    Evaluation of acute generalized rashes in travelers returning from Southeast Asia: A guide for clinicians in primary care settings

    Americans are travelling internationally in record numbers, and Southeast Asia is among the regions seeing the largest increase in tourism. Travelers to Southeast Asia are exposed to a wide array of diseases and medical conditions that they might otherwise not encounter at home. As such, it is imperative that clinicians working in primary care have the knowledge to both advise travelers of their potential risks as well as recognize diseases in the patient returning from travel. However, many primary care providers may not be sufficiently knowledgeable in travel medicine to confidently evaluate travel-related complaints. Missed diagnoses or improperly treated conditions in this context can lead to unnecessary morbidity and even mortality. Dermatological complaints are among the most frequent reasons that patients returning from Southeast Asia seek professional medical care. This article intends to serve as a resource for clinicians working in primary care who encounter the generalized rash in a patient recently returned from travel to Southeast Asia. It specifically explores how to evaluate these patients to arrive at the correct diagnosis, while highlighting some of the unique, must-not-miss diagnoses endemic to Southeast Asia

    This capstone examines the traditional approach to STEMI admissions in hospitals, highlighting the lack of flexibility in adapting admission procedures to individual patient needs. Hospitals typically follow the standard practice of triaging patients through the ED without considering each patient\u27s specific situation. This rigid approach limits opportunities for personalized care and may overlook factors that could inform more tailored admission decisions. The paper explores the idea of implementing a criteria entitled SLAC that aims to individualize the admission with the goal of decreasing intra-operative and post-operative PCI complications

    Evaluating Treatment Modalities in Concussion Management

    At least 3 million Americans get a mild traumatic brain injury (mTBI) each year, and 1 in 5 have symptoms which last more than 1 month.10 Concussions can heavily impact the lives of patients causing symptoms of confusion, memory loss, headache, nausea and vomiting, balance issues, photophobia, phonophobia, and general feeling of being unwell. Some patients may even be affected by post concussion syndrome with long lasting effects. Without proper management to efficiently maximize recovery, symptoms can be prolonged and lead to long term consequences in health and decrease in quality of life for patients. An evaluation of prolonged cognitive rest versus active rehabilitation in the recovery of concussions can help shift the guidelines in place on the management of concussions. This article will display an evidence based guideline of concussion management to lead to faster concussion recovery with less long term negative outcomes for patients by utilizing early active rehabilitation

    All breast cancer treatment leads to soft tissue changes in the treated quadrant. While these changes are chronic, they are also identifiable and manageable with care from an appropriate provider. Scarring, lymphedema, radiation fibrosis syndrome (RFS), and axillary web syndrome (AWS) may all be part of the post-treatment soft tissue changes for a breast cancer survivor. Familiarity with the likely presentation and lifelong management of a patient’s specific symptom burden is crucial. While any physical therapist can rehabilitate the neuropathies, shoulder pain, postural dysfunction, or other numerous orthopedic issues that may occur, there is an acknowledged deficit in the treatment of specific post treatment issues,1 in availability of appropriate clinicians with adequate knowledge and experience to manage these specific concerns, and in clinician willingness to treat this specialized population.2 Both providers and patients may be unaware of the lifelong functional and comfort improvements possible with early recognition of and intervention for post-treatment soft tissue and functional changes,2 as opposed to delaying referral

    Cascadian Independence: A Rising Movement for an Independent Northwest Amid Growing Regionalism in the United States and Canada

    Because of the Civil War, secession is a sore topic to many Americans. However, the idea of secession has not been confined to this notorious example. My essay focuses on the idea that Oregon, Washington, and British Columbia could be the basis for a unified autonomous or even independent entity called Cascadia. First, I discuss the history of secession as such, and the arguments that have been made pro and con as to its legality and under what situations. I then situate this within the histories of the United States and Canada and the phenomena of regionalism and independent movements. I refer to the little-known case of Vermont(independent from 1777-1791) and the 19th century discussions of an independent west coast and northwest. I argue that the idea of a unique cultural identity of people on the west coast and northwest is one foundation otf the Cascadia movement. From there, I move to more modern times and discuss the strong tensions that have arisen in the United States and Canada between the central government and various regions and localities. Lastly, I discuss the modern Cascadia movement. I ask: Who supports Cascadia, what do they want, how do they want to achieve it, and what is their ultimate goal? My research resonates with the current political moment, as people are increasingly asking themselves if Cascadia would serve their interests better than remaining within the Union or Canada

