Pain Assessment and Reassessment Documentation Improvements in Medical-Surgical Units
Problem Pain management is essential when providing quality care, and adequate documentation of pain assessment and reassessment with administration of opioid pain medications by nurses is necessary for patient safety and satisfaction. Context Nurses play a pivotal role in providing safe and effective administration of opioid pain medication and documentation of pain assessments. This quality improvement (QI) project aimed to increase opioid medication administration assessment and documentation compliance rate for nurses to 90% or greater in two medical-surgical units. Intervention A knowledge check survey was utilized to assess nurse understanding of required data needed to document, and timing for documentation to meet compliance. Interventions included visual reminders, informational posters, and instructions on how to access individual compliance reports. Measures A post-intervention survey was used to collect nurse feedback on effectiveness of interventions and to gather further suggestions. Quarterly compliance rate data was obtained to identify if improvements in compliance rates were achieved. Results Unit A scored 62% for pre-assessment compliance, and 90% for reassessment compliance. Unit B scored 77% for pre-assessment compliance and 89% for reassessment compliance. Conclusions Providing visual aids as reminders for completing pain assessment and reassessment documentation and expanding nurse education on current self-compliance rates are useful for increasing nurse documentation compliance rates in medical-surgical units. Further evaluation of pain assessment and reassessment documentation compliance rates are required to identify sustainability and opportunities for greater improvements
Improving Prolonged Antibiotics Education in a Pediatric Unit
Problem Children’s Hospital A\u27s incidence report tool, iCares, demonstrates that 50% of the issues related to prolonged intravenous (IV) antibiotics are categorized as administration issues. This leads to the medication being delivered at the wrong time or rate. Having these issues increases patient length of stay, increasing hospital costs. Context The microsystems for this quality improvement project include inpatient pediatric patient care units (PCU) 300, 360, and 400. Children’s Hospital A is a 361-bed San Francisco Bay Area hospital. Interventions The Clinical Nurse Leader students implemented a presentation defining prolonged IV antibiotics, their rationale, and a video demonstrating the correct pump setup. This presentation was shown at an in-person staff meeting on April 11, 2024, to 90 nurses and nurse managers. Measures To assess nurses’ baseline knowledge, a pre-video quiz was deployed before the intervention was presented. A post-video quiz was utilized to assess the impact of the video on nurses’ knowledge. Results When comparing baseline knowledge to post-intervention knowledge, the results went from 86.9% to 95.5%, reflecting an 8.6% improvement. Conclusions This project comprehensively assessed the microsystem using a needs assessment tool, identified the problem, and conducted a root-cause analysis. The students analyzed data to implement an educational tool that increased nurse knowledge regarding prolonged IV antibiotics. The tool is cost-effective, and easily attained ensuring sustainability. The video will be evaluated biannually utilizing iCares data to assess long-term impacts
Historical Reappearance: Tracing the impacts of Argentina\u27s Los Desaparecidos to the rise of President Javier Milei
In 1976 the Argentine government was taken over by a right wing military junta, attempting to “save” their country from communism. Military officials kidnapped, tortured and killed between 10,000 and 30,000 Argentine citizens, who were thought of as “guerrillas” supporting the leftist government and the late president Juan Perón. These victims are regarded as the disappeared, or Los Desaparecidos; due to the fact their families had no knowledge of where their loved ones were taken, or what was happening to them. Many of these young, liberal advocates were never seen or heard from again, their remains lost to history, and their families lives forever changed by grief. This thesis strives to answer the question; How might communal trauma and political unrest resulting from Argentinas Los Desaparecidos impact the rise of the current president Javier Milei. My research analyzes the communal trauma evoked by Los Desaparecidos and how the stream of said trauma influenced Argentine society, economy and government. By comparing, contrasting and clarifying, I display why Los Desaparecidos may or may not have influenced the rise of a fierce libertarian leader such as Javier Milei. Through thorough and extensive investigation of both English and Spanish media, I explore the significance behind questioning if the effects of collective trauma impact the rationale leading to the inauguration of the current radical president. My general findings support the idea that Los Desaparecidos altered the political and economic system throughout Argentina. This being said, it can not be stated that the communal trauma and intense impingement of the war crimes solely impacted the election of President Javier Milei. In addition, other factors which influenced his uprising must be acknowledged
Improving Documentation Compliance and Patient Mobility Using the Bedside Mobility Assessment Tool
Objective: Patient immobility during hospitalization contributes to avoidable complications including increased length of stay, pressure injuries, thromboembolic events, reduced functional capacity, and increased rates of discharge to rehabilitation facilities. The Bedside Mobility Assessment Tool (BMAT) was developed to empower Registered Nurses to safely assess and mobilize patients without a physical therapy evaluation. Aim: This quality improvement projects aims to enhance nurse compliance with BMAT documentation by 10% in three medical-surgical units of a 244-bed Northern California Hospital. Preliminary findings indicate an average daily patient census of 23.7 of which only 31.3% are assessed using the BMAT. Methods: Across a six-week intervention period, nurses were educated on the procedure for assessing patients utilizing the BMAT, the significance of its use, and hospital expectations regarding documentation. Preliminary data was collected using the electronic health record to analyze the number of BMATs performed across an initial four-day observation period. Post-intervention data was collected over 15-days using the same method. The Plan-Do-Study-Act (PDSA) framework was used to assess intervention efficacy. Results: The post-intervention data reflected an average daily patient census of 23.6 of which only 24.4% of patients were assessed. Conclusion: Upon conclusion of educational interventions aimed to promote BMAT significance and increase documentation compliance, it was found that the compliance with documentation decreased. To improve documentation compliance, the hospital will need to ensure the BMAT documentation is no longer optional, but part of required documentation. Until the documentation is required, nursing staff will continue to be non-compliant with documentation
