    Dietary spirulina platensis and chlorella vulgaris effects on survival and haemato-immunological responses of clarias gariepinus juveniles to aeromonas hydrophila infection

    This study investigated the effects of Spirulina platensis and Chlorella vulgaris on the growth, non-specific immunity and survival of Clarias gariepinus. C. gariepinus juveniles were infected with Aeromonas hydrophila after a 16-week feeding period with 5 different diets which served as replacements for fishmeal diet. The diets are 50% C. vulgaris (CL50%), 75% C. vulgaris (CL75%), 50% S. platensis (SP50%), 75% S. platensis (SP75%), and control (100% fishmeal). Fish (n=225; 41.85± 0.05 g) were randomly divided into fifteen tanks (150 L capacity each) with triplicate groups of 15 fish per tank. CL50% had the most significant (P0.05) in growth performance among the algae treatments. Similarly, lysozyme and respiratory burst activity and post-challenge haemoglobin, haematocrit, red blood cells, serum total protein, albumin and higher density lipoprotein cholesterol were significantly higher (P<0.05) in CL50% treatment. The present study indicates that replacing 50%-75% of fishmeal with C. vulgaris or S. platensis could improve the growth and confer significant protection against A. hydrophila in the algae-fed C. gariepinus. © 2019, BIOFLUX SRL.     Performance of a Novel Low-Cost, Instrument-Free Plasma Separation Device for HIV Viral Load Quantification and Determination of Treatment Failure in People Living with HIV in Malaysia: a Diagnostic Accuracy Study

    HIV viral load (VL) testing is the recommended method for monitoring the response of people living with HIV and receiving antiretroviral therapy (ART). The availability of standard plasma VL testing in low- and middle-income countries (LMICs), and access to this testing, are limited by the need to use fresh plasma. Good specimen collection methods for HIV VL testing that are applicable to resource-constrained settings are needed. We assessed the diagnostic performance of the filtered dried plasma spot (FDPS), created using the newly developed, instrument-free VLPlasma device, in identifying treatment failure at a VL threshold of 1,000 copies/ml in fresh plasma. Performance was compared with that of the conventional dried blood spot (DBS). Venous blood samples from 201 people living with HIV and attending an infectious disease clinic in Malaysia were collected, and HIV VL was quantified using fresh plasma (the reference standard), FDPS, and DBS specimens. VL testing was done using the Roche Cobas AmpliPrep/Cobas TaqMan v2.0 assay. At a threshold of 1,000 copies/ml, the diagnostic performance of the FDPS was superior (sensitivity, 100% [95% confidence interval {CI}, 89.1 to 100%]; specificity, 100% [95% CI, 97.8 to 100%]) to that of the DBS (sensitivity, 100% [95% CI, 89.4 to 100%]; specificity, 36.8% [95% CI, 29.4 to 44.7%]) (P < 0.001). A stronger correlation was observed between the FDPS VL and the plasma VL (r = 0.94; P < 0.001) than between the DBS VL and the plasma VL (r = 0.85; P < 0.001). The mean difference in VL measures between the FDPS and plasma (plasma VL minus FDPS VL) was 0.127 log10 copies/ml (standard deviation [SD], 0.32), in contrast to -0.95 log10 copies/ml (SD, 0.84) between the DBS and plasma. HIV VL measurement using the FDPS outperformed that with the DBS in identifying treatment failure at a threshold of 1,000 copies/ml and compared well with the quantification of VL in plasma. The FDPS can be an attractive alternative to fresh plasma for improving access to HIV VL monitoring among people living with HIV on ART in LMICs. Copyright © 2019 Pham et al

    Role of leptin as a biomarker for early detection of renal cell carcinoma? No evidence from a systematic review and meta-analysis

    Renal cell carcinoma (RCC) is the commonest from of renal neoplasm. Although surgery is a successful curative treatment for localized RCC, most patients are diagnosed with advanced or metastatic RCC, which has poor prognosis. RCC is classified by stage and grade using tissue samples. Whilst these provide good prognostic information, they are not very useful for early detection. Proteins that are dysregulated in patient's serum can be a valuable alternative and less invasive biomarker for early detection of the disease. For this reason, a hypothesis was formed that leptin is a possible biomarker for early detection and prognostication of RCC. The literature has disparate results on the usefulness of leptin as a biomarker for the early detection of RCC. Hence, a systematic review and a meta-analysis was carried out to investigate whether serum leptin could be a reliable diagnostic and prognostic factor in RCC patients. Literature on the available cohort and case-control studies on serum leptin in RCC was searched in electronic databases and included to evaluate this adipokine in the progression of RCC. The relevant studies were evaluated for the diagnostic and prognostic value of leptin in RCC patients. Overall, only 6 original research studies matched selection criteria and were included for meta-analysis. This study was hypothesised that; leptin might be a useful biomarker for early detection and prognostication of RCC. However, the data were presented in this study did not support our hypothesis. Serum leptin levels in RCC patients do not strongly associate with the development or progression of RCC, thus cannot act as a biomarker for early detection in RCC in patients. Extending our hypothesis further to include levels of obesity and RCC development may be worthwhile, but studies are currently limited. © 2019 Elsevier Lt

