Pheochromocytoma: Detecting the tumor of “fight-or-flight”
Pheochromocytoma (pheo) is a rare tumor originating from the adrenal medulla which produces catecholamines. The most abundant catecholamine produced is norepinephrine, making up an estimated 85% of overall tumor production. Causes of this neoplasm can be sporadic, familial in nature, or associated with genetic disorders. Although the tumor itself is mostly benign, the potential cardiovascular complications due to the catecholamine excess are dangerous which the diagnosis imperative. Since the clinical presentation is immensely variable due to the variety of potential symptoms of sympathetic overactivity, prompt diagnosis is uncommon. In classical descriptions of presentation, the patient may have severe hypertension as well as the “triad” of headache, hyperhidrosis, and palpitations. Promoting knowledge about pheochromocytoma with this comprehensive diagnosis and management education is the key for advancing clinicians’ suspicions in a timely manner, especially before potential lethal complications
To Wear or not to Wear: A Guide to Minimalistic Shoes
Over the last century, running has remained a popular form of exercise for enthusiasts, athletes, and causal joggers. In the last century, runners have utilized several assorted styles and forms of shoes. Among these shoes, minimalistic shoes have been increasing in popularity over the past few years. Many runners, athletes, and casual wearers are finding remarkable success with these shoes and are encouraging others to do the same. Although the claims about these shoes are appealing, not everyone fully understands the mechanism behind the shoes, who would make a suitable candidate for these shoes, and how to transition into these shoes while still minimizing injury. Currently, there are no recommendations or resources for health professionals to guide them when a patient asks about these shoes. This capstone aims to address this gap by providing a comprehensive guide of the current literature on minimalistic shoes including: the definition that defines several qualities of these shoes, body mechanics when utilizing the shoes, what disease conditions would benefit from these shoes, what disease conditions to steer away from these shoes, and finally a recommendation on where research needs to continue
Alternatives to Supine Birthing Positions: A Special Interest Topic for OBGYN Physician Assistants
This article aims to introduce alternatives to supine positioning, expand on current risks that birthing women face in the United States, review current practice guidelines on birthing positions, and discuss some recent literature that examines the outcomes of alternative positions. The goal for this piece is to bring diversity to the PA workforce’s understanding of positions that can be offered or encouraged in the second stage of labor. Utilization of more varied labor positions may be more comfortable for the birthing person and may potentially result in fewer complications. Therefore, offering alternative positions in labor is a low-risk and high reward intervention to implement in a health crisis of rising maternal mortality and concurrent obstetric provider shortage
An Introduction to Psilocybin Assisted Therapy for the Physician Assistant
Cultures have been experimenting with psychedelics for thousands of years(1). After the introduction of psychedelics in the United States in the 1950’s for the treatment of mental disorders, these substances and their use in clinical studies were later banned(1). Yet they have been experiencing a new renaissance in the last decade as more and more universities and research labs across the globe are conducting clinical studies with a multitude of different substances, including psilocybin, MDMA, DMT, and LSD, to name a few(1). Although current research has demonstrated potential benefits with many psychedelics, this paper will focus on psilocybin, the psychedelic garnering the most attention in the therapeutic world - Oregon and Colorado have both recently legalized psilocybin assisted therapy. The goal of this paper is to educate the physician assistant about the history of psychedelic assisted therapy, the use of psilocybin in the therapeutic setting, the pharmacology and metabolism of the psilocybin, the importance of the proper set and setting of a psychedelic session, and the safety profile of the drug, including contraindications

Cosimo I de\u27 Medici: Politics, Portraiture, and Propaganda
A ruler’s image was an essential part of Italian Renaissance culture, particularly in the realm of the princely court. By commissioning portraits and other forms of art, courtly elites were able to manipulate their public identity by highlighting the aspirational ideals of the time. It is thought that the Florentine senate ratified Cosimo I de’ Medici’s election because they believed him to be weak and easily manipulated. Precisely because his election was so unexpected, Cosimo I focused the resources of his court on establishing his political and social legitimacy. Cosimo employed visual imagery to deflect and neutralize challenges to his authority and looked to family precedents to give a sense of continuity and legitimacy to his regime. Through an analysis of Bronzino\u27s Portrait of Duke Cosimo I de Medici as Orpheus and Cosimo I de Medici in Armor, as well as Benvenuto Cellini\u27s Cosimo I de\u27 Medici bronze portrait bust, this thesis seeks to show that Cosimo promoted his authority, legitimacy, and the Medici name itself by propagating idealized images that obscured the nature of Cosimo’s transition to power, and instead focused on his role in ushering in an era of peace
