Efficacy of subsequent treatments in patients with hormone-positive advanced breast cancer who had disease progression under CDK 4/6 inhibitor therapy
Background: There is no standard treatment recommended at category 1 level in international guidelines for subsequent therapy after cyclin-dependent kinase 4/6 inhibitor (CDK4/6) based therapy. We aimed to evaluate which subsequent treatment oncologists prefer in patients with disease progression under CDKi. In addition, we aimed to show the effectiveness of systemic treatments after CDKi and whether there is a survival difference between hormonal treatments (monotherapy vs. mTOR-based).
Methods: A total of 609 patients from 53 centers were included in the study. Progression-free-survivals (PFS) of subsequent treatments (chemotherapy (CT, n:434) or endocrine therapy (ET, n:175)) after CDKi were calculated. Patients were evaluated in three groups as those who received CDKi in first-line (group A, n:202), second-line (group B, n: 153) and ≥ 3rd-line (group C, n: 254). PFS was compared according to the use of ET and CT. In addition, ET was compared as monotherapy versus everolimus-based combination therapy.
Results: The median duration of CDKi in the ET arms of Group A, B, and C was 17.0, 11.0, and 8.5 months in respectively; it was 9.0, 7.0, and 5.0 months in the CT arm. Median PFS after CDKi was 9.5 (5.0-14.0) months in the ET arm of group A, and 5.3 (3.9-6.8) months in the CT arm (p = 0.073). It was 6.7 (5.8-7.7) months in the ET arm of group B, and 5.7 (4.6-6.7) months in the CT arm (p = 0.311). It was 5.3 (2.5-8.0) months in the ET arm of group C and 4.0 (3.5-4.6) months in the CT arm (p = 0.434). Patients who received ET after CDKi were compared as those who received everolimus-based combination therapy versus those who received monotherapy ET: the median PFS in group A, B, and C was 11.0 vs. 5.9 (p = 0.047), 6.7 vs. 5.0 (p = 0.164), 6.7 vs. 3.9 (p = 0.763) months.
Conclusion: Physicians preferred CT rather than ET in patients with early progression under CDKi. It has been shown that subsequent ET after CDKi can be as effective as CT. It was also observed that better PFS could be achieved with the subsequent everolimus-based treatments after first-line CDKi compared to monotherapy ET
Pregnancy-Related Anxiety and Prenatal Attachment in Pregnant Women with Preeclampsia and/or Gestational Diabetes Mellitus: A Cross-Sectional Study
Objective: Health problems experienced during pregnancy may increase anxiety level during pregnancy and affect mother-baby attachment. The present study aimed to compare pregnancy-related anxiety and prenatal attachment levels between low-risk pregnant women and pregnant women with preeclampsia and/or gestational diabetes mellitus (GDM). Material and Methods: One hundred fifty women were recruited in this cross-sectional descriptive study (55 preeclampsia and/or GDM, 95 low-risk) pregnant women. Data were collected using descriptive data form, Pregnancy-Related Anxiety Questionnaire- Revised-2 (PRAQ-R2), and Prenatal Attachment Scale (PAS). Results: Prenatal attachment scores of the working pregnant women in the low- risk group were higher than those in the non-working pregnant women (p0.05). Conclusion: In line with the study results, it was found that low-risk and working pregnant women had higher prenatal attachment levels. Levels of prenatal attachment were higher in primiparous pregnant women in the high- risk group. As pregnancy-related anxiety levels of high-risk and healthy pregnant women increased, concern about one's own appearance scores increased. Therefore, reducing anxiety levels of pregnant women during pregnancy may be an effective intervention in reducing concerns about their appearance
A Late Holocene climate reconstruction from the high-altitude Lake Golcuk sedimentary records, Isparta (SW Anatolia)
A high-resolution multiproxy lake sediment dataset, comprising lithology, radiography, & mu;XRF elemental, magnetic susceptibility (MS), & delta;C-13, and & delta;O-18 measurements since ca. AD 400 is presented in this study. Changes in lithology, radiography, magnetic susceptibility (MS), & delta;C-13, and & delta;O-18 reflect wet/dry climate periods, whereas variability in log(Ca/K) can reflect warm/cold climate periods. Analyses of the multiproxy results allow the distinction of several climate periods, which may be associated with climatic phenomena such as changes in North Atlantic Oscillation (NAO) and/or solar activity. The influence of NAO-/NAO+ (negative/positive) is suggested to be related with the southward/northward displacement of the storm tracks resulting from the NAO-/NAO+ phases. For solar activity, the influence is explained through a direct increase in solar heating leading to calcite precipitation. The Dark Ages Cold Period (DACP, AD 450-750) reflects cold-dry climate conditions at this site, indicative of a positive North Atlantic Oscillation (NAO+) and low solar activity. The Medieval Climate Anomaly (MCA, AD 950-1250) exhibits wet-dry-wet and warm-cold-warm climate conditions. The wet/dry periods likely are associated with NAO-/NAO+, respectively, and the warm/cold period may reflect relatively high/low solar activity. The Little Ice Age (LIA, AD 1400-1850) is characterized by dry and cold climate conditions, suggesting the influence of NAO+ and low solar activity. Comparison of the results of this study with local and regional results suggests a generally similar climate pattern, which is indicative of similar climate mechanisms. The contradictions can be associated with age-related uncertainties, orographic differences, and/or other regional teleconnections
A novel dual-BJT avalanche pulse generator with mixer effect
