    The Effect of Fatigue on Lower Extremity Joint Kinematics and Performance

    Within recent years there has been an increase in the number of anterior cruciate ligament (ACL) injuries within female athletes (Learning how ACL, 2022). The ACL is a ligamentous structure within the knee that connects the femur and the tibia (Mayo, 2022). Its primary function is to provide stability, prevent the tibia from excessive anterior translation, and provide proprioceptive input for the lower limb (Mayo, 2022). ACL injuries are multifactorial in nature, meaning they can be the result of faulty biomechanics, individual genetic predisposition, or environmental factors (Alentorn- Geli et al., 2009). ACL injuries can be classified into two categories: contact and non-contact with non-contact being the more common injury type (Alentorn- Geli et al., 2009). Non-contact ACL injuries are classified as those which occur without the presence of another person or thing contacting the lower extremity of the injured person. One of the biggest causes of a non-contact ACL injury is an athlete moving over a planted foot with a valgus knee, but there are many other factors (Brophy, 2021). Non-contact ACL injuries are most common in females (Nasseri, 2021). Some purport that females anatomically have an increased risk of ACL injuries due to their increased Q angle, increased joint laxity, increased pelvic width, and menstrual hormone levels (Lewis, 2000). Others have found an increase in physiological stress and a decrease in self-esteem have also been shown to increase an individual’s chances of an ACL injury (Christino, 2016). A focus of most healthcare professionals including physical therapists and athletic trainers includes the screening for preventable risk factors for non-contact ACL injuries. Preventable risk factors include asymmetry between a person’s limbs, decreased hamstring strength, increased muscle fatigue, decreased landing angle of hip and knee flexion, increased valgus landing, and lack of flexibility in the hip internal rotators (Brophy, 2021). Given the short and long-term negative consequences of an ACL injury, it is important for healthcare providers to explore screening and prevention techniques to employ with at-risk athletic populations

    Activated Clotting Times and Rebleed Rates in Pediatric Patients Post Cardiac Catheterization: A Pilot Project Proposal

    Cardiac catheterization (CC) procedure is a minimally invasive procedure that requires arterial and/or venous access with the introduction of a small tube called a sheath that allows for the passage of wires and catheters into the heart. Heparin is a medication known as a ‘blood thinner’ that is administered throughout the procedure to prevent clots from forming. The activated clotting time (ACT) is a diagnostic point of care test used to measure how long it takes blood to clot. Post CC care requires patients to lie flat for several hours to ensure bleeding will not occur at the punctured vessel site. Therefore, the ACT is a significant factor to consider before pulling sheaths at the end of a CC procedure to reduce the risk of post procedure bleeding. Currently no pediatric protocol exists for appropriate ACT level for sheath removal post CC in the pediatric population. The project team proposes a pilot project to collect data by performing a retrospective chart review on whether the peak ACT and ending post procedure ACT greater than 180 seconds affect rebleed rates post procedure at a large pediatric hospital located in Central Ohio. Rebleeding post CC procedure requires extra monitoring of the pediatric patient and places the patient at risk for agitation and a prolonged hospitalization. The scholarly project proposal concludes with a discussion of the barriers endured while trying to complete the project and provides recommendations to mitigate such challenges for future projects.     Effects of Benzyl Isothiocyanate and Papain on Parascaris Spp. Larval Embryonation

    Ascarids (Parascaris equorum and Parascaris univalens) are a common type of intestinal parasite found in foals. By the age of one year, most horses have built a natural immunity to Parascaris spp. However, Parascaris spp. are known to be the most pathogenic type of parasite in foals and can cause poor growth, weight loss, colic, and even death. Unfortunately, Parascaris spp. have developed resistance to most anthelmintic drugs. Prior studies have found promising results for the use of ground papaya seeds as a short-term anthelmintic. Further research has shown similar results when determining the effectiveness of compounds found in papayas, benzyl isothiocyanate (BITC) and papain, as anthelmintics for equine strongyles. The aim of this in vitro study was to determine if compounds from papaya; BITC and papain, reduce Parascaris spp. larval embryonation. To determine whether ascarid eggs would embryonate in the presence of these compounds, eggs were incubated in different concentrations of BITC and papain in triplicate in well plates. Experimental wells were compared to control wells with either no chemical exposure or with concentrations of pyrantel (a commonly used ascarid anthelmintic). After exposure, the number of embryonated eggs were counted using an inverted microscope and were expressed as the percentage of the total and compared to each other and the control wells. A two-way completely randomized ANOVA was used to compare mean egg embryonation among treatments and chemical concentrations. Results from the two-way ANOVA test concluded that there was no statistically significant difference between the proportion of larvated ascarid eggs found in pyrantel, BITC, and papain at each chemical concentration.     Examining the Need for Change by Describing the Attitudes and Perceptions of Team Communications Related to Patient Care and Safety Among Ambulatory Clinic Healthcare Staff

