15th percentile across the whole lung. The agreement for high attenuation volume was slightly less pronounced (intraclass correlation coefficient, 0.84 [95% confidence interval: 0.73, 0.91]). Regional differences were observed, for example, concerning the mean lung density with mean differences (HUVNC minus HUTNC) of -5.71 HU [-11.07, -0.35], 1.62 HU [-2.98, 6.23] and 5.77 HU [-0.47, 12.01] for the upper, middle and lower third, respectively. Conclusion Although slight regional differences were observed, quantitative analysis can be reliably performed using VNC images

    The pace and shape of ant ageing

    Ants have been proposed as good models to study ageing and the effects of extrinsic mortality because of their long lifespans and plasticity of ageing within species. We discuss how age-dependent extrinsic mortality might influence queen lifespan, and how the effect of age-independent extrinsic mortality needs further study, accounting for different density-dependence scenarios. Based on a critical review of the available demographic data, we discuss the selective forces underlying ant ageing. We discuss differences and similarities between the life-history strategy of ants and the reproductive strategies iteroparity and semelparity. We consider how late-life fitness gains for the “superorganism” select for a delay of actuarial, and reproductive senescence, and we suggest future research directions

    Kindergarten teacher well-being: is bad stronger than good?

    Forster et al. demonstrated that school teachers’ well-being is related to their educational goals and experienced emotions for students showing undesirable behaviors: the higher the goals and the more positive the emotions, the higher the reported well-being. By contrast, the goals and emotions for students showing desirable behaviors was unrelated to school teachers’ well-being. These findings demonstrated that the principle of “bad is stronger than good” extends to the influence of student behavior on school teacher well-being. The present study examined whether this principle also applies to the well-being of kindergarten teachers who typically focus more strongly on the social–emotional development of children. We measured kindergarten teachers’ (N = 250) affective, evaluative, occupational, and psychological well-being using established questionnaires, and their educational goals and experienced emotions for children showing undesirable (e.g., children who provoke others, disrupt activities, cause physical harm) and desirable (e.g., children who share toys, comfort others, tidy up) behaviors using photorealistic pictures. Replicating the pattern observed for school teachers, the higher the goals and the more positive the emotions for children showing undesirable behaviors, the higher the well-being. By contrast, well-being was unrelated to the goals and the positivity of emotions for children showing desirable behaviors. However, the well-being of the kindergarten teachers was not completely unaffected by children showing desirable behaviors, as well-being was higher the higher the emotional arousal was in response to such children. These findings suggest that kindergarten teachers’ well-being could be improved by helping them to set high educational goals and experience more positive emotions for children showing undesirable behaviors, and to experience higher arousal for children showing desirable behaviors

    Functionalization, Fragmentation, and Expansion of cyclo‐P4R2 Ligands

    In this study, three isolobal complexes of the form [{LnM}(η3-P4R2)]+ ({LnM} = {CpMo(CO)2} (A), {Cp'''Ni} (B), {Cp'''Co}– (C), R = Ph, iPr; Cp''' = 1,2,4-tBu3C5H2) are reacted with nucleophilic carbenes (L). While C does not show any reactivity, the cationic complexes A and B undergo addition reactions. The respective products [CpMo(CO)2(η3-P4R2L)]+ (1a – d) and [Cp'''Ni(η1:1-P4R2L)]+ (3a – d) show different geometries for the L-P3PR2 ligands. Their reactivity towards EtO– results in either a simple addition (for 1) or a complex addition, ring-opening, rearrangement sequence (for 3). Moreover, [Cp'''Ni(η2-IDippPP(OEt)PPiPr2)] (4) could be methylated by the reaction with MeOTf, which affords an iso-tetraphosphine ligand, marking the first example of complete functionalization of a polyphosphorus ligand to a complexed phosphine. Mechanistic studies shed light upon the fundamental principles, which differentiate the influence of the isolobal {CpMo(CO)2}, {Cp'''Ni}, and {Cp'''Co}– transition metal units. Lastly, 3a–d were chosen as model substrates for further nucleophilic functionalization. In this regard, 3 reacts with [CN]− in a [3+1] fragmentation reaction affording the dimerized species [{Cp'''Ni}2(μ,η1:1:1:1-cyclo-P4(PR2)2)] (6a: R = Ph, 6b: R = iPr) together with IDippP-CN. In contrast, the reaction with [ECO]− (E = P, As) led to an extension of the pnictogen framework yielding [Cp'''Ni(η1:1-EP4Ph2IDipp)] (8a: E = P, 8b: E = As)

    Hierarchical clustering analysis & machine learning models for diagnosing skeletal classes I and II in German patients

