Transformation archetypes in global food systems
Food systems are primary drivers of human and environmental health, but the understanding of their diverse and dynamic co-transformation remains limited. We use a data-driven approach to disentangle different development pathways of national food systems (i.e. ‘transformation archetypes’) based on historical, intertwined trends of food system structure (agricultural inputs and outputs and food trade), and social and environmental outcomes (malnutrition, biosphere integrity, and greenhouse gases emissions) for 161 countries, from 1995 to 2015. We found that whilst agricultural total factor productivity has consistently increased globally, a closer analysis suggests a typology of three transformation archetypes across countries: rapidly expansionist, expansionist, and consolidative. Expansionist and rapidly expansionist archetypes increased in agricultural area, synthetic fertilizer use, and gross agricultural output, which was accompanied by malnutrition, environmental pressures, and lasting socioeconomic disadvantages. The lowest rates of change in key structure metrics were found in the consolidative archetype. Across all transformation archetypes, agricultural greenhouse gases emissions, synthetic fertilizer use, and ecological footprint of consumption increased faster than the expansion of agricultural area, and obesity levels increased more rapidly than undernourishment decreased. The persistence of these unsustainable trajectories occurred independently of improvements in productivity. Our results underscore the importance of quantifying the multiple human and environmental dimensions of food systems transformations and can serve as a starting point to identify potential leverage points for sustainability transformations. More attention is thus warranted to alternative development pathways able of delivering equitable benefits to both productivity and to human and environmental health
Medicinal cannabis for the treatment of chronic refractory pain: An investigation of the adverse event profile and health-related quality of life impact of an oral formulation
Introduction: Medicinal cannabis is prescribed in Australia for patients with chronic refractory pain conditions. However, measures of safety and effectiveness of different cannabinoids are lacking. We designed an observational study to capture effectiveness, adverse events (AEs), and health-related quality of life (HRQoL) measures in patients prescribed an oral medicinal cannabis formulation at Cannabis Access Clinics through the Cannabis Access Clinics Observational study (CACOS). Objectives: We aimed to evaluate effectiveness, reported AEs, and change in patient-reported outcomes in individuals prescribed a cannabinoid oil formulation for management of chronic pain. Methods: A cross-sectional analysis was conducted on patients prescribed an oil formulation of Δ9-tetrahydrocannabinol and cannabidiol for pain symptoms of at least 3-month duration. Clinician-reported AEs were organized by system, organ, class, and frequency. Analysis of patient-reported responses to a questionnaire was conducted using published minimal clinically important differences to determine meaningful change in HRQoL over time. Results: More than half (n = 91/151, 60.3%) of the participants experienced at least one AE during the observation period (mean 133 ± 116 days). No serious AEs were reported. Patient-reported pain impact scores were significantly reduced across the cohort (p = 0.034), and pain intensity scores verged on significance (p = 0.053). The majority of patients saw meaningful improvements in sleep (49.3%) and fatigue (35.6%). Conclusion: This analysis presents real-world data collected as part of standard of care. More than one-third of patients benefited from oral medicinal cannabis, which is impactful given the refractory nature of their pain. Amelioration of the impact of pain confirms continued prescribing of this formulation and validates our observational methodology as a tool to determine the therapeutic potency of medicinal cannabinoids
HLA-B*07:02 and HLA-C*07:02 are associated with trimethoprim-sulfamethoxazole respiratory failure