    Medical errors account up to 250,000 patient deaths annually. Research suggests medical errors are attributable to poor healthcare team communications. The Institute of Medicine posits that communication and teamwork are essential components to safe and successful health care environments. According to the Joint Commission poor communication is considered the root cause of nearly 80% of all serious medical errors. Consequently, the Department of Defense and Agency for Healthcare Research and Research created a team-based training program, known as TeamSTEPPS®, which has shown to improve team communication, performance, effectiveness, patient safety, satisfaction, and health outcomes in healthcare settings. Despite evidence supporting TeamSTEPPS®, one family practice clinic was not practicing standardized team communications and at an increased risk for potential miscommunications, medical errors, and adverse patient health outcomes. Consideration of perceptions and attitudes of staff before implementing system process changes like TeamSTEPPS® is important to ensure program success. Thus, the project’s purpose was to assess and describe the staff attitudes, perceptions, and intent to make change, regarding team communication as it relates to clinic patient care and safety. The descriptive scholarly project focused on staff questionnaire responses about perceived team communications and intent to change communication processes in a clinic setting. A systematic data review indicated 86% of respondents agreed the clinic was at risk for committing medical errors, 71% strongly agreed a standardized communication method would benefit the clinic, and 64% of respondents remained neutral regarding to the staff receiving TeamSTEPPS®. Thus, more research is warranted before effective implementation and sustainment can occur at the clinical project site

    Communication Assessment Tools for Emergency Department Nurses who Interact with Individuals with Intellectual and Developmental Disabilities

    Abstract/Executive Summary Problem Statement: Communication is an essential part of every nurse-patient interaction. During the best of times the patient and nurse can clearly understand each other and engage in successful interaction with positive patient outcomes. Individuals with intellectual and developmental disabilities (IDD) often use nontraditional ways of communicating that can complicate the ideal nurse-patient interaction. When a patient presents with a different method of communicating, the nurse must possess tools available to facilitate a successful interaction. Identifying such a tool or if one cannot be found building a sample tool is vital for positive patient outcomes for individuals with communication barriers. Purpose: The purpose of the project is to find a tool for emergency department (ED) nurses to use with patients who communicate differently. Methods: A web-based search for guidelines and a search to identify experts in communication to consult were conducted to find a nursing guideline. Other fields of study were investigated to create recommendations for a potential tool for ED nurses to use in practice. AGREE II was used to assess guidelines and Qualtrics was used to create and disperse the survey. Inclusion criteria: Participants need to be able to access an online survey, read, comprehend, and respond to survey questions in English. Analysis: The responses were evaluated for common themes, and descriptive statistics and thematic analysis were used to analyze the data. Implications for Practice: Identifying or building a communication tool is vital for positive patient outcomes for individuals with communication barriers

    Writing Against Progress: Frank Herbert\u27s Anti-Utopia

    I argue that there is a vitally important dimension of Utopian politics which infects the first three novels of the Dune series, Dune (1965), Dune Messiah (1969), Children of Dune (1976) which goes unacknowledged in the scholarship surrounding the Dune series. I argue that Herbert uses the Dune series to articulate what literary theorist, Fredric Jameson, describes as an anti-Utopia, a baleful vision of Utopia. He argues that the Utopian projects which seek to imagine new ends for humanity require giving up the self to some vaster collective. In the text, I explore how this baleful vision of Utopia appears: (1) in Herbert’s representations of the body and mind racing past their biological limits (2) in the formal qualities of narrative causality in the Dune series (3) in Herbert’s depiction of collectivity swallowing the individual. In the end, Herbert’s hatred of Utopian forms of progress ensures he invests his politics in inaction, conservation, and stagnation in line with various figures of movement conservativism after the second world war

    Implementation of a Diabetes Education Program in the Correctional Setting: A Project Proposal

    Diabetes is a chronic disease requiring patient responsibility to ensure good control and reduced morbidity and mortality. Incarcerated diabetic adults suffer from poor control and complications associated with their disease partly due to their limited health literacy. Arming diabetics with knowledge related to diabetes improves disease control and healthcare outcomes. The proposed final scholarly project involves the implementation of a diabetic education initiative to assess the effects of diabetic-specific education on self-care activities. The proposed quality improvement project will be framed with the Plan-Do-Study-Act and Nola Pender\u27s Health Promotion Model. The proposed final scholarly project involves obtaining a baseline assessment, implementing an educational intervention, and determining if the intervention improves glycated hemoglobin and diabetic self-management as measured by the Diabetic Self-Management Questionnaire (DSMQ). A convenience sample of diabetic incarcerated adults will be offered the opportunity to participate in an educational initiative that includes four one-hour and one individual counseling sessions. The proposed project will use multiple tools to collect data, including the DSMQ, a demographic survey, and a post-test questionnaire. Data from the pre-intervention and post-intervention DSMQ and self-reported glycated hemoglobin will be collected anonymously and aggregated. A paired t-test will determine if the educational initiative positively affects the data. Positive outcomes on this DNP project could lead to implementing a diabetic education curriculum within the correctional setting. Future research could explore alternative, cost-effective methods to implement diabetic education in the correctional setting
