 فارسی چکیده
یا ریشه های  تنگی کانال نخاع عبارت از باریک شدن فضا کانال با اثر فشاری بر طناب نخاعیزمینه و هدف:

ی مراجعه کننده نفر از بیماران مبتال به تنگی کانال نخاعی کمر 60مقطعی،  مطالعه این در  ها:مواد و روش
 T1Wاژیتال ستصویرهای اکزیال و مورد بررسی قرار گرفتند. به بیمارستان های دانشگاه علوم پزشکی کرمان 
بعد از  .تهیه شده بر اساس روش استاندارد مورد تایید انجمن های بین المللی تهیه و گزارش شد T2Wو 
استفاده لیل برای تجزیه و تح 20نسخه  SPSSافزار  نرم با وهای آمار توصیفی ها از روشداده جمع آوری
 شد.
 ۹/4۹ ± 0۳/۱5ن %( زن و میانگین سن بیمارا 7/۳6نفر ) 22%( مرد و  ۳/6۳نفر ) ۳8 بیمار،  60از  ها:یافته
بوط به بود. در مطالعه ما بیشترین فراوانی محل استنوزیس مر 6۳/۳2 ± 55/28سال و شاخص توده بدنی 
L3-L4 ۱7 ( بیرون زدگی خلفی دیسک مربوط به  ۳/28نفر ،)%L4-L5 ۱0 ( دیلولیستزیس ، اسپون %( 7/۱6نفر
 بود.  %( ۳/۱8نفر ) L2-L3 ۱۱و بیرون زدگی قدامی دیسک مربوط به   %( 7/2۱نفر ) L3-L4 ۱۳مربوط به 
ناقص وجود تبرخی یافته های این مطالعه با سایر پژوهش ها همخوانی ولی در پاره ای موارد  گیری:نتیجه
انجام شود  داشت. توصیه می شود بررسی های اپیدمیولوژی با حجم نمونه مناسب در نواحی جغرافیایی ایران
روز آن ها بتفاوت اشکال تا چگونگی روند پدیده های دژنراتیسیون مهره ها، شیوع جنسی و سنی بیماران و 
 در مقایسه با بررسی های کشورهای غربی ارزیابی شود.

Abstract 
Background and aim: Spinal canal stenosis is the narrowing of the canal space 
with a compressive effect on the spinal cord or its nerve roots and is the most 
common indication for lumbar spine surgery in people over 60 years of age. 
Therefore, we decided to conduct a study entitled MRI findings in patients with 
primary spinal canal stenosis referred to the teaching hospitals of Kerman 
University of Medical Sciences in 2020. 
Materials and Methods: In this cross-sectional study, 60 patients with lumbar 
spinal canal stenosis referred to Kerman University of Medical Sciences hospitals 
were studied. Axial and sagittal images of T1W and T2W were prepared and 
reported according to the standard method approved by international associations. 
After data collection, descriptive statistical methods were used for analysis with 
SPSS software version 20. 
Results: Out of 60 patients, 38 (63.3%) were male and 22 (36.7%) were female 
and the mean age of patients was 49.9. 15.03 years and body mass index was 
32.63 28 28.55. In our study, the highest frequency of L3-L4 stenosis was 17 
(28.3%), L4-L5 posterior disc protrusion was 10 (16.7%), L3-L4 
spondylolisthesis was 13 (7/7). 21%) and anterior disc protrusion was related to 
L2-L3 11 patients (18.3%). 
Conclusion: Some findings of this study were consistent with other studies, but 
in some cases there were discrepancies. It is recommended that epidemiological 
studies with appropriate sample size be performed in geographical areas of Iran 
to assess the development of vertebral degeneration, sex and age of patients and 
the differences in their incidence in comparison with Western studies.