We have identified an underrecognized severe adverse drug reaction (ADR) of trimethoprim-sulfamethoxazole (TMP-SMX) associated respiratory failure in previously healthy children and young adults. We investigated potential genetic risk factors associated with TMP-SMX induced respiratory failure in a cohort of seven patients. We explored whole genome sequence among seven patients representing nearly half of all reported cases worldwide and 63 unrelated control individuals in two stages: (1) human leukocyte antigen (HLA) locus variation as several other ADRs have been associated HLA genetic variants and (2) coding variation to catalog and explore potential rare variants contributing to this devastating reaction. All cases were either heterozygous (carriers) or homozygous for the common HLA-B*07:02–HLA-C*07:02 haplotype. Despite the small sample size, this observation is statistically significant both in conservative comparison to maximum reported population frequencies (binomial P = 0.00017 for HLA-B and P = 0.00028 for HLA-C) and to our control population assessed by same HLA genotyping approach (binomial P = 0.000001 for HLA-B and P = 0.000018 for HLA-C). No gene elsewhere in the genome harnessed shared rare case enriched coding variation. Our results suggests that HLA-B*07:02 and HLA-C*07:02 are necessary for a patient to develop respiratory failure due to TMP-SMX
Potential methane emission reduction strategies from rice cultivation systems in Bangladesh: A critical synthesis with global meta-data
Methane (CH4) is one of the dominant greenhouse gases (GHG) that is largely emitted from rice fields and thus, significantly contributes to global warming. Significant efforts have been made to find out suitable strategies to mitigate CH4 emission from rice culture. However, the effectiveness of these management practices is often diverse with negative, no, or positive impacts making it difficult to adopt under a particular condition. The diversity of rice cultivation in terms of agro-climatic conditions and cultivation practices makes it difficult for providing specific recommendations. Here, we collected data from a total of 198 studies reporting 1052 observations. The management practices are categorized into five different management practices i.e., water, organic and inorganic fertilizer management, crop establishment method, and agronomic practices while major categories were subdivided into different classes. To test statistically significant differences in the effectiveness between major management practices, an analysis of variance (ANOVA) was applied. The Gaussian and bootstrapping model were applied to find out the best estimate of the effectiveness of each practice. In addition, mechanisms controlling the CH4 emission reductions were synthesized. Next, the adoption potentials of these practices were assessed based on the existing rice cultivation systems in Bangladesh. Our results showed that water and organic matter management were the most effective methods irrespective of the growing conditions. When these technologies are customized to Bangladesh, water management and crop establishment methods seem most feasible. Among the rice-growing seasons in Bangladesh, there is a larger scope to adopt these management practices in the Boro season (December to May), while these scopes are minimal in the other two seasons due to their rain-fed nature of cultivation. Altogether, our study provides fundamental insights on CH4 reductions strategies from rice fields in Bangladesh
Caesarean section following antepartum stillbirth in Western Australia 2010–2015: A population‐based study
Background
There is scant literature about antepartum stillbirth management but guidelines usually recommend reserving caesarean sections for exceptional circumstances. However, little is known about caesarean section rates following antepartum stillbirth in Australia.
Aims
We aimed to describe the onset of labour, mode of birth, and use of analgesia and anaesthesia following antepartum stillbirth and to identify factors associated with caesarean section.
Material and Methods
In this retrospective cohort study, we used a population-based dataset of all singleton antepartum stillbirths ≥20 weeks gestation in Western Australia between 2010-2015. The overall, primary and repeat caesarean section rates for antepartum stillbirths were calculated and multivariable Poisson regression analyses were performed to identify associated factors, and to calculate relative risks (RRs) and 95% confidence intervals (CIs).
Results
This study included 634 antepartum stillbirths. Labour was spontaneous for 134 (21.1%), induced for 457 (72.1%), and 43 (6.8%) had a prelabour caesarean section. The overall, primary and repeat caesarean section rates were 8.5%, 4.6% and 23.0% respectively and increased with gestation (P trends all <0.01). Other factors associated with an increased caesarean section risk included: any placenta praevia or placental abruption, birth at a metropolitan private hospital, large-for-gestational-age birthweight, and any maternal chronic condition. During labour, the most frequently used types of analgesia were systemic narcotics (46.0%) and regional blocks (34.7%) while among those who had a caesarean section, 40.7% had a general anaesthetic.
