    Every single human has a past. We have all suffered our own share of trauma. Throughout the physical therapy program, we have discussed trauma-informed care. During our first semester, we reviewed the Advanced Childhood Experience (ACE) scale and took down our own scores. For those who have experienced a more traumatic childhood, their ACE scores are higher. A high ACE score indicates a higher risk for later health problems as an adult. As future and current clinicians, we need to take this score into consideration during evaluations and when we develop a plan of care. Trauma does not disappear. I have worked on my own for many years, but it continues to be a process. I am proud of the work I have done, but it has not been easy

    Airway Management And The Hypoglossal Nerve Stimulator For Obstructive Sleep Apnea Patients

    The pathophysiology of obstructive sleep apnea (OSA) relates to anatomical and non-anatomical factors. Anatomical considerations correlate to narrowing of the pharynx. When present, non-anatomical factors can intensify the severity of OSA and ease of collapsibility of upper airway tissue. The negative consequences of untreated OSA are well-defined and many treatment options are available. The novel hypoglossal nerve stimulator makes for a viable treatment option in patients who do not tolerate more traditional therapy, such as continuous positive airway pressure (CPAP), and meets eligibility requirements of device implantation as outlined by the Food and Drug Administration (FDA). The Inspire® Hypoglossal Nerve Stimulator has been FDA-approved since 2014. With countless reports of OSA patients who struggle to comply with CPAP therapy, this device appears to have widespread application. As successful implementation of the Hypoglossal Nerve Stimulator (HNS) continues to grow, the anesthesia provider should be informed on how to safely provide anesthetics for patients with the hypoglossal nerve stimulator. This article discusses the pathophysiology of the obstructing nature of OSA and evaluates how airway management could differ in the future with anesthetized spontaneously breathing OSA patients that have the novel hypoglossal nerve stimulator compared to patients managed by more traditional therapies

    Efficacy Of Dexmedetomidine As An Opioid Sparing Adjunct To Regional Anesthesia For Shoulder Surgery

    Regional anesthetics are often the preferred route for patients who are having orthopedic shoulder procedures (Rai & Bhutia, 2017). Traditionally, plain local anesthetics, like bupivacaine or ropivacaine, are used as the local anesthetic of choice because of their fast onset and relatively predictable duration of action. However, several different additives have been trialed to prolong the duration or improve the density of regional blocks. When looking at additives to regional anesthetic techniques, side effects and duration of the block can be unpredictable and unpleasant. For example, additives like fentanyl, morphine, and dexamethasone have been trialed in regional techniques and have been shown to produce itching, nausea and vomiting, respiratory depression, and hemodynamic instability (Rai & Bhutia, 2017). Will adding dexmedetomidine to upper extremity regional anesthetic techniques produce adequate anesthesia while reducing the incidence of side effects and the need for opioids in the postoperative period

    Social Determinants of Health Month: A Collaboration of Interprofessional Education on Care for Future Patients

    Throughout the month of October, various student groups will host guest lecturers, student and physician panels, and alternative opportunities for the UNE community to learn about factors impacting health. During SDoHM, students and community members will explore conditions in which people live, grow, work, learn, practice religion, and age that impact individual health, while also learning about systems of oppression that perpetuate those conditions and disparities in healthcare. The events will encourage participants to think about how we might affect positive change in our communities and in our future patient populations. Furthermore, throughout SDoHM, students will be provided resources and virtual learning opportunities about the intersections of healthcare and social injustice, environmental racism, and disenfranchisement, underrepresentation, and discrimination in medicine. Lastly, students will be able to participate in fundraisers and drives which will benefit our community and provide essential resources. This project was created in the hopes that the UNE community would engage in discussions that are difficult but necessary. We also hope that through conversations, reflections, learning (and some un-learning), UNE community members can make a commitment to accessible, diverse, inclusive, and equitable health and wellness.https://dune.une.edu/cecefall2022/1014/thumbnail.jp

    Addressing Social Needs of Rural Long COVID Patients

    Our study focuses on the social implications and role of healthcare accessibility in the treatment of Long-Haul COVID-19, specifically in the rural setting. Referred to as Long COVID, this chronic condition broadly covers a variety of symptoms experienced by those long after the initial recovery from acute sickness with COVID-19; and it is now recognized with an ICD-10 code classification. Anecdotal evidence shows a lack of social support, empathy, and resources available for individuals suffering from Long COVID in rural areas. Based on a multi-disciplinary standardized patient experience, the project focuses on social needs requiring intervention and the manner in which different scopes of medical professionals can target these issues.https://dune.une.edu/cecefall2022/1006/thumbnail.jp

    In April of 2022, Maine’s Permanent Commission on the Status of Racial, Indigenous, and Tribal Populations issued a report, One Nation, Under Fraud: A Remonstrance, outlining the history of tribal relations with the State of Maine and suggesting a framework for remedying those relations—for redressing the centuries of fraud and cultural genocide perpetrated by the state. This event will feature the report’s three co-authors, each of whom will share their unique perspective on the project and what it means for our community and the work before us today.
