Virtual Cadaver Dissection Prior to Clinical Experiences in Nurse Anesthesia
Anatomy and physiology (A&P) courses are the foundation of anesthesia training. Student registered nurse anesthetists (SRNAs) must be A&P experts as they perform clinical skills for the first time. Traditionally, cadaver dissection has been used to enhance comprehension of A&P. However, not all facilities can allocate the time, space, or financial resources for cadaver dissection within their programs of study, especially at institutions that lie outside of major medical centers. Despite the presence of a virtual cadaver at a nurse anesthesia program in the Midwest, its introduction to SRNAs as a resource had not been fully employed. Compassion Fatigue and Burnout during the COVID-19 pandemic
Abstract
The COVID-19 pandemic has pushed nurses to exhaustion while leaving them with compassion fatigue (CF) and burnout (BO). Compassion fatigue is secondary to traumatic stress and exposure, while burnout is the state of exhaustion of one’s ability to manage with their work and personal everyday stressors. This study aimed to determine whether a daily use of a meditation app could change quality of life indicators amongst front-line nurses affected by COVID-19. With the help of the ProQol assessment tool, along with a pre and post intervention surveys, participants’ awareness and personal experiences were evaluated. The study consisted of nurses at two St. Louis, Missouri regional hospitals that have been affected by the COVID-19 pandemic. Findings support the essential role of CF and BO education as well as self-care interventions in healthcare workers’ abilities to perform at the maximum level. Interventions aimed at reducing compassion fatigue and burnout can help to decrease and manage these symptoms
Care of the Healthcare Provider
Evidence has shown a strong correlation between a healthy provider and the positive care that the provider can promote to others. Many evidence-based health promotion programs have been implemented throughout many health systems. These programs have shown improvement of the providers’ overall health. And in return, these providers are able to better care for their patients. Detailed reports have shown many health consequences that healthcare providers have acquired due to long hours, intense training, lack of time, lack of sleep, and the concept of always putting their patients first. A health system in Springfield, Illinois was examined to determine if a health promotion program would benefit their providers. This health system had not yet implemented a health promotion program for providers, due to time and uncertainty if people would participate.
Surveying was taken into great detail to evaluate what areas of health providers would like to focus on improving, and how they would want this information presented to them. Providers also rated their overall health. This information was interpreted and presented to stakeholders at this specific health system. An explanation of the data collected was provided, as well as a sample health promotion program they would be able to implement. This sample program was developed using an evidence-based planning method. This specific program was developed based on survey results. An evaluation was then completed after the presentation to determine the likelihood they would implement the health promotion program, along with any potential issues. A follow-up project would be recommended to implement the actual program discussed in this project and to evaluate its effectiveness
Developing an Educational Pamphlet for Patients Undergoing Interscalene Blocks with Liposomal Bupivacaine Exparel
There are over one million shoulder arthroscopies and arthroplasties performed worldwide each year, with interscalene blocks (ISBs) the typical treatment choice for postoperative analgesia (Etkin & Springer, 2017). In 2018, Exparel® was FDA approved for use in ISBs and educational materials currently are scarce (Pacira Pharmaceuticals, 2019). The effectiveness of pamphlets in the clinical setting has been well established (Sustersic, Gauchet, Foote, & Bosson, 2017). Pamphlets improve health literacy by providing patients a tool to aid in recall and adherence to postoperative instructions (Sustersic et al., 2017). This project reviewed the available literature on Exparel® ISBs, health literacy, and pamphlet development in order to create an educational pamphlet for patients at a tertiary care center in central Illinois. The pamphlet explains home care after Exparel® ISB and includes the anesthesia department’s contact information. The Flesch-Kincaid readability scale was used to ensure the content was at or below an eighth-grade level. The language in the pamphlet scored at a seventh-grade level. The pamphlet and summary of the literature review were presented via a voiceover PowerPoint and emailed to potential participants in the anesthesia group. Participants were surveyed to collect feedback and overwhelmingly expressed the desire to adopt the pamphlet after minimal changes. Educational pamphlets have the potential to improve health literacy and lead to better patient outcomes
Standardizing Evaluation Methods for Simulation
Background: Evidence suggests that student registered nurse anesthetists (SRNAs) benefit from simulation in the educational curriculum (Turcato, Roberson, & Covert, 2008). When simulation is implemented into coursework, it is important to evaluate student learning and perceptions (Adamson & Kardong-Edgren, 2012), however, there are few standardized instruments available (Adamson et al., 2012; Doolan et al., 2016).
Objectives: The goal of this project was to provide the foundation for standardizing simulation evaluation tools within a midwestern university’s school of nurse anesthesia.
Methods: A presentation on the Value Analysis Model was developed through extensive use of the literature and collaboration with a content expert. As participants were walked through a fictional scenario, they learned how to write evaluations, interpret data, and calculate return on investment. The presentation was attended by simulation-focused faculty from the university, anesthesia students, and members of the St. Louis Simulation Interest Group. To promote project sustainability, a binder with information and resources on the Value Analysis Model was given to simulation faculty at the university for future use.