ن سنگ کلیه در جمعیت اطفال بیماری مهمی است که باعث بروز عوارض در کودکامقدمه و هدف:

 آنالیز شدند. SPSSافزار ها با استفاده از نرمدادهد. در دو گروه ارزیابی و مقایسه ش Dسطح ویتامین 
 
 در ان و در گروه سالمدرصد بیمار 5/57 نفرولیتیازیس دردر گروه  Dنتایج نشان داد سطح ویتامین : هایافته
در  (.P=180/0نداشت ) دارمعنیدو گروه تفاوت می باشد و در  ng/mL 30کمتر از درصد بیماران  5/42
 ng/mLاز  کمتر نرمال و نفرولیتیازیس و در گروه سالم میانگین سنی در افراد با سطح سرمی بیماران گروه
ی بین سطح دارمعنی تفاوت(. P=980/0و  P=878/0ی نداشت )به ترتیب دارمعنیتفاوت  Dویتامین  30
( P=676/0) سالم( و گروه P=554/0نفرولیتیازیس ) بر حسب جنسیت در بیماران گروه Dسرمی ویتامین 
( و P=998/0)در کودکان سالم و مبتال به نفرولیتیازیس بر حسب سن  Dمشاهده نشد. سطح سرمی ویتامین 
 ی نداشت.دارمعنی( تفاوت P=808/0جنسیت )

Abstract 
Introduction & Objective: Renal stones are an important disease in the pediatric 
population that causes complications in children. An increase in vitamin D has been implicated 
in hypercalciuria. However, vitamin D levels in some patients with kidney stones can be low. In 
this study, serum levels of vitamin D in children with nephrolithiasis and healthy children were 
evaluated. 
Methods: In this study, 80 children with a mean age of 3.44 years in two groups of 
nephrolithiasis (N=40) and healthy (N=40) were studied in Afzalipour Hospital in Kerman in 
1400. In the nephrolithiasis group, 47.5% (N=19) and in the healthy children group, 55% 
(N=22) were male. Vitamin D levels were assessed and compared in the two groups. Data were 
analyzed using SPSS software. 
Results: The results showed that the level of vitamin D in nephrolithiasis group in 57.5% of 
patients and in the healthy group in 42.5% of patients was less than 30 ng / mL and there was no 
significant difference between the two groups (P = 0.180). In patients in nephrolithiasis group 
and in healthy group, the mean age of individuals with normal serum levels of less than 30 ng / 
mL of vitamin D was not significantly different (P = 0.887 and P = 0.980, respectively). No 
significant difference was observed between serum vitamin D levels by gender in patients in 
nephrolithiasis group (P = 0.555) and healthy group (P = 0.676). Serum levels of vitamin D in 
healthy and nephrolithiasis children were not significantly different in terms of age (P = 0.998) 
and gender (P = 0.808). 
Conclusions: Vitamin D levels in patients with nephrolithiasis and healthy groups were not 
significantly different and age and gender did not affect vitamin D levels.

( یک ناهنجاری اورولوژیک شایع در کودکان است. تکنیک های VURرفالکس وزیکویورترال )  :معرفي
جراحی متعددی برای مدیریت این عارضه ارائه شده است ، که یکی از آنها جراحی آنتی رفالکس با 
تکنیک گیل ورنه است. این تکنیک جراحی ایمن ، ساده و نسبت به سایر تکنیک های جراحی آنتی 
ین تکنیک در گرید های باالی رفالکس ادراری کمتر مورد توجه رفالکس ادراری کم تهاجمی است . ا
قرار گرفته است. به این منظور در این مطالعه به ارزیابی اثربخشی و ایمنی جراحی آنتی رفالکس گیل 
 با گرید باال پرداخته شد.  VURورنه همراه با پلیکیشن سوراخ حالب در اصالح 
: یک مطالعه گذشته نگر که در آن اثربخشی، ایمنی و عوارض جراحی آنتی رفالکس  مواد و روش ها
گیل ورنه همراه با پلیکیشن سوراخ حالب را به عنوان یک روش انتخابی برای رفالکس با درجه باال در 
، در بیمارستان شهید باهنر کرمان تحت 1400تا شهریور ماه  1399تمام بیمارانی که از فروردین ماه 
 راحی قرار گرفتند، را ارزیابی کردیم. ج
( مورد بررسی قرار Vیا  IVبا درجه باال )درجه  ادراری  واحد رفالکس 152بیمار با  20:   یافته ها
( با رفالکس دوطرفه %80بیمار) شانزدهبودند.  Vواحد درجه  64و  IVواحد رفالکس گرید  88گرفتند. 
بیماران  از  (%90) هجده بیماردر  ادراری  طرفه بودند. رفالکس( با رفالکس یک %20بیمار ) چهارو 
امل اصالح شد و در دو بیمار رفالکس ادراری و به دنبال آن عفونت شرکت کننده در پژوهش به طور ک
 ادراری باقی ماند که تحت درمان طبی و پایش متناوب قرار گرفتند.
: نتایج ما قابل توجه و با نتایج سایر تکنیک ها سازگار و قابل رقابت است. این تکنیک  نتیجه گیری
های دو طرفه را به طور همزمان اصالح کند. بنابراین، منطقی است که  VURساده و ایمن می تواند 
کس درجه باال در نظر گرفته شود. با توجه به نتایج ما، ما روش جراحی آنتی رفال VURبرای تصحیح 
درجه باال به عنوان یک تکنیک ساده،  VURگیل ورنه همراه با پلیکیشن سوراخ حالب را برای تصحیح 
 ایمن و موفق پیشنهاد می کنیم.