This paper presents a novel, simple, and efficient pulse generation technique using BJTs in avalanche mode with mixer effect. The presented design is an improved version of the basic single transistor avalanche pulse generator, and it composes of two identical pulse generators that drive a common 50-omega load resistor at the output. The output signal exhibits wider bandwidth and narrower pulsewidth, compared to the basic single transistor circuit. The presented pulse generator also provides increased power output without using any power amplifier, and can be used in ground-penetrating radar and ultra-wideband communication systems

The main goal of the study is to produce eco-friendly furniture dyes by using bio-colorants and to determine the color stability of these dyes in outdoor conditions. In this regard, dye extracts obtained from red beet (Beta vulgaris), safflower (Carthamus tinctorius L.), and purple cabbage (Brassica oleracea), as a bio-colorant source, were applied to MDF test panels by mixing with water-based synthetic lacquer coatings. Also, three different synthetic dyes were applied to MDF test panels in order to compare the results with eco-friendly natural dyes. Natural dyes were mixed with metal and natural mordants such as iron sulfate, aluminum sulfate and vinegar. Lacquer coated test panels, coated with natural and synthetic dyes, were exposed to outdoor conditions for 50 days in Denizli/Turkey in order to assess the change in color. As a result, the maximum color stability occurred in the test panels lacquer coated with synthetic black dye, while the minimum color stability occurred in the test panels lacquer coated with synthetic light blue dye. It was determined that the thickness of the color coating layer applied to test panels increases color stability. It was also observed that the color stability performance of natural dyes is as good as that of synthetic dyes
The relationship between test anxiety, rumination, and psychological flexibility
Students’ anxiety over taking tests, which play a vital role in their educational lives, is very prevalent in our country. The purpose of this study is to reveal the pattern of relationships between test anxiety, psychological flexibility, and rumination. The survey included 247 (35%) men and 458 (65%) women from Muğla Sıtkı Koçman University’s several faculties. 705 individuals provided information. Test anxiety is classified as low, medium, or high. While psychological inflexibility and rumination were more prevalent in the high-test anxiety group, it can be demonstrated that being in the high-test anxiety group predicts psychological inflexibility and rumination. The relationship pattern between test anxiety, psychological flexibility, and rumination was investigated using structural equation modelling. Rumination was shown to be a significant variable in the relationship between test anxiety and psychological flexibility, the model analysed was found to be significant and all fit indexes were acceptable. Test anxiety has an important role in the individual’s psychological inflexibility and rumination
Psychometric properties of the Edinburgh Visual Gait Score in children with spastic cerebral palsy
BackgroundTo our knowledge, no other studies investigated the internal consistency of the Edinburgh Visual Gait Score (EVGS). The aim of our study was to determine the reliability and construct validity of the EVGS in children with cerebral palsy (CP).ResultsA total of fifty children with CP were enrolled in the study. Participants were evaluated with Gross Motor Function Classification System (GMFCS), Wisconsin Gait Scale (WGS), Gillette Functional Assessment Questionnaire (Gillette FAQ), Timed Up and Go Test (TUG), and EVGS (Rater-A and Rater-B). Slow-motion video analysis was used for the visual gait analysis. The inter-rater reliability, internal consistency, and construct validity of the EVGS were analyzed. The intraclass correlation coefficient (ICC) of the EVGS total score was 0.947 (CI: 0.90-0.97). Inter-rater reliability was excellent (ICC>0.80). The Cronbach's alpha value was 0.936, within the acceptable range (0.700.50). EVGS had moderate validity with GMFCS for both raters.ConclusionThe EVGS was reliable and valid. Internal consistency of the EVGS is high, indicating a consistent structure to assess gait in children with CP
Do Histologically Aggressive Subtypes of Papillary Thyroid Microcarcinoma have Worse Clinical Outcome than Non-Aggressive Papillary Thyroid Microcarcinoma Subtypes? A Multicenter Cohort Study
Histologically aggressive micropapillary thyroid carcinomas (PTMC) subtypes are thought to be associated with an aggressive clinical course. However, evidence for unfavorable clinical outcomes in patients with aggressive PTMC subtypes is not clear. In this study, we intended to determine the difference in clinical outcomes between patients with aggressive and non-aggressive PTMC subtypes. In this multicenter cohort study, the computer-recorded clinical and histopathological data of patients who underwent thyroid surgery between January 2000 - January 2021 in 9 referral centers and were diagnosed as PTMC were analyzed. A total of 1585 patients [female 1340 (84.5%), male 245 (15.5%), mean age 47.9 +/- 11.63 years), with a mean follow-up time of 66.55 +/- 37.16 months], were included in the study. Ninety-eight cases were diagnosed as aggressive and 1487 as non-aggressive subtypes. Persistent/recurrent disease was observed in 33 (33.7% )and 41 (2.8%) patients with aggressive and non-aggressive subtypes (p 1 cm in size. Therefore, the histopathological subtype of PTMC should be taken into consideration to tailor a personalized management plan
