    Implementation of Evidence Based Practices to Reduce Postoperative Sore Throat

    An estimated 15 million endotracheal tube (ETT) intubations occur annually in the United States. In addition to pain and nausea, one of the leading complaints by patients after intubation is a sore throat. Postoperative sore throat (POST) is caused by increased pressure on the trachea from the ETT during intubation and after the ETT cuff has been inflated. To prevent the occurrences of POST many options were explored, including cuff material, shape, filling media, and ETT tube size. Through research one of the ways to reduce POST is by monitoring the cuff pressure of the ETT against the tracheal. Typically, Anesthesia providers use indirect measurements of the ETT cuff pressure such as pilot balloon palpation or minimal leak testing instead of a direct measurement with a manometer. Direct ETT cuff pressure monitoring is shown to be an effective way to monitor ETT cuff pressures when compared to other monitoring methods. Inadequate measurement of the pressure in the ETT cuff can lead to POST in many patients. The reduction of the instances of POST is important as it can not only cause physical harm to patients but also create a financial burden to them, facilities, and providers. The goal of this project implementation is to reduce the occurrences and duration of POST by directly monitoring the ETT cuff pressure intraoperatively with a manometer

    Standardized Handoffs For Anesthesia Students

    Communication between healthcare providers is an essential aspect of caring for patients. Effective communication is vital during the patient handoff process. The patient handoff process involves exchanging patient’s medical information between a provider currently caring for the patient and the provider assuming care of the patient. If the information exchanged during the handoff process is inaccurate or essential information is omitted, that could potentially lead to harmful consequences for the patient. Currently, no standardized handoff tool exists for student registered nurse anesthesia students (SRNAs) in clinical practicum at the local level one trauma center. The lack of a standardized handoff tool at this facility suggests an increased likelihood of communication errors during the handoff process. The purpose of this quality improvement project is to implement a standardized handoff tool called the AneSBAR tool that SRNA students can utilize in their practice to ensure proper and effective handoffs in the post-anesthesia care unit (PACU) and the intensive care unit (ICU). The inclusion criteria for the project are 2nd or 3rd-year SRNA students currently enrolled full-time in the local Certified Registered Nurse Anesthetist (CRNA) program. The quality improvement project will follow a Plan-Do-Study-Act (PDSA) model. Both qualitative and quantitative data will be collected to determine the impact of the standardized handoff tool. Data collected from surveys and simulations will determine if the PDSA cycle needs repeated or if the QI measure is ready for implementation

    Development and Implementation of an Evidence-Based Practice Guideline Regarding Cricoid Pressure

    Cricoid pressure (CP) has been a standard of practice since the 1960s. The purpose is to reduce aspiration of gastric content into the lungs in high-risk patients undergoing induction of general anesthesia. Aspiration has been identified as the most common cause of airway-related mortality for patients undergoing general anesthesia. The cause of airway-related mortality can be correlated to the application of CP. Incorrect application of CP may lead to adverse clinical events and outcomes. It has been demonstrated that when administering CP in a simulation-based environment, providers are applying pressure to the incorrect anatomical location and/or applying the incorrect amount of force. The project\u27s primary purpose is the development of an evidence-based practice (EBP) guideline which can be utilized to possibly reduce anesthesia related aspiration mortality for patients undergoing general anesthesia. This will be achieved by improving the quality of the application of CP through adopting an EBP approach to educate providers on the correct application. The project also includes a plan for implementing these EBP guidelines through education and training, monitoring the outcomes, and providing changes to the guidelines if the outcomes are less than desirable. The Plan-Do-Study-Act model was used as a framework to guide this scholarly project