Conclusions
In Western Australia between 2010–2015, the caesarean section rates among women with antepartum stillbirths were low, in line with current guidelines
Development of 1,2,4-Oxadiazole Antimicrobial Agents to Treat Enteric Pathogens within the Gastrointestinal Tract
Colonization of the gastrointestinal (GI) tract with pathogenic bacteria is an important risk factor for the development of certain potentially severe and life-threatening healthcare-associated infections, yet efforts to develop effective decolonization agents have been largely unsuccessful thus far. Herein, we report modification of the 1,2,4-oxadiazole class of antimicrobial compounds with poorly permeable functional groups in order to target bacterial pathogens within the GI tract. We have identified that the quaternary ammonium functionality of analogue 26a results in complete impermeability in Caco-2 cell monolayers while retaining activity against GI pathogens Clostridioides difficile and multidrug-resistant (MDR) Enterococcus faecium. Low compound recovery levels after oral administration in rats were observed, which suggests that the analogues may be susceptible to degradation or metabolism within the gut, highlighting a key area for optimization in future efforts. This study demonstrates that modified analogues of the 1,2,4-oxadiazole class may be potential leads for further development of colon-targeted antimicrobial agents
Flow induced by a line sink near a vertical wall in a fluid with a free surface Part I: infinite depth
The two-dimensional, steady flow of an inviscid fluid induced by a line sink located near a vertical wall in a region of infinite depth is computed. The effects of surface tension are investigated. The solution in the limit of small Froude number is obtained analytically, and numerically for the nonlinear problem. The asymptotic solution is found to have a property that if the horizontal location of the sink, , a second stagnation point forms on the free surface. Numerical solution for the nonlinear problem confirms these properties. The effect of moving the sink horizontally has also been considered. The maximum Froude numbers at which steady solutions exist are computed and compared with the previous work
Employee psychological reactions to micro-corporate social responsibility and societal behavior: A structural equation modeling analysis
Employee psychological reactions to micro-corporate social responsibility (CSR) have recently been expanded in interdisciplinary management science research. The shreds of evidence in this regard are inconclusive, fragmented, and underdeveloped about how employee cognitive, behavioral, and affective dimensions of attitudes toward micro-CSR relate to each other and shape employee societal behavior (SB). In application of dual-process theories of attitude-behavior relations, we investigate the intra-relationships of perceived CSR-community (PCSRC; cognitive dimension of attitude), CSR engagement (CSRE; behavioral dimension of attitude), CSR positivity (CSRP; affective dimension of attitude). Also, we explore how these variables influence employee SB with a moderated-mediated model. Based on the opinions of 440 Bangladeshi employees as respondents, a structural equation modeling analysis confirmed the positive links from PCSRC to SB, PCSRC to CSRE, CSRE to SB, PCSRC to CSRP, CSRP to SB, and CSRP to CSRE. It also reported that CSRE mediated the relation between PCSRC and SB. It further examined that CSRP did not moderate the direct relation between PCSRC and SB, and the indirect relation between PCSRC and SB via CSRE at low, medium, or high employee CSRP. There are very crystal study implications that address policymakers to adopt CSR policy and its implementation strategies, accordingly, to employees’ psychological reactions to micro-CSR
Lurkers or posters? How teacher identity influences self-presentation on social networking sites
The ways in which educational practitioners represent themselves on social networking sites (SNSs) continue to provoke discussion and potential controversy. This study investigates how English as a Foreign Language (EFL) teachers in Vietnam manage their self-presentations within SNSs for professional learning purposes. Participants in this mixed methods study included 19 teachers in four focus-group interviews, and 393 respondents to an online survey. We applied teacher identity to interpreting teachers’ self-presentation on virtual platforms. Our findings reveal that although participants frequently use SNSs for professional learning, they prefer a lurking presence and less visible interaction. It is also evident that self-presentation on SNSs is not simply ‘represented’ or ‘reflected’. Rather, these teachers prefer their self-presentation to fit with how they are perceived in Vietnamese society. Teachers’ self-presentation within SNSs is thus robustly mediated by culture. The study contributes to the ongoing interest in teachers’ increasing use of SNSs in their professional lives
Small-scale movement and migration cues of Australian bass (Percalates novemaculeata) in an urbanised river
Urban river catchments are often severely affected by human activities but may still retain significant biodiversity. Surprisingly little is known about the behaviour of urban fishes, even those popular with anglers. Key environmental variables that trigger fish behaviour, such as river flow, are highly affected by instream structures including weirs and changes in the natural flow regime. Here, we used acoustic telemetry to examine the movements of Australian bass in a river located in suburban Sydney, Australia. We found that fish tended to be nocturnally active, however, small-scale movements were highly idiosyncratic and less associated with river flow than other factors. Larger-scale movements associated with spawning migrations were strongly correlated with winter floods. Half of the tagged fish migrated to the confluence of the river with Sydney Harbour to breed. Their return migration was hampered by a weir, but the provision of a new fish way facilitated successful return to freshwater at high tide. Despite occupying a highly affected, urban catchment fish behaviour showed similarity to those occupying more natural drainages. Our results highlight the importance of maintaining natural river flows in urban catchments and sustaining fish movement and migration capabilities through the installation of appropriate fish passage devices
