    Quality Management of Chemotherapy: Induced Nausea and Vomiting with Minimal Constipation

    Although clients diagnosed with colorectal cancer (CRC) are receiving more advanced and successful chemotherapy treatments, the side effects of these treatments continue to cause anxiety, discomfort, pain, and a diminished quality of life. Treatments must be developed to avoid some of the most unpleasant symptoms a client experiences as a result of these treatments. The purpose of this study was to examine the incidence of constipation related to the treatment of chemotherapy-induced nausea and vomiting (CINV) and to identify treatments that are successful in managing constipation while maintaining effective control of CINV. Constipation can be a significant problem with a negative impact on lifestyle during chemotherapy and needs aggressive management. Use of ondansetron as an antiemetic is associated with the development of constipation. A retrospective study was performed to identify the rate of constipation among a cohort of CRC clients and to determine effective treatment of constipation in the CRC client undergoing chemotherapy. Results of the study include increased rate of constipation in the study group, and limited documentation of symptoms and treatments. The study provided evidence that constipation is a significant side effect of CINV treatment with ondansetron and requires a comprehensive history to determine etiology. Communication is an important aspect of care, with the Clinical Nurse Specialist (CNS) informing staff of client history and insuring continuity of care.     Radburn, a planned community that was founded in 1929, was based on England’s Garden Cities. This photograph was taken on Howard Ave, named for Sir Ebeneezer Howard, whose 1898 publication Garden Cities of To-morrow influenced urban planners to design communities where people and nature lived in harmony with one another.     Trace metals (Cu, Zn, Cd and Pb) in juvenile fish from estuarine nurseries along the Portugese coast

    Organic and inorganic pollution can impact organisms directly and affect condition, growth and survival of juvenile fish which use estuaries as nurseries, and thereby affect marine adult populations quantitatively and qualitatively. Trace element contamination (Cu, Zn, Cd, Pb) in juveniles of commercial fish Solea solea, Solea senegalensis, Platichthys flesus, Diplodus vulgaris and Dicentrarchus labrax collected in putative nurseries of the main Portuguese estuaries (with diverse intensities and sources of anthropogenic pressures) was determined via atomic absorption spectrometry. Contamination was significantly different among species. Similar levels of contamination were found among estuaries, except for D. vulgaris. Cu and Zn concentrations ranged from 1.0 to 2.1 and 14 to 59 μg g-1 muscle dry weight respectively; while Cd and Pb concentrations were very low. The results indicate that juvenile migration to off-shore habitats is associated with low export of contamination, and no particular estuary increases the potential contamination of adult stocks. This knowledge is of the utmost importance in view of the ecological and economical value of these species and their use of estuarine areas as nurseries

    Does an Online Computer Based Critical Care Orientation Program Support the Learning Needs of a Multigenerational Workforce?

    The complexities of today\u27s health care system challenge acute care hospitals to provide comprehensive nursing orientation programs while meeting the needs of a multigenerational workforce. To prepare nurses to provide the highest quality of care for patients, nursing orientation must provide updated content, be cost effective, time sensitive and comprehensive, stimulate critical thinking skills, and include relevant evidence-based knowledge. Although a comprehensive nursing orientation is essential to every area of practice, specialties, such as critical care nursing, require specialized knowledge and additional skill sets that necessitate a more comprehensive training program while being sensitive to the needs of a multigenerational workforce. These considerations produce additional challenges for hospital orientation programs when considering the lack of available educators, projected nursing shortage, limited resources to accommodate multiple learning styles, and the current influence of healthcare economics. In an attempt to address orientation challenges, organizations such as the American Association of Critical Care Nurses (AACN) have developed online programs. The purpose of this paper is to evaluate one such program, the AACN Essentials of Critical Care Orientation (ECCO) program. ECCO was evaluated to determine if this online computer based orientation program would support the learning needs of multigenerational critical care nurses. Twenty-one nurses representing four generations of nurses from 11 hospitals in the United States completed an 18 question survey.     First Baptist Church

    First Baptist Church was founded by Roger Williams in 1638. The building that stands today on North Main Street was built between 1774 and 1775. This building shows Providence’s increasing wealth and power during this time period. John and Joseph Brown were responsible for the design of the church. It is an 80-ft. square building that blends two traditions together: the New England meeting house, and the Anglican parish church. The steeple derives from James Gibb’s 1728 Book of Architecture, Plate 30, an alternative design for St. Martin-in-the-Fields of 1726 in Trafalgar Square, London.https://digitalcommons.ric.edu/smolski_images/1114/thumbnail.jp
