Choosing the Route of Delivery: A Discussion of Vaginal and Cesarean Births
No full textThe percentage of cesarean deliveries in the United States has increased to a rate of 32.1%, double what the World Health Organization recommends as an ideal rate for maternal and fetal benefits. The majority of cesarean sections are performed on women who have never had the procedure before and this increases the risk for subsequent cesarean deliveries. Half of the indications for primary cesarean delivery were found to be non-reassuring fetal heart tracings and arrest of labor, both of which are subject to obstetric provider judgment and comfort level. Among these there is also an increasing incidence of elective primary cesarean deliveries. In order to mitigate the number of unnecessary cesarean sections, the morbidity and mortality of the surgery is necessary to consider for both patient and provider. In comparison to vaginal deliveries, cesarean deliveries carry an increased risk of overall maternal morbidity, including but not limited to thromboembolic events, abdominal adhesions, placental abnormalities in future pregnancies, hysterectomy, and longer hospital stay. Fetal risks include an overall increase in fetal mortality, decreased likelihood of breastfeeding, and emerging evidence of inappropriate immune system development. Conversely, vaginal deliveries carry an increased risk of hemorrhage requiring transfusion, perineal lacerations, and temporary urinary incontinence. These consequences, among others not identified in this article, are important for obstetric providers and patients to consider when electing between a cesarean or vaginal delivery

How and Why Do Race and Gender Affect ADHD Diagnosis Levels?
Get PDFAttention-deficit hyperactivity disorder (ADHD) is a disorder of the brain that impairs one’s daily functioning and focus. Previous research has shown that two groups are disproportionately under-diagnosed: females and Black people. It is important to understand why these groups, and possibly others, are under-diagnosed as those without a diagnosis do not get the treatment that they need. Literature reveals that differences in symptom presentation (inattentive vs. hyperactive), biases in symptom reports, barriers to accessing mental healthcare, and negative stigmas about, and within, certain groups lead to the under-diagnosis of females and Black people. In conclusion the diagnostic criteria of ADHD needs to be reviewed and revised to account for the under-diagnosis of these two groups. Past research states that objective measures, unaffected by bias, should be used more often in the diagnosis process as bias can further exacerbate this under-diagnosis. This is because bias could lead to an inaccurate report of symptoms, which could lead to an individual not getting a diagnosis even when they have ADHD. In addition to that, the healthcare system needs to be reformed when it comes to mental healthcare in order to break down those barriers that are keeping people from getting the support and treatment that they need

Following on the heels of the Mexican Revolution, an extended period of social upheaval and regional conflicts that transformed the country, artists and visionaries alike struggled to determine how the reborn nation would distinguish itself. While many movements in this period looked towards the future and sought utopia, there was one which concentrated instead on exploring the precolonial past and distilling the essence of “Mexicanity\u27\u27 from there. This movement was known as the Mexicanidad in Spanish; or, in the precolonial Nahuatl language, the Mexicayotl. In particular, the Mexicanidad believed that surviving indigenous civilizations had maintained a cultural identity which was independent of and reclaimed from the aggressive industrialization and de facto despotism that preceded the revolution, and thus ought to serve as a template for Mexico’s modern identity

Get PDFMore than half of all adults diagnosed with moderate-to-severe eczema suffer from the disease daily, despite initiation of treatment. It is known to be an incurable diagnosis, but the previously prescribed regimen of corticosteroids and moisturizing ointments are proving to be woefully inadequate. Patients report recurrence of flare-ups, despite adherence to topical therapy. The itch-and-scratch cycle is relentless. As the number of uncontrolled eczema cases continues to rise, providers are forced to re-evaluate the efficacy of these traditional topicals and look to other drug classes for relief. This article reviews the risks and benefits of the first FDA-approved injectable biologic for eczema called Dupixent®. So far, the response has been promising: after 4 months from the first injection, nearly 40% report almost clear skin.13 With it comes a notable improvement in quality of life, anxiety, and depression.17,18 This article further analyzes additional statistics and survey results that were gathered from the National Eczema Association, as well as from controlled experimental studies and real patient testimonies posted on the Dupixent® website. Supplementary findings were included to explicate the relationship between the pathophysiology of eczema and the pharmacodynamics of Dupixent®. Based on the evidence presented in this paper, it’s safe to presume patients with moderate-to-severe eczema can be reassured of a more optimistic prognosis

Treatment options include disease-modifying therapies (DMTs) to target the faulty immune response, and symptomatic treatments to reduce the severity of relapses. Neither of these treatment options delay the progression of the disease. Current studies show promise in the utilization of stem cell therapy to halt the progression of this disorder
