Use of High-Fidelity Simulation to Improve Confidence in Military Nurses\u27 Deployment Readiness
High-fidelity simulation (HFS) has been shown to be a successful means of skill sustainment and improved confidence across many professions in which human lives are a stake to include the fields of aviation and healthcare. More specifically, HFS technology has displayed efficacy with doctors, nurses, and other advanced practice providers. The flexibility of HFS makes for an ideal educational tool in both professional development and academia. Military medicine could benefit from HFS technology due to the dynamic nature which requires readiness at a moment’s notice. The benefit can be significant in the military nursing community due to the frequency of relocation to duty stations and positions that do not allow for frequent use of deployment critical patient care skills. These deployment skills must be carried out by confident military nurses. The functionality of HFS to provide an opportunity to repeat complicated procedures in a safe controlled setting to improve a nurses’ confidence is essential (Boling & Hardin-Pierce, 2016). The purpose of the project was to determine if the use of HFS in addition to traditional online education could improve military nurse confidence with the deployment specific skill of managing a patient with a chest tube. An educational pre-briefing, four simulation scenarios, and a debriefing were implemented and assessed using a modified confidence tool. The project demonstrated improvement in the confidence of all subjects in the assessment, intervention, and evaluation of a simulated patient with a chest tube
Knowledge of Electrolyte Replacement Protocols Among Medical-Surgical Registered Nurses: A Research Project
Electrolyte abnormalities in hospitalized patients may result from a variety of acute and chronic medical conditions. Hospital admissions related to electrolyte imbalances in adult patients may be a direct result of acute illness such as nausea, vomiting, diarrhea, and dehydration or complications related to medications or surgical procedures. Patients with deficiencies of electrolytes such as potassium, magnesium and phosphorus are at an increased risk for life threatening complications such as seizures, delirium, cardiac dysrhythmias, respiratory muscle weakness and respiratory compromise. Nurses must possess a baseline knowledge of electrolyte disturbances and recognize potential and possible complications related to deficiencies of key electrolytes. The purpose of this research project was to improve nurses’ knowledge regarding the management, treatment, and care of adult patients with electrolyte imbalances. An educational intervention was developed and a pre-posttest design was utilized to evaluate the effect of the intervention. This research project demonstrated that the implementation of an educational intervention increased nurses’ knowledge regarding the management, treatment, and care of adult patients with electrolyte abnormalities. The results of this research project validated the need for additional education for nurses which focuses on the care of adult patients with electrolyte deficiencies
Compassion Fatigue in Critical Care Nurses: An Educational Quality Improvement Project
Compassion fatigue is estimated to affect 40% percent of the 2.9 million registered nurses in the United States. There exists a critical need to explore how nurses understand compassion fatigue, how they identify it in self and others, and what strategies they enact to lessen the eventual threats to health. With the current COVID pandemic, along with the high intensity nature of the intensive care unit (ICU), critical care nurses are vulnerable to the symptoms and side effects of compassion fatigue. This quality improvement project evaluated the impact of an educational session on compassion fatigue and self-care in relation to critical care nurses’ perceived levels of compassion fatigue. Pre education session and post educational session Professional Quality of Life (ProQOL-5) survey measuring perceived levels of compassion satisfaction, burnout, and secondary traumatic stress, were administered to a sample of critical care nurses in a community hospital in Massachusetts. The educational session was conducted via a virtual audio-enabled PowerPoint presentation. Pre and post education scores were assessed between time points to determine if the education was successful at decreasing perceived level of compassion fatigue. Post ProQOL scores presented a 6.76% increase in level compassion satisfaction, 3.28% decrease in level of burnout, and an 8.66% decrease in level of secondary traumatic stress. This project illustrates the potential for targeted education with critical care nurses as a vulnerable group who experience higher levels of compassion fatigue as a group. Strategies to reduce the effects of compassion fatigue have the potential to improve mental and emotional health essential for continual safe patient care with more positive outcomes in the critical care area
Prophylactic Percutaneous Endoscopic Gastric Tubes for Head and Neck Cancer: Are They Worth Their Weight?
It is estimated that 54,000 new cases of Head and Neck Cancer will be diagnosed in the United States in 2021 (Cancer Facts and Figures, 2021). Head and Neck cancers represent those that originate from sites which include the nasopharynx, oropharynx, oral cavity and hypo-pharynx. In advanced cases, treatment modalities are combined to include surgery, radiation and chemotherapy. Expected side effects of treatment can include mucositis, dysgeusia, dysphagia, odynophagia, nausea, vomiting and xerostomia. These toxicities can contribute to dehydration, weight loss and malnutrition. This paper examines the current state of evidence as it pertains to nutritional outcomes for Head and Neck cancer patients receiving concurrent chemotherapy and radiation when prophylactic percutaneous endoscopic gastrostomy (PEG) tubes versus reactive tubes were used. A retrospective chart review was conducted to examine the following research question: Does prophylactic percutaneous gastrostomy tube placement improve nutritional outcomes in patients receiving concurrent chemotherapy and radiation for Head and Neck Cancer patients when compared to those patients with reactive PEG tubes placed. The results of this study identified that despite timing of tube placement, patients in both groups lost weight and showed signs of worsening nutritional outcomes as evidenced by decreased body mass index, total protein, and albumin levels. Advanced practice nurses are in a unique position to develop innovative approaches to enhance nutrition in this patient population. Risk assessment tools, interdisciplinary collaboration and the development of a Head and Neck cancer pre-habilitation clinic may improve nutritional outcomes in the Head and Neck cancer patient receiving cancer treatment

Effectiveness of DBT Radical Acceptance on Stress, Anxiety and Depression
In my Professional Impact Project I tested the hypothesis that DBT\u27s Radical Acceptance skills help to decrease symptoms of Depression, Anxiety and Stress. The client, who participated in this project scored high on Avoidance and Emotional Expression Inhibition. I utilized the following DBT strategies: Nonjudgmental Stance, Observe and Describe, which were applied at the initial stage of therapy (Sessions 2-7). The outcomes of the intervention were measured by DAS-21 scale - first at the baseline, and then following the six consecutive, weekly therapy sessions. The changes over time, on the measured scales, seem to support the hypothesis
