    Chronic Cavitary Pulmonary Aspergillosis: An Unusual Cause of Chronic Cough, Pleuritic Chest Pain, and Hemoptysis in a 24-Year-Old Coal Mine Worker

    Pulmonary Aspergillus infections are caused by the ubiquitous fungus mold when spores found in compost, dust, and plant material are inhaled. Manifestation of the disease is represented by a delicate balance between host and pathogen interactions, making Aspergillus infection more commonly observed in immunocompromised individuals. Chronic cavitary pulmonary aspergillosis (CCPA) is a subtype of pulmonary Aspergillosis associated with history of chronic or prior lung disease. Here we present the case of a non-immunocompromised 24-year-old male with a history of intermittent asthma with chronic pulmonary symptoms and finding of a lung mass on imaging. We describe the diagnostic challenge and surgical treatment of CCPA in medically refractory disease

    Youth throwing athletes do not show bilateral differences in medial elbow width or flexor tendon thickness

    Background: Medial elbow laxity develops in throwing athletes due to valgus forces. Medial elbow instability in professional, collegiate, and high school athletes is well documented; however, the medial elbow of young throwing athletes has received less attention. This study investigated the medial elbow and common flexor tendon during applied elbow valgus stress of youth baseball players. Methods: The study included 15 participants. The medial elbow width and thickness of the common flexor tendon were measured on ultrasound images. Results: No significant side differences in medial elbow width or common flexor tendon were found at rest or under applied valgus stress. At rest, the medial elbow joint width was 3.34±0.94 mm on the dominant side and 3.42±0.86 mm on the non-dominant side. The dominant side increased to 3.83±1.02 mm with applied valgus stress, and the non-dominant side increased to 3.96±1.04 mm. The mean flexor tendon thickness was 3.89±0.63 mm on the dominant side and 4.02±0.70 mm on the non-dominant side. Conclusions: These findings differ from similar studies in older throwing athletes, likely because of the lack of accumulated stress on the medial elbow of youth throwing athletes. Maintaining elbow stability in young throwing athletes is a vital step to preventing injury later in their careers

    Explicitness and Rationale: PurposefulCommunication in Asynchronous Online Writing Tutor Sessions

    Introduction: In the February 2018 interview in The Chronicle of Higher Education “What’s Wrong with Writing Centers,” a tutoring strategy utilized frequently in the writing center setting was problematized: nondirective tutoring (Jacobs). Much writing center tutoring pedagogy advocates for a tutoring session to function as a conversation, with the tutor posing open-ended questions about a student’s writing and the student writer revising her own writing based upon answers formulated in response to the tutor’s particular, probing questions (Ryan and Zimmerelli). Nondirection as a tutoring strategy, though, has received pushback, and some argue that direct tutoring that provides students with explicit models to emulate is a more effective way to conduct a tutoring session (Barnett and Blumner). In asynchronous online sessions, where tutors provide feedback to student writing in the form of marginalia and endnotes, the conversations that often occur in face-to-face and synchronous online sessions are not achieved in the usual way due to constraints of digital modality, medium, and student audience. Still, tutors aim to enact pedagogy that incorporates feedback that resists copyediting while incorporating a conversational style of response

    The FDA Approved Gabapentin in 1993 as a non-controlled substance and it has remained a non-controlled substance at the federal level. The drug was created as an anticonvulsant and used to treat seizure disorders. The medication has also been used to treat hot flashes, neuropathic pain, pain, postoperative nausea, substance abuse issues, and vomiting. It is estimated that approximately 1% of people in the United States misuse gabapentin. This fact has compelled certain states to generate legislative initiatives designed to monitor the use and/or reclassify gabapentin.
