    Does proprioception of knee improve after various forms of training in osteoarthritis of knee?

    The present study was aimed to investigate the role of various forms of training in improving the proprioception of the knee joint in patients with osteoarthritis of knee. Sixty participants (male and female) with knee Osteoarthritis of Grade II & III (Kellgren-Lawrence criteria) in the age range of 50-73 years were recruited from the Department of Orthopedics, Hospital Universiti Sains Malaysia. The recruited participants were taken to the Exercise and Sports science laboratory and the laboratory of School of Medical Sciences, Universiti Sains Malaysia for baseline assessment. The level of proprioception was assessed with the aid of isokinetic device BIODEX 4 System Pro. The participants were randomly categorized into four groups (Gr.) (Viz. Gr. A, Gr. B, Gr. C and Gr. D) and thus each group consisted of 18 participants. Gr. A participants received Conventional Physiotherapy consisting of strengthening exercises; stretching exercises and range of motion exercises. Gr. B received Isokinetic Training at velocities of 90° and 150°/second. Gr. C participants received EMG Biofeedback training with Mega ME 6000 device and Gr.D participants recieved a combined intervention of isokinetic training and EMG Biofeedback. The training protocol was designed for 25 - 30 minutes per session; 2 sessions per week for 10 weeks (20 sessions). The assessment was done after 5th week of training (mid term) and after 10th week of training (post) following baseline assessment protocol. A follow up assessment was performed after the 14th and 18th week to assess the after effect of training. Meanwhile the participants were not given any training after the 10 week period. They were also adviced not to get engaged in any sort of physical training, exercise or sport programme. Repeated measure of ANOVA revealed that, all interventions helped in improving the proprioception of the knee joint however post hoc analyses implicated that a combined intervention of Isokinetic Training and EMG biofeedback was beneficial in enhancing proprioception than the conventional physiotherapy (P<.05)

    Pegangan akidah adalah asas kekuatan rohani dan jasmani yang menjadi benteng umat Islam dari kerosakan, kesesatan, dan kekufuran. Bagi memelihara akidah umatnya, Islam mengajak umatnya menyeru ke jalan amar maaruf nahi mungkar serta berpesan-pesan kepada kebenaran dan kesabaran bagi mengelak kerugian dalam kehidupan duniawi dan ukhrawi. Amar maaruf nahi mungkar adalah tanggungjawab semua umat Islam terutamanya bagi para pendakwah (da’i) dan konsep ini sama dalam konsep kaunseling yang turut membimbing dan menasihati klien kepada perubahan ke arah kebaikan. Dalam perspektif Islam, kaunseling membawa klien (mad’u) memahami keterangan dan penjelasan yang seiring dengan syariat Allah tanpa merosak dan menyesatkan akidah sama ada dalam fikiran dan tindakan mereka. Hal ini menunjukkan bahawa hubungan di antara kaunseling dan dakwah amat jelas apabila proses kaunseling melibatkan usaha menyampaikan pengajaran dan pembelajaran psikososial yang berlaku antara kaunselor dan klien

    Kertas konsep ini membincangkan mengenai definisi, konsep kaunseling menggunakan pendekatan Islamik dan peranan seorang kaunselor Islamik dalam membantu klien Islam samada secara individu atau kelompok yang sedang menghadapi masalah dapat membuat keputusan seperti mana kehendak syariat Islam. Pendekatan Islamik dalam kaunseling ini diharapkan dapat memberi manfaat untuk diri klien sebagai seorang muslim yang sedang menghadapi permasalahan, pada akhirnya dapat kebahagiaan ketenangan ketenteraman kerahmatan keberkatan di dunia dan akhirat. Seseorang kaunselor Islamik perlu mempunyai ilmu pengetahuan berkaitan dengan pendekatan Islamik dalam mengendalikan sesi kaunseling, berkemahiran menggunakan kaedah intervensi akal dan roh keimanan bagi merawat klien sehingga terhasil amalan akhlak yang mulia pada diri klien. Kaunselor Islamik juga perlu mempunyai sifat-sifat personaliti keimanan akhlak yang luhur, mulia dan unggul, niat hati yang suci tulus ikhlas dalam membantu klien yang sedang menghadapi permasalahan demi kerana Allah SWT

    Hoarsness in child : a missed diagnosis

    Background: Hoarseness is not uncommon in children. It is described as a change of quality of voice. Most often it is due to self-limiting or non life threatening causes like viral upper respiratory infection or voice abuse. Foreign body in the airway can also solely presents with hoarseness of voice which later might potentially causing catastrophic event to the patient. We present a case of foreign body that impinged on the vocal cord which was missed initially. Reports: 11months old girl, with no known medical illness, had 1 choking episode followed by vomiting after taking rice. She presented to emergency department the same day as mother noticed patient started to have hoarseness of voice whenever she cried. No cyanotic episode or shortness of breath. Lungs was clear with no rhonchi. X ray was normal. She was treated for viral croup and discharged after 5 days of admission. Upon discharge, hoarseness was better but still persist. Patient presented again to hospital the next day with severe respiratory distress and stridor. Multiple nebulisation were given, however distress persist. Patient was subsequently planned for intubation. Upon intubation, emergency physician noticed there was a foreign body impinged on the vocal cord. Emergency operation was done with tracheostomy first for securing of airway, followed by direct laryngoscopy with foreign body removal. Intravenous dexamethasone was given for 1 day. Patient’s tracheostomy was removed 2 days later after repeated flexible nasopharyngolaryngoscopic examination (FNPLS) with normal laryngeal findings. Patient was discharge well 2 days later. Conclusions: Diagnosing laryngeal foreign body in infants can be challenging as they can’t complaint about it. Furthermore, the symptoms can sometimes be unclear. Doctors must always alert of such possibility to minimize the potential lethal complications it caused

    Fragmentation in MAC IEEE 802.15.4 to Improve Delay Performance in Wireless Body Area Network

    A Medium Access Control (MAC) protocol plays a significant role in enhancing the performances of Wireless Body Area Network (WBAN). This technology is commonly implemented IEEE 802.15.4 standard that works under CSMA/CA protocol. In addition, WBAN deals with probability of high traffic occurrence due to collection bulk of medical data that are generated from several bio-sensors through a continuous monitoring of patient's health. This phenomenon could result to congestion in the network which may lead to increment in delay and packet loss that could collapse the network. The increment in these two performances could lead to wrongly diagnosed of the diseases as well as late in medical help. Hence, the proposed mechanism aimed to reduce the delay through fragmenting the packets by decrementing the slot time in MAC 802.15.4 protocol under different number of number nodes with constant packet size and bit rate. The performances of the proposed mechanism are measured in terms of delay and packet loss by using OMNeT++ simulator tool. Based on the collected results, the proposed mechanism has shown a promising result in reducing the delay which means that it has improved the network performance of WBAN

    A chaotic jerk system with three cubic nonlinearities, dynamical analysis, adaptive chaos synchronization and circuit simulation

    A 3-D new chaotic jerk system with three cubic nonlinearities is proposed in this paper. The dynamical properties of the new jerk system are described in terms of phase portraits, Lyapunov exponents, Kaplan-Yorke dimension, symmetry, dissipativity, etc. Also, a detailed dynamical analysis of the jerk system has been carried out with bifurcation diagram and Lyapunov exponents. As an engineering application, adaptive synchronization of the new chaotic jerk system with itself is designed via backstepping control method. Furthermore, an electronic circuit realization of the new chaotic jerk system is presented in detail to confirm the feasibility of the theoretical chaotic jerk mode