Abstract 
Introduction: Vesicoureteral reflux (VUR) is a common urological abnormality in 
children. Several surgical techniques have been proposed to manage this complication, 
one of which is anti-reflux surgery with the Gil-Vernet technique. This surgical technique 
is safe, simple and less invasive than other anti-reflux surgical techniques. This technique 
has received less attention in high grades of urinary reflux. For this purpose, in this study, 
the effectiveness and safety of Gil-Vernet anti-reflux surgery along with ureteral plication 
surgery in high-grade VUR correction was evaluated. 
Materials and Methods: A retrospective study in which the efficacy, safety and 
complications of Gil-Vernet anti-reflux surgery combined with ureteral plication surgery 
as a method of choice for high-grade reflux in all patients from April 2020 to September 
2021 , Underwent surgery at Shahid Bahonar Hospital in Kerman. 
Results: 20 patients with 152 high-grade reflux units (grade IV or V) were studied. There 
were 88 grade IV reflux units and 64 grade V units. Sixteen patients (80%) had bilateral 
reflux and four patients (20%) had unilateral reflux. Urinary reflux was completely 
corrected in eighteen patients (90%) in the study, and in two patients with urinary reflux 
followed by urinary tract infection who underwent medical treatment and intermittent 
monitoring. 
Conclusion: Our results are significant and compatible with the results of other 
techniques. This simple and safe technique can modify bilateral VUR simultaneously. 
Therefore, it makes sense to consider high-grade VUR correction. Based on our results, 
we propose Gil-Vernet anti-reflux surgery combined with ureteral plication surgery to 
correct high-grade VUR as a simple, safe and successful technique.

( است. با این SUDسم زدایی اولین و یکی از مراحل ضروری در درمان اختالل مصرف مواد ) زمينه:
 سال محدود است. 12حال، شواهد در مورد این درمان ها در کودکان و نوجوانان به ویژه کودکان زیر 
زدایی این مطالعه با هدف بررسی اثربخشی افزودن کلونیدین و گاباپنتین به بوپرنورفین در سم  هدف:
افیونی در بخش روان  به اختالالت مصرف مواد  پزشکی کودکان بستری در کودکان و نوجوانان مبتال 
 پزشکی شهید بهشتی کرمان انجام شد.بیمارستان روان 
 16تا  5بیمار  40در یک کارآزمایی بالینی تصادفی باز در بیمارستان شهید بهشتی کرمان،  روش كار:
با بوپرنورفین و یک گروه ساله مبتال به اختالل مصر ف مواد افیونی به یک گروه کنترل تحت درمان 
مداخله تحت درمان با بوپرنورفین به همراه گاباپنتین و کلونیدین تقسیم شدند. شدت عالئم ترک قبل 
بعد از مداخله با مقیاس بالینی و ذهنی ترک مواد افیونی اندازه  14از مداخله و یک روز در میان تا روز 
 تجزیه و تحلیل شد. 22نسخه  SPSSی شد. داده ها با نرم افزار گیر
سال بودند. در هر دو گروه تاثیر زمان بر نمرات کناره گیری  12بیماران در سنین زیر  % 55 نتايج :
های مختلف، دوز و مدت زمان مصرف بوپرنورفین بالینی و ذهنی معنی دار بود. اما امتیازات در زمان