    Background Classification is one of the most common tasks in artificial intelligence (AI) driven fields in dentistry and orthodontics. The AI abilities can significantly improve the orthodontist’s critical mission to diagnose and treat patients precisely, promptly, and efficiently. Therefore, this study aims to develop a machine-learning model to classify German orthodontic patients as skeletal class I or II based on minimal cephalometric parameters. Eventually, clustering analysis was done to understand the differences between clusters within the same or different skeletal classes. Methods A total of 556 German orthodontic patients were classified into skeletal class I (n = 210) and II (n = 346) using the individualized ANB. Hierarchical clustering analysis used the Euclidean distances between data points and Ward’s minimum variance method. Six machine learning models (random forest (RF), K-nearest neighbor (KNN), support vector machine (SVM), linear discriminant analysis (LDA), classification and regression trees (CART), and General Linear Model (GLM)) were evaluated considering their accuracy, reliability, sensitivity, and specificity in diagnosing skeletal class I and II. Results The clustering analysis results showed the power of this tool to cluster the results into two–three clusters that interestingly varied significantly in many cephalometric parameters, including NL-ML angle, NL-NSL angle, PFH/AFH ratio, gonial angle, SNB, Go-Me (mm), Wits appraisal, ML-NSL, and part of the dental parameters. The CART model achieved 100% accuracy by considering all cephalometric and demographic variables, while the KNN model performed well with three input parameters (ANB, Wits, SNB) only. Conclusions The KNN model with three key variables demonstrated sufficient accuracy for classifying skeletal classes I and II, supporting efficient and still personalized orthodontic diagnostics and treatment planning. Further studies with balanced sample sizes are needed for validation

    Adenininduzierte Nierenschädigungen in Mäusen: Bedeutung, Expressionsstärke und Lokalisation von LCN2 und Kim1

    In den letzten Jahrzehnten trat die Bedeutung von LCN2 und KIM1 als Biomarker für renale Schädigungen in den Vordergrund. Beim akuten Nierenschaden lieferten viele Studien sehr vielversprechende Ergebnisse. Der Ablauf der renalen Schädigung bei chronischen Nierenerkrankungen unterscheidet sich zum Teil erheblich vom Verlauf beim akuten Schaden. Daher wären auch Untersuchungen eines möglichen Zusammenhangs der beiden Marker mit der Schwere des Schadens im Zuge der CKD notwendig. Hierbei stellte sich LCN2 als vielversprechender Marker heraus. Bei bestimmten Arten der CKD scheint ein Zusammenhang zwischen der KIM1-Expression und der GFR und anderen Nierenfunktionsparametern zu bestehen. Daher sind weiterführende Untersuchungen von KIM1 sinnvoll. Hierfür werden auch Tiermodelle benötigt um die KIM1-Expression genauer untersuchen zu können. Das Adeninmodell ist ein etabliertes Modell zur Induktion einer CKD, welches aufgrund seiner relativ einfachen Durchführung gut reproduzierbar ist. Ob die KIM1-Expression im Adeninmodell im Zusammenhang mit der renalen Schädigung steht, war bislang unklar. Die Induktion eines Nierenschadens mittels Adenin führte zu einem Anstieg der KIM1-Expression in den proximalen Tubuli. Dabei war die Expression jedoch nur segmentär erhöht. Kohybridisierungen mit Kollagen 1 und histologische Untersuchungen des Gewebes ergaben, dass es sich um die geschädigten Nierenabschnitte handelte. In der Erholungsphase nahm die KIM1-Expression schnell ab und scheint in einem engen zeitlichen Zusammenhang mit der Nierenschädigung im Adeninmodell zu stehen. Des Weiteren konnte während der Beobachtung keine extrarenale KIM1-Produktion festgestellt werden. Es ist von einer hohen Nierenspezifität von KIM1 auszugehen. Ein eindeutiger Zusammenhang der KIM1-Protein-Konzentrationen im Urin und im Plasma mit der GFR konnte nicht nachgewiesen werden. Jedoch eignete sich die KIM1-Konzentration im Urin gut um eine Schädigung der Niere anzuzeigen. Auch die LCN2-Expression nahm während der Adeningabe stark zu und erreichte nach den drei Wochen der Erholung wieder nahezu die basalen Werte. Die LCN2-Konzentrationen im Urin und im Plasma scheinen mit der GFR assoziiert zu sein. Sowohl Plasma- als auch Urin-Konzentration spiegeln den renalen Schaden wider. In der Niere wird LCN2 im distalen Tubulus und in den Sammelrohren exprimiert. Eine Anreicherung von LCN2-Protein im proximalen Tubulus konnte nicht beobachtet werden. Weiterhin konnte keine Erhöhung der extrarenalen LCN2-Expression festgestellt werden