    The Impact of Lobbying on US and UK Foreign Policy

    This paper seeks to understand the relationship between lobbying and the development and implementation of foreign policy by the United States and the United Kingdom. By examining legislation, various websites, articles, and insights gained from my personal internship experience this paper will explore how lobbying contributes to the development of policy and legislation. Additionally, case studies on foreign economic policy and foreign lobbying are used to examine the impact of lobbying. Given the central role of lobbying in policy development, it is important to understand not only what lobbying is but also how it works and what its implications are. Lobbying as an action is very similar in the US and the UK, however, the difference in the structure of government between the two leads to some differences in the execution of lobbying, particularly the transactional nature of lobbying and the interaction of lobbyists with politicians

    Managing Allergic Rhinoconjunctivitis with Accelerated Subcutaneous Immunotherapy

    Subcutaneous immunotherapy (SCIT) is a powerful treatmentoption for people with refractive allergic rhinoconjunctivitis.Immunotherapy is the only therapy that can modulate theimmune response and develop immune tolerance to commonaeroallergens such as pollen, dander, and dust mites for longerlasting results. Although the minimum treatment period forSCIT has been identified to be 3 years, there are still questionsregarding how long the beneficial effects of SCIT lasts afterdiscontinuation of therapy. There are also risks and benefits tovarious accelerated SCIT dosing schedules, including cluster and rushdosing. My presentation aims to be a comprehensive resource forproviders that need a better alternative to pharmacotherapy forrefractive allergic rhinitis and/or conjunctivitis

    Psychotherapy Approach Utilizing MDMA in the Treatment of PTSD

    At the end of 2023, results from the World Health Organization’s World Mental Health Survey revealed that approximately 3.6% of the global population experiences PTSD in any given year, with an estimated 1 in 11 people diagnosed at some point in their lifetime. Current FDA-approved treatments, including trauma-focused psychotherapies, as well as pharmacotherapies such as selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), offer moderate efficacy. However, long-term outcomes show that most patients fail to achieve sustained clinical improvements from this treatment regimen. Without effective symptom management, individuals often face persistent cognitive and mood disturbances stemming from distorted beliefs about themselves and others, which are rooted in trauma. Comprehensive treatment plans should aim to resolve these issues and improve quality of life. Emerging evidence suggests that 3,4-Methylenedioxymethamphetamine (MDMA), traditionally known as a recreational drug, shows promise as an adjunct to psychotherapy for treatment-resistant PTSD

    Early Recognition of Ehlers-Danlos Syndrome for the Prevention of Musculoskeletal Injuries and Chronic Pain

    Ehlers-Danlos syndrome (EDS) is a connective tissue disorder impacting collagen synthesis that can increase a patient’s risk for more frequent musculoskeletal injuries. Oftentimes, these injuries become chronic and affect an individual\u27s quality of life. This study demonstrates the impact of EDS on the musculoskeletal (MSK) system and the importance of early intervention to prevent chronic injury. Within this CME, information on clinical presentation, diagnosis, and treatment options for EDS are provided. Exercise based physical therapy training programs are the most effective intervention method for EDS. The Involvement of a professional physical therapist is essential within these exercise based programs for safety and efficacy. Medical Devices are most helpful during activities which involve end of range movements and help patients feel supported during activity. Overall, patients reported improved activities of daily living, less pain, and decreased fear of movement after interventions were implemented. Early identification, validation, and intervention for individuals with EDS improves patient’s long-term outcomes and overall quality of life