ABSTRACT:  
Background: Detoxification is the first and one of the essential steps in treatment of 
Substance Use Disorder (SUD). However, evidence regarding these treatments in 
children and adolescents especially children below the age 12 is limited. 
Objective: This study aimed to evaluate the effectiveness of adding Clonidine and 
Gabapentin to Buprenorphine for inpatient detoxification of children and adolescents with 
opioid use disorders in Child Psychiatric ward of Kerman Shahid Beheshti Psychiatric 
Hospital. 
Methods: In an open randomized clinical trial in Kerman Shahid Beheshti hospital, 40 
patients aged 5 to 16 with opioid use disorder were divided into a control group treated 
with Buprenorphine and an intervention group treated with Buprenorphine plus 
Gabapentin and Clonidine. The severity of withdrawal symptoms was measured by the 
Clinical and Subjective Opioid Withdrawal Scales before the intervention and every other 
day until day 14 after the intervention. Data were analyzed by SPSS software version 22. 
Results: 55% of patients were below the age of 12. In both groups, the effect of time on 
Clinical and Subjective withdrawal scores was significant. However, the scores at 
different times, the dosage and the duration of buprenorphine use were not significantly 
different between the two groups. The mean dosage of Acetaminophen and Promethazine 
was lower in the intervention group and not statistically significant. 
Conclusion: Adding a combination of Gabapentin and Clonidine to Buprenorphine in 
children and adolescent detoxification with opioid use disorder is not significantly 
superior to Buprenorphine alone.

 چکیده:
بیماری های سیستمیک ،از دست دادن دندانها ،مصرف داروها در افراد سالمند ،موجب  زمینه و هدف :
میشود این گروه سنی، یک گروه سنی ویژه  نیازمند مراقبتهای حمایتی و درمانی دندانی باشند. هدف از 
انجام این مطالعه ارزیابی آگاهی ،عملکرد و نگرش دندانپزشکان عمومی شهر کرمان در مورد مراقبت 
 های دندانی در سالمندان بود
روی دندانپزشکان عمومی در شهر کرمان انجام  این مطالعه از نوع مقطعی توصیفی بود که  روش کار:
سوال آگاهی  9شد.ابزار جمع اوری اطالعات پرسشنامه شامل اطالعات دموگرافیک و پرسشنامه شامل  
  SPSS 26سوال عملکرد بود.  داده ها وارد کامپیوتر شده و در نرم افزار آماری  7سوال نگرش و  17،
 تجزیه و تحلیل شدند. 0.05در سطح معنی داری  و رگرسیون خطی T ،ANOVAو با تستهای 
 سال 36.10±7.60سن دندانپزشکان شرکت کننده  نیانگیافراد مرد بودند.م %60مطالعه  نیدر ا یافته ها :
و    85از      59.42 ±  11.6میانگین نمره نگرش .بود 9 از 5.29± 1.49نمره دانش افراد    نیانگمی.بود
 بود.    35از  4.96±24.13میانگین نمره عملکرد         
دانش ،نگرش و عملکرد  نیعملکرد خوب داشتند.ب %34نگرش مثبت و  %18افراد دانش خوب  و  19.3%

Background and aim:  
systemic diseases ,tooth loss  and medications  in geriatric patients, making this age 
group a special group in need of specific preventive and treatment  oral health care. 
This study was aimed to assess the knowledge, attitude, practice and of general dentists 
in Kerman  to provide dental care to geriatric patients. 
 
Methods: 
 This cross-sectional study was conducted on 150  dentists using a questionnaire 
including demographic characteristics, knowledge(9 item), attitude(17item ) and 
practice( 7 item) sections. Data were fed into SPSS-26 and analysed using descriptive 
statistics, t-test, ANOVA , correlation coefficient  and regression tests .p value 
considerd at 0.05 significant level. 
Results:  
60% of the participants were men and their mean age was 36.10±7.60 years. The 
knowledge mean score was 5.29± 1.49 (out of 9). The mean score related to the 
attitudes toward geriatric was 59.42 ± 6. 11 (out of 85). The mean score of practice 
was 24.13± 4.96(out of 35). 19.3% of the participants had good knowledge, and 18% 
displayed positive attitude and 34% showed good practice. There was a significantly 
positive correlation between knowledge, practice  and attitude. There was not 
significant differences between gender and knowledge, practice  and attitude. 
However, 62% preferred to provide care to the young patients.  50% of the dentists 
had enough   experience in treating the older people 
with complex medical problems.  
Conclusion: 
 the dentists in this study had average knowledge and practice , they mostly displayed 
a positive attitude towards the geriatric . There was a significantly positive correlation 
between knowledge, practice  and attitude.
