Single Case Experimental Design Examining Cognitive Behavioural Therapy for Post-Stroke Depression
Get PDFBackground. Post-stroke depression (PSD) is common in older-age adults and is typically treated with cognitive-behavioural therapy (CBT). However, research has ascertained mixed findings regarding the efficacy of CBT for PSD in older-age adults, with limited evidence in people from global majority backgrounds. Materials and Methods. This case study used single case experimental design methodology to examine the effectiveness of CBT for PSD in a black, older-age man. The effects of health conditions, cohort beliefs, transitions in role investments, socio-cultural context and early experiences on the client’s thoughts, emotions, physical sensations and behaviours were formulated, before behavioural activation was used to increase engagement in meaningful and pleasurable activities. Results. Throughout therapy, the client’s mood, motivation and optimism for the future improved, and their depression and anxiety symptoms reduced. Conclusions. This case study offers tentative evidence that CBT for PSD may be effective in black older-age adults
Metachronous mucinous breast carcinoma and mucinous pancreatic cystadenoma in a patient with metabolic comorbidities; case report and a literature review
Get PDFMucinous breast carcinoma is a rare subtype of mammary neoplasm encountered in medical practice. It represents approximately 1-6% of all breast cancers and is more common in postmenopausal women. There are two subtypes of mucinous breast carcinoma: pure and mixed. We report the case of a 73-year-old Caucasian woman diagnosed with mucinous pancreatic cystadenoma in 2007. She underwent a surgical procedure involving pancreatectomy of the body and tail, along with multiple known cardiovascular and metabolic comorbidities. In November 2019, during a clinical-biological evaluation for associated diseases, a mucinous carcinoma of the right breast was accidentally detected and later confirmed through further investigations. For treatment, a surgical procedure was performed involving lower right quadrantectomy with right axillary lymph node dissection. The postoperative clinical course was favorable. In conclusion, we present the case of a patient diagnosed with mixed mucinous breast carcinoma, without axillary lymph node invasion. Postmenopausal women with diabetes, overweight, and obesity have an increased susceptibility to breast cancer, with our patient having all these risk factors. To date, no study has demonstrated the genetic or radiation implication in the occurrence of mucinous carcinomas, nor the possible development of mucinous carcinomas in series in the same patient. Yet, current data are insufficient to provide recommendations for the screening of malignant mucinous tumors in subjects with significant cardiometabolic risk factors
A Multimodal Intervention to Aid in Smoking Cessation
Get PDFThe Centers for Disease Control and Prevention (CDC; 2023a) notes there are 28.3 million smokers in the United States. In Indiana alone, nearly 29% of adults currently use tobacco, with 19% of the total being cigarette smokers (Indiana Department of Health, 2022). The purpose of this evidence-based practice (EBP) project was to implement a multi-modal intervention to aid in smoking cessation in adults aged 18 and older who are cigarette smokers. The intervention included physician/nurse advise using the ask, advise, refer (AAR) model, the SmokefreeTXT program, and FDA approved smoking cessation medications. The project took place at a primary care office in Northwest Indiana where 15 cigarette smokers were recruited who were between the ages of 29-63. Participants were given the Fagerstrom Test for Nicotine Dependence (FTND) questionnaire pre and post intervention. The physician/nurse using the AAR model asked each cigarette smoker about tobacco product use, then advised the patient to quit along with referring patients that were interested in quitting to the SmokefreeTXT program and incorporating smoking cessation medication, if indicated. The primary outcome of this project was measuring overall cessation rates. The secondary outcome was noting any decrease in number of cigarettes smoked. Data was analyzed using the McNemar’s test for overall cessation rates and the Wilcoxon signed-rank test to analyze for the decrease in number of cigarettes smoked. Statistical evidence showed the intervention was effective for overall cessation (c 2 (1) = 4.167; p = 0.041). Also, there was statistical evidence of a decrease in the number of cigarettes smoked per day (V = 0; p = \u3c0.001). Findings from this project support the use of a multi-modal intervention to aid adult cigarette smokers to cease or decrease smoking
“The world withers and the wind rises”: Apocalyptic Language in \u27The Homecoming of Beorhtnoth\u27 and \u27The Fall of Arthur\u27
Get PDFAs previously explored by the author, Tolkien’s 1930’s alliterative verses published as The Legend of Sigurd and Gudrún and The Fall of Arthur make direct and repeated references to the End Times, paralleling the prominence of the apocalyptic Second Prophecy of Mandos in revisions to his Middle-earth legendarium written at that time. Similar themes can be found in Tolkien’s verse-dialogue The Homecoming of Beorhtnoth. The Homecoming’s genesis dates to the same general time period, and both The Fall of Arthur and The Homecoming describe a war-tainted world in apocalyptic tones, the environment dark (literally, figuratively, and in terms of men’s deeds). A close tandem reading of both works reveals linguistic and thematic similarities, especially related to apocalyptic themes
Major Northeastward Range Extension for a Western Grasshopper Species: New State Record for Trimerotropis pallidipennis (Burmeister) in Wisconsin
Get PDFTrimerotropis pallidipennis (Burmeister) mostly occurs in the southwestern U.S., but is known to migrate significant distances periodically. It appears that some individuals of this species occasionally migrate very long distances. Here I present an extreme record for this species, over 480 km northeast of any previously known records, including any others recorded from 2022. A single male specimen was collected and photographed in Price County in north-central Wisconsin in June 2022