Results: Surveys were designed using the Value Analysis Model and responses were kept anonymous. Participants agreed that the material presented was important, relevant to their work, useful, and applicable. Participants also agreed that they gained a basic knowledge of the topic and how to apply it to simulation.
Conclusions: This project served as the first step toward developing standardized evaluation instruments for the university department of nurse anesthesia

This public dataset contains transcripts of 8 in-depth semistructured interviews with drag artists. Destiny Baxter conducted these interviews during Spring 2020. The prevalence of psychiatric and chronic pain comorbidities in fibromyalgia: An acttion systematic review
Fibromyalgia (FM) is a chronic widespread pain condition that overlaps with multiple comorbid health conditions and contributes to considerable patient distress. The aim of this review was to provide a systematic overview of psychiatric and chronic pain comorbidities among patients diagnosed with FM and to inform the development of recommendations for the design of clinical trials. Thirty-one, cross-sectional, clinical epidemiology studies that evaluated patients diagnosed with FM were included for review. None of the reviewed studies reported on the incidence of these comorbidities. Sample size-weighted prevalence estimates were calculated when prevalence data were reported in 2 or more studies for the same comorbid condition. The most prevalent comorbidity across all studies reviewed was depression/major depressive disorder (MDD) with over half of the patients included having this diagnosis in their lifetime (weighted prevalence up to 63%). In addition, nearly one-third of FM patients examined had current or lifetime bipolar disorder, panic disorder, or post-traumatic stress disorder. Less common psychiatric disorders reported included generalized anxiety disorder, obsessive compulsive disorder, and specific phobias (agoraphobia, social phobia). There were fewer studies that examined chronic pain comorbidities among FM patients, but when evaluated, prevalence was also high ranging from 39% to 76% (i.e., chronic tension-type or migraine headache, irritable bowel syndrome, myofascial pain syndrome, and temporomandibular disorders). The results of the review suggest that depression and chronic pain conditions involving head/jaw pain and IBS were elevated among FM patients compared to other conditions in the clinic-based studies. In contrast, anxiety-related disorders were much less common. Addressing the presence of these comorbid health conditions in clinical trials of treatments for FM would increase the generalizability and real-world applicability of FM research

Discharge Process Optimization
As the demands of the nurse become increased with higher acuity and documentation requirements, the delivery of discharge instructions is often one of the most, and at times, only, formal education opportunities between the nurse and the patient. It is imperative to modify the discharge process to ensure patients have the ability to confidently care for themselves after they leave the hospital. The aim of this project was to create a nursing discharge workflow that meets the educational needs of the patients. This project was a quality improvement project designed to change the nursing workflow to provide a better discharge process for patients. Staff were educated on the process by creating an interactive module. The conclusion was that the discharge process workflow was effective in increasing patient satisfaction scores related to the discharge process, however ineffective in increasing overall patient satisfaction scores for the organization. The two main limitations were related to the data collected and the impact on nursing staff. The NRC patient satisfaction data is not specific to discharge process. Additionally, the COVID-19 pandemic created challenges for healthcare workers nation-wide during the timeframe this project was implemented. It is important for healthcare to keep adapting to create a discharge process that engages patients and allows them a better understanding of their plan of care. The results suggest that implementing a standard discharge process may increase aspects of patient satisfaction, however will likely not drive overall satisfaction of the organization
Short Term Medical Mission Antibiotic Protocol
Background/Introduction: Short term medical mission trips are growing in popularity and with this growth the level of care provided must remain evidenced based. Mission groups traveling to developing countries consist primarily of volunteers. And while many of those serving on mission trips are medically trained, some have minimal healthcare knowledge. Pharmacists are not always available to dispense medications on mission trips and therefore it is common for registered nurses in the pharmacy to dispense the medications prescribed by the providers. Because the medications available on mission trips are limited, often times substitutions must be made, especially when it comes to antibiotics.
Purpose: To help ensure that the standard of care was maintained, an antibiotic protocol was developed for a short-term medical mission trip in Guatemala.
Methods: A pre-test will be given to all pharmacy team members on the trip to assess their baseline knowledge regarding antibiotics. After the pre-test, an educational session will be held to explain the protocol and how to utilize it. On the last day of the trip, a post-test will be given to the team to evaluate if antibiotic knowledge improved after learning and using the protocol to make appropriate antibiotic substitutions.
Results: There was an increase in understanding of antibiotic substitutions after the educational session, and utilizing the protocol in the pharmacy. The participants reported an ease of use and demonstrated the understanding of the antibiotic protocol on the post test compared to the pre-test.
Limitations: The long-term impact of the project cannot be evaluated on one mission trip.
Conclusions/Implications for Practice: Long-term use of this protocol on future mission trips will be helpful for continued evaluation