    Using Rasch model to assess self-assessment speaking skill rubric for non-native Arabic language speakers

    This study was conducted to assess the quality of the self-assessment speaking rubric adapted by Montgomery from Bill Heller in 2000. The rubric consisted of six aspects with a four-point rating scale and was originally written in English and aimed to be used by the English language learners. As the respondents of this research were the Indonesian students who learn Arabic language as a foreign language, the rubric was therefore modified and translated into Indonesian language. Rasch measurement model approach provides various analyses with empirical evidence about the quality of instrument by looking at the rating scale analysis, summary statistics, item fit, principal component analysis and Wright map. About 43 Arabic language learners from a university in Salatiga, Indonesia, were involved in this study. The finding showed that the four rating options were clearly understood by the respondents. All six items in the rubric were also appropriately measure students’ speaking skills. High value of person (0.84) and item reliability (0.94) indicated good quality of both respondents and instrument. The Cronbach alpha value 0.83 indicated high reliability. To sum up, the self-assessment speaking rubric has a good quality to measure speaking skills and is appropriate to be used by students to self-assess their Arabic speaking ability. © Universiti Putra Malaysia Pres

    Removal of phthalates in environmental compartments become crucial in recent years due to the growing global concern about the health effects of phthalates. In this study, a greener method based on cloud point extraction procedure was developed for the removal of selected phthalates in environmental samples using non-ionic silicone surfactant (DC193C). The parameters affecting the extraction efficiency, such as the surfactant concentration, salt types, salt concentration, temperature and incubation time were evaluated and optimized. Good linearity with correlation coefficients (R2) in the range of 0.9963 – 0.9988 for all calibration curves was obtained. The proposed method was applied in removing the diethhylhexyl phthalate and dibutyl phthalate in river water samples under optimized conditions with satisfactory recoveries in the range of 82 – 98%. © 2019, Malaysian Society of Analytical Sciences.     Predictive risk factors for complicated pneumonia in Malaysian children

    Aim: To investigate baseline characteristics associated with complicated community-acquired pneumonia (CAPc) in Malaysian children. CAPc, such as pleural effusion and/or empyema, is on the rise, especially in Southeast Asian children, and the reasons for this are unknown. Methods: A retrospective study was conducted on all children aged 2–16 years who were admitted to the University Malaya Medical Centre with community-acquired pneumonia between 2012 and 2014. Results: In this study, of the 343 children, 58 (17%) developed CAPc. Chinese ethnicity (P < 0.001), reduced breastfeeding duration (P = 0.003), not receiving outpatient antibiotic (P < 0.001) and exposure to parental smoking (P < 0.001) were identified as risk factors for CAPc. Markedly increased respiratory rate (P = 0.021) and thrombocytosis (P < 0.001) were noted as the clinical parameters for CAPc.     The effectiveness of a structured nursing intervention program on maternal stress and ability among mothers of premature infants in a neonatal intensive care unit

    Aims and objectives: To investigate the effectiveness of a structured nursing intervention program on maternal stress and NICU-related maternal ability after the admission of premature infants to a neonatal intensive care unit (NICU). Background: Mothers of premature infants may face stress having premature infants, and their infants may be admitted to the NICU for a few weeks or months. The mothers’ experience of stress would be worse if they have low knowledge and poor NICU-related maternal ability. Mothers of infants admitted to the NICU require well-planned interventions to cope with psychological matters arising after an infant hospitalisation. Design: Quasi-experimental design. Methods: A total of 216 mothers were consecutively assigned to control and intervention groups. Each group consisted of 108 mothers. The mothers in both groups received questionnaire concerning maternal stress and NICU-related ability during their first visit to NICU (within 48 hr of admission). A structured nursing intervention was implemented for 10 days on mothers in the intervention group. The control group continued to receive existing practice nursing care. Mothers of both groups were again given the questionnaire on maternal stress and NICU-related ability after 14 days of admission. Results: In the intervention group, the difference between the mean total score of maternal stress and parental role and relationship subscale decreased significantly, compared to the control group (p = 0.04; p = 0.01) respectively. Maternal ability improved significantly in mothers in the intervention group 2 weeks postintervention, p < 0.001. Conclusion: A structured nursing intervention for mothers could significantly reduce maternal stress and promote maternal NICU-related abilities. Relevance to clinical practice: The results of the study could help neonatal nurses to develop an appropriate nursing intervention for parents with premature infants in the NICU. © 2018 John Wiley & Sons Lt

    p53, stem cell biology and childhood blastomas

    Purpose of reviewChildhood blastomas, unlike adult cancers, originate from developing organs in which molecular and cellular features exhibit differentiation arrest and embryonic characteristics. Conventional cancer therapies, which rely on the generalized cytotoxic effect on rapidly dividing cells, may damage delicate organs in young children, leading to multiple late effects. Deep understanding of the biology of embryonal cancers is crucial in reshaping the cancer treatment paradigm for children.Recent findingsp53 plays a major physiological role in embryonic development, by controlling cell proliferation, differentiation and responses to cellular stress. Tumor suppressor function of p53 is commonly lost in adult cancers through genetic alterations. However, both somatic and germline p53 mutations are rare in childhood blastomas, suggesting that in these cancers, p53 may be inactivated through other mechanisms than mutation. In this review, we summarize current knowledge about p53 pathway inactivation in childhood blastomas (specifically neuroblastoma, retinoblastoma and Wilms' tumor) through various upstream mechanisms. Laboratory evidence and clinical trials of targeted therapies specific to exploiting p53 upstream regulators are discussed.SummaryDespite the low rate of inherent TP53 mutations, p53 pathway inactivation is a common denominator in childhood blastomas. Exploiting p53 and its regulators is likely to translate into more effective targeted therapies with minimal late effects for children.     Production Process and Optimization of Solid Bioethanol from Empty Fruit Bunches of Palm Oil Using Response Surface Methodology

    This study aimed to observe the potential of solid bioethanol as an alternative fuel with high caloric value. The solid bioethanol was produced from liquid bioethanol, which was obtained from the synthesis of oil palm empty fruit bunches (PEFBs) through the delignification process by using organosolv pretreatment and enzymatic hydrolysis. Enzymatic hydrolysis was conducted using enzyme (60 FPUg-1 of cellulose) at a variety of temperatures (35 °C, 70 °C, and 90 °C) and reaction times (2, 6, 12, 18, and 24 h) in order to obtain a high sugar yield. The highest sugars were yielded at the temperature of 90 °C for 48 h (152.51 mg/L). Furthermore, fermentation was conducted using Saccharomyces cerevisiae. The bioethanol yield after fermentation was 62.29 mg/L. Bioethanol was extracted by distillation process to obtain solid bioethanol. The solid bioethanol was produced by using stearic acid as the additive. In order to get high-quality solid bioethanol, the calorific value was optimized using the response surface methodology (RSM) model. This model provided the factor variables of bioethanol concentration (vol %), stearic acid (g), and bioethanol (mL) with a minus result error. The highest calorific value was obtained with 7 g stearic acid and 5 mL bioethanol (43.17 MJ/kg). Burning time was tested to observe the quality of the solid bioethanol. The highest calorific value resulted in the longest burning time. The solid bioethanol has a potential as solid fuel due to the significantly higher calorific value compared to the liquid bioethanol. © 2019 by the authors

    Marital wellbeing indicators amongst Malay Muslim couple in Malaysia: A preliminary study

    Introduction: There is a significant increase in the number of divorce cases among Malay Muslim couples over the past years which serves as a strong signal for the government to take initiatives to understand the current situation. A plethora of literature can be found on marriage satisfaction and the quality of the relationship within marriages. However, exploration on contributing factors to long lasting marriages amongst the Malay Muslim community are lacking. This paper aims to share the initial findings in an effort to explore of the strength of relationship in a lasting marriage amongst Malay Muslim couples in Malaysia. Methods: This paper adopted a quantitative approach utilizing the Eternal Love Instrument (c) 2016 which consisted of 171 items, distributed to 350 respondents based on simple random sampling technique. Results: The findings indicated that there are three main considerations in selecting a spouse which were (1) Religion (95.2%); (2) Character (responsible) (96.1%); well mannered (95.8%); and (3) Race (79.3%). Age, looks and type of jobs were the least considered factors amongst the respondents surveyed. The respondents perceived that the strongest element in their relationship was communication (69.6%), followed by tolerance (62.6%) and responsible towards family wellbeing (56.4%). Conclusion: The initial survey has indicated that a lasting marriage is a consequence of having a responsible partner with good religious background. Also, from all these five constructs, these respondents considered their spouses to be highly responsible. © 2019 UPM Press. All rights reserved
